Abstract

State spending to deal with stroke is quite large every year due to the high incidence of stroke. Data from the Ministry of Health of Indonesia mentions an increasing number in health financing every year, the country financed stroke services by 2.56 trillion in 2018. The magnitude of the incidence of stroke can be lowered by controlling the risk factors of stroke. This study aimed to identify stroke risk factors, both those modifiable risk factors and irreversible risk factors. Research is conducted with a literature review using descriptive study methods or correlation analysis with cross sectional approaches. Sixteen articles identified from Google scholars and Garuda databases in the 2015-2020 range, and were analysed using JBI critical appraisal tools-checklist for analytical cross-sectional studies and JBI critical appraisal tools-checklist for prevalence studies.  Synthesis narrative was used to analyse the articles. Out of 16 articles examined the modifiable risk factors were hypertension, dyslipidemia, diabetes mellitus, lack of physical activity, blood sugar levels, cholesterol levels in blood, alcohol and drug consumption, kidney disease, tuberculosis, heart disease, heart failure, obesity, central obesity, atrial fibrillation and also smoking are declared associated with the incidence of stroke. The irreversible risk factors are age, gender, education, family history, income employment, previous stroke history, and rural and urban distribution. Risk factors divided into two categories modifiable risk factors and irreversible risk factors. It is important to know the risk factors of stroke in order prevent the occurrence of stroke as early as possible by avoiding modifiable stroke risk factors. Hypertension, smoking, diabetes mellitus obesity, dyslipidemia, and lack of activity are the most modifiable stroke risk factors identified in the articles. The most irreversible risk factors identified in the article are age, gender, and level of education

Abstract

Hypertension was the most common disease suffered by the people of Sungai Rangas Ulu Village. One of the management of hypertension was complementary therapy in the form of a foot spa. This study aims to describe the results of nursing care in patients with hypertension through foot spa therapy. It used a case study method on one of the hypertensive patients. The individual was a community of Sungai Rangas Ulu Village chosen at random or accidentally. Individuals were given foot spa therapy, and observed their blood pressure by using a sphygmomanometer every day for six days. The results of the evaluation of foot spa therapy were that blood pressure had decreased. Blood pressure before intervention was 170/100 mmHg; on the last day of the intervention, blood pressure was found to be 141/94 mmHg. Foot spas could provide a sense of relaxation to stimulate vasodilation, lowering blood pressure. Foot spa therapy can be a complementary therapy option for people with hypertension.

Abstract

Dysphagia is a complication that often appears in stroke patients which can cause aspiration. One of the nursing interventions to treat dysphagia in stroke patients is swallowing exercises. This study aims to determine the effectiveness of swallowing exercises on swallowing function in stroke patients with dysphagia. This study was a literature review involving twelve literature obtained from three databases, namely: Google Scholar, Pubmed and ScienceDirect during 2010 to 2020, with the criteria of using a true experiment or quasy experiment design and involving stroke patients with dysphagia. The results of the review show that there were three swallowing exercise techniques found, namely effortful swallow, shaker exercise and Mandelsohn maneuver. Variation in duration of each technique was 20 minutes to 60 minutes. There were several articles that did not mention the technique and duration of swallowing exercises. Several articles combined swallowing exercise with other interventions such as electrical stimulation, vitalstim therapy, and acupuncture; with better results than a single intervention. Furthermore, it was concluded that the three swallowing exercise techniques proved effective in improving swallowing function in stroke patients with dysphagia.Keywords: dysphagia; swallowing exercises; strokesABSTRAK Disfagia merupakan salah satu komplikasi yang sering muncul pada pasien stroke yang dapat menyebabkan aspirasi. Salah satu intervensi keperawatan untuk mengatasi disfagia pada pasien stroke adalah latihan menelan. Penelitian ini bertujuan untuk mengetahui efektivitas latihan menelan terhadap fungsi menelan pada pasien stroke dengan disfagia. Studi ini merupakan literature review yang melibatkan dua belas literatur yang diperoleh dari tiga database yaitu: Google Scholar, Pubmed dan ScienceDirect selama tahun 2010 sampai 2020, dengan kriteria yaitu menggunakan rancangan true experiment atau quasy experiment dan melibatkan pasien stroke dengan disfagia. Hasil review menunjukkan bahwa ada tiga teknik latihan menelan yang ditemukan yaitu effortful swallow, shaker exercise dan mandelsohn maneuver. Variasi durasi dari masing-masing teknik adalah 20 menit sampai 60 menit. Ada beberapa artikel yang tidak menyebutkan teknik dan durasi latihan menelan. Beberapa artikel juga mengkombinasikan latihan menelan ini dengan intervensi lain seperti stimulasi listrik, terapi vitalstim, dan akupuntur; dengan hasil yang lebih baik daripada intervensi tunggal. Selanjutnya disimpulkan bahwa ketiga teknik latihan menelan terbukti efektif untuk meningkatkan fungsi menelan pasien stroke dengan disfagia.Kata kunci: disfagia; latihan menelan; stroke

