Abstract

Tumour necrosis factor alpha (TNF-α) is a cytokine produced by macrophages and other mononuclear cells, is a good antibacterial agent against Salmonella spp, especially Salmonella typhi. Centella asiatica is an alternative drug that is expected as an immunostimulant in patients with typhoid fever. Comparing the effectiveness of Centella asiatica extract the original Bali as an immunostimulant and without stimulants in mice infected Salmonella typhi in terms of TNF-α secretion. This study is an experimental study with a post test only with control group design. A total of 20 mice were divided into 4 groups. The first and second groups each given Centella asiatica extract 75 mg/20 g bw (0.5 cc) and without a given extract for 4 weeks. Both groups were inoculated orally Salmonella typhi 106 per mL of bacteria in the second week. The third and fourth groups were given thiamphenicol with Centella asiatica extract 75 mg/20 g bw (0.5 cc) and thiamphenicol without any extract for 4 weeks respectively. Both groups were inoculated orally Salmonella typhi 106 per mL of bacteria in the first day. All groups terminated on fourth week and examination levels of TNF-α by ELISA and gall culture. The mean levels of TNF-α in groups (1–4) is 86.10±2.67 pg/mL, 32.81±11.33 pg/mL, 35.87±3.90 pg/mL and 19.21±2.19 pg/mL respectively. Based on the examination of the gall cultures this study showed positive results in the first and second groups, while a negative result on the third and fourth groups. Based on the One way ANOVA analysis on levels of TNF-α, there are significant differences between the first group with the second group (p<0.05), and between the third and fourth groups also found significant differences (p<0.05) increased levels of TNF-α in mice with Salmonella typhi infection given Centella asiatica extract.

Abstract

Lipopolysaccharide (LPS) is an endotoxin from the outer membrane of Gram-negative bacteria, which has an important role in the occurrence of sepsis. Exposure to LPS will stimulate increase of nitric oxide (NO). Nitric oxide is a bioregulator of apoptosis and has some sepsis prognostic role of apoptosis regulators within the gastrointestinal cells. The objective of the study is to know if endotoxemia induces an increase in NO levels and histopathology scores as well as the existing relationship between them. This study is an observational intervention. The subjects were 48 male mice Balb/C, divided into 2 groups. The samples consisted of 24 tail as control group (group A) and 24 as treatment group (group B). The A group as well as the B group is divided into 4 subgroups according to the time of termination. The levels of NO were examined by Griess method. Histopathology score was examined by HE and read as a score of 0–5. There was a statistically significant difference between the mean NO in the treatment group with the control group at the termination of the group of 12h (p=0.009), 24h (p=0.015), 36h (p=0.014), 48h (p=0.002) and the whole group (p=0.0001), as well as between the mean histopathology score at the termination time of 12 h (p=0.0001), 24h (p=0.0001), 36h (p=0.0001), 48h (p=0465) and the whole group (p=0.0001). Increase in NO and histopathology scores in all groups of mice (r=0.527) showed a statistically significant correlation. NO levels and histopathology scores are increased during endotoxemia and thus have a significant correlation.

Abstract

Tuberculosis (TB) is an infection caused by Mycobacterium tuberculosis and health problem and cause of death worldwide. MDR-TB occurs in patients who are inadequate and irregular in treatment. WHO strategy for MDR-TB is taking anti-tuberculosis (OAT) which has the effect of second-line heavier. Ethionamide one second-line OAT is administered orally and has a side effect of hypothyroidism. A cross-sectional descriptive study was conducted on 18 patients with MDR-TB from July 2012–March 2013. One patient has been excluded due to hypothyroidism. Increased TSH levels exceeded the cut-off occurred in 2 of 4 people after 1 month of treatment, 3 of 3 people after the 5th month of treatment, 1 of 4 people after 6 months of treatment and 1 of 3 people after month 9 treatment. Only 1 person who had increased levels of TSH>10 μIU/ mL. Conclusion: in patients with MDR-TB evaluation is required to determine the levels of TSH hypothyroidism.

