Abstract

Abstract : Post partum blues is phenomenon which occurs at the first week of postpartum, the peak of symptoms occurs on third to seventh day of postpartum with duration several hours to several days. Postpartum blues are categorized as mild mental disorders thus it often ignored, undiagnosed, and finaly untreated, this situation will create uncomfortable feelings for women who suffer from it, thus the impact gets worse and continue to postpartum depression. Postpartum blues can be detected by EPDS screening. One of the techniques to prevent the occurance of postpartum blues is endorphin massage which is a technique of touch and light massage. The research objective was to find out the effect of endorphin massage on the improvement of exclusive breastfeeding and the decrease of EPDS on mothers who detected postpartum blues. The research was pre experimental with an approach of non randomized control group pre-post test or non equivalen control group design. The sample was determined by purposive sampling. The research was conducted at community health center around Mataram for 5 months (June-October) in 2016. The total sample was 20 mothers with postpartum blues who were treated, and 20 mothers were in the control group. Mothers who detected postpartum blues would be examined their exclusive breastfeeding production by assesse the indicator from infant’s factors used observational questionnaire before and after conducting treatment. The treatment was endorphin massage that given as many as 5 times by researceher since the third day to seventh day after labor. The second assessment of breastfeeding production was done immediately after the fourth treatment to determine the increasing of breastfeeding production after given endorphin massage. The collected data was analyzed by using statistical analysis with SPSS. The findings was there was a significance influence of endorphin massage on the improvement of breastfeeding (p=0.000) and the decrease of EPDS score (p=0.000). The conclusion is endorphine massage can be alternative theraphy which is effective in increasing breastfeeding production and postpartum management. The results of this study concluded, endorphin massage can be used as an effective alternative therapy in increasing milk production and postpartum blues management.

Abstract

School-age children is a golden period for embeding the values of healthy and hygienic-life behaviour (PHBS). The objective of this scientific writing was to provide an overview about the implementation of collaboration strategy between pupil and teacher in improving the behaviour of healthy and hygienic life in school-age children by utilizing the management approach of community nursing services, community nursing care, and family nursing care through the integration of community as a partner model, comprehensive school health model, family center nursing model, and management theory.  Participants were pupils of Madrasah Ibtidaiyah Terpadu (MIT) at Cisalak Pasar sub-village, Cimanggis District, Depok City. The result indicated there was a significant result to the improvement of knowledge, healthy behaviour skills, and the improvement of school role and the indepedence of family. This scientific paper is expected to be fundamental of health promotion programs for school-age children.

Abstract

Abstract: Consultation intervention using audio-visual media is one of the methods that can be carried out as an effort to enhance the society especially family (i.e. housewife) to be able to play the role as an innovator in her own household. Audio-visual is the best assistive tool nowadays. Along with the rapid development of technology, consultation using audio-visual media must also be developed at this moment. Objective of this research is to know the influence of consultations using audio-visual and conventional media on the knowledge of mothers having toddlers with non-rising weight in Penimbung Public Heath Center’s operational area, West Lombok Regency. The research was carried out in Penimbung Public Health Center’s operational area i.e. Dopang Village and Gelangsar Village in approximately 6 months, from June to November 2012. This research applied the true experimental design using pretest-posttest with control group design. The population number was 91 subjects and the samples were 60 subjects. The method of consultation used was group consultation, in which the first group (20 subjects) used audio-visual media, the second group (20 subjects) used conventional media and the third group (20 subjects) were not given any treatment at all. Data was processed using the Independent Sample t-Test statistical analysis to compare the means or average score of the knowledge of mothers among the groups, both prior to the consultation and after the consultation has been carried out. Subsequently, Paired Sample t-Test statistical analysis was used to compare the average result of the pretest with the posttest in the case group and the control group.The toddlers’ ages were between 9-57 months and half of them (50%) were first born. The mothers’ occupation were mostly (88%) housewives and some of them (38.3%) have finished junior high school/its equivalent. There was no difference in the knowledge average of mothers (posttest) after consultations using audio-visual and conventional media were conducted. There was a difference in the knowledge average of mothers (posttest) after audio-visual consultation was conducted in the control group. There was an influence seen in the increase of knowledge average of mothers when compared before and after the consultations have been conducted, both in the consultation groups using audio-visual and conventional media. The frequency of consultation conducted by the nutritionist in Penimbung Public Health Center should be increased to increase the knowledge of mothers, both by using conventional and audio-visual media. Alternative consultation media should created. One of the alternatives created is audio-visual media with an enhanced format to suit the condition of the local area because the audio-visual media that is used during this research is still simple.

