Abstract

Periodontal disease is an inflammation and degeneration of chronical dental support tissue, accumulative, and progressive that caused tooth loss. Periodontal disease is caused by bacteria that has an ability to activate host response to produce pro-inflammatory mediator. Pro-inflammatory mediator causes collagen fibers degradation or destruction in periodontal tissue. Collagen cross- link of peri odontal tissue would be broken down and released into serum, and then excreted through urine. Collagen cross-link is called pyridinium cross-link, such as pyridinoline, deoxypyridinoline, N-telopeptide, and C- telopeptide (ICTP). This study was to investiga te the level in gingival crevicular fluid as an indicator of alveolar bone resorption. This study used 24 subjects with periodontal disease and 6 healthy subjects. Dividing of periodontal disease was based on periodontal index and every subject had minimum 20 teeth in mouth. Gingival crevicular fluid was taken at mesial site of maxillary posterior tooth by paperpoint and was measured by ELISA technique. The result showedthat the lowest level of ICTP was in control group, and the highest level was in grade 3 periodontitis group. The level of ICTP increased followed by periodontal disease progression. The result of Kruskal- Wallis-H and Mann-Whitney-U test showed th at was significant difference in ICTP between subject with and without periodontitis (p<0.05). It can be concluded that ICTP level in gingival crevicular fluid can be used as indicator of alveolar bone resorption in periodont al disease subjects.

Abstract

Oral and respiratory diseases have been related with the fungal infections like oral candidiasis caused by Candida albicans and chronic rhinitis caused by Aspergillus niger. Both of these micr oorganisms are including in flora normal in the oral and nasal cavity. Aspergillus niger is rarely reported as aeroalergen in allergy rhinitis cases, but in chronic rhinitis its fungals are A. flavus and A. fumingatus. The aim of this study was to analyze the risk factors that influencing rhinitis and oral candidiasis and its relation. This research used rhinitis mucous identified various fungal used pour plate method.And then, cultured with SDA medium (Sabauraud's Dextrose Agar) to identify fung als morphology usedcultured slide.The results showed that Aspergillus niger was found more dominant than A. fumingatus and A. flavus in chronic rhinitis.In conclusion, Aspergillus niger was proved as an aeroallergen had had an important role in chronic rhinitis a nd could trigger the growth of Candida albicans.

Abstract

Radical suproxide is a kind of reactive oxygen species (ROS) which causes oxidative damage by attacking biomolecules such as protein, lipid, DNA, etc. Over production of radical superoxide becomes a centre of the generation of other radical species and oxidants, and responsible for occurring many pathological processes. The main source of radical superoxide isto activate neutrophil. Therefore, it was speculated that induction of neutrophil by periodontitis bacteria Porphyromonas gingival is potential to induce superoxide production, only a few study, however concerned with this topic. This studyaimed to know the pathogenicity of P. gingivalis by demonstrating the potency of P. gingivalis metabolites to induce production of radical superoxide by neutrophil. This study was conducted experimentally in vitro using posttest onlydesign. Superoxide radical production was demostrated by means of Nitrobluetetrazolium (NBT) slide assay and spectro photometric. The results showed that P. gingivalis metabolites induced neutrophil to produce a large amount of radical superoxide both intra and extra cellular. Its radical superoxide production affected degranulation, apoptosis, and lysis of neutrophil. In conclusion, neutrophil responses against P. gingivalis generated prooxidative and toxic condition. This mechanism may explain the pathogenicity of P. gingivalis on many pathological processes.

Abstract

Oral hygiene of removable denture users is necessarily concerned, since it may support oral health completely. Removable denture users necessitated to clean their dentures in order to prevent food debris attach on the denture. One of the methods is to inform the users about how to clean removable denture and the communication built b etween dentist and the patient, and it may be accompanied by some aids such as leaflet. The purpose of this study was to know the influence of verbal and written communication by providing leaflet on the knowledge, attitude and denture hygiene of theremovable denture users , who ordered removable dentures at the Prostodontic Clinic in Dental Hospital Jember University. Total sample was 96 subjects, the research were patients who came to Dental Hospital Jember University for removable denture. The samples were divided into 2 groups, the first group received verbal and written communication (leaflet) a nd control group achieved only verbal communication. The communicator explained the methods of maintaining a removable denture. The results showed that there was a significant differences (p=0,00) o f the knowledge behavior between the two groups and the denture hygiene. In conclusion, there were significant differences on the knowledge, attitude and denture hygiene between verbal and written communication of the removable denture user.

Abstract

Orthodontic toothbrush designed specifically to eliminate plaque around brackets, but there are still many patient who use conventional toothbrush. This study aimed to know the effect of an orthodontic toothbrush compare to a conventional toothbrush in reducing plaque index. The plaque index used was a modification of patient hygiene performance (PHP) index developed by Podshadley and Haley. The design of this study was a pre and posttest experimental design. Sample consist of 80 students of dental faculty. The results of the t- test statistical analysis s howed that there was a significant difference of reduction plaque index from 2.96 to 2.07 (p <0.05). It can be concluded that a specific orthodontictoothbrush has a better role to reduce the plaque index than a conventional toothbrush.

