Abstract

Objective: To present a case of goniotomy procedure and angle imaging on primary congenital glaucoma (PCG) in Indonesian patient.Case presentation: a 6 month old baby presenting with enlarged left eye since birth. The ophthalmological examination found good light and object fixation on both eyes. The eyes were orthophoric. There was no nystagmus and movement restriction. The corneal diameter of the right eye was 10 mm and the left eye was 13 mm. Haab striae (+) on left eye. The intraocular pressure (IOP) was 11 mmHg on right eye and 35 mmHg on left eye. There were no other abnormalities on both eyes. A goniotomy procedure was done on the left eye. The IOP of the left eye on 1 day, 1 week and 3 month post surgery were 6, 18 and 17 mmHg respectively. The cornea of the left eye was clear despite the Haab striae dan bruckner test was good. Optic nerve head examination of the left eye was good and the cup to disk ratio was 0.5.Conclusions: Surgical therapy is usually warranted in the management of PCG. Goniotomy is typically chosen as the initial surgery when the refractive media is clear and direct visualization of the angle is visible. The advantage of goniotomy over other angle surgeries for PCG is spare of the conjunctiva for filtration surgery if required in the future. Our case of goniotomy on PCG in Indonesian patient showed good results and promising prognosis post procedure.

Abstract

Pendahuluan: Refractive surprise merupakan kejadian yang tidak diharapkan pasca operasi katarak. Refractive surprise dapat ditatalaksana baik secara konservatif maupun dengan tindakan operasi berbasis lensa maupun berbasis kornea. Tujuan: Untuk melaporkan hasil akhir sebuah kasus refractive surprise yang ditatalaksana dengan implantasi lensa intraokular piggyback sekunder. Ilustrasi Kasus: Pria berusia 54 tahun datang ke unit katarak dan bedah refraktif Rumah Sakit Mata Cicendo dengan keluhan utama mata kanan buram pasca operasi katarak 4 bulan sebelumnya. Tajam penglihatan dasar mata kanan 2/60 dan mata kiri 0.5. Hasil refraktometer mata kanan didapatkan S -6.00 C-1.00 x 96 dan tajam penglihatan terbaik dengan S-6.00 adalah 0.63. Pasien kemudian didiagnosis dengan myopic surprise mata kanan, pseudofakia dengan kekeruhan kapsul posterior grade I-II mata kanan dan katarak subkapsularis posterior mata kiri. Pasien kemudian ditatalaksana dengan implantasi lensa intraokular piggyback menggunakan Sensar AR40 foldabel three-pieces hidrofobik akrilik. Tiga bulan pasca operasi didapatkan tajam penglihatan terbaik pasien adalah 0.8 tidak dapat dikoreksi. Kesimpulan: Lensa intraokular piggyback sekunder merupakan tatalaksana refractive surprise berbasis lensa yang aman, mudah diprediksi, dan bersifat reversibel.

Abstract

We report an uncommon presentation of Wyburn-Mason syndrome in a pediatric patient. An 11-year- old girl presenting with no light perception in her right eye. Examination revealed an exotropia and a large and tortuous retina vessel in the posterior part of her right eye. However, other examinations, including dermatological and neurological examination, are within normal limits. Magnetic Resonance Angiography (MRA) showed vascular malformation at the basal cistern that extends to the retrobulbar area. Therefore, a thorough examination of the eye and brain, including MRA, is mandatory in every case, even in pediatric with normal neurological examinations.