Abstract

Spasticity is a symptom of stroke which can lead to weakness and disability. A series of recovery can be done, one of which is using massage therapy. Massage differs in the type of procedure, duration of administration, duration of disease onset, and effectiveness. So it is necessary to do a literature study that aims to determine the effectiveness of massage therapy in the treatment of upper extremity hemiparesis spasticity in stroke patients. This literature review searches for articles 2000-2020 in Indonesian and English, from Pubmed, Science Direct, and Google Scholar. Assessment of article quality using JBI's Critical Appraisal Tools. Data synthesis in narrative form. The results of the study obtained 4 synthesized articles. Tuina can be given without or in addition to other interventions. Tuina is effective for reducing upper extremity muscle spasticity at disease onset 1-3 months. It was concluded that Tuina was effective in reducing upper extremity spasticity and could be given to post-stroke patients 1-3 months.Keywords: massage; spasticity; strokesABSTRAK Spastisitas merupakan gejala stroke yang dapat mengakibatkan kelemahan dan kecacatan. Serangkaian pemulihan dapat dilakukan, salah satunya adalah menggunakan terapi pijat. Pemijatan memiliki perbedaan dalam jenis prosedur, durasi pemberian, lama onset penyakit, dan keefektivitasannya. Maka perlu dilakukan studi literatur yang bertujuan untuk mengetahui efektivitas terapi pijat pada penanganan spastisitas hemiparesis ekstremitas atas pada pasien stroke. Tinjauan literatur ini mencari artikel tahun 2000-2020 dalam Bahasa Indonesia dan Bahasa Inggris, dari Pubmed, Science Direct, dan Google Scholar. Penilaian kualitas artikel menggunakan JBI’s Critical Apraisal Tools. Sintesis data dalam bentuk naratif. Hasil studi memperoleh 4 artikel yang disintesis. Tuina dapat diberikan tanpa atau dengan tambahan intervensi lain. Tuina efektif untuk menurunkan spastisitas otot ekstremitas atas pada onset penyakit 1-3 bulan. Disimpulkan bahwa Tuina efektif untuk menurunkan spastisitas ekstremitas atas dan dapat diberikan pada pasien pasca stroke 1-3 bulan.Kata kunci: pijat; spastisitas; stroke

Abstract

Latar Belakang: dalam 1 dekade terakhir, banjir pada awal tahun 2021 merupakan banjir terbesar di Kalimantan Selatan. Banjir ini mengakibatkan banyaknya masyarakat yang harus mengungsi, rumah yang terendam banjir, dan bahkan korban jiwa. Kabupaten yang paling banyak terdampak adalah Kabupaten Banjar dan Hulu Sungai Tengah. Bencana banjir ini berdampak besar terhadap kesehatan masyarakat, baik secara fisik maupun kejiwaan. Tujuan: menscreening diagnosis keperawatan dan etiologi yang muncul pada masyarakat pasca banjir di Kalimantan Selatan. Metode: penelitian ini adalah deskriptif analitik design dengan pendekatan cross sectional. Pengambilan subyek penelitian dengan teknik consecutive sampling dilakukan untuk mendapatkan 423 sample. Pengumpulan data menggunakan instrumen berupa kuesioner dalam bentuk google form yang diadaptasi dari buku NANDA-I (North American Nursing Diagnosis Association – International) edisi tahun 2018-2020 yang meliputi 85 item penilaian. Hasil: didapatkan 14 diagnosis keperawatan meliputi hambatan religiositas, ketakutan, hambatan rasa nyaman, risiko jatuh, ketidakefektifan manajemen kesehatan keluarga, kontaminasi, risiko infeksi, ansietas, ketidakefekifan koping, kerusakan integritas kulit, diare, penurunan koping keluarga, hipotermia, dan distress spiritual. Etiologi untuk masing-masing diagnosis keperawatan telah diidentifikasi berdasarkan buku NANDA-I.

Abstract

Decreased motor function is one of the problems that often arises in stroke patients. One of the nursing interventions to overcome the decline in motor function in stroke patients is to provide a motor relearning program (MRP) intervention. The objective of this study was to determine the length, duration, and frequency of giving a MRP and the effectiveness of MRP on motor function in stroke patients. This study was conducted by means of a literature review. Eight pieces of literature were identified from three databases (Google Scholar, Pubmed and Science Direct) in the 2000-2020 range. The research design studied was true and quasi-experimental with stroke patient’s population who received MRP . There were eight articles that met the inclusion criteria with different durations and frequencies of motor relearning programs. The provision of MRP interventions in post- stroke patients is effective in improving motor function. There are varying durations and frequencies of giving MRP. Each article mentions that MRP is effective restoring motor function, although with different times, durations and frequencies. Several articles also combined this intervention with other therapies such as bobath therapy and electrical stimulation, the results showed better. The provision of MRP interventions in post- stroke patients is effective improving motor function.

