Abstract

AbstractAccording to the regulation minister of health No.269, 2008,medical record is a document containing records of patient identity, examination, and treatment. Medical records should be made and completed immediately after the patient receives health care. In Pertamina Jaya Hospital, in filing a medical record still can be found incompleteness. The purpose of this research are toconduct quantitative analysis of inpatient medical record and identify factors that influence the completeness of hospitalized medical record by using fishbone method.This research is using descriptive method and technique of collecting data by observation, interview, and literature study. Based on the result of research with observation on 56 inpatient medical records in May 2017, there were 74% medical records completeness. The factors influencing the completeness of medical record can be found by conducting observation and interview. There were some factors that affect the completeness of the medical record such as the insufficient time/busyto complete the medical record, unapplied sanction, lack of the socialization, the implementation of the medical record is not based on Standar Operasional Procedure, the form of quantitative analysis not covering all basic component, and limited funding for evaluation. As a solution to improve the completeness of the medical records, doing intensive socialization and creating reward &punishment system.Keywords: factors, completeness of inpatient medical record, fishboneAbstrakMenurut Permenkes Nomor 269 Tahun 2008, Rekam medis adalah dokumen yang berisikan catatan tentang identitas pasien, pemeriksaan, dan pengobatan pasien,rekam medisharus segera dibuat dan dilengkapi setelah pasien menerima pelayanan kesehatan. Di Rumah Sakit Pertamina Jaya dalam pengisian rekam medis masih ditemukan ketidaklengkapan. Tujuan dari penelitian ini adalahmelakukan analisis kuantitatif rekam medis rawat inapdan mengidentifikasi faktor-faktor yang mempengaruhi kelengkapan rekam medis rawat inap dengan menggunakan metode fishbone. Metode penelitian yang digunakan adalah dengan metode deskriptif dan teknik pengumpulan data dengan cara observasi, wawancara, dan studi kepustakaan. Berdasarkan hasil penelitian dengan melakukan observasi terhadap 56 rekam medis rawat inap pada bulan Mei 2017, ditemukan kelengkapan sebesar 74%. Dan faktor-faktor yang mempengaruhi kelengkapan rekam medis dapat ditemukan dengan melakukan observasi dan wawancara yaitu, waktu untuk melengkapi rekam medis tidak cukup/sibuk, tidak ada sanksi yang diterapkan, kurangnya sosialisasi, pelaksanaan pengisian rekam medis belum sesuai standar proseduroperasional, formulir analisis kuantitatif belum mencakup semua komponen dan pendanaan kelengkapan rekam medis terbatas. Maka disarankan untuk melakukan sosialisasi standar operasional prosedur secara intensif, dan pengadaan sistem reward dan punishment. Abstract

AbstrakProgram pembangunan kesehatan di Indonesia  ini masih diprioritaskan pada upaya peningkatan derajat kesehatan ibu dan anak, terutama pada kelompok yang paling rentan pada  kesehatan yaitu ibu hamil,bersalin dan bayi pada masa perinatal. Angka kematian ibu di Indonesia masih tinggi dan salah satu upaya untuk menurunkan angka kematian ibu dengan melakukan pemeriksaan ibu hamil secara teratur. Tujuan penelitian ini untuk mengetahui hubungan pengetahuan ibu hamil tentang kehamilan dengan keteraturan pemeriksaan kehamilan. Metode penelitian ini adalah survey cros sectional dan jumlah sample sebanyak 86 orang secara simple random sampling. Dimensi tentang tingkat pengetahuan meliputi pengertian tentang kehamilan,diagnosis kehamilan,tanda kehamilan,tujuan pemeriksaan kehamilan. Dimensi keteraturan pemeriksaan kehamilan yaitu jumlah kunjungan ibu hamil minimal sebanyak 4 kali selama hamil. Penelitian mengunakan kuesioner dan observasi mengunakan analisis univariat dan bivariat dengan uji korelasi spearman rank. Hasil penelitian ini yaitu usia 26-35 tahun (61.6%), pendidikan ibu hamil  SMA (43.0%), pekerjaan ibu hamil sebagai ibu rumah tangga (69.8%). Tingkat pengetahuan ibu hamil tentang kehamilan yaitu (75.6%) dan teratur dalam pemeriksaan kehamilan (73.3%). Hasil uji korelasi menunjukkan ada hubungan tingkat pengetahuan ibu hamil tentang kehamilan dengan keteraturan pemeriksaan kehamilan (r = 0.344 ; p <0.05).Kata kunci: pengetahuan ibu hamil, keteraturan pemeriksaan kehamilan

