Abstract

Acute gout flare is one spectrum of gout disease. The flare is characterized by severe acute pain with signs of inflammation at the joint. Currently, there are three primary guidelines in managing gout flares, namely American College of Rheumatology (ACR), European League Against Rheumatism (EULAR), and Indonesian Rheumatology Association (IRA). The objective of this review is to analyze the management of gout flares among these guidelines. We compared three guildelines on managing acute gout flare based on their acute therapy initiation time, first-line treatment, additional treatment, and urate-lowering therapy initiation. Additional literature search was also conducted on PubMed and ScienceDirect to provide additional information to the guideline. All guidelines agreed that management of gout flares should be initiated as early as possible with colchicine or NSAIDs based on patient condition and contraindications. Oral or injected corticosteroid should be considered in patient with renal impairment or contraindications to colchicine or NSAIDs. In conclusion, most recommendations from three guidelines were similar. IRA 2018 are still suitable for managing acute gout flare for Indonesian population

Abstract

One of the Sustainable Development Goals is to reduce the Maternal Mortality Rate in 2030 to 70 deaths per 100,000 live births. In 2022 the number of deliveries in Tegal Regency is 26,679 (96.41%) vaginal deliveries 26,282 (95.6%) assisted by health workers in health care facilities 241 (0.9%) are in non-health care facilities 156 (0 .6%) assisted by non-medical health workers. In 2022, 13 maternal deaths are dominated by 60% postpartum hemorrhage, 20% embolism, 10% edema, and others. The Bumijawa Community Health Center is the largest number of delivery assistance places in Tegal Regency. In 2022 there will be 1,685 vaginal deliveries consisting of 87.7% health workers and 5% non-health workers; even though the target is 97%. The purpose of this study was to determine the involvement of traditional birth attendants in assisting with childbirth. Data collection was carried out by in-depth interviews and data reduction, demonstrated by narration, using the triangulation method, and analyzed by saturated samples. The research informants were 3 mothers who had given birth with a dukun as the main informant, 3 dukun and a midwife as triangulation informants. The results showed that the mother's decision to choose birth attendants was related to experience, knowledge, attitudes, access to services, husband and family support, local culture; This can be seen from the level of public awareness of health which is still low

Abstract

Physiological adaptation in pregnant women may increase the risk of more severe symptoms of Covid-19 than women who are not pregnant but there was not much supporting data. Aim of this study was to compare the severity category between women who are pregnant and not pregnant with positive Covid-19. The study was conducted using a retrospective approach which obtained secondary data from patient medical records after being tested positive for COVID-19 from November 2020 to December 2021. Sample size for the study consisted of 67 women COVID-19 patients who were pregnant and 67 women who were not pregnant. Samples were taken by accidental sampling. Categorization of clinical symptoms of COVID-19 was divided into 5 categories (no symptoms, mild, moderate, severe/severe pneumonia, critical). Data were analysis with the Mann Whitney test. Results showed that there were significant differences in the categories of clinical symptoms between groups of pregnant women and groups of non-pregnant women who were exposed to Covid-19 with a value of p=0.000 (p<0,05). Clinical symptoms in Covid-19 patients who are pregnant more severe than women who are not pregnant. This may be caused by increased levels of coagulation and fibrinolytic factors during pregnancy. It is hoped that the handling of Covid-19 patients, especially pregnant women, will receive more detailed attention to avoid the appearance of more severe clinical symptoms.

