Abstract

Pekalongan City is one of the endemic cities of filariasis with mf-rate > 1 %. One of many factors that can influence filariasis is environmental factor. The aim of this research was to figure out the spatial imaging of environmental health such as shrub existence, puddle, wastewater pipeline, and livestock existence with filariasis in Pekalongan City. The study was quantitative descriptive with descriptive survey. Data were analyzed with spatial analysis using GIS in 6 filariasis-endemic subdistricts, i.e. Banyurip Ageng, Jenggot, Bandengan, Pabean, Kuripan Lor, dan Kertoharjo. The study showed that filariasis transmission was in the area with shrub and wastewater pipeline existence. Risk factor of wastewater pipeline existence was in all research places. Risk factor of livestock existence was in 3 subdistricts of Bandengan, Kertoharjo, dan Jenggot. Rice fields existence was a environmental risk factor in Kertoharjo and Banyurip Ageng.

Abstract

Neonatal mortality in Indonesia declined slowly and tended to stagnant in number of 20/1000 live births. Proportion of neonatal mortality in Cilacap Regency at 2015 reached 69% of infant mortality. A quality neonatal care might decrease 34-62% mortality, but not all midwives used integrated management of neonatal and childhood illness (IMNCI) algorithm. This study aimed to figure out factors associated with midwives’ practice on using IMNCI algorithm in neonatal care. This study used cross sectional approach with purposive sampling until 60 respondents were collected. Data were analyzed with chi-square test. There were association between knowledge (p=0.000), attitude (p=0.023), availability of facilities (p=0.023), support from leader (p=0.017), length of work (p=0.011), and supervision (p=0.039) with midwives’ practice on using IMNCI algorithm. There were not assocation on age (p=0.124), employment status (p=0.124), extra work load (p=0.290), and reward (p=0.053). Factors associated with midwives’ practice on using IMNCI algorithm in neonatal care were knowledge, attitude, availability of facilities, support from leader, length of work, and supervision.

Abstract

Antiretroviral (ARV) therapy for people living with HIV/AIDS (PLHIV) may reduce mortality and improve the quality of life among them. Non-adherence of HIV/AIDS patients in ARV therapy may lead to cease of therapy, increase risk of death, complicate evaluation of ARV services, and increase the risk of ARV resistance if they wish to start over. This study aimed to determine the factors that cause non-adherence to ARV therapy among PLHIV in Pemalang Regency. The research used qualitative method with case study approach. Data collection was conducted through in-depth interviews, observations, and documentation to 6 HIV/ AIDS patients who were listed as non-adherence patient in ARV therapy. Data analysis was done descriptively with triangulation method and data analysis. Results revealed that the factors that cause non-adherence to ARV therapy among PLHIV were lack of vulnerability perception to illness in the future, lack of illness perception among informants about the severity of illness, lack of benefit perception perceived by most informants, and barrier perception of side effects, lack of facilities to access ARV, and pudency that their HIV-positive status was known by others.

Abstract

National health insurance (NHI) as a part of national social security system (NSIS) is a form of government commitment to the implementation of health insurance among people in Indonesia entirely. State-owned enterprises (SOEs) assigned specifically by the government to provide health care insurance was the BPJS for Health (social security provider for health). The aim of this study was to analyze the satisfaction of national health insurance among BPJS for Health participants in Semarang City based on education, tuition classes, and place of service. This study was analytical survey with cross sectional approach. Sample of Non PBI (Contribution Assistance Recipients) BPJS for Health were 269 respondents, with purposive proportional sampling technique. There was a significant difference regarding the satisfaction of health insurance based on education level (basic, secondary, and high) and place of service (public hospital and private hospital). There was no significant difference on satisfaction of health insurance based on tuition classes (class I, II, and III). BPJS for Health participants who did not satisfied with the national health insurance were 64.7%. The most important complaint was the process of obtaining an inpatient room.

Abstract

Methadone maintenance treatment (MMT) is aimed to reduce the harmful effects of injecting drug use. Since the first Puskesmas Poncol opened this program in 2010, there were 50 users of MMT. Currently users of MMT were 11 people, 2 people have died, no patient has recovered, and 36 others dropped out. Those did not recover, but dropped out with reasons and information that were not known by the officer. MMT is a strategic key of HIV/ AIDS prevention for injecting drug users. However, in the implementation Puskesmas Poncol was lost most of active MMT users. The research aimed to know the implementation of MMT which includes input, process, and output on heroin addict in Puskesmas Poncol. This research was qualitative research supported by quantitative data. Data collection used in-depth interviews, observation, and documentation. The result of the research showed that the service of MMT in Puskesmas Poncol was still many weaknesses, from input aspect (untrained human resources), process (methadone planning, organizing, and dose arrangement have not been well implemented), and output (services have not been evaluated).

