Abstract

Kecelakaan kapal yang tercatat dalam Kantor Kesyahbandaran dan Otoritas Pelabuhan Kelas 1 Banten periode Desember 2012 hingga Januari 2014 menyatakan bahwa jenis kecelakaan yang terjadi di Pelabuhan Banten berupa tubrukan atau benturan, kebakaran, kandas dan tenggelam. Kecelakaan kapal jenis tubrukan merupakan kecelakaan yang sering terjadi di Pelabuhan Banten dengan presentase sebesar 63,64% sebanyak 7 kejadian. Penelitan ini memfokuskan terhadap jenis kecelakaan tubrukan kapal. Tujuan penelitian ini adalah mengklasifikasikan penyebab tubrukan kapal kedalam HFACS dan menentukan rating penyebab tubrukan kapal berdasarkan AHP. Hasilidentifikasi dan klasifikasi penyebab tubrukan kapal berdasarkan faktor HFACS yaitu, unsafe acts (tindakan tidak aman) sebanyak 6 penyebab atau 40% dengan rating AHP sbesar 12.24%, preconditions for unsafe acts (kondisi tertentu penyebab tindakan tidak aman) sebanyak 4 penyebabatau 27% dengan rating AHP sebesar 8.51%, unsafe supervision (kesalahan pada pengawasan) sebanyak 2 penyebab atau 13% dengan rating AHP sebesar 8.24% dan organizational influences (pengaruh organisasi) sebanyak 3 penyebab atau 20% dengan rating AHP sebesar 10.84%.

Abstract

PT X merupakan perusahaan yang memproduksi paint bucket (ember cat) yang terdiri dari tiga jenis paint bucket, yaitu bucket polos, lid (tutup bucket) dan bucket berlabel. Persentase bucket polos cacat sebesar 1,95%, persentase lid cacat sebesar 0,65% dan persentase bucket berlabel cacat sebesar 6,28%. Peningkatan kualitas paint bucket dilakukan dengan menggunakan metode Six Sigma DMAIC. Pada tahap D (Define) dilakukan pembuatan deskripsi proses produksi, pembuatandiagram SIPOC dan penentuan critical to quality (CTQ). CTQ untuk bucket polos dan lid diperoleh sebanyak dua buah, sedangkan CTQ untuk bucket berlambel sebanyak delapan buah. Pada tahap M (Measure) dilakukan pengukuran performansi sebelum perbaikan berupa rata-rata DPMO.Rata-rata DPMO bucket polos, lid dan bucket berlabel berturut-turut sebesar 7.591,88, 3.420,77 dan 8.109,44. Pada tahap A (Analyze) dilakukan penentuan prioritas perbaikan CTQ dengan membuat diagram Pareto dan mencari penyebab terjadinya cacat pada bucket polos, lid dan bucket berlabel. Berdasarkan diagram Pareto, penelitian fokus memperbaiki 1 jenis cacat pada bucket polos dan lid, yaitu cacat susut dan 5 cacat pada bucket berlabel, yaitu perbedaan tinggi pada pertemuan foil, foil terkelupas, foil hanya menempel sebagian, penempelan tidak menghasilkan pertemuan foil dan bintik putih. Setelah diketahui penyebab terjadinya jenis cacat, dilakukan tahap I (Improve).Tindakan perbaikan yang dilakukan adalah penggunaan infrared thermometer, pembuatan alat bantu, penggunaan microfiber gloves, pembersihan jalur keluar bucket polos, dan lain-lain. Setelahdilakukan perbaikan, dilakukan tahap C (Control). Tindakan perbaikan mengakibatkan terjadinya penurunan nilai rata-rata DPMO pada bucket polos, lid dan bucket berlabel, yaitu berturut-turut sebesar 2.621,54, 1.169, dan 713,69.

