Abstract

The background of this study is a return of INACBG's claim rate files inpatient at the National Cancer Centre Dharmais Hospital was still high, where the highest return was due to the confirmation of medical coding and resume. The potential to cause losses for the hospital as a result of delayed claim payments. This study aimed to analyse the completeness of the medical resume which includes the accuracy of the components of the diagnosis, procedure and coding of the INA-CBG's inpatient claim rate at the National Cancer Centre Dharmais Hospital. In this study, we proposed a qualitative approach was used with in-depth interview techniques and a review of the medical resumes of breast cancer patients who received chemotherapy during June-July 2020. From the results, we obtain that the highest incompleteness in filling out medical resumes was related to filling in the indication for admission to care with a percentage of 68, 33%, physical examination 33.33%, and supporting examination 6.67% of the total 60 cases studied. The incompatibility rate of writing a secondary diagnosis was 30% and the procedure/action was 28.33%. Writing the main diagnosis between the medical record and medical resume was known to be in accordance. The main diagnosis coding inaccuracy was still found, with a percentage of 13.33%. The result of the inaccuracy of the main diagnosis coding was a secondary diagnosis mismatch and a procedure / action incompatibility, which resulted in a claim difference of IDR 133,763,800. Therefore, the commitment of the management of the Dharmais Cancer Hospital, namely the team involved in the final coding which was the hospital's internal team should be further strengthened in order to improve the quality of the claim file from the aspects of completeness and accuracy of diagnosis, procedure and coding so that the accurate INA-CBG claim value can be obtained.

Abstract

KAMALA Clinic is the main clinic which is a network of private clinics that provide fertility and gynecology special health services. KAMALA is under PT. Medika Sehat Utama and have committed to developing fertility and gynaecology clinics as well as offering an In Vitro Fertilization (IVF) program with a guarantee (self-insurance). However, the operation in this clinic requires risk management to protect the clinic from a significant level of risk which is hinder the achievement of Kamala's clinical vision. In achieving optimal service to patients, this clinic carries out of risk identification, risk analysis, and risk evaluation. In order to prevent and minimize the impact of risks on every part of the clinic, the risk management is used on ISO 31000 in 2018. The stages of the ISO 31000 in 2018 process, include communication and consultation, setting context, risk assessment consisting of risk identification, risk analysis and risk evaluation, risk mitigation, monitoring and review as well as recording and reporting. Meanwhile, the risk criteria used in the risk assessment are low, moderate, high and extreme/catastrophic risk. Each risk is managed by the risk owner by conducting a risk register, then assessing the likelihood of risk occurring and assessing the risk impact based on risk criteria. the results of the risk management assessment are carried out as supporting material in decision making in mitigating the impact of risks that occur so that management can improve the effectiveness of risk management implementation.

Abstract

Health financing often becomes an obstacle to accessibility of health services by the community. This situation especially occurs in situations where health financing must be borne by yourself (out of pocket) in the cash health service payment system (fee for service). Hypertension is a chronic disease that requires regular treatment and medication in daily basis. This causes a very high need of health services for hypertension patients. This study was a descriptive analysis using qualitative methods aimed to analyze unit costs of hypertension outpatients at Harapan Keluarga Hospital using the relative value unit (RVU) method. A total of 908 samples of hypertensive patients such as 292 (who used amlodipine and nifedipine), 64 (who used canderin, Abvask, and hyperil), 69 (who used concor and Abvask), 60 (who used hyperil, CPG, and Myonap), 67 (who used Abvask, CPG, and nifedipine), 55 (who used Abvask), 280 (who used amlodipine), 20 (who used nonprescription control). By this study, we can conclude that with a quality information system, top management can be helped in making the right decisions.

Abstract

Numerical solutions using the Finite Element Method (FEM) of one-dimensional bio-heat transfer of healthy and cancerous tissue together with the Magnetic Fluid Hyperthermia treatment have been worked out successfully. Each layer shows the characteristics of tissue. Each layer represents an independent biological tissue characterized by temperature-dependent physiological parameters and linear temperature-dependent metabolic heat generation. Magnetic fluid hyperthermia (MFH) is used as an external heat source to heat the cancerous area. In a transient state MFH treatments can heat up cancerous tissue without damaging healthy tissue as indicated by an increase in the temperature of the cancer tissue according to the standard temperature of the MFH treatments.

Abstract

This study aimed to discuss the flow of outpatient care process of Indonesian National Health Insurance (BPJS by customer perception and standards time at Hermina Ciruas Hospital using Lean Hospital approach. In order to achieve the result, we obtain empirical evidence of the outpatient service process and provide proposed improvements from in-depth interview in outpatient BPJS. In addition, we proposed qualitative by using time-motion studies with observation method and in-depth interview in outpatient BPJS. Here the research was carried out in an outpatient installation at Hermina Ciruas Hospital with data analysis using lean hospital approach (Visio software device). The limitations in the process of this study subjectivity in observation collection of inevitable data due to, the calculation of time in each related unit is carried out by the researchers themselves. The results show average patient service process time is 299 minutes with the assessment of activity value added 26 minutes with the assessment of non-value with activities 273 minutes.it’s identifies waste in the outpatient service process. The findings on the processing data of outpatient services BPJS is still waste and amount of non- value added is poor. Therefore, the improvement efforts are needed for service efficiency on BPJS performance. The proposed improvements through the application of future state value stream map with, proposed blueprint of BPJS outpatient services, socialization of outpatient registration using Hermina mobile app (will get the ticket code used for activation at the queue counter), and discipline the presence of doctors.
