Abstract

Catatan Perkembangan Pasien Terintegrasi merupakan salah satu berkas yang ada di lembar rekam medis. Lembar ini berisi identitas pasien, tanggal, jam, catatan perkembangan pasien, atau evaluasi tindak lanjut. Metode penelitian deskriptif kualitatif. Teknik pengumpulan data yang digunakan yaitu observasi, wawancara, studi literatur, dokumentasi. Hasil penlitian menunjukan bahwa belum adanya SOP atau struktur khusus organisasi pengisian CPPT, Tidak lengkapnya identitas pasien, jam, tanggal, no.rm pada lembar CPPT. Permasalahan nya yaitu keterbatasan waktu, miss komunikasi, kurang disiplinan dokter/ PPA, dan kurang pengetahuan pedoman MIRM 13.3. kesimpulan: membentuk tim review rekam medis, memberikan sanksi kepada petugas yang tidak melengkapi, memberikan reward/punishment kepada yang mengisi sesuai dengan dilakukan.

Abstract

This research aims to determine the effect of fillings completeness of inpatient medical discharge summary on implementation SNARS 1st edition MIRM 15 valuation element at Rumah Sakit AMC Cileunyi.The research method used quantitative method with descriptive approach, The data collecting that by distributing quessioneir, observation, interview and compeleted with literature study which has a close relevance to the problem. The sampling technique used is sampling technique saturated.From the research of this research, the problem that arise among others: 1) The high number of incomplete replenishment of inpatien medical discharge summary. 2) The absence of standard procedure operational (SPO) for discharge summary, and 3) Unfulfilled SNARS accreditation standard 1st edition MIRM 15 valuation element related discharge summary The advice given to fix the problem is 1) Make a review team every room that aims to coordinate and control the incomplete doctor registers. It is expected that the doctor may improve the completeness of the filling of medical records. 2) Socializing the new design in accordance with the provisions of the SNARS edition 1 MIRM 15 valuation element and reinforcing the filling instructions by making the SPO filling discharge summary.

Abstract

The implementation of medical records has a very broad scope of cooperation between work units, starting from hospital leaders, doctors, nurses to other health workers. One of the units involved in recording patient medical records is the Inpatient Unit. This study aims to determine the timeliness of returning inpatient medical record files to the medical record unit and the delay factor in returning inpatient medical records. This research was conducted on April 26 – June 26, 2021 descriptively with a qualitative approach with data collection methods using observations, interviews, and documentation studies. The results were obtained in the May – June period from a total of 751 inpatient medical record files that returned on time to the medical record unit as many as 594 files (61%) and 157 files (39%). The factor of delay in returning inpatient medical records is that there are still some nurse officers who do not follow the applicable SOP rules in returning inpatient files, namely 2x24 hours after the patient returns, the lack of special care personnel for the management of inpatient medical records and incomplete filling of the record form. medical treatment by doctors and room nurses. So the researcher gives suggestions to be further improved in the timeliness of returning inpatient medical record files so that there is no delay in returning inpatient medical record files to the medical record unit.

Abstract

Medical record is one of the written evidence about the service process which contains clinical data of patients during the process of diagnosis and treatment. Management of medical records in hospitals to support the achievement of orderly administration to achieve hospital goals, namely improving the quality of health services in hospitals. The purpose of this study was to determine the performance of Medical Record Officers at Salak Hospital, Bogor. This study uses a descriptive method with a quantitative approach that aims to describe the performance review of medical record officers in accordance with the facts in the field. The results of the research on the performance of the medical record officer based on the quality of work illustrate that the medical record officer has a fairly good performance, based on the quantity of work indicated that the medical record officer has a good performance, based on the supervision of the medical record officer shows that the medical record officer has a good performance, based on the presence of the medical record officer shows that the medical record officer has a good performance, and based on the conservation of the medical record officer shows that the medical record officer has a good performance.

