Abstract

ABSTRACT This study aims to determine the effect of using gelling agent and chemical penetration enhancer on the solid dispersion Ibuprofen gel in increasing the transdermal drug penetration. Inhibition of the COX-1 enzyme caused by ibuprofen in oral administration can cause side effects of gastrointestinal disorders, dyspepsia, diarrhea, upper gastrointestinal infections, nausea and bloating, resulting in a topical route to reduce side effects. The gel is prepared using hydroxy propyl methyl cellulose (HPMC) and carbopol® as gelling agents, and also propylene glycol and glycerin as chemical penetration enhancer compounds. The gel evaluation are viscosity, pH, spreadability and penetration flux rate. The value of the formula 3 penetration flux is 1.5383 ± 0.029 ug / cm2.minute, the formula 1 is 1.403 ± 0.055 ug / cm2. minute, the formula 2 is 0.756 ± 0.071 ug / cm2 minute, while the formula 4 is 0.5404 ± 0.106 ug / Cm2. minute. Abstract

Scabies is a common skin disease caused by infection of Sarcoptes scabei, which is transmitted directly human-to-human contact especially in crowded community like pesantren. The characteristics of the students in pesantren are seemingly to be associated with Scabies. The purpose of this study was to determine the prevalence, characteristics of Scabies and its association with gender and education level of students in Pesantren Nurul Qarnain Jember. This cross sectional study was conducted to all students (1120 students) in 2016. Diagnosis was made based on anamnesis and dermatology examination. Data were analyzed using chi square test. The results showed that the prevalence of Scabies was 13.48% (male 24.89% and female 5.82%; education level tsanawiyah 15.14.1% and aliyah 10.85%). Most lesions were found in the interdigital space of the hands (40.40%). Chi square test showed significant difference between the prevalence of Scabies with gender (p=0.048) and education level (p=0.023) of the students. In conclusion, the prevalence of Scabies in Pesantren Nurul Qarnain Jember was 13.48% and there was association between the prevalence of Scabies with gender and education level of the students.  Keywords: Scabies, prevalence, students, gender, educational level

Abstract

Diltiazem HCl adalah golongan benzoatiazepin penghambat kanal kalsium (calcium channel blocker) yang digunakan dalam pengobatan angina pektoris, aritmia, dan hipertensi. Diltiazem HCl mengalami first pass metabolism di hati, waktu paruh yang pendek yakni 3-5 jam dan bioavailabilitas diltiazem pada pemberian oral sekitar 40%. Sediaan mucoadhesive buccal film diltiazem HCl melepaskan obat ke mukosa buccal sehingga dapat menghindari first pass metabolism karena absorpsinya melalui sistem vena yang mengalir dari pipi. Tujuan penelitian ini adalah untuk mengetahui pengaruh polimer polivinil pirolidon (PVP) dan kitosan terhadap swelling index, waktu tinggal dan kekuatan mucoadhesive diltiazem HCl dalam sediaan mucoadhesive buccal film diltiazem HCl. Sediaan film dilakukan evaluasi swelling index, kekuatan mucoadhesive dan waktu tinggal mucoadhesive. Jumlah optimum untuk PVP adalah 1 mg dan kitosan sebesar 25 mg. Kombinasi polimer dengan jumlah tersebut dapat menghasilkan swelling index, kekuatan mucoadhesive, dan waktu tinggal mucoadhesive yaitu 3,641; 63,867 gF; dan 352,667 menit dan nilai desirability tertinggi yaitu 0,727. Abstract

Ibuprofen is included in non-steroidal anti-inflammatory drug (NSAID) which is used for the treatments of arthritis and inflammation. Ibuprofen gel is an effective approach evading ibuprofen's side effect in GI tract and first pass effect. The aims of this study was to determine the effect of HPMC and menthol combination on viscosity, penetration flux and to obtain the optimum formula.The gel evaluations were organoleptic testing, pH, viscosity, spreadibility test, assay of ibuprofen gel, and in vitro penetration rate. In vitro penetration testing used the paddle type dissolution tester. Furthermore ibuprofen which penetrated through rat skin were analyzed by UV-Vis spectrophotometer. Ibuprofen gel were prepared into four formulas based on factorial design and the data were analyzed using design expert trial 9.0.3. The viscosity testing of F1, Fa, Fb, and Fab were 183.30 ± 14.433,261.67 ± 16.07, 158.33 ± 7.637, and 310.0 ± 13.23 dPa.s. The in vitro penetration rate of F1, Fa, Fb, and Fab were 0.7225, 0.6716, 1.5587 And 1.0702 µg/cm2.minutes. It can be concluded that Fb has the optimum formula. Abstract

Ultraviolet rays (UV) has an adverse effect on the skin, which is epidermis damage. One way to prevent the adverse effects of UV rays on the skin can be done by using sunscreen. The sunscreen mechanism of action was divided into chemical absorber and physical blocker. Chemical absorber will be degradated when exposed to UV rays, while ZnO as physical blocker can increase pH value of the sunscreen. The effectiveness of sunscreen was affected by pH value, so malic acid was added into the sunscreen formula to decrease pH value. This study was aimed to determine the optimum composition of ZnO and malic acid on pH, viscosity, SPF, transmission of erythema and transmission of pigmentation using factorial design method. 0,5% ZnO was set as the lowest level and 3% ZnO was set as the highest level; 0,5% malic acid was set as the lowest level and 0,8% malic acid was set as the highest level. Selected optimum formula was 0,5% ZnO and 0,8% malic acid with the predicted SPF value of 21,242; TE of 0,000; TP of 0,000; pH of 4,820; and viscosity of 80,333 dPa.s Keywords: sunscreen, Zinc Oxide, malic acid, Sun Protector Factor