Abstract

Discrepancy of blood type is the difference result between cell grouping and serum grouping. The technical errors and various conditions are the common cause of their discrepancy, one of the conditions is mentioned as Auto Immune Haemolytic Anaemia (AIHA). Auto immune haemolytic anemia cold type can cause discrepancy due to cold autoantibody reaction. In this study the researchers conducted further testing to determine the actual blood type so the right blood transfusion can be given. A 41 years old woman with weakness and pale since a month before her admission to the present hospital, has been treated in a district hospital with the same condition. On the physical examination, the researchers found hepatomegaly. The complete blood count showed hemoglobin level was 2.1 g/dL and leukocyte count was 21.54×103/uL. The clinical diagnosis made by the emergency department was anemia pro evaluation and planned blood transfusion with 500 cc of PRC. The peripheral blood smear shown hemolytic anemia. The result of the initial blood type test on forward grouping were AB positive agglutination and on reverse grouping O positive, then followed by washing erythrocyte and the researchers found the similar result. Because of the extra antibody found in both examination, the examination was carried out at 37°C of the patient’s temperature. The researchers found that the blood type of AB rhesus positive with cold autoantibody. The indirect coombs test was positive. The patient is had been reported with ABO discrepancy cold type autoantibody (group IV) in autoimmune hemolytic anemia cold type. Further screening antibody tests are needed to confirm the specific antibody.

Abstract

Infectious diseases are still the main problem due to the potency of life threatening for neonates. Patients in the Neonatal Intensive Care Unit (NICU) are at risk for nosocomial infections resulting from the exposure to invasive procedures and medical equipments such as ventilator, catheter, Continuous Positive Airway Pressure (CPAP), incubator and antibiotics treatment. This study was aimed to know the related condition in NICU Dr. Wahidin Sudirohusodo Hospital Makassar by identification and determining the pathogenic bacteria colony counts of incubator and CPAP. The study design was a cross sectional method, by using incubator and CPAP as sample instruments, both equipments were sterilized at the NICU. The samples were collected and evaluated in July 2011. Samples were collected by usinga swab which was wiped to the instrument and put into Buffer Phosphate Saline (BPS); then diluted and put into Plate Count Agar (PCA) media for culture. The result of this study showed that there were no bacteria colony in the incubator, while in CPAP Acinetobacter calcoaceticus with bacteria colony count approximately 3.4×102 CFU/m3 were found. The researchers concluded that the incubator was sterile to bacteria, however in the CPAP bacteria from the strain of Acinetobacter calcoaceticus were found

Abstract

In glomerulonephritis there are intraglomerular inflammation, cell proliferation, and hematuria. Hematuria is characterized by more than 3 (three) erythrocytes per high-power field in the urine, which indicates the pathological processes in kidney or urinary tract. The combination of mechanical damage of erythrocyte membrane through the damaged glomerular basement membrane followed by the osmotic damage when it passes through the tubular system in the hypotonic osmotic solutions causes dysmorphic morphology. Erythrocytes trapped in the Tamm-Horsfall protein will form erythrocyte casts. Dysmorphic erythrocytes and or erythrocyte casts in the urine indicate glomerular hematuria. Various forms of dysmorphic erythrocytes in the urine can be found. Acanthocytes (G1-cells) are specific for glomerular hematury. The examination of these urinary sediments can be done natively or by using automated urinalysis analyzers.

Abstract

The incidence of metabolic syndrome (MetS) is significantly increasing. The elevated serum uric acid (SUA) level is correlated with the metabolic risk of cardiovascular. The relationship between SUA levels and components of MetS has been inconsistent. The aim of this study was to know the correlation between SUA level and component of MetS by analyzng. This study was a cross-sectional design conducted on healthy people at the Medical Check Up (MCU) Clinic, Dr. Moewardi Hospital, Surakarta from June 2012 until February 2013. MetS was defined according to the 2005 International Diabetes Federation (IDF) criteria. The statistical analysis was done by Student’s T test, Pearson or Spearman correlation coefficients. The researchers considered p<0.05 as statistically significant with a 95% confidence interval. A total of 80 healthy subjects, (range 19–57 years) were enrolled into this study. MetS was diagnosed in 40 persons where men (62.5%) were more than women and the non-MetS in women (62.5%) were more than men. In this study, a positive correlation was found between SUA and systolic blood pressure, diastolic blood pressure and waist circumference (Pearson r=0.354, 0.495, 0.399, respectively, p<0.05) in women, but not in men. The SUA was positively correlated with systolic blood pressure, diastolic blood pressure and waist circumference, particularly in women. A long term study is needed to investigate the consequence of increasing the SUA level in metabolic disorders and hormonal influences as well.

