Abstract

Breast Cancer would affect all aspects of patients’ life including spiritually. This aspect is one of the patients’ main coping sources in dealing with impacts of the disease. However, the spiritual aspect is rarely noticed by nurses, and may lead to patients’ spiritual distress. This study aimed to determine spiritual distress in breast cancer patients who had treatments in a referral hospital in West Java. This study was a descriptive quantitative with cross-sectional approach. The population was in-patient breast cancer patients. Respondents were chosen using the accidental sampling technique. During a month period, 37 patients involved in this study. The data was collected using an instrument which was developed from Spiritual Distress Assessment Tool (SDAT) and analyzed using descriptive quantitative analysis.  Almost two-thirds (62.2%) of respondents had low spiritual distress, 32.2% of respondents had moderate spiritual distress and a very small number of respondents (5.4%) had severe spiritual distress. In the term of spiritual distress dimension, “life balance” had the highest mean score (1.86) and the lowest was in “values and beliefs with sub-dimension of need maintain control” (0.89). The conclusion was the majority of patients had low spiritual distress, yet spiritual distress for moderate to severe still could be found in smaller proportion. Life balance was dimension with the highest level of spiritual distress.  The patients’ spiritual need would be achieved when patients have a good acceptance and adaptation. Nurses have a role to encourage patients’ acceptance of their illnesses through improvement of the fulfillment of the patient's spiritual needs.

Abstract

Diet is an important component in the treatment of tuberculosis (TB). The diet is a daily nutrition fulfillment. The nutrition means macronutrient and micronutrient. The lack of macronutrient and micronutrient intake in tuberculosis patients could be a high risk for patients. Lack of nutrition can be addressed from the low level of BMI in TB patients. Both malnutrition and TB influence each other, malnutrition increases the risk for TB and TB could cause malnutrition. The aim of this study was to describe the diet of tuberculosis patients in the intensive phase. The study was conducted in two primary health cares in Bandung city. This study was a descriptive-quantitative research that used consecutive sampling method. The samples were 36 TB patients in the intensive phase.  The data was gathered using 3 days food record and analyzed using the descriptive statistics method. The study’s result indicated that 86,1% of TB patients consumed calories less than their need, 80,6% of TB patients consumed carbohydrates meet the needs, 83,3% of TB patients consumed protein less than necessity,  55,6% of  TB patients consumed fat meet their needs, and 100% of TB patients consumed vitamin D less their need. The study results showed that the diet of TB patients was less than their need. This study concludes that in general, the diet in TB patients is still less than their need, especially protein and vitamin D. While carbohydrates and fat met the patients’need. Based on this result, the researcher recommends that the primary health care in Bandung city should provide counseling services about various food types that need to be consumed by TB patients to fulfill calory, protein, and vitamin D daily needs.

Abstract

Cleft lip and palate are congenital abnormalities that occur in children. Success in the management and treatment of this disorder requires the role of parents. However, often parents are embarrassed and have low self-esteem, so it would interfere with the success process of treatment and child care with clefts. This study aimed to determine the parents’ self-esteem of children with cleft lip and palate in the Yayasan Pembina Penderita Celah Bibir dan Langit-langit  (YPPCBL) Bandung. This research was a descriptive quantitative study. The study population was parents of children with cleft lip and palate. A total of 30 respondents participated in this study which was chosen with total sampling method. Parental self-esteem was measured using modified instruments based on Coopersmith's theory, with validity values of r = 0.977 and Cronbach's α = 0.989. Data were analyzed using frequency distribution. The results showed 18 respondents (60%) had high self esteem and 12 respondents (40%) with low self esteem. This research would be an evaluation material for YPPCBL Bandung in maintaining and improving parents' self-esteem of children having clefts. With high self-esteem, it is potential for nurses to increase parental participation as a primary caregiver of children with clefts, it would also improve the success of care and children’s quality of life.

