Abstract

Background: Diabetes mellitus (DM) is a significant prevalence that occurs making Indonesia ranked sixth in the world. Diabetes mellitus can totally affect the quality of life of diabetic patients with complications, ulcers are complications that affect the quality of life of DM patients. One of the factors that can affect the quality of life of DM patients is sleep quality. Purpose: Knowing the relationship between sleep quality and quality of life in patients with diabetic ulcers. Methods: This type of research is analytic research with a correlational design with a cross-sectional approach. The sampling technique in this study was total sampling with a sample size of 36 respondents. Data analysis using the Chi-square test. Results: It was found that there were 32 respondents (88.9%) experiencing moderate sleep quality and there were 30 respondents (83.3%) who had a negative quality of life. Results: statistical test of the relationship between sleep quality and quality of life showed a statistical test p value = 0.662 or a significance value> a = 0.05, which means that there is no significant relationship between sleep quality and the quality of life of patients with diabetic ulcers. Conclusion: There is no relationship between sleep quality and the quality of life of patients with diabetic ulcers at the Griya puspa wound care clinic, Sleman Regency. Researchers suggest that respondents who have poor sleep quality are expected to improve their quality of life by improving physical health, leisure time, independence, managing emotions and performing good ulcer care.

Abstract

Background: systematic literature review investigating the impact of different anesthetics on intraoperative blood glucose levels of diabetic patients. Method: Databases including ProQuest, PubMed, ScienceDirect, and Wiley were searched for RCTs (randomized controlled trials) that examined the effects of various anesthetics on intraoperative blood glucose levels in diabetic patients. In one study, two researchers independently used eligibility criteria to examine the literature, extracted the data, and used RevMan 5.3 software to perform the meta-analysis. Results: We used trials for meta-analysis with seven randomized controlled trial overall with a total sample of 519 intraoperative diabetic patients. Compared with general anesthesia, it was found that the general anesthesia-epidural combination had superior glycemic control in intraoperative blood glucose levels, based on the meta-analysis results (SMD -1.78, 95% confidence interval [CI] -4.57 to -3.41), however epidural anesthesia did not show comparable superiority. Conclusions: Combined general-epidural anesthesia offers better glycemic control for postoperative blood sugar levels compared to general anesthesia, based on existing data.

Abstract

Background: Diabetes mellitus is a group of heterogeneous disorders that have indications in the form of increased content of high blood sugar levels. WHO also confirmed DM to be the 7th trigger of death. IDF (International Diabet Federation) data estimates from survey results after North America in 2017 the 3rd place was occupied by Southeast Asia and people with diabetes mellitus with a percentage of 8.5% at the age of 20-29 years in North Africa. One of the treatments for patients with diabetes mellitus can be wound care using Modern Dressing with the Moist Wound Healing method. Methods: The method used in this wound treatment is the Modern Dressing technique with the Moist Wound Healing method. This procedure can clinically grow tissue 30-50%, increase collagen synthesis by 50%, and the rate of re-epithelialization with moist conditions is 2-5 times faster and can reduce the amount of fluid from above the wound surface. Results: evaluation shows that Modern Dressing wound care with the Moist Wound Healing method is in fact able to maintain moist conditions in the wound, reduce inflammation in the wound so that it can improve and accelerate the healing process of diabetic ulcers. Conclusion: The results showed that the Modern Dressing technique with the Moist Wound Healing method was able to improve the healing process of diabetic ulcers. The results also showed that there were significant changes between modern and conventional dressings. The modern dressing group had a much better development than the conventional dressing. The application of the Moist Wound Healing method in patients with diabetic ulcers can provide a much faster wound recovery reaction.

Abstract

Background: Diabetes mellitus is a chronic disease that ranks as the fourth leading cause of death in developing countries. The main pillars in the management of type 2 Diabetes Melitus (T2DM) are education, nutritional therapy, physical training, and pharmacological interventions. The provision of education that has had a positive impact on DM patients is the Diabetes Self-Management Education (DSME). Method: The purpose of this study was to evaluate the effectiveness of the application and web-based Diabetes Self-Management Education (DSME) method in patients with type 2 DM. This study used a systematic literature review design. Articles were collected through PubMed and Google Scholar databases using the keywords DSME Web Application, DSME Smartphone. The criteria for articles used were those published from 2014-2020. Results: the literature review show that the application-based DSME method is more effective in improving self-management of type 2 DM sufferers. Search for articles related to 9 sources that fall within the criteria. Conclusion: is that the application-based DSME method is more effective because it does not require a lot of money and a long time compared to doing direct control to health facilities, the application can also always be accessed whenever we want so that it can help improve self-management in people with Diabetes Mellitus

Abstract

Purpose: Knowing the application of chest physiotherapy techniques can manage respiratory rates in children with bronchopneumonia. Methods: This review uses the literature review method. Searches were conducted in PubMed, Garuda, and Google Scholar. The search strategy used keywords with "bronchopneumonia", "chest", "physiotherapy", "respiratory rate". Findings: There were eight articles reviewed in this literature review. Seven articles discussed the application of chest physiotherapy and one article discussed the combination of nebulization therapy and chest physiotherapy in bronchopneumonia or pneumonia patients in children. The overall results of this review show that the application of chest physiotherapy in airway clearance problems in children with bronchopneumonia or pneumonia is effective in removing sputum, diluting sputum, and stabilizing hemodynamic status. Conclusions: The application of chest physiotherapy in children with bronchopneumonia or pneumonia has been shown to stabilize hemodynamic status such as pulse frequency, respiratory rate frequency, oxygen saturation within normal limits, and can remove secretions, sputum, and reduce cough frequency.

