Abstract

The use of hormonal contraceptives relates with the incidence of breast cancer in Dr. Sardjito Yogyakarta Purpose: This study aimed to determine whether women using the contraceptive pill can cause breast cancer.Methods: A case control study was conducted August to October 2016 with 250 patients.Results: Factors that were associated with breast cancer were: type of pill with adjusted OR = 1.66 (95% CI = 1.21 to 2.28; p = 0.001), duration of consumption with adjusted OR = 2.25 (95% CI = 1.04 to 4.84; p = 0.037), and the period of last hormonal contraception with adjusted OR = 2.41 (95% CI = 1. 15 to 5.05; p = 0.020). This study showed that the use of hormonal contraceptives raises the risk of breast cancer.Conclusion: Women should play an active role in breast cancer prevention programs through early detection and treatment. Health services need to explain the potential risks of hormonal contraceptive.

Abstract

Rainfall, humidity, wind speed, availability of clean water and diarrheal cases in dry area of Kupang, IndonesiaPurpose: This study aimed to determine the relation of climate factors and the availability of clean water with the diarrhea occurrence in Kupang City from the years of 2011-2015.Methods: This study used an ecological study design with Pearson correlation with Spearman correlation tests and used secondary data consisting of diarrhea cases, water discharge, and climate data of Kupang City from 2011-2015.Results: The results of statistical analysis showed that variables that correlate with diarrhea occurrence in Kupang City were rainfall (p = 0.0001; r = -0.48), humidity lag1-3 (p = 0.000; r = -0, 5), wind speed lag2-3 (p = 0.000; r = 0,5 and clean water supply (p = 0.0002; r = - 0.47) while the air temperature variable had no correlation with diarrhea occurrence in Kupang city in 2011 -2015 (p value > 0.05).Conclusion: Based on the research results, it was concluded that the variables of rainfall, humidity lag1-3, wind speed lag2-3 and availability of clean water were correlated with the incidence of diarrhea in Kupang City from the years of 2011-2015.

Abstract

ABSTRAKLatar Belakang: Keadaan Teluk Kendari telah mengandung logam berat timbal (Pb) pada air dan sedimen yang cukup tinggi sehingga dapat terakumulasi dalam biota laut berupa ikan dan kerang. Masyarakat di sekitar perairan Teluk Kendari sering mengkonsumsi ikan dan kerang yang berasal dari Teluk Kendari. Mereka sangat berisiko untuk terpapar logam timbal dari ikan dan kerang yang dikonsumsi.Tujuan: Untuk mengetahui tingkat risiko gangguan kesehatan akibat pajanan logam timbal pada ikan dan kerang yang dikonsumsi oleh masyarakat sekitar Teluk Kendari. Metode: Penelitian ini adalah penelitian observasional analitik menggunakan desain cross sectional dengan penggabungan pendekatan Analisis Risiko Kesehatan Lingkungan (ARKL) dan Epidemiologi Kesehatan Lingkungan (EKL). Populasi dalam penelitian ini yaitu masyarakat yang bermukim di sekitar pesisir Teluk Kendari dengan pengambilan sampel yang memenuhi kriteria secara proporsional sampling sehingga diperoleh jumlah sampel sebanyak 110 responden. Biota laut dalam penelitain ini adalah ikan dan kerang yang berasal dari Teluk Kendari dengan jumlah sampel ikan sebanyak 30 sampel dari 3 jenis ikan yang paling banyak dikonsumsi dan sampel kerang diambil dari 6 tambak di Teluk Kendari. Pengukuran kadar timbal ikan dan kerang dilakukan dengan metode Atomic Absorption Spectrometry (AAS).Hasil: Hasil penelitian menunjukkan konsentrasi timbal dalam ikan berkisar antara 0,0027-0,0095 mg/l. Rata-rata intake ikan 0,0051 mg/kg/hari sedangkan untuk kerang 0,016 mg/kg/hari dan rata-rata besar risiko yang mengkonsumsi ikan adalah 1,29 sedangkan untuk kerang adalah 4,03. Variabel yang memiliki hubungan yang bermakna dengan tingkat risiko konsumsi ikan dan kerang adalah variabel konsentrasi timbal dalam ikan, laju asupan kerang, frekuensi pajanan ikan dan kerang, durasi pajanan ikan dan kerang dan jumlah asupan (intake) ikan dan kerang sedangkan yang tidak memiliki hubungan yang bermakna dengan tingkat risiko konsumsi ikan dan kerang adalah variabel konsentrasi timbal dalam kerang, laju asupan ikan dan berat badan.Kesimpulan: Masyarakat sekitar Teluk Kendari mempunyai risiko akan gangguan kesehatan (RQ>1) akibat pajanan timbal sehingga perlu dikendalikan.Kata Kunci : Analisis Risiko Kesehatan Lingkungan, Timbal (Pb), Ikan, Kerang, Teluk Kendari