Abstract

pada bagian produksi di CV. GMS. Pekerja pada bagian ini mengalami keluhan dan nyeri pada bagian lengan bawah, lengan atas, dan punggung. Pengamatan dilakukan terhadap semua pekerja yang bertugas menangani botol yang sudah terisi yang berjumlah lima orang. Pekerjaan yang diamati adalah memindahkan botol dari ban berjalan ke lantai dan mengirimkannya ke bagian berikutnya. Evaluasi dilakukan terhadap beban kerja dan jadwal kerja-istirahat berdasarkan tinjauan ergonomi. Hasil penelitian menunjukkan bahwa energi yang dikeluarkan oleh kelima pekerja untuk aktivitas yang diamati berada pada kisaran 3,70-4,07 kkal/menit. Dengan demikian, secara fisiologis aktivitas kerja para pekerja dapat diklasifikasikan sebagai kerja sedang. Waktu istirahatyang diberikan secara terjadwal pada jam 12.00-13.00 sudah mencukupi kebutuhan istirahat untuk semua pekerja. Sementara itu, evaluasi dengan Persamaan Pengangkatan NIOSH yang direvisi tahun 1991 menunjukkan Lifting Index (LI) yang lebih besar dari 3. Untuk memecahkan masalah ini, beberapa modifikasi dilakukan pada faktor-faktor pengali dengan metode coba-dan-gagal dalam perhitungan Recommended Weight Limit (RWL), tetapi hasilnya juga tidak memberikan nilai LI di bawah 1. Oleh karena itu rekomendasi yang diberikan adalah menambahkan bangku dan jalur penggelindingan pada akhir ban berjalan. Untuk mengirimkan botol air ke bagian berikutnya, pekerja dapat menggulingkannya ke bangku dan mendorongnya ke jalur yang disediakan.

Abstract

Productivity achievement at PT Adi Satria Abadi (Golf Gloves Division) is not optimal because ofthe waste. Value Stream Mapping as a Lean Manufacturing tool is used to map the production processand identify and eliminate the waste. The advantage of VSM is its capability to give visual map of valueadded flow process, necessary but non value added flow process, and non value added flow process. In thisstudy Waste Assessment Model (WAM) also have been implemented to identify the waste. The sequence ofthree greatest waste are Defect/Reject 24,73%, Unneccessary Inventory 18,80%, and Unneccessary Motion15,44%. This study can reduce production lead time 62,22 minutes and improve the production throughputby 77 pcs.

Abstract

This research uses the Consultant-Guided Search (CGS) algorithm to solve job shop schedulingproblems minimizing makespan. CGS is a metaheuristics inspired by people making decisionsbased on consultant’s recommendations. A number of cases from literatures is developed to evaluatethe optimality of this algorithm. CGS is also tested against other metaheuristics, namely GeneticAlgorithms (GA) and Artificial Immune Systems (AIS) for the same cases. Performance evaluationsare conducted using the best makespan obtained by these algorithms. From computational results,it is shown that CGS is able to find 3 optimal solutions out of 10 cases. Overall, CGS performs bettercompared to the other algorithms where its solution lies within 0 - 6,77% from the optimal solution,averaging only 2,15%. Futhermore, CGS outperforms GA in 7 cases and performs equally well inthe other 3 cases. CGS is also better than AIS in 8 cases and is equally well in only 2 cases.

Abstract

Spare parts is one of the production support components which plays an important role for the survivalof gas production in the gas processing facility owned by SKN JOB Pertamina Talisman Jambi Merang. Thehigh inventory level increased the high inventory cost for the industry which get the benefit from the efficiencyof processes and resources. This research involved consumable spare parts for Solar Turbine engine as muchas 25 SKUs with demand character patterned lumpy demand and Poisson distribution. The implementationof policies using Periodic Review (R, s, S) with Power Approximation approach in the inventory systemcapable to generate a lower total cost inventory by pressing the backorder volume, the booking volume and theinventory levels in a balanced manner. Calculation of Periodic Review (R, s, S) with Power Approximationapproach resulted inventory parameter which was able to press the total cost of inventory at 8.54% lower andincrease the service level by 1.11%.

