Public health center as the operator of immunization operations must be supported by the implementation of a good health management function. The purpose of this study was to analyze the influence of facilitating factors, enabling factors, reinforcing factors and factorsthat influence mothers in carrying out complete basic immunization for children in relation to UCI achievement at Miroto Public Health Center, Central Semarang District, Semarang City. Methods: This study is analitic observational research with cross-sectional study design. The population of this research is mothers who have a minimum age of 1 under five who reside at Miroto Health Center. The research sample was 50 mothers with simple random sampling technique. Processing and data analysis with quantitative methods with univariate and bivariate analysis using the Chi Square test. Results: The results showed that there was an effect of mother's education level, mother's level of knowledge about completeness of basic immunization for infants, mother's employment status, number of children, family support, and effect on completeness of immunization.Meanwhile, family income does not affect the completeness of immunization. Conclusion: The achievement of UCI in the Public health center area of the Miroto Health Center is only 62% so that the follow-up reaches the target set at 95%.Variables that affect mothers in carrying out complete immunization are mother's education level, mother's level of knowledge, mother's employment status, number of children, family support, and what does not affect is family income.

Abstract

Background: Reporting cases of Dengue Hemorrhagic Fever (DHF) based on case definition the Ministry of Health of the Republic Indonesia is a strategy to increase the ability to predict positive cases. Our study aims to examine the implementation of case reporting so as to improve the predictive ability of positive cases. Methods: We conducted operational research at Kertha Usada General Hospital and Buleleng District Hospital. The sample is individuals diagnosed with DHF by a hospital having their address in Buleleng District and undergoing inpatient treatment at Kertha Usada General Hospital and Buleleng District Hospital in the first week of October 2021. We collected data through a review of medical record documents using a checklist.Result: We evaluated among 10 cases, 6 cases (60.0%) of whom were over 12 years old and 7 cases (70.0%) were men. Based on clinical indicators, among 10 cases, all cases (100%) actually had fever between 2-7 days, 5 cases (50%) really had headaches, 4 cases (40%) really had abdominal pain and 8 cases (80%) actually vomiting. Based on laboratory indicators, among all 10 cases (100%) there was a decrease in platelets <100,000 u/L and in 1 case (10%) there was an increase in hematocrit >52%.Conclusion : The positive predictive value obtained is 100% with very good positive predictive ability. Reporting cases based on the Ministry of Health of the Republic Indonesia case definition is expected to be carried out an ongoing basis because it can improve the ability to predict positive cases. Periodic training and technical guidance are required to equalize perceptions of case definitions so as to improve reporting quality.

Abstract

ABSTRACTBackground: Primary hypertension is a chronic disease with the potential for complications, death, and very high medical costs. It is estimated that cases will reach 1.5 billion in 2025 if no appropriate interventions are found to suppress its growth. Wet cupping has been shown to have an effect on reducing blood pressure in patients with hypertension, but clinical analysis that supports this fact, including measurement of changes in the scores of patients’ quality of life, is still lacking. Methods: This study used a quasi-experimental method. The sample is patients with primary hypertension who consume antihypertensives regularly and are 35-64 years old. Scores of respondents' quality of life were measured before and after the intervention using the SF36 questionnaire. Data were analyzed using t test and Wilcoxon-Mann-Whitney test. Results: It was found that the mean score of quality of life in the treatment group increased by 14.40 ± 19.16 with a p value of 0.009. On the other hand, the control group experienced a decrease of 0.465 ± 0.98 in score with a p value of 0.88.Conclusion: Wet cupping is effective in improving the quality of life of patients with primary hypertension.

Abstract

Background: Tuberculosis (Tb) is a global challenge for public health problems, including Indonesia. Kupang is the city in East Nusa Tenggara (NTT) that has the highest number of Tb cases with a Success Rate (SR) about 85% in 2017. However, in 2018 the SR rate decreased to about 81%. Adherence to taking medication can have an effect on increasing the SR rate. In NTT, had many cases of withdrawal problems caused by Tb patient not taking medication regularly. The Tb control Program in NTT is based on the Directly Observed Treatment Short Course (DOTS) Strategy that focuses on case finding and cure rates. The five components of DOTS are: 1) Political commitment 2) Case finding 3) Standard treatment 4) Drug management and availability systems 5) Monitoring, recording and reporting system. The purpose of this study is to analyze the implementation of DOTS strategy in Manutapen Public Health Center (Puskesmas)Methods: This study was a descriptive qualitative method with used in-depth interviews. The main informants were three health officer participants from the Puskesmas. Triangulation informants were one participant of the Disease Control and Prevention Program of the Kupang Health Officer, one PMO participant, and one Tb patient participant.Result: Tb control with DOTS strategy encountered obstacles including a lack of political commitment related to funding new Tb cases, the high workload made the Tb officers unable to monitor the patients, and unreported cases. Conclusion: Tuberculosis control program with the DOTS strategy in Manutapen Public Health Center kupang was not optimalized.

Background: Worldwide healthcare workforce represents 12%, in Uganda, out of the 39000 healthcare workers (HCW), 500 have been deployed at Mbale regional referral hospital (MRRH). The deficient quality service delivery is attributed to adverse occupational health risk that are not addressed and acknowledged posing a great risk to patient safety. Study aims to investigate the unsafe acts and unsafe working conditions of health care workers at (MRRH).Methods: A quantitative observational study with a cross-section design was used using and a semi structured observation checklist for data collection among 120 health care workers of MRRH.Result: 45(37.5%) of the HCW never wore the necessary PPEs versus 75(62.5%) who wore, almost half 55(45.3%) worked over time and 65(54.6%) did not, as well the same figures represent those who over worked and those who did not over work respectively. 70% of the working places had defective working conditions for example no lighting, slippery floors and no danger signs. 75% of the wards had improper waste segregation measures. Conclusion : HCW and institutions should intervene to mitigate the hazards, strengthening occupational health, patient safety, hazard prevention and control

Abstract

Background: Several regions in Indonesia are classified as leptospirosis endemic including Klaten Regency, which ranked 2nd highest case of leptospirosis in Central Java in 2018. Leptospirosis in human was transmitted by animals infected by the Leptospira bacteria, which rat was the main reservoir. This study aimed to identify rat reservoir to the existence of Leptospira bacteria in endemic areas. Methods: This study was an observational survey  with cross sectional approach. The sample was rats caught by traps installed for 6 days in Dukuh Kalitengah, Kalitengah Village, Wedi Subdistrict, Klaten Regency in July 2019. The rats were identified and dissected to take their kidneys for laboratory testing using the Polymerase Chain Reaction (PCR) test to identity the existence of Leptospira bacteria.Result: The result showed that there were 17 rats and 6 Suncus murinus caught in 6 days and their species were Mus musculus (23.5%), Rattus tanezumi (41.2%), Rattus norvegicus (35.3%). The results of PCR test found 2 positive samples of Leptospira bacteria in Rattus tanezumi. Rattus tanezumi's habitat was close with humans in settlement areas where it can be a source of Leptospira infection and can spread to humans and other environments. Conclusion : The people in Dukuh Kalitengah are recommended to maintain the hygiene of their environment and improve their house’s sanitation so it not become nest and breeding grounds and also reduce the existence of rats.