Abstract

Chronic periodontitis is a chronic inflammatory disease that causes a damage of tooth supporting tissues. The incidence of chronic periodontitis disease is highly reported in Indonesia .S urvey in Java, Bali and Sulawesi reported the incidence of chronic periodontitis for 35-44 years age group was 88.67%. This disease is a major cause of tooth loss for 35 years or more age group. The results of various studies was found many chronic periodontitis diseases are common in young age, therefore the genetic role in diseases is susceptible for chronic periodontitis. The aim study was to know if the mutation of the gene Nucleotide-Binding Oligomerization Domain 2 (NOD2) is a risk factors for chronic periodontitis. Total sample was 162 subject, 81 samples with chronic periodontitis and 81 without periodontal disease as control group. The results showed that there was 12,3 % of samples that had been mutation of NOD2 gene, and in the control group only 1,2 %, and statistical l y there was a significant difference ( p=0,005). In conclusion, mutation NOD2 gen is a risk factor to chronic periodontitis.

Abstract

The periodontopathogens Porphyromonas gingivalis may directly or indirectly attack the fetus by releasing toxins into the blood stream that could reach the placental and influence fetal development . This study aimed to determine the effect of periodontal infection with P. gingivalis on fetal development in pregnant rat model. Female rats were infected with live-P. gingivalis at concentration of 109colony forming unit/ml into subgingival sulcus before and/or during pregnancy. Group I: without P. gingivalis infection ; group II: P. gingivalis infection before and during pregnancy; group III: P. gingivalisinfection before pregnancy; and group IV: P. gingivalis infection during pregnancy. They were sacrified on gestational day 14 and 20. Fetuses were evaluated for weight and crown-tail length. The results showed that the dams infected with P.gingivalis significantly decrease the mean of fetal weights, lengths and placental weights compared with the control group (p<0.05). The percentages of fetal growth restriction at the time of sacrifice were 6.66, 100, 72.97 and 87.09% growthrestricted fetuses in group I, II, III, and IV, respectively. When weights of growth- restricted fetuses of the treated groups were compared with the control group there were significant differences (p<0.05). P. gingivalis was detected by API ZYM system in the blood of umbillical cord from the treated groups. In conclusion, in pregnant rat models, periodontal infection with P. gingivalis affects fetal development. The maternal P. gingivalis infection on periodontal tissue can result in P. gingivalis dissemination to umbillical cord and induction of FGR.

Abstract

Inflammatory pulpal pain may arise due to the increased pressure inside the pulp or the release of prostaglandin E2(PGE2). In the inflamed pulp, levels of PGE2 and substance P (SP) is higher than those of normal pulp. PGE2 sensitizes all nociceptor while SP can induce the activity of cyclooxygenase-2 (COX- 2) and is an exitatory neurotransmitter. Jatropha curcas Linn latex is widely used for pulpal pain relief. The aim of this study was to evaluate the effect of the latex and extract of J. curcas on the dental pulp expression of COX-2 and SP. Thirty-six pulpitis-induced dental pulp of Macaca fascicularis , were divided into 3 groups: Group 1 served as controls, Group 2 was given latex, and Group 3 was given extracts. The ELISA assay was used to determine the levels of SP and COX-2. SP data was analyzed with ANOVA (p <0.05) while the COX-2 data was analyzed with Mann Whitney. The results showed that the levels of SP (pg/mL) of the control, latex, and extract group were 28.94; 26.22; 28.89 respectively, while levels of COX-2 (ng/ml) of control, latex, and extract group were 0.04; 0.08; 0.10 respectively. In conclusion , J. curcas can reduce the levels of SP, J. curcas latex has lower levels of SP than extract, but does not provide clear results in decreased levels of COX- 2. Further study requires the mechanism of SP, and the concentration of COX- 2 needs to be further investigated using different methods.

Abstract

Pulp irritation will form reparative dentin as a defense mechanism. Currently, the materials used to help the defense pulp still have many shortcomings. Therefore, the effect of dental pulp stem cells (DPSC) and rhBMP- 2 combination on alkaline phosphatase (ALP) expresion in rat ’s dental pulp was analyzed in this research. Lipopolysaccharide was used to irritate the pulp of 12 Sprague-Dawley rats. Materials were then applied to each group and ALP expression was performed on the first and second week. The result showed that in the first week, giving a combination of pulp stem cells with recombinant BMP-2 had not been visible to increase expression of alkaline phosphatase (ALP), a biomarker regeneration of pulp, but the increase in ALP expression occurred in the second week. In conclusion, a combination of pulp stem cells and rhBMP-2 can increase the expression of ALP on inflamed rat’s pulp.

Abstract

Children in pre-educational ages are vulnerable to the caries of the permanent first molar. Early prevention on this problem may be conducted through promotion of oral and dental health to pre-educational children such as in the education of kindergarten. The aim of this study was to observe the diff erence of the number of caries in the permanent first molar in children of 7-8 years old with pre- elementary and non pre- elementary educational backgrounds. The study was an observational study with cross-sectional approach. The population was elementary school students of 7- 8 years old at Patrang District Jember Regency. The samples were obtained using stratified random sampling. Total sample was 314 students, consit of 157 pre-elementary (kindergarten) and 157 non pre- elementary. Data was analyzed using MannWhitey Test. The result showed that caries without experience of permanent first molar in the children of 7- 8 years old with was 0.07 pre-elementary and 0.44 pre- elementary. It can be concluded that there was a significant differencebetween caries experience of the permanent first molar in the children aged 7-8 years old with and without pre-elementary educational backgrounds .
