Abstract

Gagal ginjal kronik adalah salah satu penyakit kronik menyebabkan terjadinya penurunan kualitas hidup dan memicu kecemasan yang dihasilkan dari adanya pemikiran negatif termasuk pemikiran akan kematian. Pemikiran kematian yang berlebihan menimbulkan stres tersendiri bahkan menyebabkan depresi sehingga berdampak pada perencanaan perawatan, persiapan menghadapi kematian dan sulitnya mencapai kualitas kematian yang bermartabat. Penelitian ini bertujuan untuk mengetahui gambaran kecemasan kematian pada pasien gagal ginjal kronik yang menjalani hemodialisis. Penelitian deskriptif kuantitatif ini melibatkan 73 pasien gagal ginjal kronik yang menjalani hemodialisis diambil dengan teknik consecutive sampling. Pengumpulan data menggunakan instrumen Death Anxiety Scale (DAS) Templer  yang diadaptasi untuk pasien hemodialisis. Data dianalisis dengan statistik deskriptif (mean, frekuensi, dan persentasi). Hasil penelitian menunjukkan hampir separuh dari jumlah pasien gagal ginjal kronik yang menjalani hemodialisis mengalami kecemasan kematian tingkat sedang (n=35), 27.4% responden (n=20) mengalami kecemasan rendah, dan 24.7% responden (n=18) mengalami kecemasan kematian tinggi. Berdasarkan hasil tersebut perlu dilakukan pengelolaan terhadap kecemasan kematian pasien gagal ginjal kronik yang menjalani hemodialisis. Upaya yang dapat ditempuh diantaranya dengan terapi spiritualitas, konseling kelompok, terapi kognitif (penguatan persepsi positif), dan pemberian pendidikan kesehatan terkait dengan prosedur hemodialisis dan penyakit gagal ginjal kronik.

Abstract

Tuberculosis is a contagious disease that remains a health problem worldwide. Treatment of tuberculosis is one of the main strategies in controlling tuberculosis because it can break the transmission chain, especially in families of household contacts where transmission is twice as risky as casual or unfamiliar contact. The purpose of this study was to analyze secondary data on patient adherence to pulmonary tuberculosis treatment sessions and transmission to household contact families in 2016-2017 at a private hospital in Indonesia. This research used a correlative descriptive design with Secondary Data Analysis (ADS) approach using existing data as primary data that is TB 06 and TB 09 data, TB 01 (medical card TB PKM). The population  in this research was based on the report of patient data of TB DOTS year 2016-2017 with 60 TB patients. The statistical analysis used was Pearson’s r. The result of the research showed that the patient’s adherence of pulmonary tuberculosis to treatment sessions. [D1] The value of adherence was seen from all respondents of tuberculosis patients which coming to the referral clinic according to the TB 06, and TB 09. As well as the patient’s record card at the referral clinic showed all the respondents routinely took the anti tuberculosis medication and completed the  treatment as recommended by the doctor. Investigation based on 60 respondents, there is 23.3% or 14 people diagnosed based on sputums, while remaining 46 people or 76.7% [D2] based on radiology results. Identification of household contact families members found positive sputum smear examination of 5 people (8.3%) and found to be undergoing tuberculosis treatment as many as 4 people (6.7%).The results of this study found a relationship between tuberculosis patient adherence to treatment sessions and transmission to household contact family with P value 0.001 (<0.05) with a moderate correlation (r) = 0.378%.  For further [D3] treatment, the nurse includes TB 06 and TB 09 sheets for follow up treatment at the public health service center. [D1]Edit the sentence [D2]Please use the correct sentence to explains this data [D3]Are tb patients hospitalized? Abstract

The high mortality rate in Emergency Department (ED) is consistent with the high demand for end of life (EOL) care. However, ED work system required to life saving quickly and precisely often makes EOL care is not working optimally.The purpose of this study was to determine nurses’perceptions in emergency departments related to obstacles and supportive behaviors (SB) in providing EOL care to dying patients. Descriptive-quantitative method was used with cross-sectional approach in 50 nurses Emergency Department at one of the tertiary hospitals in Indonesia. The samples were chosen using the total sampling technique. Data collection used the emergency nurse perceptions of end of life care questionnaire. Data were analyzed using univariate analysis in the form of distribution, frequency, and mean. The result of the study showed that the main obstacles in EOL care were:  ED nurses’ work loads (10.626), intra-family disagreements (9,802), and dealing with angry family members (9.472). The three main perceptions in supportive behaviors were teaching families how to act (11.962), allowing family members adequate time with the patient after patient died (11.09), and good collaboration from paramedics personnel (10.886. It can be conclude that nurses facing obstacles in taking care end-of-life patients. Based on this research, further studies are needed to minimize obstacles and improve supportive behaviors related to the nurse's workload, the provision of spiritual teams, special training for nurse and the provision of special rooms for EOL patients and their families.