Abstract

Interpersonal interaction and lecturer work stress in private universityPurposeThis paper discussed how and why the weakness of interpersonal interaction can lead to lecturer work stress.MethodThis analytic survey research was conducted with a cross-sectional approach involving 36 permanent lecturers of a private university. The independent variables were the working period and interpersonal interaction, the dependent variable was the work stress.ResultsThis paper found 50% of lecturers still lack interpersonal interaction. The problem was in communication among lecturers and university leaders, and awareness to build interpersonal interaction in the work environment. Lecturers with good interpersonal interactions have lower stress levels. Social support is very useful to strengthen the psychological aspects of lecturers in their activities.ConclusionThis paper concluded the relationship between interpersonal interaction with work stress. University need to regularly facilitate joint agenda for all staff to encourage close interpersonal interaction.

Abstract

Role of local health managers in innovating district level public health programs in Yogyakarta ProvincePurpose: The study aimed to describe the strategic decision making by the heads of the DIY health departments concerning the issue of maternal and child health and nutrition.Methods: This qualitative research was an exploratory study.Results: The district health office for Yogyakarta city in the Special Region of Yogyakarta does not have many strategic decisions to solve problems about maternal and child health and nutrition. However, the decisions are always hampered by human resources. Only the Gunung Kidul District Health Office stated that there was no political involvement in decisions by the head of the health department.Conclusions: The decisions made by heads of health department are still limited to the strategic plan and local medium term development plan (RPJMD). Not many innovations are developed to overcome the problems of maternal and child health and nutrition.

Abstract

Latar Belakang: Diabetes merupakan penyakit yang kompleks dan mahal yang dapat mempengaruhi hampir setiap organ dalam tubuh dan mengakibatkan konsekuensi yang tinggi. Data Riskesdas menunjukkan bahwa prevalensi diabetes yang terdiagnosis dokter tertinggi terdapat di DI Yogyakarta sebanyak 2,6%. Survei Dinkes Kota Yogyakarta tahun 2015, di Kota Yogyakarta terdapat 2638 orang penderita baru yang mengidap penyakit diabetes mellitus. Pada upaya meningkatkan derajat kesehatan masyarakat, Pemerintah wajib memberikan kemudahan kepada masyarakat untuk memperoleh akses terhadap informasi kesehatan.Metode Penelitian: Penelitian deskriptif analitik dengan rancangan penelitian cross sectional, menggunakan metode penelitian kuantitatif. Populasi adalah seluruh pasien diabetes mellitus yang tercatat di Yogyakarta. Sample penelitian sebanyak 336 orang pada empat puskesmas Kota Yogyakarta yaitu, Puskesmas Tegalrejo, Kotagede I, Mantrijeron, dan Wirobrajan.Hasil: Penelitan ini menunjukkan bahwa tidak ada pengaruh antara ketersediaan layanan informasi pola konsumsi dengan perilaku pengelolaan pada penderita diabetes mellitus ditunjukkan dengan p>0.25 (p=0.763). Sedangkan informasi pola konsumsi (p=0,003), informasi komplikasi gigi mulut (p=0,149), dan dukungan keluarga (p=0.148) memiliki pengaruh terhadap perilaku pengelolaan pada penderita diabetes mellitus ditunjukkan dengan nilai p<0.25.Kesimpulan: Pelayanan Puskesmas terhadap pasien penderita diabetes di kota Yogyakarta secara umum sudah baik akan tetapi masih kurangnya informasi dan kepedulian terhadap kesehatan gigi dan mulut pada penderita diabetes. Perlu adanya kepedulian oleh Puskesmas terhadap kesehatan gigi dan mulut pada penderita diabetes mellitus. Selain itu perlu inovasi pada penyampaian informasi pada pasien, terutama pasien lama.Kata Kunci: Ketersediaan Pelayanan informasi, Pola konsumsi, Gigi dan Mulut, Diabetes Melitus

