Abstract

Inflammation of the pericardium due to either an infectious or non-infectious etiology, which is known as pericarditis. About 1% of hospitalized patients had this illness, and 5% of those patients complained of chest discomfort. About one-third of individuals with acute pericarditis had cardiac involvement, suggesting that the two diseases may have similar etiologies. We describe a 19-year-old man who experienced chest discomfort and was later diagnosed with myopericarditis. Empirical treatment utilizing NSAIDs and colchicine produced excellent outcomes in terms of symptom relief, ECG improvement, and CRP decrease. To reduce morbidity and mortality from sudden cardiac death, it is critical to recognize the signs and symptoms that fit the criteria for myopericarditis..

Abstract

Background: Coronavirus Disease 2019 (COVID-19) is a disease caused by SARS-CoV-2 with flu-like symptoms that could be serious in high-risk individuals. The use of laboratory parameters is determined as a predictor in improving the quality of care and thereby reducing mortality and morbidity. One of the laboratory parameters itself is hematological examination, including leukocyte count. Several studies had shown that an increase in WBC count was independently associated with an increased risk of proteinuria.Aim: To determine the relationship between leukocytes and proteinuria in COVID-19 patients.Method: This cross-sectional study with retrospective data collection had a sample of patients with COVID-19 at Diponegoro National Hospital aged 19-60 years. The independent variable in this study was leukocyte count, while the dependent variable in this study was proteinuria. The type of data used was secondary data obtained by examining the medical records of COVID-19 patients. Data analysis between leukocyte count and proteinuria variables in patients with COVID-19 was tested using a bivariate chi-square analysis.Results: Samples that met the inclusion criteria were 34 samples with confirmed COVID-19. From the results of the chi square test, it was found that leukocytes to proteinuria had a p value of 0.601 (p>0.05), which meant that there was no significant relationship between the number of leukocytes and proteinuria.Conclusion: There was no significant relationship between leukocyte count and proteinuria in patients with COVID-19.

Abstract

Background: Infertility in men can be caused by decreased motility of spermatozoa. Electric cigarette smoke containing nicotine can produce free radicals that cause a decrease in sperm motility. Purple grape juice which contains flavonoids can act as antioxidants that can increase the motility of spermatozoa. Objective: To prove the effect of administering purple grape juice on sperm motility of male Wistar rats exposed to electric cigarette smoke.Methods: This research is an experimental study with a post-test-only control group design. Sample consisted of 28 male Wistar rats divided randomly into 4 groups, namely negative control, positive control (given electric cigarette smoke), treatment one (given electric cigarette smoke + purple grape juice 3 mL/day), and treatment two (given electric cigarette smoke + purple grape juice 5 mL/day). The adaptation was carried out for 1 week, followed by treatment for 4 weeks, then the sperm of Wistar rats were collected and the sperm motility of Wistar rats was examined. Statistical test used Saphiro Wilk and Kruskal Wallis test.Results: Progressive motility rate of K(-), K(+), P1, P2 groups consecutively: 58.33 ± 7,52; 26,66 ± 8,16; 31,66 ± 7,52; 43,33 ± 12,11. Non progressive motility rate of K(-), K(+), P1, P2 groups consecutively: 26,66 ± 10,32; 55 ± 8,36; 43,33 ± 12,11; 35 ± 12,24. Immotility rate of K(-), K(+), P1, P2 consecutively: 18,33 ± 9,83; 28,33 ± 14,71; 20 ± 6,32; 16,66 ± 5,16. Kruskal Wallis test found a significant difference in progressive motility (p=0.002) and non-progressive motility (p=0.005).Conclusion: Purple grape juice affects sperm motility of male wistar rats exposed to electric cigarette smoke.

Abstract

Background. Accuracy in archery is the ability to achieve (point) against the target or Target Face when the arrows are out of the bow using a high level of consentration in carrying out a series of shooting techniques. Based on many numbers of arrow athletes who won the championship by balancing their body balance and maintaining their stamina.Purpose. To understand the possibility of any potencies or some overviews of sleeve length, arm muscle strength, limb length, and body mass towards the archery accuracy in archery athletes.Method. This research is an analytic observational research with a cross sectional approach, which is a research that emphasizes measurement time or data observation one time at a time, which is done on the dependent variable and independent variable. This research only makes observations, without giving any intervention on researched variables. The data analysis used is pearson correlation test and using hypothesis test, and by performing a linear regression test.Result. The pearson correlation test using sleeve length towards the accuracy of archery showed the correlation score (r) equal to 0,576 (p=0,002) significant (p<0.05), limb length towards the archery accuracy with the correlation score (r ) equal to 0,631 (p=0,001) significant (p<0,05). And the hipothesis test obtained the arm length, limb length, and body mass index showed that there was not any meanignful correlation towards the accuracy of archery, and dominantly correlating with the arm muscle strength.Conclusion. The accuracy in archery has a high effect, namely with sufficient training to build good muscle strength.Key Words. Accuracy of archery, exercise, arm muscle strength.