Abstract

Introduction: The prevalence of ocular tuberculosis (TB) are 1% in patients with pulmonary TB and 20% in patients with extrapulmonary TB. The definitive diagnosis and its management are still challenging. This case highlights the diagnostic approach and successful management of ocular tuberculosis Purpose: To report the diagnostic approach and successful management of ocular tuberculosis.Case Presentation: A-15-Years-old-Girl, presented to outpatient clinic due to blurred vision on both eyes since 6 month ago. She was suffered from fever and solitaire mass along the neck lymph node 2 weeks before. The best corrected visual acuity (BCVA) of the right eye was 0.2 and the left eye 0.3. The ophthalmology examination of both eyes revealed mutton fat, 2+ flare, and 1+ cell, posterior synechia, koeppe and bussaca nodule, 4+ vitreous cells. Funduscopy were unremarkable due to vitritis. There was an elevation of erythrocyte sedimentation rate, positive mantoux test and Quantiferon-TB Gold. Chest radiograph showed fibrosis of the right lung. Fine needle aspiration biopsy of neck mass showed fibrotic tissue. Patient was diagnosed with tuberculous granulomatous panuveitis. She received anti-tuberculous therapy (ATT) along with high dose methylprednisolone 0.5 mg/BW/days on tapered dose. At 3 weeks follow up, vitritis was subsided and we revealed snowball, snowbanking, and optic disc swelling of both eyes that showed improvement at 11 weeks follow up. The BCVA become 1.0 at week-17 of therapy and remains stable until 8 months follow up. Conclusion: Diagnosis of ocular tuberculosis diagnose was made based on symptoms and signs of granulomatous panuveitis and supporting evidence of pulmonary TB ancillary tests. Significant clinical improvement was achieved after administration of ATT along with high dose oral steroid.

Abstract

Objective: To increase the awareness in diagnosing orbital pseudotumor and its management, especially in a peripheral secondary hospital with limited facilities.Case presentation: A 47 years old woman came with left eye (LE) protrusion, blurred vision, retrobulbar pain, and severe headache at regular intervals. This condition begun 8 months ago, and was diagnosed with orbital cellulitis, however the symptoms persisted until now. Physical examination of the LE showed a decreased in visual acuity, protrusion, chemosis and conjunctiva injection, increased in IOP, limited ocular motility, ipsilateral afferent pupillary defect and optic disc oedema. Plain and contrast CT-Scan showed a hypertrophy of ocular muscles with soft tissue swelling. Patient then diagnosed with idiopathic orbital inflammation and immediately prescribed with high dose steroid for 5 days with observation with glucose level observation for toxicity. Afterwards, she was given an oral corticosteroid and was tapered off for the next few weeks. At two months follow up, there were improvement in protrusion, motility and visual acuity. Conclusion: A complete history taking, physical examination and appropriate additional examination are needed. In hospital or clinical setting with limited facilities, a thorough and comprehensive examination are needed in order to establish the diagnosis and management of orbital pseudotumor.

Abstract

Introduction: Minimally invasive glaucoma surgery (MIGS) is continuously being improved after it was first initiated in the early 2000s. This promising method is hoped to fill the gap between medication and traditional surgeries in management of glaucoma.Objective: To evaluate the efficacy and safety of iStents implantation combined with cataract extraction in managing intraocular pressure in glaucoma patients. Methods: We collected data from 6 eyes of 5 patients with POAG and cataract pre-operatively. The extraction of cataract was then performed, combined with second generation iStents implantation. The follow up procedure included the change of intra ocular pressure (IOP), Visual acuity, and the need for IOP lowering Medication at 1 week and 1 month post-operatively. Results: There was a reduction of mean IOP from the baseline, which was 21.33 ± 6.15 to 18.0 ± 6.0 and 18.11 ± 5.57 at 1-week and 1-month following the surgery. The reduction of IOP was also followed by the reduction of medications needed for the patients. The mean visual acuity following the surgery was also increased from the baseline 0.53 ± 0.41 to 0.85 ± 0.17 at 1 – week and 0.91 ± 0.16 at 1 – month. For safety outcomes, there were no major adverse effect nor complication from the procedures after 1 month of follow up. Conclusion: The iStents implantation helped lowering the IOP of patients with glaucoma. It also reduced the need for further IOP lowering medication following the procedure.