Abstract

Latar belakangAngka komplikasi kebidanan di kabupaten Timor Tengah Utara tercatat 1024 kasus dari 1236 perkiraan ibu hamil dengan komplikasi kebidanan atau sebesar 82,9% pada tahun 2015. Belum semua puskesmas dan petugas kesehatan menerapkan pedoman manual rujukan kehamilan. Diperlukan pemanfaatan perkembangan teknologi berbasis komputer yang memiliki kemampuan untuk mendukung suatu keputusan rujukan kehamilan, serta memperkuat pedoman manual rujukan kehamilan.TujuanTujuan dalam penelitian ini adalah merancang  prototipe case based reasoning untuk mendukung keputusan rujukan kehamilan di puskesmas Kabupaten Timor Tengah Utara Provinsi Nusa Tenggara TimurMetodeJenis penelitian yang digunakan adalah penelitian kualitatif dengan pendekatan action research menggunakan tahap pengembangan prototipe meliputi identifikasi kebutuhan, rancangan prototipe, ujicoba dan evaluasi penilaian. HasilPerancangan sistem prototipe dibangun melalui beberapa proses diantaranya menggambarkan  diagram konteks, membuat diagram alir data, merancang diagram relasi entitas, menyusun representasi pengetahuan dan membuat menu sistem. Menu konsultasi merupakan menu interaksi antara pengguna dengan sistem inferensi penalaran berbasis kasus. Di dalam menu konsultasi terjadi proses aliran data analisa, perhitungan dan output kasus terpilih. Proses ini terjadi saat pengguna melakukan konsultasi dengan cara memilih gejala yang cocok sesuai hasil pemeriksaan pasien. Mesin inferensi akan melakukan retrieval atau penelusuran kelompok gejala kasus lama untuk mencocokkan dengan gejala kasus baru. Hasil output perhitungan kemiripan berupa kasus terpilih yang memungkinkan penggunaan ulang solusi kasus lama untuk menangani kasus baru. Pada tahap ini pengguna dapat mengambil keputusan suatu kasus dari hasil kasus terpilih. KesimpulanPengguna di puskesmas merasa terbantu dengan sistem penalaran berbasis kasus yang dapat digunakan sebagai petunjuk diagnosa dan solusi penanganan kasus. Hasil ujicoba prototipe memberikan penilaian yang positif dan pengguna mempunyai minat untuk menggunakan sebagai alat bantu dalam pengambilan keputusan rujukan kehamilan di puskesmas. Kata kunci : Prototipe, Penalaran berbasis kasus, Gejala, Rujukan kehamilan.  The design of a prototyped case based reasoning (CBR) to support pregranancy referral decision in Puskesmas Timor Tengah Utara of Nusa Tenggara Timur Province Background            The number of midwifery complications in Timor Tengah Utara Regency is noted 1024 of 1236 pregnant mother estimation cases with midwifery complication or 82.9% in 2015. Not all Puskesmas (Society Health Centres) apply pregnancy refferal manual orientation. It requires the use of computer-based technological development that provides competence to support a pregnancy refferal decision and strenghtens pregnancy refferal manual orientation. Objective            The objective of this research is to design case based reasoning prototype to support the decision of pregnancy refferal decision at Puskesmas of Timor Tengah Utara Regency, Nusa Tenggara Timur Province. MethodThe research type which is utilized is qualitative research using action research approach. It uses prototype development steps which involves need identification, prototype design, trial and assessment evaluation. ResultThe design of prototype system is built through some processes, among them are to draw context diagram, make data flow diagram, design entity relationship diagram, organize knowledge representation and make system menu. Consultation menu is an interaction menu between the user and case-based reasoning inference system. In the consultation menu, the processes of analysis data flow, case calculation and output are selected.  These processes occurs when the user performs consultation by selecting the symptom which is appropriate with patients’ monitoring result. Inference machine will perform retrieval or old case symptom group investigation to adjust it with the new case symptom. The output result of similarity calculation in form of selected case which enables the resuse of old case solution to handle new case. On this stage, the user is able to take decision of a case from the selected case result. Conclusion            The users at Puskesmas perceive that they are assisted with case-based reasoning system that can be used as a diagnosis direction and case handling solution. The result of prototype trial provides positive assessment and the user has an interest to use it as an instrument to take a decision on pregnancy refferal at Puskesmas. Keywords: Prototype, Case-based reasoning, Sympton, Pregnancy refferal

