Abstract

Dokumen rekam medis merupakan alat untuk mencatat terjadinya transaksi pelayanan sehingga harus dijaga keamanan serta kerahasiaannya dari bahaya kerusakan. Tujuan penelitian ini untuk mengetahui bagaimana pelaksanaan pemeliharaan dokumen rekam medis diruang penyimpanan Puskesmas Polonia Medan. Penelitian bersifat deskriptif dan populasi yang digunakan adalah seluruh dokumen rekam medis diruang penyimpanan Puskesmas Polonia dengan sampel berjumlah 100 berkas. Dari hasil penelitian diperoleh kerusakan dokumen rekam medis diruang penyimpanan Puskesmas Polonia Medan sebesar 56%. Faktor intrinsik yang mempengaruhi yaitu kualitas kertas, tinta dan sampul berkas. Sedangkan faktor ektrinsik yang mempengaruhi yaitu kabel listrik tidak tersusun rapi, rak terlalu penuh dan sempit, kelembapan udara mencapai 16oC, jamur, serangga, debu yang menjadi penyebab kerusakan dokumen rekam medis di ruang penyimpanan Puskesmas Polonia Medan. Disarankan kepada Puskesmas Polonia Medan agar dokumen rekam medis menggunakan sampul plastik dengan rak penyimpanan terbuat dari besi sehingga dokumen rekam medis tidak terkena jamur dan rayap yang dapat merusak dokumen rekam medis.

Abstract

Antrian merupakan kejadian dimana pelanggan harus menunggu giliran untuk mendapatkan pelayanan. Proses Antrian di apotik Rumah Sakit Tentara Padang merupakan suatu masalah yang sering kali menjadi keluhan bagi pasien. Untuk mengatasi masalah ini dibutuhkan suatu metode sistem antrian supaya mendapatkan performansi model antrian. Sehingga dapat memberikan gambaran dan solusi untuk meningkatkan kinerja pelayanan pada proses pengambilan obat di apotik Rumah Sakit Tentara Padang. Model Antrian yang digunakan adalah model antrian Single Channel Single Phase yang memiliki satu jalur dan satu pelayanan yang dialiri oleh satu antrian tunggal. Variabel yang akan diamati adalah waktu antar kedatangan, waktu mulai pelayanan dan waktu selesai pelayanan dengan mengasumsikan pola distribusi poisson. Hasil dari penelitian ini nantinya diperoleh waktu sibuk server terpanjang yaitu 94,12%, jumlah rata-rata pasien dalam antrian terpanjang perperiode (Lq) yaitu 15 pasien, jumlah rata-rata pasien dalam sistem terpanjang perperiode (Ls) yaitu 16 pasien, waktu pasien berada dalam sistem terpanjang perperiode waktu (Ws) yaitu 60 menit, waktu pasien berada dalam antrian terpanjang perperiode waktu (Wq) yaitu 56,472 menit.

Abstract

Queue is an occurrence where someone has to wait their turn to get service. Queuing processes in hospitals when processing outpatient registration medical records at the hospital often occur especially during peak hours. To overcome this problem, a solution is needed to improve service performance at the hospital. The queuing model used is the Multi Channel-single phase queuing model which has one or more services flowing by a single queue. The variables to be observed are time between arrivals, service time data and number of services assuming a Poisson distribution pattern. The results of this study will show the operator's busy time is 83.33%, the number of queues in a certain period (Lq) is 13 patients, the number of registrants in the system (L) is 14 patients, the waiting time in the queue (Wq) is 24 minutes, and waiting time in the system (W) is 30.06 minutes.