Abstract

Abstract: The delivery process through action section caesaria cause difficulties mothers initiate breastfeeding early because of rooming –in, weakness and abdominal incision in the mothers condition. Besides the feeling of mother who does not believe can provide care to the baby because the condition will cause a decrease in the hormone oxytocin that breast milk can not exit immediately after delivery. This can adversely affect a babys life because it lost the opportunity to obtain the highest nutrition early in life that colostrum. Proper lactation management can be applied if the nurse as care giver is able to understand the technique stimulates oxytocin expenditure. Based on the results of research there are several alternative methods that can be done to stimulate the hormne oxytocin t increase milk production after section caesaria. So that mothers can still provide breast milk to their babies in quality  among them is oxytocin massage methods, techniques marmet, warm compresses, massase rolling, breast care and SPEOS methods.

Abstract

a labor pain can affect maternal conditions for instance fatigue, fear, anxiety or worry and cause stress that may cause the interference on uterus contractions and influence the length of labor. One of nonpharmacologic methods that can be used to relievea labor pain is a breathing relaxation and massage techniques. The research aimed to analyze the influence of combination between breathing relaxations and massage techniques for mothers against pain intensity, duration of labor and APGAR scores for newborn at the Public Health Center of Masbagik East Lombok. This research used Quasi-Experimental with pretest-posttest control group design. The sample applied 30 mothers who were in the first stage of labor that meet the inclusion criteria. The data were analyzed by Wilcoxon Sign-Rank Test, Mann-Whitney test and t two independent samples. The result indicated the comparison of pain intensity in the the pretest and posttest of control group was p = 0.051>0.05 which means there is no difference on pain intensity before and after treatment in the control group. Although the comparison of pain intensity on the group that received the combination techniques is obtained either pretest or posttest was p=0.001<0.05 which means there is a difference which signed by the reduction of pain intensity before and after the treatment by 2.93. Besides, there was a correlation between the implementation of the technique against duration of labor by cervical opening as indicator (p = 0.000). APGAR scores on newborn on the group that received the technique of relaxation and massage was 7-10 (normal). In short, the implementation of the technique can be used as an integral part of providing basic help for mothers to relieve labor pain and to accelerate delivery process.

Abstract

Abstract: Patients in critical care unit inserted by ventilators could potentially develop complications as known by Ventilator Associated Pneumonia (VAP). The preventions of VAP include non-pharmacological and pharmacological intervention. Pharmacological prevention of  VAP is the use of antiseptic drug in oral hygiene procedure. Kinds of anticeptic drugs are hexadol (hexetidine) and chlorhexidine. The purpose of this study was to evaluate the effects of oral hygiene with hexadol and chlorhexidine in patients with mechanical ventilator in ICU. The study was a quasi-experiemntal two group design. There were 30 subjects recruited by purposive sampling in the ICU at Pantiwilasa hospitals on street Dr. Cipto and Citarum Semarang. The subjects were divided randomly into 2 groups: 15 subjects given by hexadol 0,1% and 15 subjects given by chlorhexidine 0,2% for twice a day. VAP was assessed by Clinical Pulmonary Infection Score(CPIS).                    The results show that CPIS ≤ 5 was no VAP at 96,67% and ≥ 6 was VAP at 3,33%. There was no significant difference between oral hygiene with hexadol and chlorhexidine (U=78; p<0,14). Oral hygine with hexadol twice a day was as effective as oral hygiene with chlorhexidine to prevent VAP.

