Abstract

We present the case of a 23-year-old woman, G1P0A0, Gemelli (twins) with a gestational age of 38 weeks. The patient was referred to a type C referral hospital with complaints of low back pain without other symptoms. Antigen Rapid Test Swab examination with positive results and nasopharyngeal and oropharyngeal swab sampling for Reverse Transcriptase-Polymerase Chain Reaction (RT-PCR) examination of patients gave positive results with asymptomatic infection (without symptoms). Gemelli's pregnancy, which is considered a high risk and COVID-19 infection in the patient, resulted in parturition being performed by Sectio Caesarea. Both babies were born alive, but the first baby died on the first day, followed by the second baby on the second day after birth and then the mother experienced a worsening of the condition, namely in a coma placed in the ICU, on the third postpartum day she experienced respiratory and cardiac arrest and was declared dead.

Abstract

Vitamin K is a vitamin fat-soluble with the main function as an anticoagulant. The active form of vitamin K can be divided into two categories, namely vitamin K1 (phylloquinone) can be found in photosynthetic plants and vitamin K2 (menaquinone) comes from bacteria, source in foods we can found this in fermented foods like natto or soybean product which fermented by Bacillus natto. This literature review was conducted through searching in database like PubMed, and Elsevier using keywords like vitamin K, Immune system, and inflammation. The role of vitamin K in physiological functions is well known worldwide, such as blood coagulation, bone metabolism, and regulation of several types of enzymes. Apart from this role, vitamin K also acts as a cofactor in several plasma proteins, so this vitamin can influence immune responses. Recent studies have found vitamin K links to inflammatory diseases. Vitamin K also induced autophagy pathway. However, information regarding the role of vitamin K in the immune system is limited.

Abstract

The healing process of diabetic ulcers is influenced by the nutritional status of the patient, especially the nutritional status of the patient's protein intake.  This study was conducted to determine the relationship between the amount of dietary protein intake of DM patients with the healing process of diabetic ulcers at the Ampel Sehat Inpatient Primary Clinic. This study uses a non-experimental quantitative method with a cross-sectional approach, conducted in May - June 2021 with the research subjects being diabetic ulcer patients who underwent treatment, either outpatient or inpatient stay 1 week with the number of subjects 32 patients aged more than 45 years with this type of male and female, patients with type 2 diabetes mellitus, diabetic ulcers in the leg region, the degree of diabetic ulcers according to Wagner with a score of 1-3, and the patient has full consciousness. Data collection on the amount of dietary protein intake of DM patients was obtained through interviews and questionnaires. The data were analyzed by using the chi-square correlation test or Fisher's exact. The research subjects on average did not have sufficient daily protein intake and did not experience the healing process of their diabetic ulcers. The relationship between the amount of dietary protein intake in DM patients and the healing process of diabetic ulcers was found. This study concludes that there is a relationship between the amount of dietary protein intake of DM patients with the healing process of diabetic ulcers at the Ampel Sehat Inpatient Primary Clinic. Abstrak Proses penyembuhan ulkus diabetikum dipengaruhi oleh status gizi pasien, terutama status gizi asupan protein pasien. Penelitian ini dilakukan untuk mengetahui hubungan antara jumlah asupan protein diet pasien DM dengan proses penyembuhan ulkus diabetikum di Klinik Pratama Rawat Inap Ampel Sehat. Penelitian ini menggunakan metode kuantitatif non eksperimental dengan pendekatan cross sectional, dilakukan pada bulan Mei – Juni 2021. dengan subjek penelitian adalah pasien ulkus diabetikum yang menjalani pengobatan, baik rawat jalan maupun rawat inap 1 minggu dengan jumlah subjek 32 pasien berusia lebih. dari 45 tahun dengan tipe ini pria dan wanita, pasien diabetes mellitus tipe 2, ulkus diabetik di daerah kaki, derajat ulkus diabetik menurut Wagner dengan skor 1-3, dan pasien memiliki kesadaran penuh. Pengumpulan data jumlah asupan protein diet pasien DM diperoleh melalui wawancara dan kuesioner. Analisis data menggunakan uji korelasi chi-square atau Fisher's exact. Subyek penelitian rata-rata tidak memiliki asupan protein harian yang cukup dan tidak mengalami proses penyembuhan ulkus diabetikumnya. Ditemukan hubungan antara jumlah asupan protein diet pada pasien DM dengan proses penyembuhan ulkus diabetikum. Penelitian ini menyimpulkan bahwa ada hubungan antara jumlah asupan protein diet pasien DM dengan proses penyembuhan ulkus diabetik di Klinik Pratama Rawat Inap Ampel Sehat         This is an open access article under the CC–BY-SA license. _______