Abstract

AbstractIndonesia through the Ministry of Health has begun implementing Hospital Information System based computer, although it is still growing rapidly but gradually it will be done in several hospitals. One application of Hospital Information Systems including electronic prescription (e - prescription) where the doctor wrote a prescription through the data entry process is automated using software that connects at the hospital pharmacy. Hospital " X " is the new hospital . Has been in operation since December 2010 located in West Jakarta , type B with a number of 200 beds and currently has as many as 130 operational beds,  BOR in 2012 by 60 % and the average number of 400 outpatients per day. Since its establishment, the medical records of the patient has been treated using a semi- electronic paper -based medical record / manual , although it has a Hospital Information System which can be used electronically. Frequent delays in preparing the prescription and administration of medication errors to the attention of the great directors of the hospital. This research is an observational analytic cross-sectional study design. This study is a comparative quantitative study , the research aimed to compare the quality of pharmaceutical services to the adoption of electronic prescribing in order to find differences or by testing the hypothesis that the effect can be deduced about the meaning of the data obtained. Target population of this study is the entire recipe before and after implementation of an electronic prescribing in hospitals X. While the study sample is a sample that meets all inclusion criteria that had been spiked with prescription receipt and completion time of service. Through this study found that the number of samples in the implementation of electronic prescribing prescription amounted to 64 % ( 340 ) , while the number of 192 ( 36 % ) do not use electronic prescribing prescription . From the results of the estimation interval can be concluded that 95 % believed the average waiting time is between. 18.03 minutes to 21.40 minutes. Average waiting time for service of a prescription before using electronic prescriptions for 27.12 minutes . While waiting period after the application of the electronic prescription service an average of 15.53 minutes . Based on the results of the independent t-test of significance for values obtained pvalue 0.000 ( < 0.05), meaning that there is a significant difference in waiting time between the use of electronic prescribing and electronic use . Advice from research in order to conduct electronic prescription services for all outpatient services . Besides the quality of pharmacy services not only to measure the waiting time prescription services also exist other indicators such as drug administration errors that are critical to patient safety . Therefore, further research needs to be done to the influence of other factors such as quality and quantity of human resources, facilities available and the information system itself .Keywords: electronic prescribing, health information systems, medical records

Abstract

AbstractDetermination of diagnosis and code of the cause of death are one of the functions in medical records unit that requires accuration. In determining code of the cause of death must be precise and accurate in accordance based on ICD 10. Rules determinant of the accuracy of diagnosis codes are completeness of the writing in a medical certificate of the death causal. The object of this study to known the association between the completeness of the medical certificate of death cause and accuracy of diagnosis code of patients death cause in SumberWaras Jakartahospital in 2016. This study is a quantitative study with cross sectional observation. The study sample are 74 death certificates on (January-March) 2016. Data were analyzed using chi square test. The results showed that the completenessof the medical certificate cause of death is complete was 55 (74.3%), and accurationof diagnosis code was 49 (66.2%).The P value = 0.010 (0.010 <0.05) and the  OR = 4.027. there is about association between the completeness of the medical certificate of death cause and accuracy of diagnosis code of patients death cause in SumberWaras Jakartahospital in 2016.Keywords: code the cause of death, the completeness writing a diagnosis, ICD-10 AbstrakPenentuan diagnosa dan kode penyebab kematian merupakan salah satu fungsi yang penting dari fungsi unit kerja rekam medis yang membutuhkan ketelitian. Dalam menentukan kode penyebab kematian haruslah tepat dan akurat sesuai dengan aturan ICD 10. Salah satu penentu ketepatan kode adalah kelengkapan penulisan diagnosa di sertifikat medis penyebab kematian. Mengetahui hubungan antara kelengkapan sertifikat medis penyebab kematian dengan ketepatan kode diagnosa penyebab kematian pasien di Rumah Sakit Sumber Waras Jakarta tahun 2016. Jenis penelitian ini adalah penelitian kuantitatif. Dilaksanakan dengan observasi secara cross sectional terhadap sertifikat kematian pasien untuk melihat kelengkapan penulisan diagnosa penyebab kematian dan ketepatan kode diagnosa. Sampel penelitian 74 sertifikat kematian pada triwulan (januari-maret) tahun 2016. Analisis data menggunakan uji chi square. Penelitian ini menunjukan bahwa kelengkapan sertifikat medis penyebab kematian yang lengkap adalah 55 (74.3%) dan ketepatan kode diagnosa yang tepat adalah 49 (66.2%). Nilai P value = 0,010 (0,010 < 0,05) dan nilai OR = 4,027. Sehingga menunjukan adanya hubungan antara kelengkapan sertifikat medis penyebab kematian dengan ketepatan kode diagnosa penyebab kematian pasien.Kata kunci : Kode Penyebab Kematian, kelengkapan penulisan diagnosa, ICD-10

