Abstract

Abstract

This research explores issues related to mainstreaming the 2015 Regional Budget (APBD) of Riau Province amounted IDR 10.7 trillion. Based on the data obtained from the Riau Finance and Asset Management Board (BPKAD), Riau could only spend APBD 37.58% of APBD until October 2015. Meanwhile, the Ministry of Home Affairs of the Republic of Indonesia calculated Riau spent only 59.6% throughout 2015 and it wast the lowest regional spending in Indonesia. This poor expenditure rate demonstrates a problem in the implementation of Riaus APBD which was inneficient and not yet relevant. An assessment by government expenditure theory reveals that these problems indicate a gap in APBD allocation that affect on the non-performance regional development as targeted in mid-term regional development plan (RPJMD). This APBD seems to perform only a budget plan, not an implemented budget policy yet.

Abstract

Introduction: In 2014 globally 9.6 million people are infected with TB, including 1.2 million people living with HIV. HIV positive patients have a risk of Tuberculosis (TB), infection or latent TB infection (LTBI) about 5-8% per year and the risk of developing active TB 3 times higher than the immunocompetent individuals. The impact of HIV infection on the immune system is the destruction of lymphocyte T CD4 and declining nutritional status. Nutritional status can be assessed by clinical signs, anthropometry and biochemical indicators such as serum albumin levels. Methods: Cross-sectional observational epidemiological research in RS Dr. Moewardi, Surakarta, to patients with HIV, from April to May 2017 to observe the association of risk factors with LTBI in HIV patients. The first step is patient and family anamnesis, history of past and family illness, duration of antiretroviral therapy, Cluster of Differentiation 4 (CD4) count, BMI count, serum albumin concentration, immunologic Tuberculin Skin Test (TST) for LTBI diagnosis. Results: Total of 93 people meet the criteria. There was no association close contact history with active tuberculosis patients with ITBL in HIV (p = 0.5, OR 1.268; 95% CI 1.139-1.411); no association of BMI classification with LTBI in HIV (P=1.000; OR=0.894; 95%CI=0.326-2.457), no association of serum albumin grade with ITBL in HIV (P=0.19; OR=1.284; 95%CI=1.147-1.436); there was a significant association of CD4 classification with LTBI in HIV (P=0.033; OR=3.560; 95%CI=1.214-10.433), there was no association between duration of antiretroviral therapy with LTBI in HIV (P=0.636; OR=0.969; 95% CI=0.244-3.848). Conclusions: HIV patients with advanced CD4 and severe immunosuppressive CD4 classification (CD4+

Abstract

Pengawasan ketat timbulnya efek samping gangguan pendengaran dan gangguan fungsi ginjal pasien usia lebih dari 40 tahun dan perempuan pada pengobatan TB RO.Kata kunci: Efek samping, gangguan fungsi ginjal, gangguan pendengaran, kanamisin, TB resistan obat Effects of Risk Factors on the Onset of Kanamycin’s Adverse Events in Drug Resistant TuberculosisKanamycin is a therapy for drug-resistant tuberculosis (TB) which may cause hearing loss, impaired kidney function, and electrolyte disorders, especially hypokalemia. The objective of this study was to assess patient survival and the effects of risk factors such as age, previous history of streptomycin therapy, and weight on adverse events due to kanamycin administration in patients with drug resistant TB. A retrospective cohort study was conducted in TB patients by using medical records from January 2011 to April 2017 in Dr. Moewardi Hospital Surakarta. Cox regression analysis was used to analyze the relation between risk factors and adverse events. Of 238 patients, 143 patients experienced hearing loss, 147 patients experienced impaired kidney function, and 169 patients had hypokalemia. Age over 40 and female gender had higher risks for hearing loss and impaired kidney function (HR: 2.419 (95% CI: 1.716–3.409; p= 0,000) and HR: 1,892 (95% CI: 1.353–2.646; p=0,000); HR: 1.549 (95%CI: 1.089–2.202; p=0,015), and HR: 1.667 (95% CI: 1.179–2.357; p=0.004)), respectively. History of streptomycin and body mass index (BMI) were not risk factors for  kanamycin’s adverse events. Therefore, closed monitoring on hearing loss and impaired kidney function is necessary for drug resistant TB patients aged over 40, and female patients. Key words: Adverse events, drug resistant tuberculosis, hearing loss, impaired kidney function, kanamycin

Abstract

Background: Uric acid can be used as a biomarker to distinguish transudates from pleural fluid exudates. Uric acid is soluble in liquid media, elevated levels of uric acid in blood plasma become a predisposing factor for uric crystal deposits in tissues and body fluids. Increased permeability due to changes in capillary pleural pressure in the formation of transudates is a cause of increased levels of uric acid in pleural fluid. The aim of the study was to analyze the examination of pleural fluid uric acid levels and the ratio of uric acid levels to pleural fluid to serum to distinguish exudates and transudates in pleural effusion. Methods: A cross sectional diagnostic test with a cross sectional study design in pleural effusion patients at RSUD Dr. Moewardi Surakarta in July-August 2019. Subjects were examined for pleural fluid levels, serum uric acid ratio, and pleural fluid analysis. Results: A total of 30 pleural effusion patients were included in this study. The level of agreement between the uric acid pleural fluid against Light criteria was in the moderate category and statistically significant (Kappa = 0.595; P=0.001). The level of agreement between the ratio of uric acid to pleural fluid and serum to Light's criteria was weak and not statistically significant (Kappa = 0.200; p = 0.195) Conclusion: The level of concordance between pleural fluid uric acid with Light criteria is in the medium category and statistically significant. There is no match between the ratio of serum pleural fluid uric acid levels with Light criteria. (J Respir Indo. 2020; 40(2): 82-6)