Abstract

The most frequent and accurate cataract surgery had been the phacoemulsification method. Currently, phacoemulsification had been developed using various techniques including divide and conquer phaco chop and supra capsular. This research aims to assess the cataract surgery time or phaco time at various degrees of lens hardness. All cataract patients at the Kharisma Paramedika Hospital, Kulon Progo, Yogyakarta Special Region during the period June-December 2021, had cataract surgery using the horizontal chop phacoemulsification technique, by an experienced eye specialist. The machine used the Millennium from Bausch & Lomb. Parameter settings on the machine are as follows: bottle height 100 cm; vacuum 230 mmHg; 25% power; multi burst module. The patient gave topical anesthesia with 0.5% pantocaine, then an incision was made with 2.75 mm keratoma, then trypan blue gave into the anterior chamber of the eye, then sodium hyaluronate was added without rinsing. Then the operator performs an anterior capsulotomy in a way of continuous circular capsulorhexis (CCC). The second port incision was made with a 15-degree knife, then hydro dissection was performed. The phaco time value was recorded on the machine at the end of each operation. There were 40 subjects (57%) male and 30 subjects (43%) female. Subjects aged 50-65 years reached 56 subjects (80%) and those aged >65 years were 14 subjects (20%). The average value of phaco time was obtained at 54.83 seconds. As Conclusion, Phacoemulsification cataract surgery with horizontal chop technique at various degrees of resolution density obtained an average phaco time of 54.83 seconds. ABSTRAK Pembedahan katarak yang cukup akurat dilakukan adalah dengan metode fakoemulsifikasi. Berbagai teknik fakoemulsifikasi, antara lain: divide and conquer, phaco chop (terdiri dari: horizontal chop, vertical chop, serta stop&chop), dan suprakapsular. Teknik horizontal chop dan vertical chop cukup efektif dilakukan, namun teknik ini cukup sulit bagi pemula. Terdapat parameter yang dapat dinilai pada operasi katarak, salah satunya yaitu phaco time. Penelitian ini bertujuan menilai waktu operasi katarak atau phaco time pada berbagai derajat kekerasan lensa. Semua pasien katarak di RS Kharisma Paramedika, Kulon Progo, Daerah Istimewa Yogyakarta (DIY), selama periode Juni-Desember 2021, dilakukan operasi katarak dengan teknik fakoemulsifikasi horizontal chop, oleh seorang spesialis mata yang sudah berpengalaman. Mesin yang digunakan adalah Millenium dari Bausch & Lomb. Parameter pengaturan pada mesin sebagai berikut : tinggi botol 100 cm ; vacum 230 mmHg ; power 25% ; modul multiburst. Pada pasien diberikan anestesi topikal dengan pantocain 0,5%, selanjutnya dilakukan insisi dengan keratom 2,75 mm, diberikan trypan blue ke dalam kamera okuli anterior, kemudian tanpa pembilasan dimasukkan sodium hyaluronat. Operator melakukan kapsulotomi anterior dengan cara continuous circular capsulorhexis (CCC). Pembuatan insisi second port dilakukan dengan pisau 15 derajat, lalu dilakukan hidrodiseksi. Pencatatan nilai phaco time dilakukan setiap akhir operasi. Hasil penelitian kami, sebanyak 70 subjek penelitian, terdiri dari 40 subjek (57%) laki-laki dan 30 subjek (43%) perempuan. Subjek yang memiliki usia 50-65 tahun berjumlah 56 subjek (80%) dan yang berusia >65 tahun berjumlah 14 subjek (20%). Nilai rata-rata phaco time yang didapatkan adalah 54,83 detik.  Sehingga dapat kami simpulkan operasi katarak fakoemulsifikasi dengan teknik horizontal chop pada berbagai derajat kepadatan lensa didapatkan nilai rata-rata phaco time sebesar 54,83 detik.

