Abstract

Intention of teenager in consuming milk at milk cafe of Sleman  YogyakartaPurposeThis study aimed to explore the intentions of adolescents who come into the cafe milk in Sleman.MethodsThis study used a qualitative method with an exploratory and phenomenological design, with purposive sampling by considering the prescribed criteria. Informants of this study were thirteen informants. They consisted of nine adolescents, one manager of the cafe, two employees cafe and one academic nutritionist. Data collection was done from March-May 2015 by observation and interviews.ResultsResults showed that intention of adolescents who come to the cafe were for refreshments, using the facilities such as wifi (coursework), and for an organization's activities (meetings). Adolescent attitudes were positive and support the cafe milk and milk consumption behavior. Subjective norms included among others that adolescents wanted to socialize, to be recognized, to share information and build closer relationships with their friends.ConclusionThe health aspect is not the main intention of adolescents to come to the cafe milk. Adolescents come to the cafe milk with different intentions. Adolescents consume the milk in milk cafe because it has many benefits. However, the health benefits were only the last intention. The consumer intention was influenced by the positive attitudes of adolescents towards the cafe milk which includes media promotion of health and the subjective norm of adolescents toward the consumption of milk in the milk cafe.

Abstract

Analysis of 5-year survival of patients treated for breast cancer at Sardjito Hospital in Yogyakarta province, IndonesiaPurposeThis study aimed to assess the length of life of breast cancer patients as well as factors related to the prognosis of survival of patients.MethodsThis research was a retrospective study. Samples in this study were patients with breast cancer who were first diagnosed with breast cancer from January 1, 2009 until December 31, 2009 at the Dr. Sardjito Hospital, Yogyakarta. The Kaplan Meier method was used for the data analysis, and final factors were reviewed and those that showed significant association were entered in a Cox regression model. Calculations were performed in Stata 12.0.ResultsResults showed the 5 year survival rate was 51.07%. In bivariate analysis, there was a correlation between the age of diagnosis, education, clinical stage, tumor size and tumor location on 5-year survival in patients with breast cancer. For multivariate analysis, age of diagnosis had the most powerful correlation (HR = 3.73; 95% CI = 1.0 to 13.6) on survival (p = 0.046).ConclusionSurvival rates of women with breast cancer aged less than 50 years were lower. Young women with breast cancer tended to have more aggressive breast cancer growth and recurrence risks were greater. Efforts are needed for early detection of breast cancer to improve survival in breast cancer patients.

Abstract

Seroprevalence of human toxoplasmosis in Gianyar Bali provincePurposeThe present study was designed to determine the seroprevalence and risk factors of human toxoplasmosis.MethodsThe research used a cross-sectional design and was conducted in Gianyar District, Bali Province. A total of 240 sera samples were collected and examined by indirect ELISA methods for IgG Anti-T. gondii. Demographic characteristics and risk factors were collected by interviews using a structured questionnaire. Data were analysed using the Poisson regression test with robust variance estimators.ResultsSeroprevalence of human toxoplasmosis in Gianyar District were 56.7%. Multivariate analysis showed that there was an association between meal of raw meat/Lawar (aPR: 1.40; 95%CI: 1.05-1.86); Male (aPR1:42; 95%CI: 1.12-1.81); existence of animals/wild cats (aPR: 1.48; 95%CI: 1.04-2.09); often washing their hand (aPR: 1.25; 95% CI: 1.01-1.56); elevation (aPR: 0.99; 95%CI: 0.99-0.99); and distance to the river from the house (aPR: 0.99; 95%CI: 0.99 to 0.99).ConclusionThere was found high seropositive toxoplasmosis in Gianyar District. Patients with seropositive toxoplasmosis  should receive treatment and health information. There should be continued prevention efforts to increase health knowledge about toxoplasmosis and transmission.

Abstract

Intervention on midwife behavior about early initiation of breastfeeding practice using short message servicePurposeThis research examined the effects of an intervention by delivering short message service towards midwife behavior.MethodsThis research was a quasi-experimental study with pre and post test design using control group. A total of 64 midwives in Palembang participated until data extraction. About 18 themes of short messages were delivered through participants’ mobile phone. Data were analyzed by univariate, bivariate, and linear regression tests.ResultsResults showed that the intervention improved early breastfeeding perceptions by about 7.3% (p= 0.0308). Participants’ knowledge was increased after intervention although there was no significant mean difference between groups. Almost all participants (over 80%) practiced imperfect early breastfeeding, especially in giving information since antenatal care, short-duration, and directing baby for sucking mother’s nipple.ConclusionNot all of participants facilitated early breastfeeding in every birth because of other factors such as unsupported mother and infant’s condition. Early initiation of breastfeeding is not easily implemented by health staff. It is better that midwives could motivate mothers to breastfeed within 1 hour since antenatal care. The important keys are having high commitment and confidence which contribute to midwives’ belief for practicing every step in every birth. We concluded that this research should be continued for long-term-analysis to give more significant results.

