Abstract

Background : Increased TB prevalence with and without diabetes mellitus affects the quality of life of the patients. Appropriate action on TB patients was needed in health status improvement and quality of life of the patients. SF-36 questionnaire as an instrument to assess the quality of life. Aim : To compare Quality of Life (QoL) between TB with diabetes mellitus patients and TB without diabetes mellitus patients before and after the treatment of tuberculosis. Methods : A cross-sectional design was performed in this study by collecting data from the medical record of patients in Kariadi Hospital Semarang who were diagnosed with tuberculosis. Research subjects were 44 people with 22 TB with diabetes mellitus patients and 22 TB without diabetes mellitus patients. The direct approach was carried out using the SF-36 questionnaire. The SF-36 questionnaire was filled out using telephone interview. All questions were scored on a scale from 0 to 100, with 100 represents the highest level of function with interpretation the higher the score the higher the level of function, and vice versa. Statistical tests use Wilcoxon Test and Independent Samples T test. Results : Wilcoxon test on quality of life of TB patients with and without diabetes mellitus before and after treatment of tuberculosis was significant (p = 0.0040). Mean improvement in quality of life difference before and after treatment of tuberculosis in TB without diabetes mellitus patients (p = 41.72) was more significant than TB with diabetes mellitus patients (p = 26.38). Conclusion : There was a significant difference in quality of life between TB with and without diabetes mellitus patients before and after the treatment of tuberculosis as indicated by the SF-36 questionnaire score. TB without diabetes mellitus patients have more significant changes in quality of life compared to TB with diabetes mellitus patients.

Abstract

Background: Acrylic resin was a material of denture base which frequently used, but it still had a weakness that it was porous, then, it was often the breeding ground of Candida albicans. Virgin coconut oil (VCO) contained fatty acid which was effective to hamper the growth of C.albicans. Objective: This research was to identify the effect of VCO on the growth of C.albicans on acrylic resin denture. Methods: This research was an experimental laboratory research which exerted post-test control group design. The total sample 24 which were divided into 3 groups of treatment and 1 group of control. The treatment covered acrylic plates soaking which have been contaminated with C.albicans suspension into VCO in concentration level 25%, 50%, 75%, and sterile aquades for 8 hours. The acrylic plate was vibrated to knock out C.albicans and was then continued by colony counting on the media of Sabouraud Dextrose Agar (SDA). The statistic test employed Kruskal-Wallis and continued to the Post-Hoc test which employed Mann-Whitney. Findings: The VCO in concentration level 25%, 50%, and 75% affected to inhibit the growth of C.albicans. In the Kruskal-Wallis test, it was found a significant difference between the number of C.albicans on the control group and the VCO in concentration level 25%, 50%, and 75% (p<0,05). The Post-Hoc Mann-Whitney test indicated no significant difference between VCO in concentration level 25% and 50%, 25% and 75%, also 50% and 75% (p>0,05). Conclusion: The VCO in concentration level 25%, 50%, and 75% affected was able to hamper the growth of C.albicans. Further, there was no significant difference in each VCO concentration.

Abstract

Background: Dengue viral infection can infect people of all ages, including infants less than 1-year-old. The Disease severity of dengue infection consists of asymptomatic infection and symptomatic infection. The symptomatic infection has varying degrees of severity: undifferentiated fever, dengue fever, DHF, and Dengue Shock Syndrome (DSS). Method: The data obtained by interviewing the patients' parents for the symptoms and patients' medical records for information on the treatments and examinations.  Case: A 7 months old baby arrived at the hospital with a continuous abrupt 3 days fever, the condition is accompanied by watery stool two times a day, vomiting once a day.  Discussion: The patient arrived at the hospital with a continuous abrupt 3-day fever. The results of the physical examination showed a positive tourniquet test and palpable liver. The laboratory test results showed that the patient has thrombocytopenia, and the chest x-ray results showed that the patient has pleural effusion. Conclusions: The patient was diagnosed with DHF without shock and sent home after the patient got fluid therapy, supportive, and symptomatic treatments.