Abstract

Perceived barriers to the application of health protocols are defined as obstacles that arise in carrying out health protocols. The smaller the obstacles, the more people will implement health protocols. Perceived barriers in carrying out health protocols are influenced by demographic areas, education level, employment status, sources of information, and positive experiences of Covid 19. This study aims to determine the factors that influence the perception of the community’s barrier in carrying out health protocols. This study used an analytic observational method with a cross-sectional design. The study was conducted in Semarang, Central Java Province, Indonesia, in November 2021. The research subjects were taken by consecutive sampling as many as 88 people. Perceived barrier questionnaires were prepared, previously through validity and reliability tests. Test the analysis with Mann Whitney and Chi-Square. The majority of people with low perceived barrier are people who live in urban areas (66,7%), have higher education (76,7%), work (93,3%), receive information related to Covid 19 from social media (46,7%), and have no experience of Covid 19 (63,3%). There was a significant relationship between area of residence (p=0,026), education (p=0,003), employment status (p=0,000), experience (0,003) on the perceived barriers from health protocols. The perception of a small barrier is shown in urban communities, higher education, work status, easy media access, and experience that do not intersect with Covid 19. There will be easier to receive education and implement health protocols.   ABSTRAK Hambatan yang dirasakan dalam penerapan protokol kesehatan didefinisikan sebagai hambatan yang muncul dalam melaksanakan protokol kesehatan. Semakin kecil hambatannya, semakin banyak masyarakat yang menerapkan protokol kesehatan. Hambatan yang dirasakan dalam melaksanakan protokol kesehatan dipengaruhi oleh wilayah demografi, tingkat pendidikan, status pekerjaan, sumber informasi, dan pengalaman positif Covid 19. Penelitian ini bertujuan untuk mengetahui faktor-faktor yang mempengaruhi persepsi hambatan masyarakat dalam melaksanakan protokol kesehatan. . Penelitian ini menggunakan metode observasional analitik dengan desain cross sectional. Penelitian dilakukan di Semarang, Provinsi Jawa Tengah, Indonesia, pada bulan November 2021. Subjek penelitian diambil secara consecutive sampling sebanyak 88 orang. Kuesioner hambatan yang dirasakan disiapkan, sebelumnya melalui uji validitas dan reliabilitas. Uji analisis dengan Mann Whitney dan Chi-Square. Mayoritas masyarakat dengan persepsi hambatan rendah adalah masyarakat yang tinggal di perkotaan (66,7%), berpendidikan tinggi (76,7%), bekerja (93,3%), menerima informasi terkait Covid 19 dari media sosial ( 46,7%), dan tidak memiliki pengalaman Covid 19 (63,3%). Ada hubungan yang signifikan antara daerah tempat tinggal (p=0,026), pendidikan (p=0,003), status pekerjaan (p=0,000), pengalaman (0,003) terhadap persepsi hambatan protokol kesehatan. Persepsi hambatan kecil ditunjukkan pada masyarakat perkotaan, pendidikan tinggi, status pekerjaan, akses media yang mudah, dan pengalaman yang tidak bersinggungan dengan Covid 19. Abstract

COVID-19 pneumonia with critical symptoms causes acute respiratory distress syndrome (ARDS) and multiple organ dysfunction syndromes (MODS) which can be severe complications until death. One therapy for Covid-19 critical was to suppress the effects of cytokine storms, which was administered by Mesenchymal Stem Cells therapy. In this case, we will report a case of COVID-19 with a critical patient that was given MSC therapy there were good outcomes in this case which were improvement of the clinical symptoms and prevent death.   ABSTRAK Pneumonia COVID-19 dengan gejala kritis menyebabkan sindrom gangguan pernapasan akut (ARDS) dan sindrom disfungsi organ multipel (MODS) yang dapat menjadi komplikasi berat hingga kematian. Salah satu terapi kritis Covid-19 adalah menekan efek badai sitokin, yang dilakukan dengan terapi Sel Punca Mesenkim. Dalam kasus ini, kami akan melaporkan kasus COVID-19 dengan pasien kritis yang diberikan terapi MSC ada hasil yang baik dalam kasus ini yaitu perbaikan gejala klinis dan mencegah kematian

Abstract

The rapid and sudden arrival of the COVID-19 pandemic has overwhelmed everyone. In this article, the author summarizes various issues and problems in Indonesia that arose during the COVID-19 pandemic. These issues included the desecuritization of the COVID-19 issue; the masks scarcity and high price of masks; public confusion over the policy of testing, tracing, treatment (3T), social restriction policies, and terms related to COVID-19 that often change; social problems and poverty; rumors and fake news that accompany vaccination program; shortage of medical equipment and health resources; and the problems experienced by medical personnel such as fatigue, mental health issues, and even death. It is known that COVID-19 is a new virus. Therefore, at the pandemic's beginning, no one knew its characteristics and how to deal with it. This confuses policy makers around the world in formulating a formula to deal with COVID-19. Subsequently, the policies often change according to the development of the virus and science itself. Thanks to the stakeholders' role, the pandemic situation in Indonesia has gradually recovered. However, there are some notes that we can learn from Indonesia's pandemic to avoid the same problem if similar conditions occur in the future. As the title of this paper, "a lesson learned is a lesson earned". Finally, the authors summarize four formulas to minimize similar problems in the future, including: 1) strengthening medical resources and infrastructures; 2) disease prevention through health promotion and education; 3) strengthening the national emergency institutions; and 4) improving crime and corruption surveillance system.