Abstract

Rawat jalan merupakan pelayanan yang diberikan kepada pasien untuk berobat atau keperluan lainnya tanpa tinggal diruang rawat inap rumah sakit. RSU Imelda Pekerja Indonesia Medan juga berpotensi menjadi salah satu rumah sakit rujukan di Kota Medan Sumatera Utara. Tempat pendaftaran rawat jalan RSU Imelda Pekerja Indonesia Medan dimulai dari pendaftaran pasien hingga pasien mendapatkan pelayanan. Berdasarkan survey diketahui bahwa jika pasien BPJS tidak melengkapi persyaratan seperti foto copy KTP dan KK, surat rujukan, foto copy SKP dan foto copy kartu BPJS dalam sehari maka pasien tersebut tidak segera mendapatkan pelayanan lebih lanjut. RSU Imelda Pekerja Indonesia Medan belum mempunyai prosedur pendaftaran secara khusus untuk pasien rawat jalan BPJS sehingga masih sulit untuk menelusuri dokumen rekam medis diruang filling dan juga pendistribusian dokumen rekam medis ke poliklinik masih sangat lama. Hal ini menyebabkan pelayanan pasien BPJS tidak segera terpenuhi. Tujuan penelitian untuk mengetahui alur prosedur pelayanan pasien rawat jalan peserta BPJS. Jenis penelitian yaitu deskriptif, subyek yang diteliti yaitu pasien rawat jalan BPJS dengan obyek penelitian yaitu RSU Imelda Pekerja Indonesia Medan. Instrumen yang digunakan dengan wawancara dan observasi langsung ke lapangan.

Abstract

Analysis of the medical record workload is very necessary in improving health services in a hospital. This aims to determine which workloads need to be streamlined. In addition, with the analysis or measurement of workloads, scientific evidence-based decision making can be made. Seeing the large role of medical records in health services in hospitals, it is necessary what things are needed to improve the best quality in medical record installations. The purpose of this study was to determine the need for medical record personnel based on the workload in the filling section of Mitra Sejati General Hospital Medan. This type of research is descriptive with a cross sectional approach with a population of 4 medical record officers. The place where the research was conducted was at Mitra Sejati General Hospital Medan because the hospital had problems regarding work requirements in the filling section. The available working time is based on the number of effective working hours in the medical record section, which is 2200 hours/year, in 1 day working hours are 8 hours. The above regulation is based on the number of working hours of medical record officers at Mitra Sejati Hospital Medan, the number of outpatient visits is 6098 patients/year and for inpatients is 14789 patients/year while the standard time allowance for officers is 0.01 minutes. Based on the results of the calculation of the need for filling officers using the WISN method, the total human resource requirements in the outpatient filling unit were 2.89 or 3 officers while the outpatient filling unit officers at Mitra Sejati General Hospital Medan were 2 people. Meanwhile, the total human resource requirements for the inpatient filling unit are 3.11 or 3 officers, while the inpatient filling staff at Mitra Sejati General Hospital Medan are 2 people, so there is still a need for additional medical records officers specifically in the filling section.

Abstract

Storage (Filling) is an activity of storing, structuring, or storing medical record files facilitate retrieval. The aim of this research is to determine the overview of the storage system with the length of time for retrieval of medical record files at RSIA Artha Mahinrus Medan. This research is descriptive qualitative to see by describing the results as they are and comparing them with the theory which is then drawn conclusions. The time of the study was carried out in May at the Artha Mahinrus Mother and Child Hospital, Medan. The population and sample used are medical record employees in the registration and storage division (filling). The instruments used in data collection were observation and interviews. Based on the results of this study, it shows that the storage system at the Artha Mahinrus Mother and Child Hospital Medan is a centralized storage system, by combining medical record files in one room. The time of taking medical record files is very influential on education, age, years of service and storage shelves. The education of medical record file storage officers is not all D3 medical record graduates. But the storage process continues to run well. for the medical record retrieval process, it is hoped that the retrieval process from registration to the polyclinic is expected to be improved again with a time of 10 minutes.

Abstract

The coding audit is a review to ensure that the results of the diagnosis and treatment coding produced by the clinical coder are accurate, consistent and timely when compared to the information contained in the patient's medical record. The purpose of this study was to determine the implementation of the coding audit and to find out the efforts made to minimize the non-compliance of claims to the BPJS Health at Imelda Pekerja Indonesia Hospital in 2021. The type of research used was qualitative research with the number of informants as many as three coders who took Total Sampling. The research instrument used was an interview guide with qualitative descriptive data analysis techniques. Based on the results of the study, it was found that the level of knowledge of coding audit officers about coding audits is quite good but coding audits are still carried out post-claims and there are no instruments in implementing coding audits while the person in charge of coding audits at Imelda General Hospital is the Medical Committee, Verifier Internal, Quality Control and Cost Control Teamand Koder. Suggestions from researchers should Imelda Pekerja Indonesia Hospital socialize the coding audit SPO, then the coding audit is carried out pre-claim and in its implementation a coding audit instrument is made.
