Abstract

ABSTRACT The long-term use of insecticides causes resistance to targeted insects, so that a study of its resistance in Indonesia is needed. A cross-sectional study was conducted to determine the resistance status of Aedes aegypti to organophosphates and pyrethroids in 102 districts in 2015 with reference to WHO standard bioassay test. Sampling and testing of Ae. Aegypti larvae from 100 houses were carried out from three area of each health center. Test results on 0.8% malathion revealed that 84% districts were resistant, 13% districts were tolerant, and 3% districts were vulnerable. The test results for temephos of 0.02% also found that 49% districts were resistant, 29% districts were tolerant and 22% districts were vulnerable. While for 0.05% cypermethrin insecticide test; 98% were resistant, and 1% was tolerant. The test on 0.025% alpha cypermethrin found that 40% were resistant, 51% were tolerant and 9% were vulnerable. Test on 0.025% deltamethrin found that 65% were resistant, 22% districts were tolerant and 14% districts were vulnerable. With the high resistance to organophosphate and pyrethroid groups, it is recommended to rearrange the application of insecticide types in each region cyclically. Keywords: Insecticides, resistance, organophosphates, pyrethroid, Aedes aegypti ABSTRAK Penggunaan insektisida dalam jangka waktu lama menyebabkan resistensi terhadap serangga sasaran, sehingga diperlukan uji status kerentanan Ae.aegypti di Indonesia. Jenis penelitian adalah deskriptif dengan desain potong lintang. Pengambilan sampel uji (jentik Ae. aegypti) dilakukan di 102 Kabupaten/kota di Indonesia pada 100 rumah di setiap tiga wilayah kerja puskesmas. Metode uji kerentanan menggunakan bioassay test standar WHO. Hasil uji terhadap malathion 0,8% mendapatkan 86 (84%) kabupaten telah resisten, 13 kabupaten (13%) toleran dan 3 (3%) kabupaten rentan. Hasil uji terhadap temephos 0,02% menunjukkan 50 (49%) kabupaten resisten, 30 (29%) kabupaten toleran dan 22 (22%) kabupaten rentan. Hasil uji insektisida sipermetrin 0,05%, sebanyak 100 (98%) kabupaten resisten, dan 1 (1%) kabupaten toleran. Hasil uji terhadap alfa sipermetrin 0,025%, sebanyak 18 kabupaten (40%) resisten, 23 (51%) kabupaten toleran dan 4 (9%) kabupaten berstatus rentan. Hasil uji terhadap deltametrin 0,025% sebanyak 66 (65%) kabupaten resisten, 22 (22%) kabupaten toleran dan 14 (14%) kabupaten rentan. Dapat disimpulkan bahwa telah terjadi resistensi terhadap insektisida golongan organofosfat dan piretroid. Disarankan untuk mengganti kedua jenis insektisida tersebut di setiap wilayah yang teridentifikasi resitensi tinggi. Kata kunci: Insektisida, resitensi, organofosfat, piretroid, aedes aegypti

Abstract

ABSTRACT Efforts to break transmission of filariasis chain, through elimination program with preventive mass drug administration (POPM) and vector control, which is currently taking place in all provinces in Indonesia. Known as elephantiasis or lymphatic filariasis, it is a filarial worm with the main species being Wuchereria bancrofti and Brugia sp. The aims of study was to identify mosquitoes that have the potential become vector filariasis. Known as elephantiasis (Elephantiasis) or Lymphatic filariasis is a major species of filarial worms with that Wuchereria bancrofti and Brugia sp. Mosquito collection conducted in 23 districts / cities in Indonesia using modificated Human Landing Collection (HLC) method that is double net to feed humans inside, while the method of Transcription Insulated Isothermal Polymerase Chain Reaction (iiPCR) was used to detect the presence of filarial worms in the mosquitoes. The number of samples collected was 26,276 mosquitoes, 38 were identified. The results of the examination used iiPCR showed that positive W. bancrofti was found in 6 districts / cities and positive Brugia sp were found in 9 districts / cities where the study was conducted. There are other mosquito species that are not confirmed as filarial vectors, but found DNA traces both the parasite in mosquitoes that have not been confirmed as vectors. Keywords: Microfilariae, Wulcheria brancrofti, Brugia malayi, Mass Drug Administration (MDA) ABSTRAK Upaya pemutusan rantai penularan filariasis melalui program eliminasi dengan pemberian obat pencegahan masal (POPM) dan pengendalian vector, saat ini sedang berlangsung di semua provinsi di Indonesia. Di kenal sebagai penyakit kaki gajah (Elephantiasis) atau Filariasis limfatik merupakan cacing filaria dengan spesies utama yaitu Wuchereria bancrofti, dan Brugia sp. Penelitian ini bertujuan untuk mengetahui nyamuk yang berpotensi menjadi vector filariasis. Pengambilan sampel nyamuk dilakukan di 23 kabupaten/kota di Indonesia menggunakan metode Modificated Human Landing Collection (HLC) yaitu kelambu ganda dengan umpan manusia, sedangkan metode Transcription Insulated Isothermal Polimerase Chain Reaction (iiPCR) digunakan untuk mendeteksi keberadaan cacing filaria dalam tubuh nyamuk. Jumlah nyamuk yang berhasil dikumpulkan sebanyak 26.276 nyamuk dengan 38 spesies yang teridentifikasi. Hasil pemeriksaan DNA menggunakan iiPCR menunjukkan spesies Wulchereria bancrofti ditemukan di 6 kab/kota dan spesies Brugia malayi ditemukan di 9 kab/kota tempat penelitian. Terdapat spesies nyamuk lainnya yang tidak terkonfirmasi sebagai vektor filaria, namun ditemukan jejak DNA kedua parasite tersebut dalam nyamuk yang belum terkonfirmasi sebagai vektor. Abstract