Abstract

Natural disasters and forensic cases are increasing, requiring methods to identify victims and criminal cases that are easier, more efficient, and accurate. Lip print pattern is one way to help the process of individual identification. Lip print analysis using facial photo analysis method helps identify a person's characteristics. To determine the differences in proportion and the uniqueness of the lip print patterns in relation to ABO blood group between the Mongoloid dan Australomelanesoid populations of Indonesia. A total of 124 Mongoloid and Australomelanesoid population respondents who met the inclusion criteria were included in the study consisting of 31 males and females from each population. After the respondents signed the informed consent, they were interviewed and photos of their faces and lips were taken, then lip print pattern analysis was performed. Statistical analysis using Chi Square and Cohen's Kappa test.  From 124 research respondents, the Cohen's Kappa consistency test obtained p> ​​0.8 which means it has a very good match.  The research shows that the majority of the Mongoloid population has a type I' pattern while the Australomelanesoid has a type IV pattern. The majority of the Mongoloid population has AB blood, while the majority of Australomelanesoid has O blood. There is a non-significant difference in the proportion of lip print patterns in distinguishing between ABO blood groups and the Mongoloid and Australomelanesoid populations in Indonesia.   Abstrak Bencana alam dan kasus forensik yang semakin meningkat, membutuhkan metode untuk mengidentifikasi korban dan kasus kriminal yang lebih mudah, efisien, dan akurat. Pola sidik bibir merupakan salah satu cara untuk membantu proses identifikasi individu. Analisis sidik bibir dengan metode analisis foto wajah membantu mengidentifikasi karakteristik seseorang. Tujuan dari penelitian ini adalah untuk mengetahui perbedaan proporsi dan keunikan pola sidik bibir dalam kaitannya dengan golongan darah ABO antara populasi Mongoloid dan Australomelanesoid di Indonesia. Sebanyak 124 responden populasi Mongoloid dan Australomelanesoid yang memenuhi kriteria inklusi diikutsertakan dalam penelitian, terdiri dari 31 laki-laki dan perempuan dari masing-masing populasi. Setelah responden menandatangani formulir informed consent, mereka diwawancarai, foto wajah dan bibir mereka diambil, kemudian dilakukan analisis pola sidik bibir. Untuk signifikansi statistik, data dianalisis dan dinyatakan dalam proporsi menggunakan uji konsistensi Cohen's Kappa dan uji chi-square. Dari 124 responden penelitian, uji konsistensi Cohen's Kappa diperoleh p > 0,8 yang berarti memiliki konsistensi sangat baik. Hasil penelitian menunjukkan bahwa mayoritas populasi Mongoloid memiliki pola tipe I', sedangkan Australomelanesoid memiliki pola tipe IV. Mayoritas populasi Mongoloid memiliki darah AB, sedangkan mayoritas populasi Australomelanesoid memiliki darah O. Terdapat perbedaan proporsi pola sidik bibir yang tidak bermakna dalam membedakan golongan darah ABO dengan populasi Mongoloid dan Australomelanesoid di Indonesia.

Abstract

Dyslipidemia is associated with cardiovascular disease as well as hyperglycemia and insulin resistance. One of the natural ingredients for controlling dyslipidemia is kaffir lime peel ( Citrus hystrix ), which contains flavonoids and polyphenols that act as antioxidants and regulate blood glucose. This study aims to prove the effect of kaffir lime peel in lowering blood glucose levels. Methods : Post-test only controlled group design study , using 25 wistar rats divided into 5 groups, namely group (K-) without high-fat diet and extract (K+) was given a high-fat diet alone, (P1) was given kaffir lime peel extract at a dose of 35 mg/kg BW rats/day, (P2) was given extract at a dose of 70 mg/kg BW rats/day and (P3) was given extract at a dose of 140 mg. /kgBW rats/day. The intervention was given for 3 weeks and then blood glucose levels were measured using the GOD-PAP method. The data were analyzed using the Kruskal Wallis test and continued with the Mann-Whitney test. Results : The results showed a significant difference in the mean GDS levels between groups p=0.011 (p<0.05), Mann-Whitney test showed K(-) against P1 (ρ=0.009), K(-) against P2 (ρ=0.016) , K(-) to P3 (ρ=0.028), and P1 to P2 (ρ=0.028).

Abstract

Chronic osteomyelitis is a condition of acute osteomyelitis that has failed to heal because appears as long-lasting and refractory disease in which bacteria may produce biofilms consisting of matrix proteins and polysaccharides to protect the bacteria from antibiotics and monocyte-macrophage system obliteration.1,2 We report a 40-years-old man with a history of postoperative open fracture of the right femur 6 months ago. Pain (+). X-ray examination showed the results: attached plate and screw with nonunion old fracture of the femoral dextra alignment and less apposition with sclerotic features, osteodestruction of the left femoral system, leading to osteomyelitis. Then performed a partial internal fixation removal operation. From the results obtained from the patient, it was concluded that there was osteomyelitis in the femur due to complications from a postoperative open fracture of the right femur.