Abstract

Background Postpartum haemorrhage is identified as one of main causes of maternal mortality especially in low income countries including Indonesia. The use of rectal misoprostol is a considerable intervention to prevent postpartum haemorrhage. However, administering misoprostol would cause various considerable responses that need intensive observation to prevent high risk condition of post-partum women. Limited studies in Indonesia assessed responses of post-partum women’s after misoprostol administration. This study aimed to identify physical responses of post-partum women after misoprostol administration. Methods, This descriptive quantitative study involved 30 post-partum women. They were chosen using consecutive sampling technique. The data collection instruments were an observation form and a semi-structured interview form. Using those instruments women’s physical responses observed on the minute of 15, 30, 45, 60, 90 and the hour of 2, 3, and 4 after post- partum. The data were analysed using frequency distribution. Results The study found that there were different physical responses of women after rectal misoprostol administration including nausea, vomiting, diarrhoea, headache and fever, especially in the minute of 15 and the hour of 2. The most prominent physical responses were nausea, and followed by headache and fever. Conclusion This study finding is a significant information for health professionals especially nurses. Nurses could develop an anticipatory guide in anticipating any effects that may risk to postpartum patients.

Abstract

Introduction, the success of treatment in chronic renal failure patients with hemodialysis required the existence of self-management and self-efficacy. Previous studies showed that self-efficacy had the influence to bring up self-management behavior. The purpose of this study was to determine patients’ self-management and self-efficacy in chronic renal failure patients who had hemodialysis at Advent’s Hospital Bandung. Methods, this study was a quantitative descriptive research.  The study samples were chosen using consecutive sampling technique. The number of samples was 75 patients. Data were collected using the SMBQ and PEPPI+Life Options DeNovo instruments. Data were analyzed using the quantitative descriptive approach with mean value and frequency distribution. The results of self-efficacy in patients with chronic renal failure with hemodialysis was   high (54.7%) and the self-management behavior was in a good category (53.3%). The conclusion of this study is that the patient has a good self-management as well as self-efficacy.  However, this study found the poor category of the partnership in caring aspect. Recommendation There is a need for a strategy to improve the self-management in the partnership in caring aspect by providing emotional support, educating patients related to treatment and care, and evaluating patients’ health development periodically.

Abstract

An intestinal stoma is an opening of the intestinal tract into the abdominal wall for patients with feces production problems. The process might change patients’ physical, psychological, social, and spiritual. Patients have limitations in daily activities, changes in elimination, depression, feeling hopeless, ashamed with the colostomy bag, and may also affect to their religious and spiritual. For example their religious rituals and belief in God. The aim of this study was to identify the spiritual need of patients with a colostomy in a public hospital in Bandung. This was a descriptive quantitative study, The population was patients with a colostomy who hospitalized in a public hospital in Bandung. The samples were chosen using the accidental sampling (n=42) technique. The study instrument was Spiritual Needs Questionaire (SpNQ), that consists of 19 questions with 4 dimensions (religious needs, inner peace, existential, and actively giving). Data were analyzed using statistics descriptive and presented in a table of distribution frequency. The study found that patients with a colostomy had the spiritual need for all dimension including the religious dimension (88,1%), inner peace (57,1%), existential (54,8%), and actively giving (52,4%). Patients with a permanent colostomy had higher scores on the spiritual needs than those temporary colostomy patients. It can be concluded that the religious dimension is the highest relevance of the spiritual needs. According to this study results, it is expected that nurses can improve the spiritual services in nursing care including assessing patients’ spiritual needs and facilitating patients to fulfill their spiritual needs. The facilitation includes providing an opportunity for praying together, spiritual books, helping patients for ritual worship, establishing therapeutic communication, giving patients their own time, and creating a quiet place.

Abstract

Diabetes Mellitus (DM) is a chronical community health problem, and mostly suffered by the elderly. The elderly with DM have several symptoms at night that caused a sleep disorder and it may be affected by the poor quality of sleep.  The poor quality of sleep has several negative impacts to elderly including sleepy in the daylight, a lack of attention, memory disorders, depression, easy to fall, and reduce of the quality of life. There is a need for a study that assesses the sleep quality of elderly with DM. This study used descriptive quantitative approach. The samples were chosen using total sampling technique. This study involved 47 elderly with DM in the primary health center (PHC) Garuda. The data were collected using Pittsburgh Sleep Quality Index (PSQI) Questionnaire that has been adapted for elders. The data were analyzed using distribution frequency. The results showed that the majority of elderly had a poor sleep quality (51%). All elders in this study had sleep disorders, and some of them had behavior that affected   their sleep quality, such as sleeping in a bright room (n=25), sleeping in low temperature room (n=27), and consuming caffeine (n=28). Nurses in the community setting are expected to provide health education to maintenance a good sleep quality, such as the importance of sleeping in the dark and warm temperature room, and avoiding to consume caffeine.

