Abstract

Background: Genu osteoarthritis (OA) is the most common arthritis disease caused bya degenerative joint process that causes knee pain and functional disorders. One of therecommended therapies for the treatment of OA genu from several studies ishydrotherapy exercises. Hydrotherapy exercises can reduce pain and improve thepatient's quality of life. This study aims to determine the effect of hydrotherapyexercises on pain intensity and functional ability in OA genu patients. Methods: Thisstudy is a quasi-experimental study with a one group pretest-posttest design conductedby the Medical Rehabilitation Installation of dr. Mohammad Hoesin Palembang duringthe month of October-November 2019. Primary data was collected using interviews toassess pain intensity based on the Numerical Paon Rating Scale (NPRS) score andfunctional ability based on the WOMAC (Western Ontario and McMaster UniversitiesOsteoarthritis Index) questionnaire before and after training. hydrotherapy for 4 weeksfor one therapy a week. The data was carried out by the Shapiro-Wilk normality test,then analyzed using the paired T-test or Wilcoxon test using the SPSS tool. Results: Asmany as 31 study subjects, the results showed that there was a significant effect beforeand after hydrotherapy exercises on reducing pain intensity (p = 0.000) and improvingfunctional ability (p = 0.000) in patients with OA genu for 4 weeks. Conclusion: Thereis an effect of hydrotherapy exercise on pain intensity and functional ability in genuosteroarthritis (OA) patients..

Abstract

In Indonesia, the cases of cervical cancer are estimated at around 50 per 100.000 people. It was estimatedthattherearemore than1 millionwomenworldwidewho have cervical cancer,andmostofthemhave not been diagnosed yet or do not have access to screening and medical treatment. P53 codon 72 polymorphism can affect the risk of cervical cancerthrough the regulationofproliferationandcellapoptosis.The purpose of this research was to investigate the association between p53 codon 72 polymorphism and cases of cervical cancer. This research was observational analytic research. The research was done by examining in the laboratory of Molecular Biology, Faculty of Medicine, Universitas Sriwijaya by using a case-control study approach.Thesampleoftheresearchconsistedof 70subjects,andtheyweredividedintotwogroups:35in case group and 35 in the control group. The determination of genotype and allotype was done by using PCR-RFLP technics. The results of the research showed there was a significant difference between p53 codon 72 polymorphism between the case group and control group. The results of genotypes of p53 codon 72 polymorphism in the case group were seven respondents (20.0%) with Pro/Pro genotype, five respondents (14.3%) with Arg/Arg genotype, and 23 respondents (65.7%) with Pro/Arg. While in control group, there were 28 respondents (80.0%) with Pro/Pro genotype, 0 respondent (0.0%) with Arg/Arg genotype, and 7 respondents (20%) with Pro/Arg. The frequency of the Prolin allele in case group was 37 (52.9%), and the Arginin allele was 33 (47.1%), while the frequency of the Prolin allele in the control group was 63 (90%), and the Arginin allele was 7 (10%).TheChi-Squareofgenotypewas valued0.000,OR1,304andCI95%1,071-5,891,whiletheallelewas valued 0.000,OR8.027 andCI95% 3.228-19.962. Abstract

Miss AN, female, 17 years old, Muslim, Betawi ethnicity, graduated from elementary school, currently no longer in school. The first febrile seizure patient was four years old. Five to seven years old, patients with recurrent seizures without fever, diagnosed with epilepsy and do not see a doctor regularly. At 14 years, the patient was raped, and since then, the patient has had free sex with many different men. At the age of,16, the patient had seizures up to five times an ay, and one seizure was approximately five minutes. The patient went to a neurologist and was regularly treated with valproic acid and carbamazepine. At the age of 17 years, the patient often got angry when teased and threatened with knives or scissors, had free sex with many men and went to a psychiatrist. The patient has been treated five times in different mental hospitals because he often runs away from home. Currently, the patient is being treated in the psychiatric ward and is receiving 2 x 500 mg of valproic acid, 3 x 200 mg of carbamazepine, 2 x 0.5 mg of Clonazepam and 1 x 50 mg of sertraline. At the time of treatment, the patient is known to be HIV positive and does not need ARV therapy. Patients also planned behavioural therapy and involved patients with activities at Yayasan Pelita Ilmu. It is hoped that this therapy can help overcome the patient's behavioural and emotional problems.