Abstract

Palliative care is one of the practice areas that is perceived difficult by nurses and nursing students. Students tend to avoid seeing patients with the deteriorating condition. This research aimed to identify the self-efficacy of internship nursing students in facing palliative patients within communication, management of the patient, and multidisciplinary teamwork components. This study was a descriptive quantitative. Samples were recruited from a population of internship nursing students at Faculty of Nursing Universitas Padjadjaran using a total sampling technique (n=159). Data were collected using Self-Efficacy Palliative Care scale (SEPC). Data were analyzed using mean scores calculated from the total scores of each respondent. The results showed that more than half of the respondents had a low level of self-efficacy. In terms of its components, students’ self-efficacy in communication,  multidisciplinary team collaboration, and patient management had a mean score of 38.24 (SD 10.35), 39.21 (SD 9.44), and 48.18 (SD 9.56) respectively. Based on the results, it is recommended to develop an active learning strategy for internship nursing students, especially in the aspects of communication, patient management, and multidisciplinary team collaboration.

Abstract

Nurses who work in intensive care unit and emergency rooms have excessive workload, and they are required to carry out nursing interventions appropriately and quickly. The heavy workload has an impact on the nurse's mentality, however limited information about the nurse's mentality related to workload. The aim of the study was to identify the mental workload of nurses in intensive care and emergency rooms. Method, this research is quantitative descriptive study with total sampling technique, 63 nurses who worked in ICU, PICU / NICU, and IGD of RSUD Kota Bandung involved in this study. The research instrument was NASA-TLX (National Aeronautics and Space Administration-Task Load Index). Data were analyed using descriptive analysis in the form of frequency distribution and mean. The results showed that nurses in the emergency room (42.9%) had high mental workload, and the moderate mental workload identified in nurses who works the PICU / NICU and ICU rooms. Conclusion, critical care rooms and emergency room should consider nurses’ working balance and fulfill their rest needs. Further research is needed to examine the efforts that can be made to reduce nurses' mental workload.

Abstract

Trauma is a circumstance in which a person experiences or witnesses death, feels threatened or seriously injured or a threat of physical integrity to oneself or others. Children and adolescents often had violence (harassment, neglect, embezzlement, or kidnapping) that can bring into trauma. The traumatic occurrence can have a serious psychological impact on children. To deal with this psychological hazard could be assisted by Cognitive Behavioral Therapy. This literature review aimed to identify the effect of CBT in supervising trauma in children and adolescents who became victims of violence to reduce the impact of trauma. Articles collected by Google Scholar, Science Direct and Pubmed Health used some keywords such as children, adolescent, CBT, trauma, and violence. The inclusion criteria consisted of quantitative research, systematic review, original research, full text, and published in English during 2007-2017. It identified 6 articles with 5 articles that were specifically related to CBT to supervise trauma. The results of the review of the five articles noted that 4 articles indicated that CBT is very effective in dealing with trauma in children and adolescent as the victims of violence, while 1 article showed that CBT is less effective in dealing with violence in children and adolescents. Thus, CBT was a potential intervention in addressing trauma in children and adolescent who became victims of violence, especially in West Java.

Abstract

In this technological era, various health technology including media, instruments and tools are created in area maternal and babies’ health. However, comprehensive information about these technology products are limited. This literature review aimed to find health technology in area maternal and baby especially in detecting high risk conditions in pregnancy. Method, this is a systematic review, the articles were searched from two databases including PubMed and CINAHL using several keywords. The keywords were high risk, pregnant women and technology OR instrument OR tools. Inclusion criteria are primary research, English language, and publications in the last 5 years. After the critical appraisal, a total of 16 articles included in the analysis. Result: The literature related of technology divided into two criteria which are the questionnaire development and health tools or devices. None of technology came from Indonesia. The entire technology showed effectiveness in detecting risks, and interventions to improve prenatal care. The target user for health tools mostly health professionals. Conclusion: various health technologies in area maternal in baby health are identified, however the majority of them provided from health professionals. Technology development is needed specifically, in the form of simple and practical tools or devices to assess foetal health with target user is pregnant women with high risk pregnancy.

