Abstract

Penyembuhan luka merupakan mekanisme pertahanan jaringan, dengan epitelisasi sebagai salah satu parameternya. Kulit jeruk manis memiliki potensi dalam penyembuhan luka karena mengandung nutrisi yang memiliki peran dalam mempercepat penyembuhan luka. Penelitian ini bertujuan untuk mengetahui efek aplikasi topikal gel ekstrak kulit jerukmanis terhadap epitelisasi pada proses penyembuhan luka gingiva labial tikus Sprague Dawley. Dua puluh tujuh ekor tikus Sprague Dawley, berumur 2-3 bulan, dibagi dalam 3 kelompok, kontrol positif, perlakuan, dan kontrol negatif. Perlukaan pada gingiva labial mandibula dibuat dengan punch biopsy berdiameter 2,5 mm. Luka pada kelompok kontrolpositif diaplikasikan Aloclair, kelompok perlakuan diaplikasikan gel ekstrak kulit jeruk manis 10%, dan kelompok kontrol negatif diaplikasikan CMC-Na, masing-masing 2 kali sehari selama 1 menit secara topikal. Tiga ekor tikus dari tiap kelompok didekapitasi masing-masing pada hari ke-3, 7, dan 14. Jaringan luka diambil dan dibuat sediaan histologis dengan pengecatan Hemaktosilin Eosin. Pengukuran ketebalan epitel dilakukan dengan menggunakan Optilab yang dipasang pada mikroskop cahaya. Data ketebalan epitel dianalisis dengan menggunakan ANAVA dua jalur. Hasil pengukuran ketebalan epitel menunjukkan perbedaan yang tidak signifikan pada kelompok kontrol positif dengan perlakuan (p>0,05), namun menunjukkan perbedaan yang signifikan pada kelompok perlakuan dengan kontrol negatif (p<0,05) pada hari 3, 7, dan 14 setelah perlukaan. Kesimpulan dari penelitian ini adalah aplikasi topikal gel ekstrak kulit jeruk manis mempercepat epitelisasi pada proses penyembuhan luka gingiva labial tikus Sprague Dawley. Effect of Topical Application of Sweet Orange (Citrus sinensis) Peel Extract Gel on Epithelialization of Labial Gingival Wound Healing: In Vivo Studies in Sprague Dawley Rats. Wound healing is a defense mechanism from complex biological phenomenon, in which epithelialization occurs as one of its parameters. Sweet orange peelcontains nutrients that have role in enhancing wound healing process. The objective of this research is to determine the effect of topical application of sweet orange peel extract gel on epithelialization of wound healing process in labial mandibular gingiva of Sprague Dawley rats. Twenty seven Sprague Dawley rats were divided into 3 groups: positivecontrol, treatment, and negative control. Labial mandibular gingival was wounded using 2.5 mm diameter punch biopsy. Topically, each wound of positive control group was administered Aloclair, treatment group was administered 10% sweet orange peel extract gel, and negative control group was administered CMC-Na, twice a day for 1 minute. Three rats from each group were sacrificed for histological evaluation at 3, 7, and 14 days and the specimens were stained with HE. The measurement of the epithelial thickness used Optilab installed on microscope. The data obtained from the measurement was analyzed using two-way ANOVA. There was insignificant difference between positive control group and treatment group (p>0.05) while there was significant difference between treatment group and negative control group (p<0.05) observed at 3, 7, and 14 days. The conclusion of this research is that the application of sweet orange peel extract gel accelerates epithelialization of wound healing process in labial mandibular gingiva of Sprague Dawley rats.

