Abstract

Fraktur patologis tulang panjang ekstremitas pada pasien kanker metastasis tulang mengakibatkan morbiditas yang sangat serius. Kejadian ini terus meningkat dengan semakin baiknya harapan hidup pasien kanker. Tujuan pengobatan pasien penyakit kanker pada stadium lanjut adalah meningkatnya kualitas hidup yaitu : hilang rasa nyeri, mencegah terjadi fraktur patologis dan memudahkan perawatan medis. Intervensi bedah lebih agresif pada pasien dengan metastasis tulang yang risiko fraktur (impending fracture) dapat mencegah pasien jatuh ke keadaan yang lebih buruk dan pasien dapat melajutkan terapi tambahan.Pada evaluasi, didapat 24 pasien fraktur patologis pada pasien kanker metastasis tulang. 17 pasien dilakukan operasi sisanya 7 pasien tidak dioperasi. Dari 24 pasien, 9 pasien fraktur patologis ekstremitas atas dan 15 pasien fraktur patologis ekstremitas bawah. Paska operasi, pasien dapat mobilisasi dan rasa nyeri hilang. Penatalaksanaan bedah pada fraktur patologis metastasis tulang sebaiknya berpegang pada keadaan umum pasien, bukan pada harapan hidup pasien.Kata kunci: fraktur patologis, kanker, risiko fraktur

Abstract

Berbagai penelitian terhadap kanker serviks uteri telah dilakukan, khususnya di negara barat. Dari penelitian tersebut didapatkan adanya variabel-variabel yang kemudian dikenal sebagai faktor-faktor resiko yang kemudian diketahui peranannya terhadap kanker serviks uteri.Sebagai negara yang terletak di Asia dimana angka kejadian kanker serviks uteri, khususnya pada stadium lanjut masih tinggi dan semakin meningkat, maka pengetahuan terhadap kanker pada umummnya dan kanker serviks uteri pada khususnya haruslah semakin baik.Untuk mendapatkan angka survival dari penderita kanker serviks uteri dan mengevaluasi faktor prognostik jenis histopatologik, umur pada penderita kenker serviks yang berobat di Rumah Sakit Kanker "Dharmais", Jakarta.Penelitian ini merupakan penelitian kohort retrospektif dari penderita kanker serviks uteri. Data didapat berdasarkan catatan medik penderita yang berobat di Rumah Sakit Kanker "Dharmais", Jakarta pada tahun 1993/4 - 1997. Tercatat sebanyak 710 kasus tetapi hanya 175 kasus yang dapat dilakukan pemantauan selama 2 tahun. Karsinoma sel skuamosa 128 kasus (73,12%) dan adenokarsinoma (termasuk adenoskuamokarsinoma) 14 kasus (8%), 33 kasus tidak dinyatakan jenis histopatologi sel kankernya).Dari 175 kasus yang dapat dievaluasi didapatkan jenis histopatologik adenokarsinoma tinggi pada stadium I dan residif juga pada kelompok umur muda (< 40 tahun), perbedaan tidak bermakna (p = 0,342 dan p = 0,396). Stadium mempengaruhi survival : stadium awal (IA-IIA) 85,2%, stadium lanjut (IIB-IV) 50,4% dan residif 36,7% (p = 0,001). Kelompok umur muda (< 40) dan jenis histopatologik yang mengandung kelenjar mempunyai nilai survival 2 tahun yang lebih baik dibandingkan kelompok umur yang lebih tua, jenis histopatologik skuamosa.Stadium awal (IA-IIA), jenis histopatologik yang mengandung kelenjar dan kelompok umur muda (< 40 tahun) mempunyai angka survival yang lebih baik selama 2 tahun "follow up"Kata kunci: Stadium, jenis histopatologik, umur, survival .