Abstract

Background: Cases of cerebral toxoplasmosis continue to burden people living with HIV/AIDS (PLWHA). This surge in cases is primarily due to patients’ delayed definitive treatment, missed follow-up, and insufficient diagnostic equipment in some regions, particularly in developing countries such as Indonesia.Case Presentation: We report on a 35-year-old male referred to the emergency department with impairment in four extremities from a secondary healthcare facility. Specific atypical symptoms that mimicked subacute meningitis were identified during the physical examination. Internists, neurologists, and rehabilitation medicine professionals were associated with the management.Conclusion: We suggest that improved early diagnosis and treatment of HIV patients to prevent opportunistic infections, including cerebral toxoplasmosis, is needed, particularly in primary healthcare facilities in developing countries.

Abstract

Abstract Background: Hand, foot and mouth disease or HFMD is a common childhood infectious disease mainly caused by coxsackievirus A16 (CVA 16) and enterovirus 71 (EV71). This HFMD disease can spread throughout the world and also cause outbreaks. Occurrence of this disease is mostly reported in the summer and fall. However, in Indonesia itself, HFMD is still not getting enough attention due to its self-limiting nature, so there is not enough epidemiological data.Objective: This research was conducted to find out the description of hand, foot, and mouth disease or HFMD.Methods: Conducting a literature review of previous journal articles related to hand, foot, and mouth disease or HFMD.Results: HFMD caused by coxsackievirus A16 or CVA16 and enterovirus 71 or EV71 which are self-limiting. Contact with saliva, fluids, and feces of HFMD patients can trigger transmission. Meanwhile, vaccines and antivirals have not been found that can be a way to treat and prevent EV71.Conclusion: One solution that is considered effective for preventing HFMD is to always practice clean living. Keywords: Hand, foot and mouth disease; HFMD; eEV71; CVA16; A16

Abstract

 Background: Klatskin tumor is a rare liver cancer that involves the bile ducts. Its incidence begins to increase over time and is usually only diagnosed in patients who are already at an advanced stage. The annual incidence of klatskin tumors is not more than 1:100,000. Klatskin tumor is suspected based on signs of biliary obstruction, ultrasonography showing a stricture or bile mass, cholangiography with ERCP and MRCP, abnormal liver function tests, and elevated tumor marker CA 19-9. Case Report: A 65-year-old man presented to the hospital with complaints of yellowing of the eyes and body that began one month ago, as well as a lump in the right upper abdomen that had grown larger over the previous five months. Physical examination revealed yellow eyes, and a palpable lump in the upper quadrant of the patient's abdomen. The patient defecates looked like putty and urinates looked like tea. The diagnosis leaded to a klatskin tumor because the results of MRCP showed a mass in the hilar region of the liver that extended to the proximal common bile duct, which described the appearance of a klatskin tumor. There were an increasing CA 19-9 level and liver function tests. Conclusion: Patient has been diagnosed with klatskin tumors, has choledocholithiasis risk factor, supported by appropriate MRCP images and increasing CA 19-9 level

Abstract

Background : Uric acid is the end product of purine catabolism carried out in the liver. Increased level of uric acid in blood is called hyperuricemia, which might cause gout if it is not treated properly. Melinjo (Gnetum gnemon L.) plant is a native plant of Indonesia that contains flavonoids and stilbenoid potential as antihyperuricemia. Aim : This paper aimed to study the effect of melinjo seed extract on uric acid levels. Methods : Thirty-six male wistar rats were randomly divided into 6 groups, consists of healthy control group, hyperuricemic control, Allopurinol control, and treatment groups (3 groups).  Hyperuricemia was induced by the administration of block broth and potassium oxonate for 4 weeks. Melinjo seed extract doses of 250, 500, and 2000 mg/kg BW, and allopurinol 90 mg/kg BW was given orally for 2 weeks. Statistical analysis was conducted to evaluate differences among groups before and after the intervention. Results : Three doses could significantly reduce uric acid levels (p<0.05) from 5.61±0.95 to 2.45±1.21; 5.86±1.84 to 2.04±1.11; and 5.95±0.91 to 3.59±1.58, respectively. However, there is no significant difference between the three doses. The effectiveness of melinjo seed extract at a dose of 500 mg/kg BW reduced uric acid levels by 65%, a dose of 250 mg/kg BW by 56%, and allopurinol 90 mg/kg BW by 10%. Conlusion : Melinjo seed extract can reduce uric acid levels even though there is no effect of graded dose.

Abstract

Background: Esophageal varices are dilated submucosal veins that connect the portal and systemic circulations to the esophagus. It occurs because of portal hypertension (most caused by cirrhosis), portal blood flow resistance, and increased portal vein blood flow. Esophageal variceal bleeding has a higher morbidity and mortality rate than other upper gastrointestinal bleeding types. In cases of esophageal variceal bleeding, an endoscopic examination is required to make a diagnosis, assess varicose veins, and plan management based on the underlying disease. This case report reviews the diagnosis and management of the esophageal varices at ENT-KL department. Case Presentation: We present a case in which a 54-year-old woman presented to the emergency department of Cut Meutia Hospital with complaints of painful swallowing and a swollen neck. The patient has had this complaint for 5 years, and is getting worse by the day. A barium swallow examination revealed esophageal varices in one-third of the distal esophagus between the seventh and ninth thoracic vertebrae. Conclusion: The patient was diagnosed with esophageal varices and was treated several times before being transferred to a gastroenterology department.
