Abstract

Tuberculosis (TB) remains a major public issue in Indonesia including in North Sumatra province. Despite reported good efficacy of TB treatment in the region, the success of treatment depends on many factors including nutritional status. This study aims to determine the relationship between Body Mass Index and Albumin level with sputum conversion in AFB-positive pulmonary TB patients.The study was done in two primary health centres in Medan between October and November 2018. A total of 39 newly diagnosed TB patients with confirmed AFB-positve were included in the study.Participants received TB treatment according to the national guidelines. The proportion of participants with below normal, normal and above normal BMI status were 13 (33.3%), 21 (53.9%) and 5 (12.8%), respectively. Level of albumin was determined as normal in 25 participants (64.1%), and the remaining as low. Normal BMI status was significantly associated with increased albumin level (P<0.05). At 2 months follow-up, the sputum conversion was observed in 24 individuals (61.5%), and the conversion was significantly associated with normal BMI and/or normal albumin level (P<0.05). We concluded that nutritional status is an important factor in the success of TB treatment.

Abstract

The stress nurses face in hospitals at work will greatly affect the quality of nursing services provided to patients. This study aims to analyze what factors affect the work stress on nurses. This research is a quantitative study with a research design using the analytical survey method with a Cross Sectional approach. The research was conducted at Rantauprapat Hospital, Labuhanbatu Regency. The sample was 82 nurses obtained by purposive sampling. Data analysis was performed using a univariate analysis test, Bivariate using a chi-square test, and Multivariate using multiple Logistic regression. The results showed that in the univariate analysis of 82 respondents, most of the respondents were 36-45 years old, namely 36 (43.9%), and had a D3 Nursing education level, namely 45 (54.9%). In the Bivariate analysis, it was found that there was the effect of workload on nurses' work stress (p= 0.000), there was an effect of the work environment on nurses' work stress (p= 0.000), there an effect of career development on nurse stress (p= 0.000), there was the effect of role conflict double the work stress of nurses (p= 0,000). In the multivariate analysis, the most dominant variable affecting the work stress of nurses was career development with a p value of 0.007, t count = 2.273 (r table = 0.2146). Based on the results of this study, it is recommended that the hospital provide the duties and responsibilities given should not be less or excessive so that work stress on nurses.

Abstract

The accuracy of the diagnosis code is important as a determinant in the financing of health services. The delay in payment of BPJS Health claims can occur if there is a difference in the diagnosis code. This study aims to describe the differences in the ICD 10 diagnosis code between hospitals and BPJS Health Verifiers and the factors that influence them. Qualitative research with data collection techniques through in-depth interviews and documentation studies. The research informants consisted of the main informants as many as 3 Doctors in Charge of Service (DPJP) and the triangulation informants as many as 3 people, namely the Internal Verifier, Coder, and Head of the Medical Record Section. Processing and analyzing data using the content analysis method. The results of the study were 187 claim files experienced delays in payment due to differences in diagnosis codes, no quality audit of diagnosis coding was carried out, no clinical protocol, lack of cooperation between DPJP, coder, and internal verifier, lack of understanding of DPJP, lack of hospital management role involving DPJP in writing the code diagnosis, the absence of hospital policies/SOPs related to writing the ICD 10 diagnosis code, not providing diagnostic coding training. The conclusion of the study is that there are differences in the ICD 10 diagnosis code due to the absence of quality audits for diagnosis coding, clinical protocols, and the role of hospital management which is lacking in preparing policies and training on diagnosis coding causing delays in claim payments.

Abstract

The coronavirus pandemic (COVID-19) is designated as a Public Health Emergency that is troubling the world, much remains unknown about the virus that causes COVID-19, but in general, this virus is transmitted through direct contact with droplets from the respiratory tract of an infected person (which exits through coughs). And sneezes). People can also become infected by touching a surface contaminated with this virus and then touching their face (eg, eyes, nose, mouth). The school health program (SHP) is a health program implemented at the school level. The main activities of SHP are carried out through health education, health services, and fostering a healthy school environment with the primary target of students, educators, educators, and the school community. Efforts to maintain health can be started from the school environment, which emphasizes changing behavior. Therefore this SHP program is essential because it includes early health education. This community service aims to provide education and socialization to prevent the spread of the COVID-19 virus to teachers and students based on UKS at Amanah SMPS, Binjai District, Langkat Regency. Before being given education, assessing the COVID-19 prevention behavior that they have implemented is carried out using a valid questionnaire. Then proceed with educational activities. The educational method carried out was by lecturing and discussing the prevention of COVID-19 and simulating the proper use of masks, after which several participants were asked to practice it and witnessed by other participants. It is hoped that with this community, serving students and teachers will increase their understanding and be able to prevent COVID-19 both for themselves and prevention in the community.