Abstract

PT WLS is a company that produce solid surface product. The variance of shape and design that canbe produced is very high and can be customized to the demand. The problem that occur in this company isthat sometimes the product fractured during delivery and production. To cope this problem, an experimentdesign with General Factorial Design is used to determine the best material composition that can increase thetoughness of the material. Two factors are determined to be the main factors that determine the toughnessof the materials which is the amount of hardener and the type of material used. Charpy test was used todetermine the toughness of the samples with a 55 mm x 10 mm x 10 mm dimension. Based on the analysisdone, the amount of catalyst, the type of material used, and the interaction between those two factors have asignificant effect to the materials toughness value and to get the optimal result for a 100 mm x 100 mm x 9mm sample, it was advised to use 3 ml of hardener and Al(OH)3 with HWF specifications.

Abstract

A staticwork position with sitting is common work posture founded in industry, particularly small industryin Indonesia. The problem that review in this paper is the activity of workers sitting static for processingemping in Yogyakarta. Activity done by means sitting on the floor without using table. The activity carriedin a continuous long term, so as to cause disease risks muscle and bone especially lower back. Ergonomic workfacilities is implemented to solve the work problem with table and chair. The design of work table is developedwith concept low cost improvement and consider with secure, health, and convenience of worker. The designprocess using participatory approach by involving worker to get best design. The result of facilities implementationcan be achieved a decrease ih the risk of work. The potential risk of work posture can be repair thatshown of the RULA score from 6 to 4, and no more complain the back and waist from worker activity.

Abstract

The purpose of this paper is to analyze work system improvements of home industries in Yogyakarta City,based on participatory ergonomics approach. WISH (Work Improvement for Safe Home) action checklisthas been used as the instrument to analyse and evaluate the working conditions. WISH programme isan action manual published by ILO (International Labour Organization) to improve work conditions ofhome manufacturing by using a participatory approach. The working conditions in WISH action checklistcovers five aspects; i.e. materials storage and handling, machine safety, work stations, physical environment,and welfare facilities and work organization. The early step in this study was identification of generalcharacteristics of home industries in Yogyakarta City. The results of identification were used as input indeveloping improvement actions. Analysis was done to determine the priority among all improvement aspectsand improvement actions based on WISH action checklist. The findings show the most priority aspect ofimprovement was Work Stations aspects with index priority 0.172. The action needed for the most priorityimprovement aspect was developed by consideration of home industries characteristics.

Abstract

Achievement of plant capacity and reducing bottlenecks in the production process at wet blanketworkstation determined on the level worker of musculoskeletal disorders and the cycles time of the productionprocess per slabs. Capabilities and limitations of workers, equipment, task, and environmental organizationsmust be harmonized for level of workers musculoskeletal disorders and the cycles time of the productionprocess per slabs reduction. The problem scale priority based on total ergonomics must be applied early withidentification of 8 aspects of ergonomics, planned and considered systemic, holistic, interdisciplinary andparticipatory and apply the concept of appropriate technology become one of problem solution about this case.Reduction of level of worker musculoskeletal disorders and cycles time of the production process per slabsbased on ergonomics; include redesigning of the ergonomics table and standing-seat chair folding work wetblanket, setting the pattern system of pair work, giving an active break, providing additional nutrition inthe form of sweet tea and snack pempek, provision of personal protective equipment, and redesigning of thephysical work environment. Experimental research was using the treatment by subject design, with involved17 workers samples who perform activities on the conditions of activity before and after the redesign basedon ergonomics. The level worker of muskuluskeletal disorders, and the cycle time of the production processper slabs data were analyzed using Two Pair Sample t-test at a significance level of 5%. The results of studyshowed that after redesign of wet blanket workstation based on ergonomics there was a significant reductionof the level worker of muskuluskeletal disorders as 21.02%, and reduction of the cycles time of the productionprocess per slabs as 21.85%.
