Abstract

In improving the total coverage of elderly healthcare, public health centers (puskesmas) provide services off-building, one of which is elderly posyandu. In Semarang city, the average elderly attendance rate to posyandu was still meager. It impacted healthcare's low coverage and decreased elderlies' health status quality. This study aimed to analyze factors affecting the elderly posyandu utilization in working areas of six puskesmas of Semarang city. This study was quantitative with a cross-sectional design. The population was 42.679 elderlies with 70 elderly samples. The sampling technique was accidental sampling using questionnaires. The univariate analysis used a frequency distribution, while the relationship analysis used the rank spearman test. The study results showed relationships of attitude (p= 0.048), trust (p= 0.000), access (p= 0.000), cadres role (p=0.000), healthcare workers role (p=0.000), family support (p=0.015), facility (p=0.000), and disease susceptibility perception (0.002) to elderly posyandu utilization in working areas of six puskesmas of Semarang city. It is necessary to maximize funding for activities and facilities, innovate the development of gymnastics, conduct a singing together activity, and provide an understanding of disease risk factors.

Abstract

Pemerintah Kota Semarang membuat strategi pendampingan ibu hamil selama pandemi COVID-19 menggunakan dua model pendampingan yaitu pendampingan langsung dan telemedicine sebagai pencegahan terhadap penyebaran kasus COVID-19. Tujuan dari penelitian ini adalah untuk melihat perbedaan kepuasan antara ibu hamil yang mendapatkan pendampingan langsung dan ibu hamil yang mendapatkan model telemedicine. Metode penelitian kuantitatif dengan pendekatan cross-sectional. Penelitian ini terdiri dari 110 sampel ibu hamil yang dibagi menjadi dua kelompok secara merata baik untuk model pendampingan langsung maupun model telemedicine. Pengambilan data dilakukan melalui online menggunakan google form yang disebarkan secara personal kepada ibu hamil melalui whatsapp mesengger. Terdapat dua metode yang digunakan untuk menunjukan tingkat kepuasan dalam penelitian ini, yaitu analisis univariat dengan tabel frekuensi dan analisis bivariat menggunakan uji Mann-Whitney U. Analisis univariat menunjukan tingkat kepuasan yang lebih baik dari mereka yang menerima model telemedicine dibandingkan dengan model pendampingan langsung. Analisis bivariat menunjukan adanya perbedaan tingkat kepuasan antara dua kelompok sampel (sig. = 0,020), pada aspek hubungan petugas-ibu hamil (sig. = 0,013), aspek kenyamanan (sig. = 0,013), serta aspek pengetahuan dan kompetensi petugas (sig. = 0,008). Dengan demikian disarankan Dinas Kesehatan Kota Semarang untuk melakukan mix model pendampingan, mengembangkan teknologi telemedicine, dan meningkatkan pendampingan model telemedicine untuk masa depan.

Abstract

Dharmarini Public Health Center has a high number of COVID-19 cases and there are COVID-19 epidemiological surveillance activities that are not in accordance with those stated in the guidelines, specifically in health monitoring and in terms of Community-Based Surveillance that vulnerable populations have not been recorded and reported. Based on a preliminary study, Dharmarini Public Health Center only has a surveillance officer and experienced obstacle in cross-sectoral coordination. This study aims to analyze the management aspects of COVID-19 epidemiological surveillance at Dharmarini Public Health Center include planning, organizing, actuating, and controlling. This is a qualitative research with analytical descriptive approach. Primary data collection was done by in-depth interviews and research subjects were selected by purposive sampling. The result shows that management aspects in COVID-19 epidemiological surveillance activities at Dharmarini Public Health Center have been carried out from planning, organizing, actuating, to controlling, but there are still shortcomings including in actuating that not all officers involved have received training, there is obstacle in cross-sectoral coordination, the implementation of health monitoring can still be improved, there are refusals in tracing, Community Based Surveillance has not been fully implemented, supervision has only been held once, and in controlling aspect that the evaluation carried out is still not comprehensive. Dharmarini Public Health Center is suggested to optimize the actuating and controlling aspects in COVID-19 epidemiological surveillance.