Abstract

Breast cancer is a major public health concern worldwide. The incidence of breast cancer remains high. The mortality rate and prognosis for breast cancer can be improved with early diagnosis and therapy. Self-examination of the breasts is a common method of early diagnosis for women's health issues. This study aimed to explore the characteristics of female students in Allied Health Sciences Institution who performed regular breast self-examination in Indonesia and Pakistan. After institutional ethics review board approval and informed consent, a quantitative survey recruited 430 female students from Allied Health Sciences institutions in Indonesia and Pakistan. This study strictly followed the highest level of ethical standards proposed by Helsinki Declaration (Revised 2013). A closed and open-ended questionnaire was used to collect data on their demographics and characteristics. The questionnaire was distributed online using a google form. Data were analyzed using descriptive analytics . The findings of this study showed that n=395(91.86%) were aged 18-35 years, and 51% were from rural. 49% from urban areas. The majority of participants were single or never married (88%). Most participants were undergraduate students (95.35%). Almost half participants did routine BSE once a month (48.6%). Only 16% were very confident about doing a breast self-examination. Nearly half respondents had never seen a doctor (47%).  A positive attitude was seen among all the students. Additionally, most students performed a weekly breast examination. Evidently, more than half of the participants lacked self-assurance when it came to performing a breast assessment. We found a significant percentage who had never been to a doctor for medical attention. Public campaigns to raise awareness and urge female student in allied health science to regularly see their doctors and engage in BSE are so crucial

Abstract

Adolescents pregnant mothers can potentially have a problem related to their immature physics and psychology. Their health-care-seeking practice is essential to ensure that adolescent pregnant mothers receive adequate care during their pregnancy. During the COVID-19 pandemic, online media should be perceived as an alternative way to help adolescents seek care during pregnancy. This study aimed to explore the experiences of pregnant adolescent mothers who had practiced health-care seeking and followed online counseling using WhatsApp groups. This qualitative survey recruited 16 Indonesian pregnant mothers aged 15-19 who had followed WhatsApp online counseling. An open-ended questionnaire was used to collect data, which were analyzed using thematic content analysis. There were three themes related to pregnant adolescent care-seeking practices using WhatsApp groups: experience, benefits, and advice and hope. The experience had three sub-theme: present experiences, previous experiences, and hindering factors.  The benefits had sub-themes: benefits of WhatsApp counseling and benefits  of Group WhastApp. Their hopes and advices were:  no advice, be polite, be consistent, be patient, and be duplicated by others. Adolescent pregnant mothers had good experiences in practicing health-care seeking using WhatsApp groups. They were more enthusiastic about following the counseling process and found beneficial information and solution regarding their problem during pregnancy. The findings show that providing WhatsApp groups for adolescent pregnant mothers could benefit them.

Abstract

Taro leaves (Colocasia Esculenta (L.) Schott.) contain alkaloids, saponins, tannins, flavonoids and polyphenols. Utilization of taro leaves requires new innovations by developing ways of making tablets, which increase the convenience of use through more comfortable use and more stable storage compared to liquid pharmaceutical preparations or suspensions. The aim of this study was to determine the effect of the comparison of variations of Polyvinylpyrrolidone (PVP) binder on the physical properties of granules and tablets and to obtain the optimum formula. Tablets were made by wet granulation process with 1%, 3% and 5% PVP. Granules were tested for water content, flow time test, angle of repose and tapping. The resulting tablets were then tested for their physical properties. Research result; Weight uniformity with 5% PVP fulfills the physical requirement of 600 g. Size uniformity fulfills the physical requirements with a yield of 2.91 mm. Hardness with 3% PVP meets the physical requirements of 5.95 kg. Fragility meets the physical requirements of 1% PVP, namely 0.4%. The disintegration time with 1% PVP met the physical requirements, namely 7.88 minutes. The results stated  that variations in the concentration of the PVP binder could be formulated into tablets and the tablet binder resistance was measured which met the requirements of the best tablet physical properties. Optimal formulation is achieved in formula III with binder containing 5% PVP.

Abstract

Asthma is an inflammatory respiratory disease caused by narrowed airways resulting in wheezing, shortness of breath, and chest pain. Knowledge of asthma is essential to prevent asthma recurrence, so patients can avoid precipitating factors that can cause asthma recurrence. This study aims to describe the knowledge about the prevention of recurrence in asthma patients. This descriptive study used the accidental sampling technique and involved 41 patients treated at the Pulmonary Polyclinic at Bendan Hospital, Pekalongan City. The Tullu questionnaire was used. This study indicated that most patients seeking treatment at the Pulmonary Polyclinic at Bendan Hospital Pekalongan City had poor knowledge of 27 respondents (65.9%). In contrast, some patients have good knowledge and as many as 14 respondents (34.1%) regarding the prevention of asthma recurrence. The majority of respondents in this study have less knowledge.
