Abstract

PCP is one of the most common opportunistic infection in HIV and AIDS patients. A definitive diagnosis of PCP in HIV and AIDS patients in Dr. Soetomo General Hospital Surabaya has not been previously done. A definitive diagnosis have the role in the therapy and to prevent the illness as well. PCP is diagnosed by using a microscope to indentify Pneumocystis jerovecii in the lung fluid or tisuue. The objective of this study was to know how to detect Pneumocystis jerovecii in the sputum samples and to know the determination of the prevalence of PCP in HIV and AIDS patients suffered with pulmonary symptom who were hospitalized in Dr. Soetomo General Hospital Surabaya. This research was carried out by a cross sectional study utilizing waste sputum samples from HIV and AIDS patients. The detection of Pneumocystis jerovecii used Giemsa stain. Six sputum samples among 18 sputum samples (33.33%) HIV and AIDS patients were Pneumocystis jerovecii positive. Based on this findings HIV and AIDS patients with pulmonary symptoms should be suspect having the possibility of PCP as opportunity infection in HIV and AIDS existed, and there for the detection of the Pneumocystis jerovecii in sputum sample must becoming routinelycarried out in the laboratory examination for HIV and AIDS patients which also suffering pulmonary symptom.

Abstract

Lipid profiles are used as the regular tests which performed to predict and evaluate the risk of coronary heart disease. The routine lipid tests include total cholesterol, LDL, HDL and triglyceride level. The total cholesterol/HDL and LDL/HDL ratio are often used as predictors of coronary heart disease. This study was performed to know the lipid level and ratio by comparing the condition in myocardial infarction patients and young adult controls and to observe the percentage of their lipid level and ratio exceeding the recommended optimal value. A retrospective study was performed using secondary data of 73 myocardial infarction patients, whom hospitalized in the Intensive Cardiac Care Unit Dr. Wahidin Sudirohusodo Hospital Makassar from June 2010 to July 2011 and 21 healthy young adult as controls. The mean level of the total cholesterol, LDL, HDL, triglyceride, ratio of total cholesterol/HDL and LDL/HDL of myocardial infarction patients are compared with the control subjects were 209.99±51.70 vs 151.42±27.90 mg/dL (p=0.000), 141.18±41.92 vs 76.76±20.45 mg/dL (p=0.000), 36.96±10.47 vs 47.05±8.24 mg/dL (p=0.000), 132.27±65.67 vs 142.14±54.93 mg/dL (p=0.155), 5.99±1.71 vs 3.26±0.59 (p=0.000) and 4.06±1.45 vs 1.66±0.44 (p=0.000), respectively. It was shown that the percentage of myocardial infarction patients with total cholesterol, LDL level, total cholesterol/HDL and LDL/HDL ratio exceeding recommended optimal value were 57.53%, 63.01%, 73.97% and 86.30%, respectively. The level of total cholesterol and LDL, ratio of total cholesterol/HDL and LDL/HDL in myocardial infarction patients were significantly higher, but the HDL level was lower compared to the control subjects. The percentage of myocardial infarction patients with LDL/HDL ratio exceeding recommended optimal value was higher than the other lipid levels and ratio. Therefore, it is concluded that the LDL/HDL ratio is better in predicting coronary heart disease than other routine lipid markers.

Abstract

Protein disulfide isomerase (PDI) is a member of thioredoxin superfamily that is secreted in the lumen of the endoplasmic reticulum (ER) in mammalian cells. PDI catalyzes disulfide bond formation, reduction, or isomerization of newly synthesized proteins in the lumen of the RE. PDIA4 is a member of the PDI family, which is also expressed in the mammary gland. Low expression of PDIA4 probably as a result of the stress of RE in hypoxic conditions. These conditions will enhance the ability of tumor cells in mammary tissue to divide aggressively and metastasize. A total of 61 breast cancer patients aged 23−90 years (60 women and 1 man) in Jakarta, Indonesia have participated in this study after signing the informed consent letter. The diagnosis of metastasis is confirmed by the examination of bone scanning. Measurement of PDIA4 concentration is performed by the quantitative enzyme-linked immunosorbent assay (ELISA) using a commercial kits. Based on the examination of the bone scanning, there were 23 (62.5%) subjects with metastatic and 38 (37.5%) subjects non-metastatic. The mean of age of the subjects with metastatic were significantly higher than non-metastatic subjects (p=0.028), which is 51.4 (45.9–56.8) and 44.2 (40.4–48.0). The median of PDIA4 concentrations in serum of subjects with metastatic, i.e. 7.7 (1.0–37.2) ng/ml was significantly lower than non-metastatic subjects (p=0.044), i.e. 9.4 (2.2–69. 6) ng/mL. The concentration of PDIA4 in serum of breast cancer patients with metastatic were significantly lower than non-metastatic patients. It shows that the concentration of PDIA4 have decreased in incidence of metastatic breast cancer.