Abstract

ABSTRAKLatar belakang: Hipertensi yang dibiarkan dapat menyebabkan komplikasi terhadap jantung, otak, mata dan ginjal. Prevalensi hipertensi menurut jenis pekerjaan pada pegawai 20,6%, wiraswasta 24,7%, petani, nelayan dan buruh: 25%. Aktivitas fisik pekerja berdasarkan data Riskesdas 2013 yang masih kurang yaitu pegawai 23,3%; wiraswasta 18,3%; dan petani, nelayan, dan buruh 13,4%. Variabel luar yang mempengaruhi hipertensi yaitu faktor risiko yang dapat diubah dan tidak dapat diubah. Tujuan: untuk mengetahui pengaruh aktivitas fisik dan variabel luar terhadap hipertensi pada pekerja.Metode: Penelitian kuantitatif non eksperimen menggunakan data Riskesdas 2013 dengan rancangan cross sectional. Populasi sebanyak 1.027.763 dengan sampel yang memenuhi kriteria inklusi yaitu responden mempunyai pekerjaan utama, berusia ≥ 15 tahun, diukur tinggi badan, berat badan, lingkar perut, tekanan darah, dan tidak sedang hamil, sebanyak 371.713. Data dianalisis secara univariabel, bivariabel dan multivariabel.Hasil: Aktivitas fisik mempengaruhi kejadian hipertensi pada pekerja dengan OR sebesar 1,25 (CI 95%: 1,21-1,28), aktivitas fisik dengan melibatkan variabel luar menjadi 1,16 (CI 95%: 1,13-1,19). Variabel luar yang menjadi faktor risiko hipertensi meliputi umur, obesitas, obesitas abdominal, kebiasaan merokok setiap hari, jumlah rokok 10-20 batang/hari, kebiasaan konsumsi makanan berlemak, konsumsi sayur < 3 porsi/hari, stres, riwayat DM, gagal ginjal kronis dan batu ginjal.Kesimpulan: Aktivitas fisik berpengaruh terhadap kejadian hipertensi pada pekerja.

Abstract

Insecticide use and Aedes aegypti susceptibility status in endemic areas of PurbalinggaPurpose: This study aimed to identify the correlation between behavior towards insecticide use and susceptibility status of Aedes aegypti mosquito and to identify the level of mosquito susceptibility from an endemic area in Purbalingga.Methods: This research used a cross-sectional design and was conducted from January 2017 until April 2017 in four endemic villages of Purbalingga. 40 respondents were involved in the research. Samples from each village were taken using purposive sampling. Ovitraps were installed and interviews were conducted using questionnaires for selected households. Mosquito susceptibility status was described based on the results of WHO’s standardized bioassay test.Results: The results of susceptibility test indicated that Aedes aegypti from three endemic villages (Purbalingga Lor, Penaruban, Sempor Lor) were resistant, while mosquitoes from Kembaran Kulon Village were tolerant to cypermethrin 0.05%. Variables correlated with susceptibility status of Aedes aegypti mosquito were respondent's negative attitude (PR = 0.55; 0.37 - 0.80) and behavior towards insecticide use (PR = 1.24; 1.05 - 1.47).Conclusions: Misuse of insecticides increased susceptibility status of Aedes aegypti mosquito and becomes the main risk factor for resistance. It is necessary to rotate the insecticide use for programs and households through different modes of action and to provide public health education about insecticide use, selection of active ingredients, dosage, mode of application and periodic replacement of insecticides.

Abstract

Parents' communication about sexuality to premarital sexual behavior in adolescentsPurposeThis research aimed to examine the relationship between parent-child communication about sexuality and adolescents premarital sexual behavior.MethodsThis research involved 205 students in Senior High School "A" Yogyakarta. Data analysis was conducted with chi-square and logistic regression tests.ResultsThis research showed that there was no significant correlation (p > 0.05) between parental communication about sexuality and premarital sexual behavior in adolescents with OR score 1.43 (95% CI; 0.78- 2.63). The results of parental communication analysis of sexuality after considering other variables: such as gender, self-efficacy, and alcohol/drug used accounted for 5.7% in at-risk premarital sexual behavior.ConclusionThere were no significant correlations between parent-child communication about sexuality and adolescents’ premarital sexual behavior.