Abstract

Metastasis is the major cause of increased morbidity and mortality in breast cancer. Interleukin-8 has a contribution in the spread of the tumour cells. This study was aimed to know IL-8 levels in patients with metastasic and non metasatic breast cancer by analyzing. A cross sectional study was performed at the Dr. Wahidin Sudirohusodo, Ibnu Sina and Labuang Baji hospitals during September to December 2011. Laboratory examinations were done in the Research Centre of the Medical Faculty, University of Hasanuddin. Fiftysix (56) subjects with breast cancer were examined. The results showed that the levels of IL-8 in metastatic and non metastatic breast cancer were 13.73 pg/mL and 10.48 pg/mL respectively. Mann-Whitney test showed no significant difference of IL-8 level between them, (p=0.09). When the subjects were divided into early and advance stages, the mean levels of IL-8 were 8.035 pg/mL and 12.052 pg/mL, respectively, showing a significant difference by Mann-Whitney test (p=0.009). Based on this research, the level of IL-8 can be considered as a marker in predicting the prognosis of breast cancer. A further research in cohort studies which specific metastasis crawl method is advised by the researchers, these are suggested to allow the determination of the risk factors.

Abstract

The transfusion of thrombocyte concentrate is one of the important medical approaches to make the platelet count raised in patients with thrombocytopenia. The TC could change during the storage process, so the in vitro storaging should be considered to minimalize the alterations on the platelet count. The study was aimed to know the impact of storage time to maintain the platelet count stability in TC of transfusion blood by evaluating it. A study with Time Series design was performed in 30 samples of TC of transfusion blood collected from 30 blood donors. The samples was collected from the bag tube of TC and the test is performed as soon as possible by using haematological analyzer (Sysmex KX-21) with an impedance method, the rest of the samples was stored in an agitator on 22±2°C to perform a repeated count on day 5th and 7th. The collected data was analyzed with “Paired T Test”. The results of this study showed that the platelet count in TC of transfusion blood was decreased in the fifth day compared to the first day, but the decreased matter is insignificant, statistically (p=0.13). While after seventh (7) day, the platelet count is decreased significantly (p=0.00). The researchers concluded that the storage of TC for seven (7) days could not maintain the stability of the platelet count of the transfusion blood. There fore is suggested, a further study to evaluate the impact of storage time on platelets viability. So the quality of platelets in TC of transfusion blood can be evaluated.

Abstract

Stem cells have a remarkable potential to act as a self renewal in the related system. The proliferation of stem cells can be stimulated by Platelet Rich Plasma (PRP) growth factor. PRP has a potential application for aesthetics, traumatology and maxillofacial surgery. The optimation method could be carried out by a double centrifugation, which is reliable and simple in producing an optimal PRP. The samples were obtained from healthy volunteers; 10 mL of blood was needed to produce 1 mL PRP. CBC test was then performed, in order to know the platelet count before and after double centrifugation. This method used four variations of speed and time. Four variations of speed and time were used to find the optimation result, which yield the highest platelet count and the highest PDGF level released after platelet activation. The measurement of Platelet Derived Growth Factor (PDGF-BB) level was done using ELISA method. Twenty first samples, resulted in a variation of platelet counts, the mean was 0.89 times. About 60% of platelet counts showed a decrease with the lowest value 0.01 times and 40% of the platelet counts showed an increase with the highest value 3.94 times. The study was repeated using “I” tubes, the highest increase of platelet count and PDGF-BB level was obtained by centrifugation at 900 g, 5 minutes duration, then followed by 1500 g, for 15 minutes. In this study protocol, the production of optimal PRP was not yet found, however it provided some important information. In this case, the influence of separation process and skill was more important than the centrifugation speed. The researchers suggested that the measurement of PDGF-BB level should be done immediately after PRP harvest.