Abstract

Malaria is still a global health problem around the world particularly in Indonesia. Chloroquine is one of the anti-malarial drugs which is still used in Indonesia. The aim of this research is to know the synergic effect of Chloroquine and N-Acetyl Cysteine towards parasitemia and erythrocyte malondyaldehyde levels in mice infected with Plasmodium berghei. This was an experimental research using Plasmodium berghei and BALB/c strain mice, 6−8 weeks old, with body weight 20−30 grams. After infected with Plasmodium berghei, the mice were divided into eight (8) groups: control, chloroquine, NAC 0.25 mg/gBb; NAC 0.5 mg/gBb; NAC 1 mg/gBb and combined drugs of chloroquine and NAC 0.25 mg/gBb; NAC 0.5 mg/gBb; NAC 1 mg/gBb. On the 3rd, 5th, and 7th day after treatment, the mice were killed and their parasitemia and Malondialdehyde (MDA) levels were measured. By using MANOVA and Tukey HSD test, a signifi cant difference in parasitemia level was found between the control and chloroquine group, as well as the control and combined drug groups. In the NAC groups, there was no decrease. The decrease of MDA level began on the 7th day. It can be concluded that there was a synergic effect between the chloroquine and N-Acetyl Cysteine in decreasing parasitemia and malondyaldehyde levels in mice BALB/c infected with Plasmodium berghei.

Abstract

In pregnant women who previously did not have hypertension pre-eclampsia is a pregnancy complication, which characterized by the increased of blood pressure, proteinuria and/or oedema. One of theory about aetiology of pre-eclampsia suggests that this is caused by endothelial damage that can lead to adhesion and platelet aggregation. The aim of this study was to know the platelet count and indices by analyzing, such as mean platelet volume (MPV), its distribution width (PDW) and the large cell ratio (P-LCR) for diagnostic confirmation in pre-eclampsia patients. This research was a retrospective study using secondary data of routine blood test from medical records of pre eclampsia patients who were treated at Dr. Wahidin Sudirohusodo Hospital during the period of January to December 2011 and the normal pregnant women data that were taken as control. The data obtained were platelet count, MPV, PDW, P-LCR and analyzed using independent T test. The results showed that the mean platelet count in pre-eclampsia patients was lower than the control but had no statistically significant difference (p=0.325) whereas the mean of MPV, PDW and P-LCR in pre-eclampsia patients increased compared to the control group and was statistically significant with p value of MPV (p=0.003), PDW (p=0.002) and P-LCR (p=0.010). In conclusion, platelet indices can be used as diagnostic confirmation markers in pre-eclampsia patients.

Abstract

Chronic myeloid leukemia (CML) is a type of Chronic myeloproliferative disorders in pluripotencial stem cell haematopoiesis cell disease caused by somatic mutation chromosomal translocation of the Abelson (ABL) and Breakpoint Cluster Region (BCR) genes on chromosomes 9 and 22. The Breakpoint Cluster Region Abelson Kinase (BCR-ABL) gene encodes different fusion transcripts of messenger Ribo Nucleic acid (m RNA)/type of fusion gene that vary in size depending on the breakpoint in the BCR gene. The majority of CMLcases have been shown to have either b3a2 or b2a2 fusion gene. This research is a preliminary study designed to know how to identify a quantification BCR-ABL gene and expression fusion of gene and its relation to routine haematological parameters. The researchers analyzed 12 adults who were positive using a quantification ratio BCR-ABL and Glucose-6-Phosphate Dehydrogenase (G6PDH) as a house keeping gene by real-time quantitative polymerase chain reaction (RQ PCR) of chronic phase of CML patients, qualitative of translocation of BCR-ABL gene by gel agarose and routine haematological tests by a haematologic analyzer. The average quantification ratio of BCRABL gene and G6PDH was 0.0881, 50% patients had b3a2 fusion gene, 41.6% had b2a2 dan 0.4% had e1a1. Fusion gene b3a2 showed a quantification ratio, haemoglobin level and leukocyte count higher compared to b2a2 fusion gene.