Abstract

The hospital has a mission to provide quality and affordable health services to the community in order to improve the health status of the community, one of which is through fast patient waiting times. This study aims to analyze the length of waiting time (arrival process, service, human resources) for outpatient services at the Internal Medicine Polyclinic at Salak Hospital, Bogor. During the service process, there are several obstacles that occur, including the patient does not bring a complete file/guarantee, inadequate room, and limited human resources who are experts in refraction and medical records. The waiting time at the Salak Hospital in Bogor is still quite long(> 60 minutes) due to the large number of patients, interference with the insurance network which makes the waiting time longer, the distribution of medical record files is often late. It can be concluded that a person or group of people has the opportunity to make people aware of their own abilities, while at the same time implementing the actions of certain people or groups. In addition, it can be interpreted as the ability to control, dominate, or manipulate the actions of others (Rollo May 1972), thus it can be concluded that a patient can affect the length of waiting time in Health Service Agencies.

Abstract

Medical Record is a file containing records and documents about patient identity, examination, treatment, other service actions that have been given to patients. Medical resume is a summary of the entire period of care and treatment of patients carried out by doctors to patients. The completeness of the medical resume is a reflection of the quality of medical records and services provided by the hospital. This study aims to describe the completeness of the medical resume of outpatients at Meilia Hospital. The research method used is by using observation techniques. The population of the data is medical records of inpatients of 81 medical records obtained from the results of calculations using the Slovin formula from the research conducted, the following results were found: that the population of incomplete medical resumes at Meilia Cibubur Hospital reached 27% and completeness of medical resumes at the Hospital Meilia Cibubur reached 73%. The conclusion that can be drawn from this research is that the analysis of the completeness of the inpatient medical resume has gone well, suggestions need to be re-socialized for the importance of authentication in writing and completing the medical resume because it is related to legal aspects, and sanctions are given if the doctor is not disciplined in filling out the medical resume

Abstract

This study aims To explain the medical support result completeness of an information system design for covid-19 patients after being hospitalized The research method used is a descriptive method by conducting observations, interviews, and literature review. The software method used is the Waterfall development method. From the results of this study, the following problems were found: The process of recording the completeness of the information system for the results of medical support for Covid-19 patients still in manual method so that the recording gets to be less successful, the time wasteful in recording since it still employments the manual method, the difficulty in making a completeness report because the completeness of the medical support results are written randomly, the identity of the medical support result is often not in accordance with what is proposed. Therefore, the researchers designed an information system for the completeness of the results of medical support for covid-19 patients after being hospitalized using Microsoft Visual Studio 2010 to make it easier for officers to process the completeness of medical support results.

Abstract

Latar belakang : Dalam memberikan pelayanan kesehatan yang baik, klinik harus memiliki sumber daya manusia kesehatan yang memadai dan berkualitas untuk mendukung efektivitas fungsi klinis. Tujuan penelitian : Untuk mengetahui kebutuhan sumber daya manusia (SDM) kesehatan di Klinik Pratama Green Care. Metode penelitian : Penelitian ini merupakan deskriptif dengan menggunakan wawancara, observasi, perhitungan menggunakan stopwatch dan kalkulator untuk mengetahui waktu kerja yang tersedia, standar beban kerja, dan standar tugas penunjang berdasarkan metode analisis beban kerja kesehatan (ABK Kes). Penggunaan metode ABK Kes bertujuan untuk mempermudah pemenuhan kebutuhan sumber daya manusia (SDM) kesehatan di Klinik Pratama Green Care. Hasil penelitian : Menunjukkan bahwa HRK di Klinik Pratama Green Care membutuhkan 3 orang bidan sedangkan jumlah bidan yang tersedia hanya 2 orang sehingga perlu ditambah 1 orang dan untuk tenaga non medis rekam medis membutuhkan 1 orang sedangkan yang tersedia tidak, sehingga perlu merekrut tenaga kesehatan baru di bidang rekam medis.

