Abstract

Imunoterapi spesifik (ITS) merupakan pilihan terapi pada rinitis alergi yang tidak memberikan respons perbaikan klinis dengan medikamentosa. Peran zat adjuvan diduga dapat meningkatkan efektivitas dan efikasi ITS. Penelitian ini bertujuan menganalisis peran adjuvan vitamin D3 pada ITS terhadap percepatan timbulnya respons imun toleran. Penelitian analitik komparatif numerik dua kelompok tidak berpasangan dengan rancangan uji klinis acak terkontrol ganda ini dilakukan di Klinik Rinologi-Alergi THT-KL RSUP dr. Hasan Sadikin Bandung sejak Mei 2016–April 2017. Sampel terdiri atas 18 pasien menerima ITS+kalsitriol dan 18 pasien menerima ITS+plasebo yang memenuhi kriteria penelitian diambil secara consecutive sampling. Dilakukan pemeriksaan jumlah sel Tregulator dengan flowcitometry pada baseline, minggu ke-8 dan 15 dan skor gejala hidung.  Analisis dengan Uji Mann-Whitney, uji Friedman, uji T berpasangan dan analisis Post hoc. Perbandingan kenaikan rerata median jumlah sel Treg kelompok intervensi dengan kontrol pada minggu ke 8 menunjukkan nilai p=0,04, rerata MFI Treg pada kelompok intervensi dengan kontrol nilai p=0,002. Analisis post hoc terhadap MFI Treg menunjukkan peningkatan pada kelompok intervensi dengan p=0,001 pada minggu ke-8. Disimpulkan perubahan di tingkat imunologi mulai terjadi pada minggu ke-8, tetapi perbaikan gejala klinis terlihat setara.Kata kunci: Imunoterapi spesifik, rinitis alergi, sel t-toleran perifer, sel T-regulator, vitamin D3 Role of Adjuvant Therapy with Vitamin D3 on T-Regulator in Allergic Rhinitis Patient with Subcutaneus Immunotherapy Allergen-specific immunotherapy (AIT) is a potentially disease-modifying therapy that is useful for the treatment of allergic rhinitis, especially in those who do not response to pharmacotherapy. Currently, the role of adjuvant is expected to increase the clinical efficacy in AIT.  The aim of this study was to analyze the role of vitamin D3 adjuvant in AIT to induce immune tolerance. This was an unpaired comparative analytic research with a randomized controlled trial dstudy conducted at the Rhinology-Allergy Clinic ORL-HNS of Dr. Hasan Sadikin General Hospital Bandung from May 2016 to April 2017. Eighteen subjects with AIT+ calcitriol treatment and 18 subjects with AIT+placebo treatment who met the study criteria were selected through consecutive sampling. All subjects were evaluated for Treg cells using flow citometry at baseline, week 8, and week 15. Nasal symptom score was also evaluated.  Statistical analysis performed in this study included Mann-Whitney test, Friedman test, T-Paired test,and Post hoc analysis. There was a significant different of Treg cells average results between the intervention group and control at week 8 with a p-value of 0.04. The mean delta value on MFI Treg reflected significant differences within intervention group (p=0.002). Post hoc analysis results presented a significant increase in MFI Treg in the intervention group at week 8 (p=0,001). Therefore, immunological changes start in week eight but the improvement of clinical symptoms looks similar. Abstract

Skrining pendengaran pada anak sekolah dapat mengetahui lebih dini gangguan dengar akibat otitis media sehingga hasil pengobatannya akan lebih baik. Audiometer skrining medan bebas merupakan alat skrining pendengaran yang sederhana, terjangkau, ringkas, mudah dan singkat, tetapi belum diukur tingkat ketepatannya untuk anak sekolah dasar. Penelitian ini bertujuan untuk menguji tingkat ketepatan audiometer skrining medan bebas untuk mendeteksi gangguan dengar pada anak sekolah dasar yang menderita otitis media. Penelitian uji diagnostik dengan rancangan penelitian potong silang dilakukan di Sekolah Dasar Sejahtera dan Poliklinik Departemen Ilmu Kesehatan Telinga Hidung Tenggorok–Bedah Kepala dan Leher RS Dr. Hasan Sadikin Bandung periode bulan Juni–Agustus 2011. Dari 479 anak sekolah dasar, didapatkan 86 subjek sesuai kriteria inklusi. Terdapat gangguan dengar pada 56 anak yang diperiksa audiometer skrining medan bebas dan 51 anak yang