Abstract

AbstractFrom the results of observations and reports on patient visits in the first semester of 2015 concluded that outpatients at the Tiara Mother and Child Hospital in Tangerang Banten were still very low after the hospital operated again, the impact on hospital income was far from expected, action was needed changes to improve patient visits. The purpose of the study was to determine the relationship of the influence of interpersonal-based medical services on service quality, trust and satisfaction of patients. The type of research used is quantitative descriptive with quantitative analysis using primary and secondary data obtained through documentation and questionnaire studies. Measurements using a Likert scale, testing the research hypothesis using SEM (Structural Equation Modeling) analysis; medical services provided well during interpersonal meetings will affect service quality; medical services that are well provided during interpersonal meetings will affect patient satisfaction; the quality of services provided to patients well will affect the patient's trust; and the factor of trust among high patients greatly influences patient satisfaction.Keywords: interpersonal, quality of medical care, trust and patient satisfactionAbstrakDari hasil observasi dan data laporan kunjungan pasien pada semester 1 tahun 2015 terlihat Kunjungan pasien rawat jalan di RSIA Tiara Tangerang masih sangat rendah setelah rumah sakit beroperasional kembali hal ini sangat berdampak pada pemasukan rumah sakit yang jauh dari yang diharapkan sehingga perlu dilakukan penelitian lebih lanjut untuk dapat meningkatkan pelayanan agar kunjungan pasien menjadi lebih baik lagi. Tujuan penelitian untuk mengetahui hubungan pengaruh pelayanan medis berbasis interpersonal terhadap mutu pelayanan, kepercayaan dan kepuasan pasien. Jenis penelitian yang dipakai adalah kuantitatif deskriptif dengan analisa kwantitatif, data yang digunakan adalah data primer dan data sekunder yang diperoleh melalui studi dokumentasi dan kuesioner. Pengukuran menggunakan skala likert,  Hipotesis penelitian diuji dengan menggunakan analisis SEM(Structural Equation Modelling) yaitu; pelayanan medis yang diberikan dengan baik saat pertemuan interpersonal akan mempengaruhi kualitas layanan; pelayanan medis yang diberikan dengan baik saat pertemuan interpersonal akan mempengaruhi kepuasan pasien; kualitas pelayanan yang diberikan kepada pasien dengan baik akan mempengaruhi kepercayaan pasien; dan faktor kepercayaan dikalangan para pasien yang tinggi sangat mempengaruhi kepuasan pasien.Kata Kunci : interpersonal, kualitas pelayanan kesehatan, kepercayaan dan kepuasan Pasien.