Abstract

Human resources (HR) planning is associated with an institution’s strategic plan, so the study of the required number of support personnel is in line with the direction of business development planning of the Laboratory Installation Unit. WISN (work load indicator staff need) method is a calculation based on the analysis of health manpower needs of the workload (work load analysis) and is considered as the most accurate method to calculate labour requirements in short-term. Based on the results of the analysis of manpower needs at the Laboratory Installation of Lung Health of the Community Hall Surakarta, it is known that there is still a lack of a number of five (5) persons of the health laboratory staff. The persons still lacking consist of: one (1) laboratory person related to clinical pathology laboratory, one (1) laboratory person for smear microscopy laboratory, and still three (3) microbiology laboratory persons. These persons are needed to be able to carry out the activities for a good laboratory service, and quality which is in accordance with the related applicable standards.

Abstract

Coronary Artery Disease has a high prevalence and is frequently occurred and associated with the high mortality and morbidity. Dyslipidemia is one of the risk factors of Coronary Heart Disease (CHD). ApoB contained in very low-density lipoproteins (VLDL), intermediate-density lipoproteins (IDL), LDL and small dense LDL (sd-LDL), with one molecule of apoB in each particle. Apo A-I is the major apolipoprotein in HDL particles. The ratio of apoB/apoA-I is a balance between apoB-containing particles and potentially atherogenic apoA-I that is antiaterogenik. This study is carried to know the determination whether there are differences between apoB/ apoA-I ratio in patients dyslipidemia with ACS and non ACS. The research used a cross-sectional study design with patients dyslipidemia subjects suffering Acute Coronary Syndrome (ACS) and non ACS who enter to the Laboratory of Pathology Clinic at Dr. Moewardi Hospital between July and November 2011. To determine the pattern of data distribution, the researchers used Kolmogorov Smirnov test. For the analysis of differences in mean apoB/apoA-I ratio in the two population groups is used the T test, using a computer program, with the significance level p<0.05, 95% confidence interval. From 74 samples examined the mean age is 56.42 year old. This patients consisted of 33 males (44.6%) and 41 women (55.4%). All subjects are grouped into two groups, dyslipidemia ACS and non dyslipidemia ACS. The results showed apoB/apoA-I ratio significantly different in patients with dyslipidemia with ACS and non ACS. The mean apoB/apoA-I ratio of women and men subjects in both groups, including groups at high risk of myocardial myokard and higher than the cut-off ratio of apoB/ apoA-I (men 0.9 and women 0.8). It can be concluded that the apoB/apoA-I ratio of women and men subjects in both groups, included the high risk category for infarct myokard although lipid abnormalities are still not demonstrated to the risk of infarct myokard.

Abstract

Serum albumin measurement can be used for assessing prognosis, determining initial therapy and treatment efficacy for liver cirrhosis patients. The discrepancy result of albumin measurement using different methods can make bias for the albumin concentration. The measurement of albumin was carried out in 20 sera of liver cirrhosis patients using bromcresol green (BCG), BCG conversion as well as bromcresol purple (BCP) methods including protein electrophoresis. The results showed that BCP method has a lower albumin concentration than the other kind (1.68±0.35), BCG (1.94±0.31), BCG conversion (2.09±0.45) as well as protein electrophoresis (2.14±0.35). BCP method has a good correlation with protein electrophoresis as well as BCG conversion (r=0.935 and r=0.90 respectively). It can be concluded in this study, that albumin measurement with BCP method has a good correlation with protein electrophoresis. It is important to know that the measurement results of BCP method is lower but more accurate in the albumin concentration, because there is no cross reaction with globulin. That means, there is no overestimation of albumin concentration in hypoalbuminemia condition of the patients.

Abstract

Iron overload disorder is a condition due to an excessive rate of iron acquisition compared to the rate of body iron loss that happens constantly. The clinical manifestations vary depending on the location of the damaged organs. Consequently, the symptoms of iron overload may mimic many diseases such as cirrhosis, diabetes mellitus, arthropathy, and skin pigmentation. The laboratory results which may point to iron overload are as follows: transferrin saturation >45%, serum ferritin level >300 ng/ml in men and >200 ng/ml in women. It is thus imperative that the molecular mechanism of iron metabolism and pathogenesis of iron overload are well understood in order to correct the interpretation of the laboratory results for iron overload. The standard management is blood removal by phlebotomy to maintain the level of the serum ferritin at 50 ng/mL in order to prevent irreversible hepatic cirrhosis