Abstract

Individual factors as predictors of diabetes mellitus in SalatigaPurposeThe purpose of this study was to determine the relationship of the individual factors with the incidence of diabetes mellitus in Salatiga.MethodsThe research was a case control study conducted from April to June 2016 around the existing health centers in the Salatiga. The total sample was 156 respondents. Data analysis used McNemar’s Chi-square and multiple logistic regression conditional tests.ResultsThe variables associated with the incidence of diabetes mellitus was a family history of diabetes. People who have a family history of diabetes have a risk 41 times more likely to develop diabetes mellitus compared with people who do not have a family history of diabetes mellitus in Salatiga.ConclusionThis study concluded that the incidence of diabetes mellitus is affected by family history. Primary health care is expected to cooperate with health office to conduct prevention efforts such as activities that encourage physical activity.

Abstract

Latar belakang: Penyakit demam berdarah dengue adalah penyakit yang disebabkan oleh virus dan ditularkan lewat nyamuk dan merupakan salah satu masalah kesehatan masyarakat di Indonesia, yang cenderung semakin luas penyebarannya sejalan dengan meningkatnya mobilitas dan kepadatan penduduk. Provinsi dengan IR demam berdarah dengue tertinggi tahun 2013 yaitu Bali sebesar 168,48, DKI Jakarta sebesar 104,04, dan DI Yogyakarta sebesar 95,99 per 100.000 penduduk. Selama dua tahun terakhir, kasus demam berdarah dengue tertinggi terdapat di Kabupaten Bantul yakni pada tahun 2014 sebanyak 622 kasus dan tahun 2015 sebanyak 1417 kasus. Salah satu kecamatan yang endemis demam berdarah dengueyaitu Kecamatan Banguntapan. Berdasarkan data Dinas Kesehatan Kabupaten Bantul, terjadi peningkatan kasus sebanyak tiga kali lipat dari tahun 2014 ke tahun 2015 yakni 93 kasus menjadi 282 kasus. Determinan penyakit demam berdarah dengue adalah sanitasi lingkungan dan keberadaan jentik Aedes Sp. Penelitian ini bertujuan untuk menganalisis hubungan antara sanitasi lingkungan dan keberadaan jentik Aedes Sp. dengan penyakit demam berdarah dengue di Kecamatan Banguntapan Kabupaten Bantul.Metode: Penelitian ini merupakan jenis penelitian observasional dengan rancangan case control yang mengkaji hubungan antara efek tertentu dengan faktor risiko. Jumlah sampel pada penelitian adalah 52 kelompok kasus dan 52 kelompok kontrol. Analisis data dilakukan dengan uji chi-square α = 0,05.Hasil: Hasil analisis bivariat didapatkan pengelolaan sampah padat (p=0,002 ; OR=3,73; CI 1,46-9,76), TPA (p=0,003; OR=3,6; CI 1,38-9,70), pengurasan TPA (p=0,001; OR=4,09; CI 1,54-11,4), keberadaan jentik Aedes sp (p=0,000; OR=4,42; CI 1,66-12,31), keberadaan jentik di dalam rumah (p=0,019; OR=3,12; CI 1,07-9,84), keberadaan jentik di luar rumah (p=0,038; OR=2,67; CI 1,94-7,96), keberadaan breeding place di luar rumah (p=0,005; OR=3,50; CI 1,31-9,81) berhubungan secara statistik terhadap kejadian demam berdarah dengue. Hasil analisis multivariat diperoleh variabel keberadaan jentik di luar rumah (p=0,020; OR=17,29; CI 1,57-190,07).Kesimpulan: Faktor risiko yang paling berhubungan dengan kejadian demam berdarah dengue adalah keberadaan jentik Aedes sp. di luar rumah.