Abstract

AbstractMassage is a tactical stimulation that gives biochemical effects and physiological effects on various organs, when done correctly and regularly in infants will benefit the baby's growth, improve the baby's emotional connection and parents. The purpose of this research is to know the effectiveness of infant massage on growth and development of infant age 6-12 month in Nifuboke Village Noemuti Sub District by 2017. The approach is non randomized pre and post test with control group design. The number of samples is 16 people, by assessing the growth and development of infants before and after 30 days of treatment. The test used is independent t-test. The results of this study were obtained in the control group of infants able to lift the chest as much as 50%, lifting the neck as much as 37.5%, weight gain> 400 gram as much as 12.5% and the addition of body length> 3 cm as much as 12.5%. While the intervention group raised the chest as much as 87.5%, lifting the neck as much as 75%, weight> 400 gram as much as 75% and body length> 3 cm as much as 562,5%. T-test results showed a baby massage 11 times more likely to lift the chest and both hands as a buffer, 12 times more likely to retain the neck when pulled slowly into a sitting position. The conclusion is that infant massage has great effectiveness on the growth and development of infants aged 6-12 months.Keywords: Baby Massage, Growth and Development, Parents AbstrakPijat merupakan stimulasi taktis yang memberikan efek biokimia dan efek fisiologi pada berbagai organ tubuh, apabila dilakukan secara benar dan teratur pada bayi akan memberi keuntungan dalam tumbuh kembang bayi, meningkatkan hubungan emosional bayi dan orangtua. Tujuan penelitian adalah untuk mengetahui efektifitas pijat bayi terhadap pertumbuhan dan perkembangan bayi usia 6-12 bulan di Desa Nifuboke Kecamatan Noemuti tahun 2017. Pendekatan yang dilakukan adalah non randomized pre and post test with control group design. Jumlah sampel sebanyak 16 orang, dengan menilai pertumbuhan dan perkembangan bayi sebelum dan sesudah 30 hari perlakuan. Uji yang digunakan adalah independent t-test. Hasil penelitian diperoleh pada kelompok control bayi mampu mengangkat dada sebanyak 50%, mengangkat leher sebanyak 37,5%, mengalami peningkatan berat badan >400 gram sebanyak 12,5% dan penambahan panjang badan >3 cm sebanyak 12,5%. Sedang kelompok intervensi mengangkat dada sebanyak 87,5%, mengangkat leher sebanyak 75%, berat badan >400 gram sebanyak 75% dan panjang badan >3 cm sebanyak 562,5%.  Hasil uji t-tes menunjukkan pijat bayi 11 kali lebih besar kemungkinan mengangkat dada dan kedua tangan sebagai penyangga, 12 kali lebih besar kemungkinan mempertahankan leher ketika ditarik perlahan ke posisi duduk. Kesimpulannya adalah pijat bayi memiliki efektivitas besar terhadap pertumbuhan dan perkembangan bayi usia 6-12 bulan.Kata Kunci : Pijat Bayi, Pertumbuhan dan Perkembangan, Orangtua

Abstract

AbstractDiarrhea in toddlers is one of the diseases that become a health problem in the world, especially in developing countries and if the handling is not right it can lead to death. Geographical Information System (GIS) is a system that can be used to obtain epidemiological data information and this information when mapped will be a useful tool for mapping the incidence of an illness. This study aims to map the distribution of risk factors for diarrhea among toddlers in Kendari City. Type of research is observational analytic with cross sectional design. Data analysis using chi square test and GIS device to determine the distribution of diarrhea patients based on risk factors for diarrhea in toddlers in Kendari City. The results of the analysis showed that there was a significant relationship between environmental factors and the incidence of diarrhea in toddlers, including drinking water source factors p-value 0.002, latrine conditions p-value 0.001, sewerage channels p-value 0.03 and landfills p-value 0.04. The mapping results show that the majority of respondent families who have drinking water, and SPAL are not eligible in the village puuwatu. The majority of area that use family latrines are not eligible in the Kelurahan Watu-Watu, Lahundape and Bende.Keywords: disesase mapping, geographic information system (GIS), diarrhea; children under five years AbstrakDiare pada balita merupakan salah satu penyakit yang menjadi masalah kesehatan di dunia terutama di negara berkembang dan jika penanganannya tidak tepat dapat berujung pada kematian. Sistem Informasi Geografis (SIG) salah satu sistem yang dapat digunakan untuk memvisualisasikan dan mendapatkan informasi data epidemiologi dan informasi ini ketika dipetakan akan menjadi alat yang berguna untuk memetakan kejadian suatu penyakit. Penelitian ini bertujuan untuk pemetaan sebaran faktor risiko diare balita di Kota Kendari. Jenis penelitian ini yaitu observasional analitik dengan rancangan cross sectional. Analisis data menggunakan uji chi square & perangkat SIG untuk mengetahui distribusi penderita diare pada balita berdasarkan faktor risikonya. Hasil analisis menunjukkan ada hubungan signifikan faktor lingkungan dengan kejadian diare pada balita, diantaranya faktor sumber air minum yaitu p-value 0.002, kondisi jamban p-value 0.001, dan saluran pembuangan air limbah (SPAL) p-value 0.03. Hasil pemetaan menunjukkan bahwa mayoritas keluarga responden yang memiliki sumber air minum dan saluran pembuangan air limbah yang tidak memenuhi syarat terdapat pada kelurahan puuwatu. Mayoritas wilayah yang memanfaatkan   keluarga yang tidak memenuhi syarat yaitu terdapat pada kelurahan Kelurahan Watu-Watu, Lahundape dan Bende.Kata Kunci: pemetaan penyakit; sistem informasi geografis (SIG); diare; balita