Abstract

Abstract: Pregnancy is the natural and normal process. Pregnant women get common changes that occur both physically and physiologically. Those changes cause them discomfortort and one of them is back pain. It can be solved by non-pharmacological therapy which is done by doing some activities such as distraction technique, self hypnosis, reducing the pain perception, massaging treatments, relaxing in a hot bath and heat or cold therapy. This research intended to investigate the effect between Endorphin Massage and warm compress therapy to reduce the back pain during the third trimester of pregnancy. This research was a quasi experimental research with two group pre test and post test design in which this experiment was done in two different groups. The sample utilized pusposive sampling technique and conducted at all public health center or puskesmas in Mataram during July to October 2017. The sample consisted of 30 pregnant mothers with third trimester of pregnancy who detected with the back pain through Numerical Rating Scale (NRS). This scale was done before and after they got Endorphine Massage and warm compress for 20 minutes in five days. Afterward, the data was analyzed by using test statistic of independent t test. The result found that mean score before and after the Endorphin Massage was 1.933 while the mean score before and after warm compress was 0.733. Those results indicated that Endorphin Massage was more effective than to reduce the back pain during the third trimester of pregnancy. Endorphine Massage was also used as the alternatif therapy of non-pharmacological to solve the back pain. Therefore all the midwives can apply Endorphine Massage through the midwifery service to the pregnant mothers in which it can help them to solve the back pain problems during the third trimester of pregnancy.

Abstract

Abstract: One of the causes of expectant mother and fetus’ morbidity and mortality is severe preeclampsia and asphyxia in newborns. Asphyxia in newborns is the first cause of infant death in Indonesia, and the second cause of infant death after Low Weight Birth in West Nusa Tenggara province. This research was conducted in the delivery room of RSUDP NTB which aimed to determine the effect of age, another complication and types of birth with newborn asphyxia to mother who suffers severe preeclampsia. The collection of secondary data is obtained by searching delivery room registration and patients’ medical record at RSUDP NTB. The research design used in this study was observational analytic with cross sectional design, sample in this study  were all birth mothers with severe preeclampsia who delivered  alive single newborn, and  were recorded at delivery room registration and medical records of RSUDP NTB in 2013 and 2014  was 111 cases. The analysis was logistic regression. Based on the results of the study, it can be percieved that 111 expectant mothers with severe preeclampsia, the most types of deliveries were cesarean section (SC) as many as 55.9%, and the babies who were born do not suffer asphyxia (73%). Variables which connected and affected to the incidence of asphyxia is the kind of labor with the asphyxia newborn on severe preeclamsia mothers at RSUDP NTB years 2013-2014 with p value = 0.003 and OR = 4.623

Abstract

Abstract: Background: Traditional medicine herb ingredient or ingredients are derived from growing-plants, animals, and minerals, galenik preparations or mixtures of these materials that are passed down through generations has been used for the treatment of. Sugar Palm roots (Arenga pinnata (Wurmb.) Merr.) nutritious as deciduous deciduous urine and menstruation. The purpose of this research is to know the diuretic activity of root extract of white rat strain aren wistar (Rattus norvegicus), and knowing the optimal levels of root extract of Palm that has a diuretic activity of white rats wistar strain. As many as 25 test animal's tail is divided into 5 groups: control group i.e treatment of negative (aquadest), a positive control group (furosemid suspension), group root extract aren 100 mg/kg, group root extract aren 200 mg/kg and groups of Palm root extract 300 mg/kg. Diuretic effect of testing done by looking at the volume of urine is issued for 18 hours. The results showed issuing Palm root extract during the time of observation going on increased volume of urine. Data were analyzed with spss 16, unlike any real treatment tested by one way ANOVA. Increased concentrations of root extract aren showed better results. Based on these results, it was concluded that root extract has a diuretic effect of areca palm (p < 0.05) and optimal levels of root extract of Palm that has a diuretic activity of white rats wistar strain is 300 mg/kg.

Abstract

Abstract : the adherence of physical exercising plays important role in therapeutic management of diabetic patients. The research objective was to identify the factor which influences the adherence of diabetic patiets to exercise. The research was Quantitative research with cross sectional approach. Total sample was 122 respondents. Modifier factor data, an individual perception, and action used questionnaire. Data analysis utilized Chi Square and multiple logistic regressions. The result indicated factor which influenced the adherence of diabetic patiets to exercise was gender (p = 0026), a knowledge (p = 0.013). Benefit perceptions (p = 0.008), obstacle perceptions (p = 0.002) and family supports (p = 0.00). The most dominant factor was family supports (OR = 10.047). Therefore, it is expected that health services develop family management to improve the patients’ adherence.