Abstract

ABSTRAKPenurunan kunjungan rawat inap terutama ruang rawat inap kebidanan di RSIA Kurnia Cilegon, penurunan jumlah pasien lama dan penurunan jumlah persalinan memperkuat indikasi loyalitas pasien di Poliklinik Obstetri dan Ginekologi menurun. Adanya persaingan rumah sakit swasta menempatkan bauran komunikasi pemasaran penting dalam meningkatkan kunjungan dan membangun hubungan jangka panjang dengan pasien. Penelitian ini bertujuan untuk mengetahui nilai promotional mix (bauran komunikasi pemasaran) dikaitkan dengan loyalitas pasien. Jenis penelitian observasional dengan pendekatan cross sectional. Variabel yang diteliti meliputi komunikasi personal, periklanan, publisitas dan hubungan masyarakat, materi pembelajaran, rancangan korporat dan kepuasan pasien. Data penelitian diolah secara kuantitatif dengan metode univariat, bivariat dan multivariat dengan uji analisis regresi logistik dilengkapi dengan analisis secara kualitatif (content analysis). Hasil penelitian menemukan bahwa persepsi kurang baik pada komunikasi personal sebesar 62%, persepsi kurang baik pada periklanan sebesar 55%, persepsi kurang baik pada publisitas dan hubungan masyarakat sebesar 63%, persepsi kurang baik pada materi pembelajaran sebesar 54%, persepsi kurang baik pada rancangan korporat sebesar 53% dan persepsi kurang baik pada kepuasan pasien sebesar 72%. Hasil analisis pengaruh bersama-sama menunjukkan bahwa komunikasi personal, periklanan, publisitas dan hubungan masyarakat, materi pembelajaran, rancangan korporat dan kepuasan pasien berpengaruh terhadap loyalitas pasien. Hasil tersebut dapat dibuat persamaan regresi yaitu Y= 0,871 + 2,145 variabel komunikasi personal + 1,879 variabel periklanan + 1,044 variabel publisitas dan hubungan masyarakat + 1,758 variabel materi pembelajaran + 1,233 variabel rancangan korporat + 1,445 variabel kepuasan pasien. Komunikasi personal memiliki pengaruh yang paling kuat sedangkan publisitas dan hubungan masyarakat memiliki pengaruh yang paling lemah terhadap loyalitas pasien. Disarankan manajemen RSIA Kurnia Cilegon untuk meningkatkan komunikasi personal bidan dan informasi sms gateway.Kata kunci : Promotional Mix, Loyalitas Pelanggan, Perilaku Konsumen. ABSTRACTThe decrease of inward visit specifically in the obstetrics inward room of Kurnia maternal andchild hospital (RSIA) Cilegon, the decrease of the number of old patients, and the decrease of the number of deliveries strengthen the indication of decreasing patient’s loyalty in the obstetrics and gynecology policlinic. Competition among private hospitals placed a promotional mix an important matter to increase patient visit, and to build long term relationship with patients. Objective of this study was to identify promotional mix value in relation with patients’ loyalty. This was an observational study with cross sectional approach. Study variables were personal communication, advertising, publicity and public relation, teaching materials, corporation design, and patient satisfaction. Data were analyzed quantitatively using univariate and bivariate methods; and multivariate analysis was conducted by applying logistic regression test. Qualitative analysis was implemented, and content analysis was applied. Results of the study showed that perception on personal communication was not good (62%), perception on the advertising was not good (55%), perception on publicity and public relation were not good (63%); perception on teaching materials was not good (54%), perception on corporate design was not good (53%), and perception on patient satisfaction was not good (72%). Result of multivariate analysis showed that personal communication, advertising, publicity and public relation, teaching materials, corporate design, and patient satisfaction influenced the patient loyalty. The regression formula was Y= 0.871 + 2.145 communication personal + 1.879 advertising + 1.044 publicity and public relation + 1.758 teaching materials + 1.233 corporate design + 1.445 patient satisfaction. Personal communication had the strongest influence, publicity and public relation had the weakest influence to patient loyalty. It is suggested to the management of RSIA Kurnia Cilegon to improve midwives personal communication and sms information gateway.Keywords : promotional mix, customer loyalty, consumer behavior