Abstract

Introduction, early detection of cancer is an important action in addressing the high incidence breast cancer cases. The easiest early detection that women could do is breast self-examination(BSE). Previous research indicated a gap between knowledge and behavior towards breast self-examination. Intrinsic motivation is needed by women to perform breast self-examination. This study aimed to identify the intrinsic motivation of women reproductive age in performing breast self-examination in Kebonwaru District Bandung. Methods, this study was a descriptive quantitative study, the study population was women in reproductive ages. The samples were chosen using the simple random technique (n = 344). Data was collected using Intrinsic Motivation Inventory instrument which has been back-translated. Data were analyzed using descriptive analysis of frequency distribution and percentage. The results showed that the proportion of respondents who had the high intrinsic motivation and low intrinsic motivation were almost the same (54.1% and 45.9% respectively). The majority of respondents (84% of) had a high perception of importance and usability of BSE. 66.3% of respondents had a perception of suitability and interest along with high competence perception. In addition, 56.4% of respondents had a high perception of effort and choice. However, 57.4% of respondents reported an increased sense of stress and nervous when performing BSE. Conclusion, there is a need to maximize cancer prevention and promotion programs.  For example, health education related to cancer prevention and treatment,  accompaniment, and counseling, and providing information about the importance of early detection.  That information could increase the intrinsic motivation of women to perform breast self-examination so a number of women’s mortality caused by breast cancer could be decreased and breast cancers could be detected earlier.

Abstract

In Indonesia, thalassemia is a major disease which increases its number every year.  There were 7,238 patients of thalassemia major in Indonesia in 2016. The highest number was from West Java, 3,039 patients. Thalassemia is preventable. The increasing number of Thalasemia patients impacted the bigger cost of supportive treatment such as blood transfusion, iron chelation for a lifetime, and the cost of treatment when complications occur. In addition, patient and family also should deal with the psychology burdens. This study was to identify parents’ awareness of thalassemia towards thalassemia prevention in a public hospital in Bandung. This study applied quantitative descriptive method. It involved 99 parents of thalassemia children. The samples were chosen using the accidental sampling technique. Data were analyzed using descriptive statistic. The data were collected using a modification of Thalassemia Awareness Quesionnaire (TAQ). The results showed that the majority of parents 53 (53.4%) had low awareness of thalassemia, and 47 (46.5%) of parents had high awareness. Similarly, the majority of parents had low awareness of thalassemia prevention 58 (58,5%), and only 42 (41,4%) of parents had a high category of awareness. Developing of strategy or program to improve the awareness of parents with thalassemia towards thalassemia prevention is needed. Nurses have a role as a provider and educator of thalassemia health information and prevention.

Abstract

The quality of sleep is required by breast cancer patients to regenerate and repair the body cells. When patients’ sleep is disturbed, it may be affected to the physiological conditions, such as decreased appetite, weight loss, anxiety, irritability, and changes in natural and cellular immune functions. Breast cancer therapies can affect the quality of sleep. This study aimed to determine the sleep quality of breast cancer patients that had treatments in a public hospital in Bandung. This research used the quantitative descriptive approach. The sample was selected using the consecutive sampling technique (n = 31). Data were collected using the Pittsburgh Sleep Quality Index (PSQI) questionnaire. Sleep quality was categorized as good when the total score was ≤ 5, and sleep quality was poor when the total score was > 5. The results of this study indicated that all respondents (100%) had poor sleep quality. The components that contributed to the assessment were sleep latency, sleep duration, sleep efficiency, and daytime dysfunction. The worst sleep quality with the highest score (18) occurred in respondents who had radiotherapy and mastectomy (2 persons), and respondents in advanced stage (3 persons). This study concludes that the pharmacological therapies and the stages of cancer contributed to the sleep quality of breast cancer patients. It is expected that health professionals and hospitals notice the patient’s sleeping needs during treatment at the hospital.