ABSTRACT The prevalence of confirmed COVID-19 cases is high and tends to increase, becoming the main cause of morbidity and mortality. This study aims to determine the relationship between comorbidities and individual characteristics on the incidence of COVID-19 cases in Bogor City in 2020. The research method is case controlith a sample of 289 consisting of 148 cases and 141 controls (1:1). Datas were collected online using a structured questionnaire, taken from the medical record of 5 hospitals in Bogor City who had been treated. Samples were obtained from March to September 2020. Data were analyzed univariate to see the descriptive picture, bivariate to obtain crude OR, andultivariate using logistic regression method to obtain adjusted OR. The results of multivariate analysis showed a relationship between confirmed cases of COVID-19 and several risk factors include marriage factor (OR = 2.69 at 95% CI 1.54-4.70 with p value = 0.00), diabetes Mellitus (OR=3.07 at 95%CI 1.27-7.41 with p=0.01) and risk age group factors (OR=3.44 at 95%CI 2.00-5.90 with a p=0.00). In conclusion the risk factors for the incidence of COVID-19 cases are married residents, people suffering diabetes mellitus, and residents at the risk age group (18-59 years) in Bogor City. Keywords: COVID-19, comorbid, individual characteristics, Bogor city ABSTRAK Prevalensi kasus konfirmasi COVID-19 yang tinggi dan cenderung meningkat menjadi penyebab utama kesakitan dan kematian. Banyak hasil penelitian tentang faktor risiko terjadinya kasus COVID-19 yang hasilnya sangat bervariasi. Penelitian ini bertujuan untuk mengetahui hubungan antara komorbid/penyakit penyerta dan karakteristik individu terhadap kejadian kasus COVID-19 di Kota Bogor tahun 2020. Rancangan penelitian adalah case-control dengan sampel sebanyak 289 yang terdiri dari 148 kasus dan 141 kontrol (1:1). Data dikumpulkan secara daring menggunakan kuesioner terstruktur melalui aplikasi yang didapat dari data rekam medis 5 Rumah Sakit di Kota Bogor. Sampel diambil dari bulan Maret s.d. September 2020 yang pernah di rawat di RS. Data dianalisis secara univariat melihat gambaran deskriptif, secara bivariat mendapatkan crude OR dan multivariat dengan metode regresi logistic untuk mendapatkan adjusted OR. Hasil analisis multivariat menunjukkan hubungan antara kasus konfirmasi COVID-19 dengan beberapa faktor risiko di antaranya: faktor perkawinan (OR=2,69 pada 95%CI 1,54-4,70 dengan nilai p=0,00), faktor menderita diabetes mellitus (OR=3,07 pada 95%CI 1,27-7,41 dengan nilai p=0,01) dan faktor kelompok umur berisiko (OR=3,44 pada 95%CI 2,00-5,90 dengan nilai p=0,00). Kesimpulan bahwa faktor risiko kejadian kasus COVID-19 ialah penduduk yang sudah menikah, penduduk yang menderita sakit diabetes mellitus dan penduduk pada kelompok umur yang berisiko (18-59 tahun) di Kota Bogor. Abstract

ABSTRACT Domestic solid waste and livestock waste management are part of 2019-2025 Citarum Watershed Pollution and Degradation Control Action Plan. Domestic and livestock activities in the livestock area in Bandung regency have the potential to cause Citarum river pollution. This analysis aim to know the method of domestic solid waste and livestock waste management in the livestock area of Tarumajaya Village, Kertasari District, Bandung Regency. This research was conducted in 2019, with a quantitative and qualitative approach. The result is some people still manage domestic solid waste by burning or throwing it into the river, and managing livestock waste by throwing it into the river. Alternatives for eco- friendly domestic solid waste management through ‘waste banks’ and livestock waste management with biogas technology, have been known and practiced by some people. The current challenge is to maintain the continuity of the program. It is recommended to the local government, especially the Health Office and Health Center, as well as the Agriculture Office to periodically provide socialization, technical and management training, consultation and monitoring of the environment friendly programs. Keywords: Citarum river, waste bank, biogas ABSTRAK Permasalahan sampah rumah tangga dan limbah peternakan merupakan bagian dari Rencana Aksi Pengendalian Pencemaran dan Kerusakan Daerah Aliran Sungai Citarum 2019-2025. Aktivitas domestik dan peternakan di kawasan peternakan Kabupaten Bandung ditengarai memiliki potensi penyebab pencemaran Sungai Citarum. Analisis ini bertujuan untuk mengetahui pola pengelolaan sampah rumah tangga dan limbah ternak di kawasan peternakan Desa Tarumajaya, Kecamatan Kertasari, Kabupaten Bandung. Penelitian ini dilaksanakan tahun 2019, dengan pendekatan kuantitatif dan didukung pendekatan kualitatif. Hasil analisis menunjukkan sebagian masyarakat masih mengelola sampah rumah tangga dengan cara dibakar atau dibuang ke sungai, dan mengelola limbah ternak dengan cara membuang ke sungai. Alternatif pengelolaan sampah rumah tangga yang ramah lingkungan melalui bank sampah dan pengelolaan limbah ternak dengan teknologi biogas, telah dikenal dan dipraktekkan sebagian masyarakat. Tantangan saat ini adalah menjaga kontinuitas program tersebut. Disarankan kepada pemerintah daerah khususnya Dinas Kesehatan dan jajarannya, serta Dinas Pertanian secara periodik memberikan sosialisasi, pelatihan teknis dan manajemen, konsultasi dan monitoring program ramah lingkungan tersebut.