Abstract

Latar belakang: Penggunaan nevirapine (NVP) dan efavirenz (EFV) sebagai basis terapi antiretroviral memiliki efikasi klinis yang sama, namun berbeda efikasi virologi. Kedua obat tersebut dinyatakan mampu menekan jumlah virus dan meningkatkan jumlah CD4 pada pasien terinfeksi HIV, namun tidak diketahui manakah dari kedua obat tersebut yang memiliki respon imunologi yang lebih baik untuk meningkatkan jumlah CD4. Tujuan: Penelitian ini membandingkan respon peningkatan jumlah CD4 menggunakan terapi antiretroviral (ART) berbasis NVP dan EFV pada pasien terinfeksi HIV. Metode: Penelitian dengan desain retrospective cohort study. Data penelitian diperoleh dari rekam medis pasien terinfeksi HIV di RSUD Dr. Haji Moch. Ansari Saleh Banjarmasin, dari Desember 2004 sampai Januari 2015. Pasien dianalisis adalah pasien yang memenuhi kriteria inklusi dan eksklusi yang telah ditetapkan. Dilakukan pengukuran peningkatan jumlah CD4 ≥25%. Hasil: Seratus limabelas pasien (115) memenuhi kriteria inklusi dan eksklusi, terbagi atas 66 pasien menerima ART berbasis NVP (47 pasien mengalami peningkatan CD4 ≥25%, 1 pasien meninggal dunia), dan 49 pasien menerima ART berbasis EFV (33 pasien mengalami peningkatan jumlah CD4 ≥25%). RR ART berbasis NVP terhadap ARV berbasis EFV adalah 1,08 (CI 95% 0,85-1,38; p>0,05). Rerata peningkatan CD4 dari baseline pada masing-masing kelompok berbeda signifikan (p<0,05), tetapi perbandingan rerata perubahan absolut CD4 pada kedua kelompok tidak berbeda signifikan (p>0,05). Waktu yang dibutuhkan untuk meningkatkan jumlah CD4 ≥25% adalah 12 bulan setelah terapi menggunakan basis NVP (CI 95% 6,62-17,38), dan 11 bulan setelah terapi menggunakan basis EFV (CI 95% 8,50-13,50). Tidak terdapat perbedaan bermakna pada waktu yang dibutuhkan meningkatkan jumlah CD4 ≥25% (p>0,05). Kesimpulan: Baik penggunaan ART berbasis NVP, ataupun berbasis EFV mampu meningkatkan jumlah CD4 dari baseline secara siginifikan, namun tidak terdapat perbedaan peningkatan rerata jumlah CD4 secara bermakna antara kedua kelompok basis terapi ARV.

Abstract

The effect of lactation counseling towards mother’s knowledge, ability and success rate in breastfeedingPurposeThis study aimed to identify the effect of lactation counseling towards mother’s knowledge, ability, and success in breastfeeding in Panembahan Senopati General Hospital, Bantul.MethodsThis research was a quasi-experimental study with a post test-only non-equivalent control group design. Sample collection technique used consecutive sampling with 32 respondents as total samples in each group (intervention group and control group). ResultsBivariate analysis used chi square tests with significance level p<0.05.  The intervention group was given lactation counseling (p = 0.000 [<0.05]). There was a significant difference in mother’s ability to breastfeed between the intervention group and control group after they were given lactation counseling (p=0.012 [<0.05]; RR=1.917). There was also a significant difference in the success rate of breastfeeding between the intervention and control group after counseling (p= 0.006 [<0.05]; RR=2.500).ConclusionLactation counseling can positively affect the mother’s knowledge, ability, and success rate in breastfeeding.

Abstract

Differences in the number of germs in the palms of nurses according to the level of knowledge and compliance of handwashing in private hospitalsPurposeThis study aimed to measure the difference of germs on the hands of nurses according to level of knowledge and compliance of handwashing in PKU Muhammadiyah Hospital of Yogyakarta.MethodsThis study used observational analytic method with a cross sectional study design. The population in this research was nurses who work in PKU Muhammadiyah Hospital of Yogyakarta totalling 68 people. Measurements used questionnaires and direct observation of handwashing, as well as direct sampling for examination of the number of germs in the palms of nurses.ResultsThere was no difference in the number of germs in the palms of the nurses with knowledge level (p = 0.541), and individual characteristics (p > 0.05). There was a difference in the number of germs in the nurse's hands (p = 0.000) with adherence. The result of observation showed that the lack of compliance of hand washing was due to the lack of awareness of nurses about handwashing.ConclusionThere are differences of germs in the hands of nurses with compliance with handwashing. Therefore, it is necessary to check and supervise handwashing routinely in order to increase awareness of the importance of handwashing by health personnel especially nurses.
