Abstract

Gout is a picturesque presentation of uric acid disturbance. The clinical picture of gout is divided into asymptomatic hyperuricemia, acute gouty arthritis, intercritical period, and chronic tophaceous gout. The tophus found on gouty arthritis has clinical resemblance to neoplastic lesions such as epidermoid cysts therefore it is often mistaken. Diagnosis of gout is based on the history, the laboratory result, ultrasonography and histopathological examination. Management of gout includes management of flares, chronic gout, as well as management of comorbidities. Keywords: Gout arthritis, distal fibula, tophus, bone tumour

Abstract

Background : Transradial access is preferred approach for coronary angiography and  PCI. Although, access site-related complications can occur following transradial access. RAO is the most common postprocedural complication. It has been reported that nitroglycerin can dilate radial artery in patients undergoing transradial coronary catheterization without affecting blood pressure. Objective: This study aimed to evaluate the effect of nitroglycerin on RAO after transradial catheterization. Methods : We searched RCT from Cochrane, PubMed, Science Direct, NCBI, and BMJ. Titles, abstracts, and full texts were screened using inclusion criteria to filter out irrelevant studies. Quality assessment were undertaken using Jadad scale and the results have been presented as a narrative overview with figures. Results : Three studies were included after screening 206 records. Two studies demonstrated statistically significant results that giving nitroglycerin reduced the risk of RAO after transradial catheterization, compared with the comparator groups (p < 0.05). These studies were Chen Y et al 2018 (95% CI 0.116 - 0.998; p = 0.04) and Dharma S et al 2015 (OR : 0.62; 95% CI: 0.44 – 0.87; p = 0.006). Conclusions : This study demonstrated benefit of using nitroglycerin for reducing incidence of RAO after transradial catheterization.

Abstract

Background: Lifestyle behavior modification is a strategy in the management of obesity. The use of financial incentives is part of the external motivation in weight control programme. This intervention pattern is used to encourage healthy lifestyle behavior by providing direct rewards that depend on individual involvement in healthy lifestyle behaviors. Objective: To examine the overall effect of a study using a 6-month financial incentive intervention on weight loss control. Methods: Data were analyzed from studies reporting the use of financial incentive interventions with the resulting weight changes in the PubMed database. Then a meta-analysis was performed to assess the overall effect. Results: The meta-analysis involved a total of 13 studies. The overall effect shows that the financial incentive intervention has a significant effect on weight loss. There was a significant difference in weight loss in the group that was given financial incentive intervention for 6 months compared to the control group (SMD: -0.49; 95% CI). Statistical heterogeneity was found (I2 = 95.20%, P = 0.00). The random effects model was used in the analysis. Conclusion: There was a significant effect between the financial incentive intervention for 6 months and weight loss although heterogeneity was found between studies. Abstract

Amyloidosis is an infiltrative disease characterized by the deposition of abnormal fibril proteins in various tissues and organ systems, including the heart. Cardiac amyloidosis is a very rare disease. The diagnosis of amyloidosis is highly dependent on the suspicion of a clinician. This disease is often asymptomatic until eventually advanced which will give symptoms and symptoms can vary widely. As a result, the patient's diagnosis is often delayed. Cardiac ATTR amyloidosis can be diagnosed in the absence of histology in the setting of typical echocardiographic/CMR findings when 99mTc-PYP scintigraphy shows Grade 2 or 3 myocardial uptake of Radiotracer. Once cardiac ATTR amyloidosis is confirmed, genetic counselling and testing should be performed to assess for the presence of TTR mutations in order to differentiate between ATTRwt and ATTRv. Genetic testing should be performed even in elderly patients, as a significant number can have TTR mutations. 99mTc-PYP can differentiate between CA types and specific for ATTR types. Imaging modalities other than nuclear imaging, cannot differentiate the types of amyloidosis

Abstract

Background: Quality of life in people living with HIV/AIDS (PLWHA) is an important component of evaluating treatment. In other hand, nutritional status plays an important part in disease progression, which eventually affects the quality of life. Therefore, the correlation between nutritional status and quality of life must be further studied. Objective: to study the correlation between nutritional status and quality of life of PLWHA receiving ART Methods: cross-sectional analytical research using Body Mass Index (BMI) and WHOQOL-HIV BREF Questionnaire to evaluate the quality of life of PLWHA. Bivariate and multivariate analysis were done using chi-square and logistic regression, respectively. Results: The results showed that there was no relationship between nutritional status and quality of life. This study found a relationship between age, stigma, and depression with quality of life in people living with HIV who received ARVs. Conclusion: The integration of psychologists in the treatment of HIV/AIDS is also recommended considering that depression plays a role in the quality of life of PLWHA. Education to the community to reduce stigma should also be carried out. Keywords: Nutritional status, Quality of Life, People Living With HIV/AIDS, Antiretroviral Therapy