Abstract

Pendahuluan. Perawatan ortodontik terutama yang cekat sudah banyak digunakan, bahkan akhir-akihir ini tidak hanya untuk kepentingan perawatan maloklusi gigi, tapi juga untuk estetika. Namun banyak pasien mengeluh adanya peradangan, bau mulut bahkan sampai terjadinya periodontitis berat, sampai kegoyahan gigi. Tujuan penulisan. Untuk mengetahui dampak yang ditimbulkan dari pemakaian alat ortodontik terhadap kesehatan jaringan periodontal. Dampak tersebut ditinjau dari efek alat ortodontik yang dapat meningkatkan akumulasi plak, iritasi yang ditimbulkan pada pemakaian band dan braket pada alat ortodontik, juga akibat tekanan yang ditimbulkan pada jaringan periodontal. Kesimpulan. Untuk meminimalkan dampak pemakaian alat ortodontik tersebut, seharusnya ditekankan pentingnya plak kontrol, juga harus dipertimbangkan tekanan yang diberikan pada jaringan periodontal.  Introduction. Fixed orthodontic appliances is widely used, not only for treatment dental malacclusion, but also for aestetically. But many patient complain of gingival inflamation, severe periodontitis, event to luxation tooth. The aim of this review. Was to know the side effects of orthodontic appliances for periodontal healthy. Which was evaluated the effect of orthodontic appliances could increased the number of plaque retention, irritation of use appliances and also the pressure of this orthodontic appliances on periodontal tissues. The conclusion. To minimize the side effect of using orthodontic appliances, beside instruction for controle plaque, also consider the pressure exerted for periodontal tissues.

Abstract

Latar belakang. Warna gingiva normal, mencerminkan kesehatan mulut dan mempunyai peranan penting dalam kedokteran gigi estetik. Jika terdapat kelainan warna, seperti gingiva yang kehitam-hitaman yang sering dikeluhkan pasien, dapat merupakan suatu masalah walau bukan suatu masalah medis. Kadang-kadang pewarnaan gingiva ini diperparah dengan adanya mahkota gigi klinis yang pendek. Ada beberapa macam teknik depigmentasi untuk mengatasi masalah ini. Tujuan. Laporan kasus ini bertujuan menghilangkan hiperpigmentasi gingiva dengan menggunakan teknik sederhana dalam waktu minimal serta tidak terlalu sulit dilakukan tapi masih cukup memuaskan bagi pasien. Kasus. Pasien perempuan umur 18 tahun mengeluh gingiva berwarna gelap dan mahkota gigi tampak pendek yang mengganggu penampilan dan ingin diperbaiki. Diagnosis dari kasus ini adalah hiperpigmentasi gingiva dengan mahkota gigi klinis pendek. Pada kasus ini digunakan teknik bedah skalpel untuk mengkoreksi hiperpigmantasi gingiva dan mahkota gigi klinis pendek tersebut. Pasien sangat puas dengan penampilan gingivanya yang baru. Kesimpulan. Teknik bedah skalpel dapat mengatasi hiperpigmentasi gingiva dan mahkota gigi klinis pendek.

Abstract

Treatment of infrabony pocket makes use of bone graft material demineralized freeze dried bone allograft (DFDBA) from different individuals which has undergone demineralization process and it is osteoinductive. Enhancement of growth factor was done by adding platelet rich fibrin (PRF) and platelet rich plasma (PRP). PRP is activated with an addition of calcium chloride CaCl2) to form gel. The method used to apply the bone grafting material is open flap debridement OFD). This research aimed to reveal the differences in the results of infrabony pocket treatment using PRF and PRP gel with an addition of DFDBA. The sample was taken from 20 infrabony pocket points divided into 2 groups, 10 infrabony pocket were treated with OFD+DFDBA+PRF and the other were treated with OFD+DFDBA+PRP gel. Pocket depth (PD) was measured on the baseline and the first and third month after treatment. Alveolar bone height was measured using cone beam computed tomography (CBCT) radiograph on the baseline to the third month after treatment. The results of this research showed that there was difference in the results of infrabony pocket treatment using PRF and PRP gel with an addition of DFDBA which could be observed from a reduction in PD from the baseline, month 1 and month 3 as well as reduction in alveolar bone height from the baseline to month 3. This research concluded that infrabony pocket treatment PRF application yields better results than PRP gel application in terms of PD and alveolar bone height reduction.