Abstract

Severe malnutrition may increase the risk of mortality, morbidity from infectious diseases, and delayed physical likewise cognitive development. The incidence of severe malnutrition in Cilacap in 2013 was 112 cases and increased in 2014 to 116 cases. Therefore, it was necessary to know and analyze the implementation of severe malnutrition management, in terms of input, process, and output components. This study used qualitative research, using case study design on 6 informants selected with eligibility criteria. Data collection was conducted by observation, in-depth interviews, and document studies. Validity test was done with triangulation. Data were analyzed with Miles and Huberman analysis model. It showed that the management of severe malnutrition was good enough. It could be seen from the input components (the number of health volunteers and the infrastructure facilities have been fulfilled, while the number of nutrition officers was still less and their role have not been well implemented), the process components (preparation stage was good, while organizing has not well structured, implementation and severe malnutrition service flow have not well run as procedure), and the output components (malnourished children under five have treated for 3 months and there was an improvement to better nutritional status).

Abstract

Health surveillance officers of dengue hemorrhagic fever (DHF) was one of DHF prevention and control programs Semarang City. The aim of this research was to know the effectiveness of DHF surveillance officers to determine larvae-absence rate in Semarang City. This research was quantitative supported by qualitative study with descriptive survey design. Samples in this research were 57 DHF health surveillance officers determined by cluster sampling technique and 3 of them as informants to complete the quantitative data. Techniques of collecting data were interviews and documentation. Data analysis was done descriptively by determining the effectiveness formula and using Miles and Hubberman model for qualitative data. The study showed that DHF health surveillance officers in Semarang City has been effective. It showed from effectiveness result score of each component compared with the percentage table of program effectiveness that input aspect was effective (71%), process aspect was very effective (92%), and output aspect was effective (77%), even though there were still obstacles in each aspect.

Abstract

Almost 65% of high blood pressure cases in old women is caused by estrogen deficiency, while 35% can be influenced by lifestyle factors and other factors. This study aimed to analyze lifestyle factors on hypertension at the age of menopause in Tlogosari Wetan Public Health Center of Semarang City. As many as 50 people has used as respondent randomly by cross sectional design study. Analysis of the data in this study using univariate, bivariate using Spearman rank test, and multivariate analysis using logistic regression test. The results in this study are there was a significant relationship between physical activity, stress , fat intake, the intake of sodium (p<0,05) against hypertension at the age of menopause. The results of logistic regression showed that sodium intake have a higher level of risk to hypertension (0.021). It can be concluded that food choice, stress, and physical activity are the lifestyle factors that contribute significantly to the risk of hypertension in menopause women.

Abstract

Vaginal discharge is one of reproductive tract infections that commonly occure among teenage girls. It actually does not need a treatment, but its impact will lead to infertility and cervical cancer. The aim of this study was to determine factors associated with the occurrence of pathologic vaginal discharge among high school girls in Kayong Utara Regency. This study was an analytical obervational research with cross sectional approach. Sampling technique used was simple random sampling. Sample size were 59 respondents. Data were collected by interview using questionnaire. Data analysis was done by chi square test. Results showed that there were significant association between knowledge of vulva hygiene (p=0.036), vaginal cleaning motion (p=0.025), use of vaginal cleanser (p=0.002), tight underwear (p=0.007), and use of public toilets (p=0.021) with pathologic vaginal discharge. There were no significant association between obesity and pathologic vaginal discharge (p=0.587). Factors associated with pathological vaginal discharge were knowledge of vulva hygiene, vaginal cleaning motion, use of vaginal cleanser, use of tight underwear, and use of public toilets.

Abstract

Stunting is often referred to as short stature as a manifestation of chronic malnutrition. It is often not realized at the age of five, and realized at primary school age. Prevalence of stunting children in Indonesia at 2007 was 37%, whereas in Pekalongan Regency reached 42,2%. Based on previous study, risk factors related with stunting were family factors, diet, and socioeconomic conditions. This study aimed to analyze the association between genetic factors, intake, socio-demography, CED malnutrition, and anatomical defects with the incidence of stunting among primary school students in Pekalongan Regency. This research was cross sectional. Sampling technique used was proportional random sampling until 93 respondents were collected. Stunting data were collected by anthropometry assessment, while socio-demography data were collected by indepth interview with open ended question to parents. Data were analyzed with chi square test. It showed that the parents’ height (genetic) was significantly associated with stunting (p=0.000). However, socio-demographic (p=0.093), intake (p=0.093), CED malnutrition or head circumference status (p=0.119), and anatomical defects (p=0.133) were not significantly associated with stunting. Parents’ height factors (hereditary) was associated with stunting among primary school students.