Abstract

Deteksi dini gangguan jiwa di Puskesmas Banyuurip sebagai tindakan pencegahan dan intervensi dini faktor risiko. Deteksi dini diharapkan mengurangi prevalensi Orang Dengan Gangguan Jiwa (ODGJ) Kabupaten Purworejo yang menempati peringkat tertinggi di Riskesdas 2018 di Jawa Tengah. Pencapaian Puskesmas Banyuurip tahun 2019 adalah 33,02% pada usia produktif dan 80,6 % pada usia lanjut. Capaian tersebut menunjukkan adanya aspek-aspek yang perlu diperbaiki. Tujuan dari penelitian ini adalah mengevaluasi pelaksanaan deteksi dini gangguan jiwa melalui model CIPP. Metode penelitian ini menggunakan wawancara mendalam dan observasi. Penelitian ini merupakan penelitian kualitatif pendekatan deskriptif. Aspek evaluasi meliputi context, input, process dan product. Hasil penelitian pada evaluasi aspek context bahwa kebutuhan, masalah, dan sasaran deteksi dini gangguan jiwa menjadi latar belakang perencanaan. Evaluasi aspek input menunjukkan perencanaan cukup baik walaupun terdapat keterbatasan tenaga, dana, dan infrastruktur. Evaluasi aspek process bahwa pelaksanaan edukasi, rujukan, dan pelaporan kurang optimal dilaksanakan. Evaluasi aspek product dalam kriteria cukup baik dengan beberapa aspek perlu ditingkatkan. Antar aspek memiliki keterkaitan sehingga perbaikan satu aspek harus diikuti oleh aspek lainnya.

Abstract

Background: Treatment adherence among patients with severe mental disorders is needed to avoid drop-outs, relapses, and shackling. Kedungmundu Primary Health Center in Semarang City is known to have a low treatment adherence to patients with severe mental disorders. This study aimed to determine the factors associated with treatment adherence among patients with severe mental disorders.Methods: A quantitative study with a cross-sectional approach. The samples were 73 patients with severe mental disorders at The Kedungmundu Primary Health Center. Data were collected using questionnaires and analyzed using frequency distribution analysis and Spearman Rank Correlation Test.Results: The results indicated that knowledge (p = 0,000; r = 0,715), attitudes (p = 0,000; r = 0,839), family support (p = 0,000; r = 0,427), assessment of health workers (p = 0,000; r = 0,456), and the need for health services (p = 0,000; r = 0,716) have a positive correlation with treatment adherence among severe mental disorders patients.Conclusion: Kedungmundu Primary Health Center is suggested to provide psychoeducation to patients and families, improve the implementation of mental health programs, and empower mental health cadres.

Abstract

Background: Planning becomes the most important management function because it arranges all management activities. Non-compliance with the standards of the planning process has an impact on the quality of planning that is unable to bridge needs which ultimately lead to dissatisfaction. PHC as public organizations must meet public service standards. Organizational factors are determinants of organizational performance as they relate to the provision and arrangement of vaious resourcesAims: The purpose of study was to determine affect of organizational factors on compliance with the standards of PHC planning process.Methods: An observational study with cross sectional design. All 56 PHC in the 2 districts in Central Java province (Semarang and Kudus) became the samples. The respondents are officer in charge of the PHC planning programme.. Data collection by interviews using questionnaires and observations. Data were analyzed descriptively and statistically.Results: Only 55.4% of PHCs comply with standard planning process. Weaknesses and inconsistencies still occur at every stage, especially with regard to inaccuracy of time, incomplete and inappropriate data, and low understanding of members of the team. Study shows that the aspect of manpower, infrastructure, organizing and monitoring-evaluation function are positively correlated with adherence to standard planning processes, where monitoring-evaluation factors are the dominant factor.Conclusion: All the gaps identified as the cause of puskesmas non-compliance to the standard planning process can be overcome through increasing staff commitment and competency by providing structured managerial training and routine socialization. Strengthening the monitoring-evaluation function needs to be pursued consistently to ensure the quality of the puskesmas planning process and the implementation of the work plan produced