diperiksa audiometri nada murni. Hasil uji diagnostik audiometer skrining medan bebas didapatkan sensitivitas 90,2%; spesifisitas 71,4%; indeks Kappa 0,630. Perbandingan pemeriksaan pendengaran antara audiometri skrining medan bebas dan audiometri nada murni didapatkan p=0,302. Simpulan, tingkat ketepatan audiometer skrining medan bebas sama dengan audiometer nada murni untuk mendeteksi gangguan dengar anak sekolah dasar yang menderita otitis media. [MKB. 2013;45(1):62–8]Accuracy of Portable Screening Audiometer for Detecting Hearing Loss in Elementary School Children with Otitis MediaHearing skrining in school children can detect hearing loss due to otitis media earlier therefor treatment will give better outcome. Portable screening audiometer is a device which are simple, less expensive, brief, easy to use and has shorter examination time but has not been measured its accuracy for hearing screening in elementary school children. The objective of this study was to measure the accuracy of portable screening audiometry for detecting hearing loss in elementary school children with otitis media. A diagnostic test study with cross-sectional method was conducted at Sejahtera Elementary School and at Out-patient Clinic Otorhinolaryngology-Head and Neck Department Dr. Hasan Sadikin Hospital Bandung period June until August 20011. From 479 children, there were 86 children match with inclusion criteria. Hearing loss was found in 56 children examined using portable screening audiometer and in 51 children using pure tone audiometry. Diagnostic test result for portable screening audiometer were 90.2% sensitivity, 71.4% specificity, with Kappa index 0.630 and p value was 0.302 compared to pure tone audiometry. In conclusion, the accuracy of portable screening audiometer is as good as pure tone audiometry in order to detect hearing loss in elementary school children with otitis media. Abstract

Otitis media merupakan peradangan lapisan mukoperiosteum di telinga tengah tanpa melihat penyebab atau patogenesisnya. Angka kejadiannya bervariasi, di Bandung dan sekitarnya mencapai 6,9%. Penyebab otitis media karena terganggunya fungsi tuba eustakius dapat ditimbulkan oleh pemakaian pipa nasogastrik (PNG). Dilakukan penelitian analisis observasional untuk melihat pengaruh PNG pada kejadian otitis media efusi pada penderita rawat inap di Departemen Ilmu Kesehatan Telinga, Hidung, Tenggorok-Bedah Kepala dan Leher Rumah Sakit Hasan Sadikin. Sebanyak 34 orang subjek didapatkan dalam periode Januari 2007 yang memenuhi kriteria inklusi diikutsertakan dalam penelitian. Pada 34 subjek dilakukan pemeriksaan fisik THT dan timpanometri sebelum dan selama pemasangan PNG. Pemeriksaan timpanometri diulang tiap 24 jam sampai PNG dilepas. Timpanogram dibaca dan data yang diperoleh dianalisis menggunakan uji Friedman untuk membandingkan nilai tengah tekanan dan compliance antara sebelum dan sesudah pemasangan PNG. Uji Spearman correlation untuk mengetahui hubungan tekanan dengan compliance di telinga tengah. Besarnya kejadian ganguan otitis media efusi diuji dengan uji binomial. Terjadi penurunan puncak tekanan dan compliance di telinga tengah sampai hari ke-3 (p=0,197), sedangkan pada hari ke-4–ke-7 terjadi peningkatan kembali. Simpulan, penggunaan PNG tidak berpengaruh pada tingkat otitis media efusi, tetapi pada penurunan puncak tekanan (daPa) dan compliance (mmho) telinga tengah. [MKB. 2011;43(1):42–8].Kata kunci: Compliance, otitis media, pipa nasogastrik, tekanan, telinga tengah, tuba eustakiusThe Effect of Using Nasogastric Tube on Incidence of Otitis Media with Effusion Otitis media is an inflammation of the middle ear mucoperiosteal without reference to its cause or pathogenesis. The incidence rate in Bandung area was 6.9%. Otitis media caused by Eustachian tube dysfunction might be induced by the use of nasogastric tube (NGT). An observational analytic was conducted to know the effect of NGT on incidence rate of otitis media with effusion in hospitalized patients at Department of Otolaryngology-Head and Neck Surgery, Dr. Hasan