Abstract

Guide tissue regeneration (GTR) is the treatment of infrabony pockets for soft and hard tissue regeneration. Membrane is used as a barrier and prevents apical migration of the cells in epithelial tissues. Membrane fixation is one of the procedures in GTR treatment because resorbed membrane is less stable. Simple sling suture technique for membrane fixation has 1 anchorage, located in coronal flap, while periosteal vertical mattress suture technique has 2 anchorages in periosteum, making it more stable. This study aimed to study the differences in the effectiveness of membrane fixation using periosteal vertical mattress suture and simple sling suture techniques in terms of probing depth, relative attachment loss, and alveolar bone height in the treatment of infrabony pockets. The samples were divided into 2 groups. The first group was open flap debridement (OFD) with demineralized freeze dried bone allograft (DFDBA) application and membrane fixation with simple sling suture, while the second group was OFD with DFDBA application and membrane fixation with periosteal vertical mattress suture, observed on day 0, 30th day, and 90th day. The results of the study showed significant differences in the probing depth and relative attachment loss (except from the 30th day to the 90th day), and there were no significant differences in the alveolar bone height from the baseline to the 90th day, between the group of membrane fixation using simple sling suture and that of periosteal vertical mattress suture techniques on flap surgery. This study concluded that membrane fixation in the treatment of infrabony pocket with periosteal vertical mattress suture technique is more effective in reducing the probing depth and relative attachment loss, but there is no difference in increasing the alveolar bone height when compared to simple sling suture technique.

Abstract

Periodontitis is periodontal inflammation in response to plaque bacterial antigens, causing damage to periodontal ligament and alveolar bone resorption. Bone graft material combination i.e. demineralized freeze-dried bone allograft (DFDBA) and hydroxyapatite (HA) using sandwich bone augmentation (SBA) method will support each other and will be beneficial to be used as a scaffold. The body takes long time to resorb HA so this could complement DFDBA which is more easily dissolved. This study aimed to reveal the effect of bone graft addition using SBA method on the treatment of infrabony pocket with open flap debridement in terms of probing depth, relative attachment loss, alveolar bone height, and density. This study was carried out to 20 infrabony pockets, where 10 of them were treated using open flap debridement with HA addition, while the other 10 groups were treated using open flap debridement with DFDBA and HA using SBA method. Probing depth  and relative attachment loss were measured on days 0, 30 and 90. Bone height and density were measured using cone-beam computed tomography (images on day 0 and 90). The study showed that probing depth reduction on SBA group was greater than HA group. There were significant differences in probing depth and relative attachment loss examinations. However, bone height and bone density reduction did not show any significant difference. The conclusion from this study is open flap debridement using SBA method yields better regeneration in terms of probing depth and relative attachment loss than open flap debridement with HA addition. There is no difference in bone height and bone density between the two groups.

Abstract

Platelet-rich plasma (PRP) contains growth factors that speed up a healing process. The liquid form of PRP is difficult to be applied, so it needs material as a carrier of collagen. Collagen can be used as carrier materials and capable to activate growth factors and increase the concentration of the PRP to become a gel. Collagen often losses its shape and size due to degradation when exposed to body fluids. In the selection of materials, collagen can synthesize with chitosan. Collagen-chitosan hydrogel has potential as scaffold. Many formulations for proper hydrogel applied in the oral cavity need to pay more attention to several conditions, such as a neutral pH, high viscosity and ideal swelling. The aims of this study was to examine the influence of various formulations of collagenchitosan hydrogel PRPagainst pH, viscosity, and swelling. The study samples consisted of 25 collagen-chitosanhydrogel formulations. The samples were divided into five groups: 100/0; 75/25; 50/50; 25/75; 0/100. The first stage of the test was pH, viscosity and swelling measurement of the collagen-chitosan hydrogel to determine which ones matched the criteria. Collagen-chitosan hydrogel formulations were added with PRP with a ratio of 1:1. The second stage was to measure the pH, viscosity, and swelling measurement to see the results of the physical parameters of the hydrogel after mixed with PRP. The observation data were analyzed by t-test for pH and viscosity, while twoway ANOVA and post hoc LSD for swelling test. The results showed that collagen-chitosan hydrogel formulations of 25/75 and 0/100 met the criteria of pH, viscosity, and swelling. The pH of both groups showed no significant difference (p>0.05), but the viscosity and swelling variables showed significant difference (p<0.05). To conclude, collagen-chitosan hydrogel formulations mixed with PRP had no effect on pH, but had an effect on viscosity and swelling test. The collagen-chitosan hydrogel formulations mixed with PRP which matched the criteria were 25/75 and 0/100.