Abstract

Indonesia targeting that at least 70% of the population vaccinated and formed the herd immunity. However, there are several challenges in reaching that target, such as the anti-vaccine movements, disinformation or hoaxes, fear of the Adverse Event Following Immunization (AEFI), and fear of needles. Based on the preliminary study, the area of Bandarharjo Public Health Centre was in the lowest 3rd place related to the COVID-19 vaccination coverage in Semarang. This was due to lack of education to the community regarding the vaccination, the high resistance to vaccination due to concerns about AEFI, and so on. Health cadres are one of the main actors in the supporting the health programs in PHC, therefore the role of health cadres need to be optimized in education and management of AEFI related to COVID-19 vaccination. This community service program was carried out offline in 30 health cadres in the working area of Bandarharjo PHC. The activity began with a pre-test, continued with the presentation of the topic, discussion session, and ends with post-test. The results of the pre-test and post-test showed that there was an increase in the knowledge of cadres between before the presentation and after the presentation session. Health cadres also received booklets containing general information about COVID-18 vaccination and also related to AEFI. Booklets given to cadres not only to increase their knowledge but also can be used to educate the community. Hopefully, with the increase in knowledge and understanding of cadres, the community will be easily get the right information. The health cadres commitment to educate the community regarding COVID-19 vaccination also hoped to increase the community knowledge related to the vaccination and also the willingness of community to receive the COVID-19 vaccination, especially in the working area of Bandarharjo PHC, Semarang City, Central Java Province.

Abstract

Physical health problems that are often left unattended are often cases of mental illness and suicide. Early detection includes targeted prevention. Early detection of mental disorders is for the productive age and old age. Puskesmas Banyuurip carries out early detection of mental disorders in community-based mental health services. Implementation involves elements of the community. While planning by the Puskesmas. This research is a qualitative research with a descriptive approach with in-depth interview method. This research was conducted in February-April 2020. Informants were key informants and triangulation informants. The aspects analyzed include input, process, and product. The input aspect shows that there are limited staff and funds but this is an opportunity in planning, namely the development of cadres and the use of non-government funds. This aspect of the process shows a lack of implementation in education and referrals so there is a need for supporting facilities. The product aspect is responded to by a 100% target as a must. Achievements in 2019 will be an improvement in the implementation of the next period. Kader can be trained with early detection training on classification methods. Comprehensive education for all targets requires a cross-sectoral role. The Health Department can improve coordination with other sectors as well as the private sector.

Abstract

Problems regarding bad hygiene and healthy behavior (PHBS) are still common in elementary school age children. The results of health screening at primary schools in the Kedungmundu’s Community Health Center in 2018 indicate that there are still students who have poor nutritional status and are at risk of developing anemia. In addition, there was a decrease in level in 2018 to 28 UKS minimum level and 2 UKS standard level. This study aims to determine the factors associated with UKS level in the application of healthy schools at the elementary school level. This research is a quantitative study with a research design using cross sectional study. Samples were taken by means of a total population of 29 elementary schools. The results showed that the related variables were environmental conditions (p = 0,008), characteristics and capabilities of implementing agencies (p = 0,001). While the unrelated variable is the relationship between organizations (p = 0,061), organizational resources 0,272). From these results schools should be expected to form a UKS implementation team, increase the number of small doctors, complete the UKS facilities, make collaboration with relevant parties, and hold regular meetings held by the UKS implementation team.