Abstract

Indicators of success in oncology traditionally include cure, survival, and tumor response. In advanced stage however, Quality of life (QOL) has become an important outcomes.20'12 Despite the broad use of the term QOL, it is difficult precisely define.10'12 It is multidimensional, dynamic and subjective concept.3'5'10 The numerous questionnaire for QOL measurement lead to a challenge for its application.12 Most of QOL instrument include physical symptoms, functioning, psychological and social well-being.12 In advanced stage existential, meaning, fulfillment, purpose and grief become more prominent.11'12 Besides the contain of the questionnaire, validity and reliability need to be considered in deciding which instrument will be applied.12This paper discusses the definition of QOL, the purpose of measuring QOL, various QOL instruments, the McGill Quality of Life Questionnaire as a measure suggested in clinical practice and the reasons for its application.Key words; Quality of Life, Palliative, Cancer Patients

Abstract

Background: Cervical cancer and breast cancer are the two top leading cases of female cancer in Indonesia. Being the 2nd largest city in Indonesia, Surabaya is being populated by more than 1.5 million females in 2017. This study aimed to describe the demographic characteristic of women living with cervical and breast cancer in order to show the current picture of female cancer situation in Surabayan communities nowadays. Substantial to it, the correlation between time of first diagnosis and type of therapy with current stage of cancer were analyzed to evaluate the cancer management effectiveness in community context.Methods: This cross-sectional study involved six (9.68%) Public Health Centers (PHCs) among 62 PHCs in Surabaya, Indonesia, which were selected by one stage cluster random sampling. There were 140 and 139 women living with cervical and breast cancer participated in this study respectively (n=279). Self-developed demographic questionnaire was used to collect the data. Descriptive statistic, Spearman Rank correlation test, and Mann-Whitney U test were used in data analysis (α<.05).Results: Overall, most respondents were married (77.06%) and were housewives (59.14%). They were high school graduated at majority (44.44%). Family support came from spouse and children (70.25%). Those with breast cancer mostly aged middle-up adulthood (56.12%), while older aged were found in respondents with cervical cancer (72.86%). The majority had been diagnosed with cancer for less than five years (81.36%), but there was a tendency that the longer life expectancy was found in those with cervical cancer. More cervical cancer respondents with advanced stage were found to be long-term survivors (25%). The first time of diagnosis and the type of therapy were significantly different between cases (p=.039 and p=.000 respectively), but the current stage of cancer was not significantly different (p=.239). There was a quite strong correlation between time of first diagnosis and current stage of cancer (ρ = -.409; p=.000), but there was no significant correlation between type of therapy and current stage of cancer (p=.147).Conclusions: The demographic characteristic of women living with cervical and breast cancer has been slowly shifting from elderly or late adulthood, to middle and early adulthood. The first time of diagnosis and the type of therapy were significantly different between cases, but the current stage of cancer was not significantly different. There was a strong significant correlation between time of first diagnosis and current stage of cancer in cervical and breast cancer.

Abstract

Ekstravasasi merupakan suatu keadaan yang bisa muncul pada pasien kanker yang mendapat kemoterapi, yang menimbulkan rasa sakit dan eritema, ulkus, serta kerusakan jaringan. Beberapa obat sitostatika dapat bersifat vesicant, iritan, dan nonvesicant. Tata laksana ditentukan dari stadium ekstravasasi, banyaknya cairan yang terpapar, dan ketersediaan antidotum yang spesifik.Kata kunci: ekstravasasi, kemoterapi, tata laksana.

Abstract

Patients with life threatening illness area characterized by multi-aspect of suffering. These various aspects of suffering area interrelated require a total comprehensive care by an interdisciplinary team. Palliative care means to address the total suffering of the patients and to maintain a rational hope when cure is absurd. Palliative care emphasizes the essential role of the family. Although the awareness about palliative care has been increasing, its development in Indonesia is still left behind compared to that of other Asian Countries. This paper discusses the definition of palliative care, total suffering, interdisciplinary approach, integration of palliative care in cancer management & barriers to Palliative Care.

Abstract

Dari 62 kasus kanker lambung satu kasus dijumpai metastase pada tulang proximal femur dan sakrum pasca bedah total gastrektomi 4 tahun sebelumnya. Pada pasien ini 2 tahun pasca gastrektomi dilakukan tiroidektomi karena adenokarsinoma tiroid papiliferum. Metastase tulang pada kanker lambung di RS. Kanker "Dharmais" ditemukan sebanyak 1.6 % kasus.Kata kunci: kanker lambung, metastase, metastase tulang.