Abstract

Oxygen hyperbaric therapy is a clinical treatment by inhalation of pure oxygen (FiO2 = 100%), resulting in increasing oxygen levels in the blood and tissues. Currently, hyperbaric oxygen therapy is used for additional management in diabetes mellitus. The purpose of this study was to know the effect of hyperbaric oxygen therapy in case of diabetes mellitus to changes in blood glucose level. Type of research was pre-experimental one group pre-test post design. The population in this study was diabetes mellitus patients, so that the sample obtained 47 respondents. Patients were given 100% oxygen inhalation with a pressure of 2.4 ATA in chamber hyperbaric oxygen. Blood glucose levels were measured before and after hyperbaric oxygen therapy. Glucose levels before the average therapy were 229 mg / dl and after treatment averaged 159 mg / dL. Statistical analysis with t test showed p value 0.000 <α (0,05), so there was the influence of hyperbaric oxygen therapy with decreasing blood glucose level. There are improvements pancreatic beta cell function and increased tissue sensitivity to insulin due to increased oxidative phosphorylation process in pancreatic beta-cell mitochondria to increase ATP production in pancreatic beta cells.   Checks need to be done periodically in one period with using the time series method, so that long-term effects can be known and need to be reviewed by the last use of anti-diabetes drugs and insulin

Abstract

AbstrakDosen bertanggung jawab untuk melaksanakan Tridharma Perguruan Tinggi dengan beban kerja 12 SKS sampai 16 SKS setiap semester. Namun, banyak dosen yang merasa bahwa beban kerjanya sangat berlebih. Tujuan penelitian ini adalah untuk mengetahui gambaran beban kerja mental pada dosen Fakultas X Universitas X. Metode penelitian ini adalah deskriptif kuantitatif dengan menggunakan teknik total sampling yang melibatkan 43 dosen Fakultas X Universitas X. Data dikumpulkan menggunakan instrumen NASA-TLX. Analisa data menggunakan statistik deskriptif. Hasil penelitian menunjukkan bahwa sebanyak 74,4% responden memiliki beban kerja mental yang tinggi pada tugas pendidikan dan pengajaran, sebanyak 76,7% responden memiliki beban kerja mental yang tinggi pada tugas penelitian, dan sebanyak 74,4% responden memiliki beban kerja mental yang tinggi pada tugas  pengabdian kepada masyarakat. Dimensi Effort memiliki nilai mean tertinggi yaitu sebesar 51,8, sedangkan dimensi yang memiliki mean terendah adalah dimensi Perfomance yaitu 9,4. Simpulan dari penelitian ini menunjukkan hasil sebagian besar dosen memiliki beban kerja mental yang tinggi. berdasarkan hasil penelitian ini, disarankan dosen Fakultas X Universitas X untuk menyeimbangkan jumlah tugas sesuai dengan kemampuannya, menyeimbangkan waktu bekerja dengan rekreasi, dan memenuhi kebutuhan istirahat. Abstract

The needs of nursing care for Myocardial Infarction (MI) patients who performing Cardiac Rehabilitation (CR) Program have been investigated previously. Despite, nutritional counseling and risk factors management were reported as the most needs of MI patients who performed CR Program, information related to the MI patients’ needs on nutritional counseling and risk factor management are very limited. Moreover, the relationship between patients’ needs on nutritional counseling and risk factor management are not clear yet. To investigate patients’ needs on nutritional counseling and risk factor management among MI patients and its relationships those factors. This was quantitative approach with descriptive correlational and convenience sampling was used for this study. The participants included 38 MI patients admitted to the Cardiac Rehabilitation Unit. Data collection employed from questionnaires that were filled in by the respondents using a retrospective technique at the end of cardiac rehabilitation program.  Data were analyzed using descriptive and correlational statistics. The study signified that MI patients required cardiac rehabilitation towards risk factors management and nutritional counseling from nurses (92.1%).The finding also indicated that there is a statistically positive relationship between nutritional counseling needs and risk factor management (r = 0.503, p <0.01). Patients’ needs on nutritional counseling and risk factor management among MI patients are correlated positively. Therefore, the information regarding nutrition and risk factor management for MI patients should be provided simultaneously to complement each other.