Abstract

Caffeine is one of alkaloids that have the potential to skin cancer treatment. Caffeine is hydrophilic with log-P value of 0.07 that makes difficult to penetrate to the stratum corneum. Nanoemulsion is one of system which made to increase drug penetration. One of the important component in the nanoemulsion is a surfactant that acts as a unifier between the oil and water phases to form one phase that isotropic and thermodynamically stable. The presence of lipophilic component in the surfactant can affect the skin barrier disturb, thus increasing penetration. The aims of this study was to find the best composition of the PEG-400 and lecithin as surfactant combination to obtain a nanoemulsion that is safe, stable, and increasing penetration of caffeine. The experiment was conducted using a factorial design method which observed responses of flux value and viscosity deviation. The viscosity deviation and flux were analyzed using Design Expert 10.0 trial to determine the optimal formula of caffeine nanoemulsion. The results obtained optimal formula of nanoemulsion that has composition of PEG-400 as 32% and lecithin as 7% . Prediction of viscosity deviation and flux value of optimal formula was 0,034 cP and 0.514 µg cm-2min-1. Keywords: caffeine, nanoemulsion, PEG-400, lecithin

Abstract

Ibuprofen is a non-steroid anti inflammatory drug (NSAID) used for rheumatoid arthritis treatment. In this study, formulation, evaluation, and optimization of ibuprofen gel formula were conducted in order to determine the best ibuprofen penetration rate through the rats skin using propylene glycol and ethanol as penetration enhancers and to find out the optimum formula to get the best penetration rate with simplex lattice design method. Evaluations included organoleptic, pH, viscosity, gel dispersive, assay of gel formulations, and ibuprofen penetration rate assay. Penetration rate was determined by paddle type dissolution and the samples were analyzed by spectrophotometry UV-Vis. The results of ibuprofen penetration rate assay were analyzed and optimized using design expert trial version 10. The results of ibuprofen penetration rate were F1 1.524 ± 0.121 µg/cm2.minute; F2 1.945 ± 0.0104 µg/cm2.minute; and F3 1.173 ± 0.216 µg/cm2.minute. Penetration rate optimization resulted in one optimum solution formula which was 18.000% propylene glycol and 12.000% ethanol with a maximum flux of 1.957 ug/cm2.minute and desirability 1. It can be concluded that the best ibuprofen penetration rate was F2 with flux 1.945 ± 0.0104 µg/cm2.minute with 15% propylene glycol and 15% ethanol. Abstract

Basil (Ocimum basilicum) is a plant from Lamiaceae family that has the potential for typhoid fever treatment. Basil essential oil is a hydrophobic compound being difficult to dissolve in the gastrointestinal tract. SNEDDS is one system that can solve this problem. The vital components of SNEDDS are surfactants and cosurfactants to increase the stability of the SNEDDS system by forming small droplets. This research determines the best composition of Tween 80 and Propylene glycol (PG), which can form nanoemulsion spontaneously in the gastrointestinal tract with a droplet size of nanometers. This research used a factorial design method to observed responses transmittance and emulsification time. Response transmittance and emulsification time then analyzed using Design Expert 11.0 to determine the optimum formula of SNEDDS basil essential oil. The optimum formula of SNEDDS basil essential oil obtained was a formula with a composition of 2.05 mL of Tween 80 and 1.35 mL of Propylene glycol. The optimum formula characterization test results showed a pH value of 7.01 ± 0.007; particle size 46.1 ± 0.61 nm; polydispersity index 0.46 ± 0.016. The antibacterial activity test optimum formula showed the ability to inhibit the growth of Salmonella typhi bacteria with an inhibition zone diameter of 20.50 ± 0.59 mm.

Abstract

Rheumatoid Arthritis is a chronic and systemic inflammatory disease that can destruct joint tissues. The disease progression is associated with elevated levels of interleukin 1ß (IL-1ß) produced by macrophages and dendritic cells. Gamavuton-0 (GVT-0) has been known to have anti-inflammatory effect related to this activity inhibit COX-2, anti-oxidative, and radical scavenger. GVT-0 can modulate the expression of inducible inflammatory cytokines. Administration of GVT-0 significantly suppressed the progression of Rheumatoid Arthritis and inhibited the production of IL-1β. GVT-0 can be developed as a potential therapy for Rheumatoid Arthritis due to the activity inhibits the production of IL-1ß.

Abstract

Kelompok ibu-ibu di di Dusun Padasan belum memiliki aktivitas yang produktif, padahal mereka memiliki peluang dengan mengoptimalkan budidaya rimpang yang telah dilakukan dalam skala rumah tangga. Program Pengabdian Kemitraan (PPK) ini dilakukan dengan kelompok ibu-ibu di Dusun Padasan, Desa Darsono, Kecamatan Arjasa sebagai mitra. Pemberdayaan dilakukan dengan melatih dan mendampingi kelompok ibu-ibu di sana agar mampu menghasilkan bahan baku jamu berupa rimpang kunyit, jahe, dan temu kunci sesuai dengan kaidah Cara Pembuatan Obat Tradisional yang Baik (CPOTB) serta diversifikasi produk berbasis rimpang tanaman obat, yaitu kunyit, jahe, dan temu kunci. Hal ini dimaksudkan untuk mendukung program Saintifikasi Jamu yang telah dicanangkan pemerintah. Pelatihan yang diberikan adalah pelatihan tentang struktur organisasi dan tata kelola Posdaya; budidaya rimpang dan khasiatnya; pembuatan obat tradisional yang baik; serta pemasaran produk yang telah dihasilkan. Di akhir kegiatan, telah terbentuk Posdaya Darsono yang memiliki 3 jenis produk awal berbasis rimpang, yaitu teh herbal Wedang Uwuh, Jahe, dan Temulawak.