Sadikin Hospital. There were 34 subjects in January 2007 were included in this study. Before NGT insertion all subjects had physical examination and tympanometry. Every 24 hours tympanometry was performed till exertion the NGT. Tympanogram was collected and analysed statistically by Friedman test to compared median value of pressure before and after NGT insertion, and that of compliance. Spearman correlation test to identify correlation between peak pressure and compliance in the middle ear, and binomial to test hypotesis. There was decreasing pressure and compliance in middle ear until day 3 (p=0.197) and increased on day -4 and -7. In conclusion, the incidence rate of otitis media with effusion is not affected by using of NGT. The using of NGT is associated with reduced peak middle ear pressure (daPa) and peak compliance (mmho). Abstract

Radiasi berperan penting pada pengobatan kanker kepala leher karena reseksi bedah sering tidak memungkinkan, tetapi menimbulkan efek samping gangguan dengar sensorineural. Penelitian observasional rancangan longitudinal ini untuk mengetahui pengaruh radiasi terhadap gangguan dengar sensorineural penderita karsinoma nasofaring di Bagian THT-KL Rumah Sakit Hasan Sadikin Bandung periode Februari–Agustus 2006. Didapatkan 28 laki-laki dan 7 perempuan, yang satu atau kedua telinganya tidak terganggu pendengaran sensorineural, usia 12–72 tahun, dan memenuhi kriteria inklusi. Seluruh penderita mendapat radiasi dan pemeriksaan audiometri serta timpanometri sebelum, durante 2.000 cGy, 6.600 cGy, dan satu bulan pascaradiasi. Data dianalisis secara statistik menggunakan uji Chi-kuadrat, Mc-Nemar, dan eksak Fisher. Hasil penelitian menunjukkan kejadian gangguan dengar sensorineural durante 2.000 cGy adalah 7 kasus (10%), 6.600 cGy 22 kasus (31,4%), dan pascaradiasi 24 kasus (34,3%). Hubungan antara durante 6.600 cGy dan 2.000 cGy pada kelompok preradiasi normal sangat bermakna (p=0,001), sedangkan antara pascaradiasi dan durante 6.600 cGy tidak bermakna (p= 0,5). Pada usia >30 tahun gangguan dengar sensorineural 37,0% durante 6.600 cGy (p=0,031) dan 40,7% pascaradiasi (p=0,018). Simpulan, radiasi karsinoma nasofaring dapat menyebabkan gangguan dengar sensorineural selama dan pascaradiasi, serta usia >30 tahun merupakan faktor prognosis gangguan dengar sensorineural. [MKB. 2010;42(3):108-14].Kata kunci: Gangguan dengar sensorineural, karsinoma nasofaring, radiasiNasopharyngeal Carcinoma External Radiation As Causal of Sensorineural Hearing LossRadiation has an important role on nasopharyngeal carcinoma therapy because surgery is often difficult, however it cause sensorineural hearing loss as side effect. Longitudinal observational study was conducted to know the effect of radiation on sensorineural hearing loss of nasopharyngeal carcinoma patients at Ear, Nose, and Throat Department, Hasan Sadikin Hospital, February-August 2006. Twenty eight male and 7 female, with no sensorineural hearing loss in one or both ears, age 12–72 years, and met inclusion criteria, were included in this study. All patients received >radiation and underwent audiometry and tympanometry prior-,during-radiation with a 2.000 cGy and 6,600 cGy, and one month postradiation. Data was analyzed using Chi-square, Mc-Nemar, and exact Fisher test. The results showed that incidence of sensorineural hearing loss were 7 cases (10%) on 2,000 cGy, 22 cases (31.4%) on 6,600 cGy, and 24 cases (34.3%) on postradiation. The relationship between duration 6,600 cGy and 2,000 cGy in the normal preradiation group were significant (p= 0.001), whereas postradiation and duration with 6,600 cGy was not significant (p= 0.5). Sensorineural hearing loss on >30 years was 37.0% on duration 6,600 cGy (p=0.031) and 40.7% postradiation (p=0.018). In conclusion, radiation on nasopharyngeal carcinoma can induce sensorineural hearing loss during- or postradiation and age >30 years is prognostic factor for sensorineural hearing loss. [MKB. 2010;42(3):108-14].Key words: Nasopharyngeal carcinoma, radiation, sensorineural hearing loss
