Abstract

Misfile is an error in the placement of medical record documents. Misfile events will have an impact on patient care that is longer and can result in a decrease in the quality of health services in hospitals effectively and efficiently. The purpose of this study was to determine the management of medical record files so that medical record file misfiles do not occur at the Buton District General Hospital in 2019 based on 5 M (Man, Material, Mechine, Money, Method). The design of this research is descriptive qualitative. The results of the study found that the incidence of misfile in the Buton District Hospital was caused by the man and money factors. The conclusion of this study is that the cause of misfiled events at the Buton district hospital is because the medical record officer does not have a medical record education background and has never attended medical record training, as well as a small amount of funds for the medical record room so that there are still many shortcomings in the medical record unit, especially the filing section.

In the Money aspect, the patient's external causes code has not had a significant effect on the patient's INA-CBGs claim and for the Methods aspect, there is no SOP related to external causes. For this reason, officers should be given training, external causes codes are taken into account in making INA-CBGs claim reporting and RL4a reports related to morbidity, and SPOs about coding external causes are compiled and applied.

The results showed that the drug planning at Makkasau Health Center has been in line with the drug management guidelines. Drug requests at Makkasau Health Center have also been carried out according to available procedures. This can be seen in the implementation of drug requests on the health office through LPLPO to regency/city pharmacy warehouses (in Indonesian: Gudang Farmasi Kabupaten/Kota (GFK)) based on the guidelines for drug management. The drug acceptance at Makkasau Health Center has been carried out as well according to the procedure. Drug storage at Makkasau Health Center has not been categorized in the good drug storage standards. This is because the available medicine warehouse has minimal space and does not have enough shelves. The drug distribution at Makkasau Health Center has not been according to the procedure. This can be seen from the distribution of medicines from district/city pharmacy warehouses that is not based on the guidelines for drug management. Drug use at Makkasau Health Center has been according to the standard procedures. This can be seen by the implementation of rational drug prescribing based on the drug management guidelines. Drug recording and reporting at Makkasau Health Center have been according to the procedure. This can be seen by the implementation of daily recording and monthly reporting using the Drug Usage Report and Request Sheet (in Indonesian: Laporan Pemakaian dan Lembar Permintaan Obat (LPLPO)

Abstract

Nurses are one of the hospital resources, which are quite large in number and have a very decisive role in the quality of hospital services. Nurses in carrying out their work use a lot of bending and twisting movements, especially around the lower back, lifting heavy objects and transferring patients are the biggest risk factors for low back pain. The purpose of writing this article is to determine ergonomic risk control efforts in cases of low back pain in nurses in hospitals. The method used in this paper is a literature study approach. The collected data is then analyzed carefully. The results of the analysis from these journals are that the nonergonomic position of the nurse can cause low back pain. Nurse activities that are often carried out and have a high risk of low back pain are lifting and moving patients (patient mobilization), stitching wounds and installing infusions.