Abstract

Indonesia mempunyai masalah yang sama dengan negara berkembang lainnya yaitu pasien kanker payudara yang datang ke rumah sakit dalam stadium lanjut. Oleh karena itu diperlukan upaya peningkatan kepedulian khususnya di lingkup tenaga medis terhadap kanker payudara melalui penyuluhan dan ketrampilan pemeriksaan payudara. Tujuan penelitian ini adalah mengumpulkan data awal untuk menentukan metode yang tepat untuk pelatihan pemeriksaan payudara bagi tenaga medis khususnya perawat, bidan dan dokter umum.Dilakukan penyuluhan bagi staf medis yang berminat dan pelatihan pemeriksaan payudara pada tenaga medis (perawat dan bidan) di RS.Kanker Dharmais periode April - Desember 2005. Untuk meningkatkan kepedulian dan kemampuan ketrampilan pemeriksaan payudara para peserta kemudian diminta untuk memberikan penyuluhan di lingkungannya dan mengajak wanita yang bersedia dilakukan pemeriksaan payudara dan USG payudara di RS. Kanker Dharmais.Sebanyak 25 orang perawat dan bidan serta 1 orang dokter umum yang berpartisipasi aktif, 856 wanita bersedia dilakukan pemeriksaan payudara dan USG payudara. Pemeriksaan payudara oleh tenaga medis yang telah dikonfirmasi dengan USG payudara ditemukan hasil normal 783 ( 91.5 %) dan 73 (8.5%) abnormal. Adapun dari hasil abnormal tersebut adalah 39 (53.4%) kista, 17 (23.3%) lesi solid benigna dan 8 (10.9%) dicurigai maligna. Sedangkan kelainan payudara lain yang ditemukan adalah mastitis, galactocele, duktal ektasis serta mamaria aberans.Pelatihan pemeriksaan payudara membutuhkan tidak hanya pengetahuan dan ketrampilan pemeriksaan payudara tetapi juga ketrampilan berkomunikasi dan menjalin kerjasama dengan para wanita di lingkungannya sehingga partisipasi masyarakat dapat ditingkatkan.Kata kunci: kanker payudara, deteksi dini, pemeriksaan payudara, USG payudara.

Abstract

Pada bulan Oktober 2007 Rumah Sakit Kanker "Dharmais" genap berdiri selama 14 tahun dan merupakan tantangan bagi pihak rumah sakit untuk menjawab berbagai masalah yang timbul khususnya di bidang kanker. Kebijakan Departemen Kesehatan untuk menurunkan angka kesakitan dan kematian akibat penyakit kanker, memperpanjang umur harapan hidup serta meningkatkan kualitas hidup penderita dilaksanakan dengan strategi sebagai berikut; menggerakkan dan memberdayakan masyarakat untuk hidup sehat , meningkatkan akses masyarakat terhadap pelayanan kesehatan yang berkualitas, meningkatkan sistem surveilans, monitoring dan informasi kesehatan dan meningkatkan pembiayaan kesehatan.

Abstract

Thyroid ultrasonography is the modality of choice to detect, and to evaluate the morphology of thyroid nodule. ThyroidImaging Reporting and Data System (TIRADS) is a system to evaluate risk of malignancy of thyroid nodule, that firstproposed by Horvath et al. There are various TIRADS proposed by other authors. To the author knowledge, until thewriting of this study there is not yet found a study to evaluate implementation and diagnostic value of TIRADS inIndonesian.Methods: Diagnostic study with cross sectional approach using retrospective evaluation of thyroid ultrasonographydata and post operative histopathology data to evaluate conformity relationship between TIRADS and post operativehistopathological result of thyroid nodule.There is comparable result (p=0.065) between TIRADS result and post operativehistopathological result of thyroidnodule. TIRADS resulted in 96.4% sensitivity; 83.0% specificity; 85.7% positive predictive value; dan 95.7% negativepredictive value.Conclusion: TIRADS can be implemented in evaluation, and reporting of thyroid ultrasonography result with gooddiagnostic value.