Abstract

Penilaian mutu pelayan kesehatan antara lain dimensi kehandalan(reliability), daya tanggap(responsiveness), jaminan(assurance), empati(emphaty) , dan bukti fisik (tangible). Penelitian ini dilakukan karena menurunnya kunjungan pasien yang berobat jalan ke klinik kesehatan kulit dan kelamin dalam setahun ini, hal ini apakah ada hubungannya dengan ketidakpuasan pasien terhadap pelayanan farmasi, karena dari wawancara sebelumnya oleh peneliti, menunjukkan ada beberapa ketidakpuasan pasien, seperti menunggu obat yang lama, kurangnya obat, suasana yang tidak nyaman dan lainnya. Penelitian ini bertujuan untuk menganalisis kepuasan pasien klinik kesehatan kulit dan kelamin terhadap pelayanan farmasi berdasarkan perspektif pasien dan petugas kesehatan di RSUD Dr.R.M.Djoelham Binjai. Penelitian ini merupakan penelitian deskriptif dengan pendekatan kualitatif, dimana pengumpulan data menggunakan teknik observasi, wawancara, catatan lapangan dan dokumentasi. Responden sebanyak 10 informan yang terdiri dari, 4 informan dari pasien yang berulang berobat jalan ke klinik kesehatan kulit dan kelamin, 2 informan dari dokter , 2 informan dari pelayanan farmasi/apoteker dan 2 informan dari manajemen rumah sakit. Hasil wawancara pada penelitian ini menunjukkan pasien merasa tidak puas dengan pelayanan instalasi farmasi di RSUD Dr.R.M.Djoelham Binjai. Manajemen pengelolaan sediaan farmasi dan farmasi klinik (sarana dan prasarana) harus ditingkatkan dan menambah petugas pelayanan farmasi pada RSUD Dr.R.M.Djoelham Binjai agar pasien menjadi puas.

Abstract

Knowledge and social stigma can influence the behavior of TB sufferers in seeking treatment in order to free themselves from the disease and obtain health recovery. This study aims to analyze the effect of knowledge and social stigma of tuberculosis on treatment-seeking behavior. This research is quantitative research conducted in the working area of ​​Hutabaginda Public Health Center, North Tapanuli Regency. The sample in the study was 60 people consisting of 30 TB patients and 30 people from the community. Research data in the form of primary data, secondary data, and tertiary data. Data were analyzed by univariate, bivariate, and multivariate analysis. The results showed that there was no effect of patient knowledge on the behavior of seeking treatment for tuberculosis patients with p = 0.563 > 0.05. There is no influence of internal stigma on the behavior of seeking treatment for tuberculosis patients with a p-value = 0.565 > 0.05. There is an influence of public knowledge on the behavior of seeking treatment for tuberculosis patients with a p-value = 0.027 <0.05. There is an influence of external stigma on the behavior of seeking treatment for tuberculosis patients in the working area of the Hutabaginda Public Health Center, North Tapanuli Regency with p = 0.030 <0.05. Based on this, it is recommended that the results of this study be used as input for the Hutabaginda Public Health Center, North Tapanuli Regency to make programs and evaluations in dealing with health problems, especially cases of pulmonary TB.

Abstract

Introduction: Stunting is a chronic malnutrition condition caused by a numeuros of factors, including inadequate maternal nutritional nutrition during pregnancy, inadequate exclusive breastfeeding, and infectious diseases. There are also some indirect factors .Objective: The objective of this study was to investigate the risk factors for stunting in children aged 6-36 months in the Sambirejo Health Center area. study. Method: This study is an observational analytic study using a cross-sectional approach that gathers data via a questionnaire. Sampling Techniques using the Non- Probability Sampling technique, and the sample size was estimated using the Lemeshow formula. The Prevalence Ratio (PR) was calculated to evaluate the strength of the link between the factors assumed to be linked with stunting. Result and Discussion: Of the 65 samples,  Four children were stunted, while 61 were not. Infants who were not exclusively breastfed, infants with a history of LBW, infants with a history of SBL, and family planning of 2 years both were risk factors for stunting in this study. Conclusions: Some variables in this study had no impact on the incidence of stunting. In this study, the risk factors for stunting were LBW, SBL, and infants without exclusive breastfeeding, and child spacing was a neutral factor for the occurrence of stunting in children aged 6-36 months.

Abstract

This study analyzes the impact of outpatient health services on the satisfaction of BPJS Health participants at the Huraba Community Health Center, East Angkola District in 2023. Factors including responsiveness, reliability, guarantee, care, and physical evidence of service are explored to determine their influence on patient satisfaction. The research method used is quantitative with a cross sectional study approach. Primary data was collected through questionnaires distributed to patients, while secondary and tertiary data were obtained from documentation and trusted sources. The results of the analysis show that the aspects of responsiveness, reliability, assurance and caring have a significant influence on patient satisfaction, while physical evidence does not show a strong correlation.