Abstract

AbstractDiagnostic coding should be appropriate, because if it is improper will affect to the clinical data management, cost recovery, as well as the matters relating to health care. Based on this case, the researcher conducted research on the accuracy of diagnosis code at Pertamina Jaya Hospital by taking NIDDM case of inpatient in 2016. The purpose of this study is to get an idea of the accuracy of coding diagnosis case of NIDDM (Non Insulin Dependent Diabetes Mellitus) in 2016. The research started from June to August 2017. The research method used is descriptive. Researcher took 59 samples obtained from the formula of proportion estimation. Based on the total of 59 samples studied, there were 58 less precise NIDDM codes (98.31%) and 1 exact NIDDM code (1.69%). For the smallest digit accuracy is on the 4th digit (complication) with the exact number of 4 codes (6.78%) and the largest digit accuracy is on the dagger and asterisk (etiology and manifestation) with the exact number of 49 codes (83,06%). It can be concluded that the coding of NIDDM case diagnosis in Pertamina Jaya Hospital is still relatively low. The main factor that becomes obstacle in coding of diagnosis is knowledge factor of coder with nursing background. The coder should be a graduation of Diploma degree of Medical Record that has the competence of coding diagnosis.Keywords : medical record, diagnostic code, NIDDMAbstrakPengkodean diagnosis harus tepat, karena jika tidak tepat akan mempengaruhi manajemen data klinis, penagihan kembali biaya, beserta hal-hal yang berkaitan dengan asuhan pelayanan kesehatan. Berdasar hal tersebut, peneliti melakukan penelitian mengenai ketepatan kode diagnosis di RS Pertamina Jaya dengan mengambil kasus NIDDM pasien rawat inap tahun 2016. Tujuan penelitian ini adalah untuk mendapatkan gambaran mengenai ketepatan pengkodean diagnosis kasus NIDDM (Non Insulin Dependent Diabetes Mellitus) tahun 2016. Penelitian dimulai dari bulan Juni - Agustus 2017. Metode penelitian adalah metode deskriptif. Peneliti mengambil 59 sampel yang didapatkan menggunakan rumus estimasi proporsi. Dari total 59 sampel yang diteliti, terdapat 58 kode NIDDM kurang tepat (98,31%) dan 1 kode NIDDM tepat (1,69%).  Untuk jumlah ketepatan digit terkecil yaitu ketepatan pada digit ke-4 (komplikasi) sebanyak 4 kode tepat (6,78%), dan jumlah ketepatan  terbesar yaitu pada dagger dan asterisk (etiologi dan manifestasi) sebanyak 49 kode tepat (83,06%). Dapat disimpulkan bahwa pengkodean diagnosis kasus NIDDM di RS Pertamina Jaya masih masih tergolong rendah. Faktor  utama yang menjadi kendala ketepatan kode adalah faktor pengetahuan petugas rekam medis bagian koding dengan latar belakang perawat. Sebaiknya petugas koding lulusan D-III Rekam Medis yang memiliki kompetensi pengkodean diagnosis.Kata kunci: rekam medis, kode diagnosis, NIDDM