Abstract

The minimally invasive surgical technique (MIST) is a method for periodontal regenerative treatment by minimizing trauma so it can preserve vascular supply of the interdental papillae. This study aims to determine the effectiveness of the MIST method with the addition of a combination of hydroxyapatite and β-tricalcium phosphate (HA + β-TCP) in treatment of infrabony pockets in terms of alveolar bone density and osteocalcin levels of gingival crevicular fluid. The study sample was taken from 20 teeth with infrabony pockets (n = 20), which were divided into two groups: 10 teeth treated with MIST and the other group with open flap debridement (OFD). Both groups received combination of HA + β-TCP. The osteocalcin levels of gingival crevicular fluid were checked on day-0 prior to the flap surgery, day-7 and day-14 after flap surgery using the Human Osteocalcin Elisa Kit. Radiological evaluation of alveolar bone density at day-0 and day-90 was done using cone beam computed tomography. Data of osteocalcin levels were analyzed using two-way ANOVA and continued with LSD Post Hoc test, while data of alveolar bone density reduction were analyzed using the Independent t-test parametric test. The results showed that there was no significant difference (p > 0.05) in the values of alveolar bone density between the MIST and OFD groups on day-0 and day-90, while the osteocalcin levels in both groups showed an increase from day-0 to day-7 and a decrease from day-7 to day-14. There was a significant difference (p < 0.05) on day-0 and 7 and day-7 and 14 between MIST and OFD groups. The MIST method with HA + β-TCP was effective and further increases alveolar bone density and osteocalcin levels of gingival crevicular fluid.

Abstract

Various biomaterials have been utilized as additional material to help tissue regeneration in the open flap debridement (OFD) procedure. Platelet-rich fibrin (PRF) is an autologous platelet concentrate that contains many growth factors (GFs) and generally used as an additional biomaterial in OFD. However, PRF has several weaknesses, such as its consistency, which is hard to mix with another biomaterial, and its abundant sediment of regenerative cells resulted from high-speed centrifugation. Injectable Platelet Rich Fibrin (I-PRF) is one of the PRF derivatives that contain more regenerative cells. Due to the low-speed centrifugation, I-PRF has a liquid form. Certain GFs were also found in I-PRF, such as PDGF, TGF-β1, VEGF, IGF, EGF dan COL-Ia. In this study, we measured probing depth (PD), relative attachment level (RAL), and bone height as the healing parameter, to see the effectiveness of I-PRF addition and bone graft in the OFD for infrabony pocket as the aim of this study. Twenty (20) periodontal pockets were chosen (PD= 5-7 mm) and divided into OFD+I-PRF+bone graft (bg) group (group I) and OFD+PRF+bg group (group II) equally. The clinical evaluation was measured at the baseline, day-30, and day-90. Results showed that PD (p= 0.022) and RAL (p= 0.008) significantly increased in group I (PD 2.80 ± 0.42; RAL 2.50 ± 0.52) compared to group II (PD 2.10 ± 0.73; RAL 2.00 ± 0.67); however, there was no significant difference in the bone height (p= 0.194). Taken together, our data revealed that I-PRF+bg induces soft tissue regeneration in infrabony pocket treatment.