Abstract

The lack of interest of nurses to used screening instruments for dysphagia at stroke centers and nerve spaces wasa strong reason for the development of the dysphagia screening website. This was to facilitate nurses in screeningpatients with dysphagia quickly. This study aimd to see the level of nurse satisfaction on the performance ofdysphagia screening websites. Quantitative research was used with total sampling techniques to 25 nurses. Itwas conducted on July to August 2019. Data collection used a nurse satisfaction level questionnaire after usingthe dysphagia screening website with results 8.0% were quite satisfied, 68.0% were satisfied and 24.0% werevery satisfied for the dysphagia screening website. As many as 88.0% chose dysphagia screening websites toscreen for stroke patients, 8.0% chose others that wanted this website to be developed into an offline applicationso that it could be used at any time and 4.0% chose to continue to screen for dysphagia by manual or check liston paper. Most respondent expressed the satisfaction in using dysphagia screening websites as seen from thelevel of satisfaction obtained.

Abstract

Diabetes mellitus is a chronic disease indicated by abnormal increase of blood glucose level. Diabetesmellitus requires long-term medical attention and treatment to prevent complication and manage the disease.The risk of complication can be reduced by self-management. The success of self-management hygieneconsiting of nutritional, pharmacology, physical training, blood sugar monitoring and foot care aspects areaffected by the motivation of diabetes mellitus patient. This research was located in Sungai Besar Village,from June to July 2019. The aim of this study was to determine the description of the motivation and selfmanagement of type 2 diabetes mellitus patients. It was a descriptive observational research using nonprobability sampling method with consecutive sampling technique. The instrument was questionnaire with atotal of 101 respondents. Result the study showed that most respondents’ motivation was good (72.3%) andmost respondents’ self-management was good (92.1%). It showed that high motivation was indicated bybehaviors. The motivation of type 2 diabetes mellitus patients encouraged them to do self-management ofdiabetes mellitus. Motivation and self management of type 2 diabetes mellitus patients in Sungai BesarVillage were good. Howover, only 2,0% of respondents dry between of their toes after washing and cleaningthem on the aspect of foot care. This result can be used as a reference for health workers to socialize abouthealth education related to the importance of foot care avoiding foot problems. Diabetes patient wereexpected to have great motivation doing self management.

Abstract

Background: The increasing of stroke prevalence is high year by year, and Indonesia is the number onefor cause of mortality. After three months, stroke survivors require long-term care. A stroke is a lifelongchange for both the stroke survivor and the family. Family caregiver is included in multidisciplinary thatshould provide the patients with stroke because of the unique demands on that population. There is lack ofstudy about stroke care in Indonesia.Objective: to explore the role and function of family in care of patients with stroke in community,Banjarmasin.Method: A critical ethnography design used in this study to explore and critically analyze role andfunction of family in care of patients with stroke in community. There were 15 key informants (familycaregivers) in this study using purposive sampling. Content analysis was used to answer the researchquestion.Result: Daily care activity, spiritual activity, rehabilitation and offering medicine, decision maker, andfinancial support are the role and function of family in care of patients with stroke in community setting.Conclusion: Family caregivers are playing an important role in caring for their relatives who havesuffered from strokes. They should work together with health workers to help the patient for enhancingthe quality of life of patient.

Abstract

Mortality and morbidity were caused by stroke. In ASEAN, stroke is the fourth leading andIndonesia is number one. South Kalimantan is the top four for high prevalence in Indonesia. Strokepatients require long-term care at their home and their quality of life will be good if they get theappropriate long-term care and support from caregivers. Quality of life of caregivers also need tobe considered. The purpose of study was to determine the supportive care needs of strokecaregivers when take care at home. A descriptive study used at neurological clinic, with 112caregivers, and using questionnaire. Data analysis used frequency distribution. The results showedthat the supportive care needs among stroke caregivers in the community is at a moderate level(100%, n = 112); with emotional domain (27.11 ± 8.47), psychosocial domain (26.84 ± 6.36),informational domain (23.74 ± 4.20), practice domain (16.3 ± 4.24), spiritual domain (6.43 ±1.30), and physical domain (3.06 ± 1.17). Nurses in the community need to improve the quality andthe health care system to help stroke caregivers in taking care for long term.