Abstract

Latar Belakang: Penambahan Rak Penyimpanan Dokumen Rekam Medis diharapkan dapat menunjang fasilitas dan memudahkan pengelolaan data di Rumah Sakit tersebut. Tersedianya rak penyimpanan dapat mempermudah dalam penyimpanan dokumen rekam medis agar tidak terjadi penumpukan dokumen di ruangan tersebut. Tujuan: Tujuan dari peneliti untuk mengetahui Perhitungan Kebutuhan Rak Pernyimpanan Dokumen Rekam Medis di Ruang Filing RSIA Humana Prima Bandung. Metode: Jenis penelitian yang digunakan Deskritif dengan metode Observasional dan studi pustaka dengan pendekatan Cross Sectional. Populasi yang digunakan yaitu dokumen rekam medis pasien Rawat Inap dan Rawat Jalan bulan Januari-Mei 2021 yang berjumlah 15.439 dokumen rekam medis. Sistemastika sampel yang dipakai berupa sistematik Random Sampling yang berjumlah 50 dokumen rekam medis. Hasil: Hasil penelitian yang digunakan untuk memperoleh luas ruangan yang dibutuhkan 58,32 m2 dengan panjang 10,8 m dan lebar 5,4 m, dengan ukuran rak panjang 4,5 m dengan jumlah muka 2 sharft 5 memperoleh penambahan 7 rak untuk 5 tahun yang akan datang. Rak di RSIA Humana Priama Bandung saat ini berjumlah 9 rak. Telah diperoleh perhitungan penambahan rak berupa rak roll o’pack sebanyak 7 rak. Jadi, total rak diruangan tersebut terdapat 16 rak. Jarak yang di usulkan selebar 90 cm antar rak. Kesimpulan: RSIA Humana Prima Bandung memiliki keterbatasan dalam penyimpanan (filing) dan memiliki ruang penyimpanan yang kurang memadai. Jumlah rak penyimpanan rawat inap dan rawat jalan di RSIA Humana Prima saat ini yaitu 9 rak yang terdiri dari rak besi dan rak roll o’pack.

Abstract

Latar Belakang: Pada pandemi Covid-19 sangat penting dalam melakukan prevensi,deteksi dan respon di dalam pencegahan dan pengendalian Covid-19. Penyebaran Covid-19 inipun berdampak ke berbagai sektor, mulai dari ekonomi, pariwisata, transportasi, politik hingga pelayanan publik termasuk kesehatan khususnya perekam medis, disanalah kontak pertama kali petugas dengan pasien, ditempat itu pula penyebaran Covid-19 dari pasien ke petugas sangat mungkin terjadi. Tujuan: Penelitian ini bertujuan untuk menganalisis pengaruh pandemi Covid-19 terhadap pelayanan kesehatan di rumah sakit XX cimahi. Metode: Penelitian ini merupakan penelitian kualitatif dengan metode wawancara mendalam. Data yang dikumpulkan dengan metode wawancara mendalam, praktek kerja lapangan (PKL) dan observasi langsung dan penelusuran. Penelitian ini dilakukan di rumah sakit XX Cimahi pada bula Mei-Juni Tahun 2021. Hasil: Berdasarkan hasil penelitian mengenai Pengaruh Pandemi Covid-19 terhadap pelayanan kesehatan bagian rekam medis di rumah sakit XX Cimahi ini memperoreh hasil yaitu berdasarkan hasil wawancaran dengan N1, N2, N3 mengatakan bahwasannya dengan adanya pandemi Covid-19 ini petugas khususnya dibagian Rekam Medis juga memperketat protokol kesehatan seperti memakai masker, menjaga jarak, menggunakan sarung tangan dan sering mencuci tangan agar petugas dapat mencegah penularan Covid-19 yang mungkin saja dibawa oleh pasien yang sedang mendaftar ataupun yang menempel pada berkas rekam medis. Dan banyak nya kasus pasien control dan pasien umum menurun sedangkan pasien Covid-19 meningkat sehingga meningkatkannya pasien yang harus di daftarkan di UGD. Kesimpulan: Pandemi Covid-19 dapat mempengaruhi pelayanan kesehatan khususnya dibagian rekam medis seperti terjadinnya perubahan alur seperti adanya test suhu tubuh sebelum mendaftarkan diri untuk mendapat pelayanan kesehatan, memperketat protokol kesehatan, tingginya beban kerja dikarenakan jumlah pasien Covid-19 yang meningkat sedangkan pasien umum menurun, jumlah petugas dikurangi guna meminimalisir keramaiaan dalam satu ruangan.