Abstract

Background: Uncorrected refractive errors that could seriously affect children’s future. The compliance of spectacle-wear has been a problem in refractive errors management in developing countries in terms of the cost, availability and service range of both medical professionals and optical service. Aim: To study the compliance inferiority of ready-made spectacle-wear compared to custom spectacle-wear in refractive errors screening program.Method: Non-inferiority trial research, cluster-randomized trial was conducted to children aged 11-15 years old. Participants with corrected refractive errors (presenting visual acuity <6/12, spherical equivalent -6.00 until +6.00 Diopter (D), astigmatism ?1.00 D, anisometropic ?1.00 D) without other ocular abnormalities were given ready-made spectacles (RMS) and custom spectacles (CS). The observation was carried out in the first (1st) and third (3rd) month; subsequently, non-inferiority test was conducted with 20% margin. Result: Of 1009 school children, 365 were detected with uncorrected refractive errors. Among this number, 220 participants that met the inclusion criteria were prescribed RMS (n = 110) and CS (n = 110). The most common type of refractive errors found was myopia with mild degree of refractive errors. Compliance rate on the first (1st) month for RMS-wear was 63.6% and for CS-wear was 75.5% with d = -11.9% (95% CI -17.95% until -5.85%). Compliance rate on the third month (3rd) for RMS- wear was 65.5% and for CS-wear was 72.7% with d = -7.2% (CI 95% -12.03% until -2.37%). Conclusion: Based on the compliance observation performed in the first (1st) and third (3rd) month, there was no inferiority found among RMS-wear compared to CS-wear. This result could be used as the foundation of RMS use as an alternative for refractive errors management in refractive errors screening program to solve problems in the area of cost, availability and service range of both medical and optical service.

Abstract

Introduction: Uncorrected refractive error remains a leading cause of blindness and visual impairment among different population groups worldwide. In school children, it has a considerable impact on quality of life including social development, school performance and academic achievement that can affect their future. Management of refractive errors in developing countries is not adequate related to the cost, availability and distance of health and optical services. Hence, early detection and intervention with ready-made spectacles at the time of school children vision screening programs is expected to help solving these problems. Objective: To evaluate outcome measure of wearing ready-made spectacles (RMS) and custom-made spectacles (CS) on children’s quality of life.Method: A non-inferiority trial with a margin of 25%, cluster-randomized trial. Children aged 11-15 years in Bandung Regency, that meet the inclusion criteria were randomized to receive RMS and CS. Main study outcome was global score on the National Eye Institute Refractive Error Quality of Life- 42 after one month of wearing study glasses, comparing both group. Result: A total of 220 eligible subject were allocated to RMS (n = 110) and CS groups (n = 110) respectively. Baseline and end line score data were available for 220 (100%) of subjects. Improvement of quality of life in RMS group was 19.48±9.33 and CS was 24.14±9.68 with D = -18.11(CI 95% - 19.36 to -16.64). Conclusion: Improvement of quality of life in RMS group is non-inferior compare with CS group

Abstract

Purpose: To compare intraocular pressure (IOP) after water drinking test in primary open angle glaucoma patients controlled with latanoprost 0.005% and trabeculectomy.Methods: Observational study with cross-sectional design of 28 eyes with primary open angle glaucoma (POAG) patients. Subjects were divided into two groups, 14 patients controlled with trabeculectomy and 14 patients with latanoprost monotherapy. All subjects were under clinical therapy and had an intraocular pressure (IOP) equal or lower than 18 mm Hg, monitored during the follow up period. All subjects were submitted to the water drinking test. The results of the water drinking test, which were peak and fluctuation of IOP, were then compared between groups. Results: There were no significant difference between IOP peak between two groups (p=0.88). IOP fluctuation also showed no significant difference between groups (p=0.84). In both groups, baseline IOP strongly correlated with peak IOP in water drinking test with correlation coefficient r=0.96 (p<0.001) and r=0.71 (p=0.04), while baseline IOP in trabeculectomy group was strongly correlated with IOP fluctuation with correlation coefficient r=0.86 (p<0.001). Conclusions: Peak and fluctuation of IOP after water drinking test in primary open angle patients controlled with latanoprost and trabeculectomy showed no significant difference.