Abstract

The existence of limitations for children with mental retardation causes an inability to protect themselves so that they often become victims of sexual harassment. Preventive efforts are something that needs to be improved for prevention, considering the worse risks to children. The importance of an effort to prevent sexual abuse of children with mental retardation in connection with the lack of knowledge about sexual abuse and the inability of children to face the dangers that threaten themselves. From the results of research conducted in the United States in 1994, nearly 90% of children with mental retardation disorders had experienced sexual abuse, as many as 49% had experienced sexual abuse more than ten times. The high incidence of sexual abuse experienced by children with mental retardation is due to various factors, namely the condition of children's limitations, lack of supervision and protection of children by parents and the environment. Efforts that can be made are increasing knowledge about sexual abuse and self-protection skills through the Behavioral Skills Training Group Program, where children are trained to recognize threatening objects and situations (sexual abuse). Increasing knowledge about sexual abuse and providing training (self-protection skills) is a significant primary preventive measure to reduce the risk of sexual abuse in children with mental retardation.

Abstract

Bipolar disorder in children and adolescents is a clinical disorder that causes public mental health problems that need attention. In the last decade, bipolar disorder in children and adolescents has become a trendy field, both in the clinical area and in research, especially in terms of diagnosis, which is still controversial. The controversy that remains is whether it is possible to diagnose bipolar disorder in prepubertal children. Based on the DSM-IV-TR diagnostic criteria, the prevalence of the bipolar disorder in children scarce rare. Epidemiological studies report the lifetime prevalence of bipolar I and II disorders in late adolescence is about 1 per cent. Various studies in a large population have shown a prevalence rate of 0.1% -2%. The onset of bipolar disorder in children and adolescents is often accompanied by a more severe disease course, compared to bipolar disorder with onset in adulthood. This case report presents a case of bipolar 1 in children accompanied by comprehensive management.

Abstract

Pemphigus Vulgaris (PV) is an autoimmune disease characterized by vesicles and bullae on the skin and mucosa resulting from an autoantibody reaction to desmosomal adhesion molecules desmoglein (Dsg) 1 and 3, which function as strong adhesions between keratinocytes. Pemphigus Vulgaris is more common in the fifth and sixth decades of age. The lesions are fragile blisters on the mucosa and skin. Diagnosing PV requires anamnesis, physical examination, and investigations such as histopathologic examination and direct immunofluorescence. Therapy generally uses steroids. Adjuvant treatment is given to reduce the side effects of corticosteroids. We reported a male, 59 years old, was treated with blisters that break easily into blisters on the head, face, chest, back, groin and buttocks accompanied by burning and itching. In the scalp, facial, anterior et posterior trunk, inguinal, and gluteal regions, multiple erythematous macules were found with lenticular-plaque shape; it was partially confluent with thick brown crusts which were challenging to remove. The histopathologic examination found the presence of suprabasal bullae with lymphocyte inflammation cells. The patient was diagnosed with PV and treated with corticosteroids with the sparing agent mycophenolate sodium and showed clinical improvement. The first-line treatment for pemphigus Vulgaris is systemic corticosteroids. Adjuvant sparing agent therapy is given to reduce the side effects of corticosteroids. Sodium mycophenolate sparing agent was selected because of its minimum side effects. In systemic management, the dose of corticosteroid and sparing agent mycophenolate sodium was gradually decreased. The patient experienced initial remission after treatment

Abstract

Zenker Diverticulum is a multifactorial disorder of the Killian’s triangle in the hypopharynx characterized by the main symptoms of dysphagia affecting the seventh and eighth decades of age and rarely under 40 years of age. The existence of factors that cause an increase in the intraluminal pressure of the pharynx will lead to the formation of a pocket located in the Killian’s triangle, an area with weaker tone of the hypopharyngeal wall and a decrease in relative pressure in the retropharyngeal space. Zenker Diverticulum was first described by Ludlow in 1769, who reported abnormal dilatation of the posterior pharyngeal wall on postmortem examination of a patient who complained a lifelong dysphagia. Zenker Diverticulum is thought to be associated with increasing age in relation to motor muscle coordination for swallowing functions and GERD (gastroesophageal reflux disease). Symptoms include dysphagia, regurgitation, malnutrition, and recurrent aspiration pneumonia. Management includes invasive surgery and non-invasive endoscopy.