Abstract

Colorectal cancer (CRC) is the development of cancer from the colon or rectum. There are two kinds of surgery which are laparotomy and laparoscopy. This course of action had a significant effect on the length of stay, surgical site infection, and intraperitoneal adhesion. The purpose of this study is to find out the comparison between laparotomy and colorectal cancer laparoscopy with the length of stay, surgical site infection and intraperitoneal adhesion. The researcher used a cross sectional design. The sample for this study was taken from the medical record data of colorectal cancer patients in Dr. Kariadi Semarang from 2013 to 2018. The data were collected using purposive sampling technique. There were 57 samples that included inclusion and exclusion criteria with 28 details of laparoscopy and 29 samples of laparotomy. Numeric-scale data were tested for normality of data by Kolmogorov-Smirnov and found abnormal data using the Mann Whitney test. Nominal scale data were processed using the non-parametric Chi-Square test and it significant if p<0.05. There is a difference in length of stay and frequency of occurrence of surgical site infections and intraperitoneal adhesion between laparotomy and laparoscopy.

Abstract

Background: Inappropriate use of antibiotics not only cause resistance, but also indirectly increase morbidity, mortality, and prolong the length of stay, which can lead to increased health care costs. Pneumonia is one of the infectious diseases with a high prevalence in Indonesia. It requires various antibiotic treatment that makes the antibiotic selection need to be considered in both clinical terms and cost-effectiveness. Aim: This study aimed to determine the association between the quality of antibiotic and cost-effectiveness in hospitalized pneumonia patients at Diponegoro National Hospital Semarang in the period 2015 - 2019. Methods: A cross-sectional study was performed in fifteen patients with community-acquired pneumonia (CAP). The medical records as samples were retrospectively obtained by total sampling method. The categories of antibiotic quality (appropriate and inappropriate) were determined according to the Gyssens criteria and analyzed using a cost-effectiveness table. The outcome measure was length of stay (LOS).  Results : Mean LOS, mean antibiotic cost, and median total direct hospital cost of appropriate and inappropriate antibiotic quality respectively 3,27 days versus 6 days (p = 0.275), 123,302 IDR versus 286,000 IDR (p = 0.020), and 3,317,100 IDR versus 3,878,600 IDR (p = 0,602). Based on the cost-effectiveness table, the position of appropriate antibiotic quality was in column G (dominant), while the position of inappropriate antibiotic quality was in column C (dominated).  Conclusions: The appropriate usage of antibiotics was more cost-effective than inappropriate. There was a significant differences of LOS between both categories.

Abstract

Background: Oxidative stress is a condition which were free radicals harm biological molecules, like lipid, protein, and DNA. Consume paracetamol can increase oxidative stress, which can cause increased level of AST and ALT serum. Longan pericarp (Dimocarpus longan L.) is one of natural ingredients that contain phenolic and flavonoids as an antioxidant to prevent oxidative stress. This study aims to prove the effect of longan pericarp (Dimocarpus longan L.) on AST and ALT serum levels of Wistar rats induced paracetamol. Methods: Experimental research with post-test only control group design. Thirty males wistar aged 2-3 months weighing 140-250 grams randomly divided into 5 groups. Group K1 was normal control group. Group K2 was induced by paracetamol 2000 mg/kgBW. Group P1, P2, P3 were induced by paracetamol 2000 mg/kgBW administered by longan shell extract (Dimocarpus longan L.) in multilevel dose 5 mg/200 gBW, 10 mg/200 gBW, and 15 mg/200 gBW. Blood samples in each group were collected to measure AST and ALT serum levels of wistar rats. Results: There were signifficant differences groups of AST serum. There were signifficant difference groups of K1-K2 (p=0,000), K2-P2 (p=0,041), and K2-P3 (p=0,010). There were signifficant differences groups of ALT serum. There were signifficant differences groups of K1-K2 (p=0,001), K2-P2 (p=0,032), and K2-P3 (p=0,006).  Conclusions:  The longan pericarp extract can lower the AST and ALT serum levels of Wistar rats induced by paracetamol.