Abstract

Purpose: This study aimed to identify the incidence of hypotension after spinal anesthesia induction in SC patients during intra-anesthesia based on Body Mass Index (BMI), age, and baseline systolic blood pressure at Juanda Kuningan Hospital. Methods: This study used a quantitative descriptive method with a cross sectional approach. The sample in this study were all SC patients with spinal anesthesia techniques who met the criteria. The sampling technique used was purposive sampling with the slovin formula resulting in 90 respondents. And data analysis using univariate analysis. Findings: The results showed that the majority of respondents who experienced mild hypotension with excessive BMI status were 50 respondents (58.8%), the majority of respondents who experienced mild hypotension with early adulthood were 52 respondents (61.2%) and the majority of respondents who experienced mild hypotension with initial systolic blood pressure were at initial systolic blood pressure <120 mmHg and 120-130 mmHg as many as 31 respondents (36.5%). Conclusions: The results of this study illustrate that the majority of respondents will experience hypotension after spinal anesthesia induction. This is because spinal anesthesia can block sympathetic nerves, resulting in a decrease in circulating arterial tone, this condition causes peripheral arterial vasodilation. This vasodilating effect can result in a decrease in vascular resistance (hypotension). For further researchers to be able to develop this study to look for other variables that may be associated with the incidence of hypotension after induction of spinal anesthesia in SC patients so that the results of the study will show more variation.

Abstract

Purpose: to determine the description of changes in Mean Arterial Pressure (MAP) of patients after general anesthesia at Juanda Kuningan Hospital, West Java. Methods: This research is descriptive quantitative with a cross sectional approach. The sample in this study were all surgery patients with general anesthesia who met the criteria, while the sampling technique used was total sampling resulting in 100 respondents. Data analysis using univariate test. Findings: The results of the analysis showed that the most dominant age of respondents was early elderly (34%), with female gender (58%) and the most dominant Body Mass Index (BMI) was normal (91%). Normal MAP (70-100 mmHg) was obtained before induction of anesthesia as much as (77%), normal MAP (70-100 mmHg) after general anesthesia in the recovery room as much as (93%%). Conclusions: These results describe that changes in MAP values after general anesthesia can occur and the majority of pre-induction anesthesia MAP values of respondents will experience a decrease in MAP values during post-general anesthesia in the recovery room. So it is very important to maintain normal MAP values (70-100 mmHg) so as to reduce the risk of postoperative complications that can occur in patients.

Abstract

Purpose: Determine the effect of progressive muscle therapy on the anxiety level of pre-anesthesia patients at RSI Banjarnegara. Methods: This type of research is pre experiment pre test and post test with one group design. The sample technique in this study used purposive sampling of 37 pre-anesthesia patients. Data were taken using measurements of anxiety before and after progressive muscle therapy. This research instrument used questionnaires and SOP for progressive muscle therapy. Result: The anxiety level of respondents before being given progressive muscle therapy was the majority of 26 respondents (70.3%) experienced moderate anxiety. The anxiety level of respondents after being given progressive muscle therapy was the majority of 20 respondents (54.1%) experienced mild anxiety. The results of data analysis in this study with paired t test showed a value of (p) 0.000 ˂ 0.05. Conclusion: This shows that there is a significant influence on the provision of progressive muscle therapy at RSI Banjarnegara. Based on the results of the study, it is expected that the Banjarnegara RSI service will use progressive muscle therapy to reduce pre-anesthesia anxiety levels

Abstract

Background Evidence Based Practice (EBP) is scientific evidence with clinical expertise, pathophysiological knowledge, knowledge of psychosocial issues, and patient preference decision making. Based on data from WHO and the European Commission, health and social services must be supported by the best research evidence. Evidence Based Practice (EBP) plays an integral role in health care and has been recognized internationally as an ideal problem-solving approach. The ability of students to engage in understanding Evidence Based Practice is also still low, faculty in Iran who explain that knowledge of students involved regarding Evidence Based Practice is still low (47.1%). Meanwhile, undergraduate students' understanding of the main evidence-based practices in terms of assessment, discovery, and research applications is still low, namely (39%). Objective: To determine the level of knowledge of students of the Fellowship Study Program in the 7th semester of the applied undergraduate program regarding the Evidence Based Practice 2 course at Harapan Bangsa University. Methods: This study used a quantitative method with a cross sectional approach. The sample in this study used a non-probability sampling technique with a total sampling of 151 respondents. Results : The most dominant age of respondents with a good level of knowledge in the Evidence Based Practice 2 course in this study was 21-22 years old, with a percentage of 112 respondents (94.1%), gender with a good level of knowledge in the Evidence Based course The most dominant Practice 2 in this study was female, as many as 75 respondents with a percentage of (93.8%), the level of knowledge of respondents in the Evidence Based Practice 2 course in this study. The most dominant were respondents with a good level of knowledge, as many as 138 respondents with a percentage (91.4%).

Abstract

Purposive: Determine the effect of audio-visual health education using video media about spinal anesthesia on the anxiety level of preoperative patients with spinal anesthesia at Hj. Anna Lasmanah Banjarnegara hospital. Methods: This type of research is pre experiment pre test and post test with one group design. The sample technique in this study used purposive sampling of 92 pre-anesthesia patients. Data were taken using measurements of anxiety before and after provision of audio visuals on spinal anesthesia. This research instrument used questionnaires and audio visual media. Result: Before being given audio-visual health education spinal anesthesia most respondents experienced moderate anxiety of 45.7% and after being given some respondents experienced mild anxiety of 51.1%. The results of this study showed that there was an effect of providing audio-visual health education on the anxiety level of preoperative patients with spinal anesthesia as evidenced by a p value of 0.000 (0.000 0.005. Conclusion: There is an effect of providing audiovisual health education on the anxiety level of preoperative patients with spinal anesthesia at Hj. Anna Lasmanah Banjarnegara hospital.