Abstract

AbstractThis article discusses about Indonesian influence diagnostic related group (ina-drg) to resume charging completeness medical patients in hospitals Kalianda jamkesmas South Lampung. In the health care system was an enacted JAMKESMAS payment pattern by applying INA-DRG system. Financing system diagnosis related group (DRG) is a system or method of payment by the funders of health care providers (health providers) for convening services, large costs are not calculated based on the type or amount of health services being offered to each patient, but by agreement price according to the diagnosis of diseases in which a group of patients who are being treated are located. INA-DRG (Diagnostic Related Group Indonesian) using the basics in coding using ICD 9 CM to ICD 10 and the action to diagnose the disease, the pattern of the rates specified amount of an average patient stay and long stay patients in accordance with the code of the disease suffered by patients treated so long hospitalization was determined and adjusted to the prevailing rates in the INA-DRG. Medical resumes charging completeness prior to implementation of INA-DRG still shows that there is still low awareness of health practitioners in the medical resume charging completeness. This is because there is no requirement in terms of the completeness of filling the whole medical resume. Completeness of medical resumes charging after INA-DRG implementation has already shown that there are very significant and meaningful changes, but there are certain parts that are still not discovered the change, namely the charging of physical examination items. Abstract

Abstract Birth attendant by health worker is one of the important indicators in reproductive health aimed at reducing maternal and child mortality. The cause of not optimal birth attendant by health worker was still lack of knowledge and public awareness about health service benefit of mother and still high trust to shaman (paraji) which is part of belief system and culture of society. The purpose of this research was to analyze the social determinants of birth attendant behavior among Baduy Luar tribe women, Banten province. This research was analytic study with cross sectional approach. The population in this study was Baduy Luar mothers who had children under five years old in Kadungketug village, Banten province. Sampling method used was purposive sampling with the number of respondents is 60 people. Dependent variable was birth attendant behavior and independent variable were age, education, occupation, parity, and distance of health service. Data were collected by questionnaire and analyzed using univariate and bivariate analysis (statistical test χ²). The results: most of the respondents were 20-34 years old (90%), uneducated (100%), weaver (50%), one parity (50%), average distance of health service was 7,4 km, and birth attendant by shaman (90%). Based on χ² statistical test, occupation and parity were social determinats of birth attendant behavior (p <0,05). The conclusion, there was need to continuous communication, information and education efforts on birth attendant behavior by health worker among Baduy Luar tribe woman.Keywords: birth attendant behavior, social determinants, maternal health AbstrakPerilaku pertolongan persalinan oleh tenaga kesehatan merupakan salah satu indikator penting dalam kesehatan reproduksi yang bertujuan untuk mengurangi angka kematian ibu dan anak. Penyebab masih banyaknya masyarakat yang persalinannya tidak ditolong oleh tenaga kesehatan yaitu kurangnya pengetahuan dan sikap mengenai manfaat pertolongan persalinan oleh tenaga kesehatan dan masih tingginya kepercayaan terhadap dukun (paraji) dalam menolong persalinan. Tujuan dalam penelitian ini yaitu untuk menganalisis determinan sosial perilaku pertolongan persalinan pada masyarakat suku Baduy Luar, Propinsi Banten, Jawa Barat. Jenis penelitian ini adalah studi analitik dengan pendekatan cross sectional. Populasi dalam penelitian ini adalah ibu-ibu suku Baduy Luar yang mempunyai anak dibawah usia lima tahun di desa Kadungketug, Banten. Metode pengambilan sampel yang digunakan yaitu purposive sampling dengan jumlah responden sebanyak 60 orang. Variabel dependen yaitu perilaku pertolongan persalinan dan variabel independen yaitu umur, pendidikan, pekerjaan, paritas, dan jarak pelayanan kesehatan. Pengumpulan data dilakukan dengan kuesioner dan dianalisis menggunakan analisis univariat dan bivariat (uji statistik χ²). Sebagian besar responden berumur 20-34 tahun (90%), jumlah paritas satu (50%), tidak berpendidikan (100%), pekerjaan menenun (50%), jumlah paritas satu (50%), rata-rata jarak pelayanan kesehatan adalah 7,4 km, dan persalinan ditolong oleh dukun (90%). Berdasarkan uji statistik χ², pekerjaan dan paritas merupakan determinan sosial perilaku pertolongan persalinan (p <0,05).
