Sesuai dengan standard ISO 45001 bahwa karyawan harus berpartisipasi dalam melakukan pencegahan kecelakaan. Untuk itu perusahaan telah menetapkan Program Hazob (Hazard Observation) untuk mengidentifikasi bahaya dan melakukan tindakan koreksinya. Penerapan Program Hazob masih dengan metode konvensional, mengisi lembar form, sehingga tidak efektif, efisien dan tidak berintegrasi dengan sistem lain. Transformasi digitalisasi diperlukan dengan merubah bisnis proses pelaporan dari konvensional ke aplikasi website atau mobile Apps. Hasil dari penelitian ini terjadi peningkatan kinerja operasional keselamatan kerja dengan program Hazob (i) pengisian form secara manual bertransformasi ke system digitalisasi aplikasi website dan mobile Apps; (ii) leading indikator pelaporan hazob meningkat menjadi 1.364 dari seluruh lokasi. Ini bermakna, bahwa perusahaan telah mengidentifikasi bahaya sejumlah tersebut dalam kuran waktu 3 bulan, dan melakukan tindakan koreksi sebelum terjadi kecelakakan. Data persentasi kontribusi menunjukan bahwa semua pihak bagian di perusahaan berkontribusi untuk melakukan pelaporan hazob. Hal ini menunjukan komitmen semua tingkatan untuk melakukan pecegahan kecelakaan. Angka terbesar pada tingkatan karywan ( 63 %) berkontribusi ; (iii) dampak implementasi berupa tingkat penurunan angka kecelakaan dengan Lagging Indikator sebagai berikut : LTIR = 0 (Nol), TRIR, yang sebelumnya 0.87 di bulan juni, turun menjadi 0 (nol) dari bulan Juli sampai September 2020, dan IFT, turun terus setiap bulan dan di bulan September di angka 12.33.

Abstract

PT. Pallet is a waste processing company that uses plastic waste as the main material to make plastic pallet products. Based on initial observations, this company experienced problems in material management, thereby reducing cost-effectiveness and efficiency. The purpose of this study is to identify problems in material management and the procurement of raw materials for plastic waste used in the production process of making plastic pallets, as well as to find out some of the causal factors for the occurrence of deficiencies/excesses in the procurement of raw materials for production waste. This research uses a descriptive quantitative approach, and the researcher acts as a research instrument or data collector. The analytical method used is Economy Order Quantity (EOQ), Safety Stock, and Reorder Point (ROP). The EOQ method has a lower total inventory cost. After calculating using the EOQ method, it is clear how much material must be ordered, the time to place the order and the total costs that must be incurred. The number of economic orders in the 3 months was 1,153.57 kg in October, 1,153.02 kg in November and 1,262.22 kg in December. The safety stock needed to expedite the production process is 394,345 kg in October, 376,306 kg in November and 99,652 kg in December. The results of the EOQ calculation produce a lower frequency of reorders, namely 203 times in one month, compared to the frequency of orders without EOQ, namely 378 times in one month.

Abstract

The Belawan Main Kesyahbandaran Office is a Technical Implementation Unit under the Directorate General of Sea Transportation, Ministry of Transportation which has duties in accordance with the Minister of Transportation Regulation Number PM 34 of 2012 concerning Organization and Work Procedures of the Belawan Main Kesyahbandaran Office which has the task of carrying out supervision and law enforcement in the field of shipping safety and security, as well as coordinating government activities at the port. In this study the authors analyzed the Leave Submission Procedure at the Belawan Main Kesyahbandaran Office directly, using several methods to obtain data from the office such as conducting interviews and observations. In taking leave, employees of the Belawan Main Kesyahbandaran Office must comply with the applicable flow or procedure and also the Operational Standards. In addition, there are also several obstacles in applying for employee leave which can cause leave applications not to be ACC by superiors / leaders

Abstract

Assessing possible risks in construction projects helps predict risks and plan risk management strategies. In construction projects, many factors can affect project completion, including the risk of work accidents. This research aims to look for Occupational Health and Safety risk factors for COVID-19 in construction projects by conducting a literature study on several previous studies. The research methodology is a literature search using the Publish or Perish application. Relevant literature is then used as a source of research. Based on the results of a literature study, twenty-eight risk factors are considered to have the potential to occur in construction projects consisting of eighteen OHS risk factors and ten COVID-19 risk factors. The five most common risk factors for OHS in previous studies were: falling from a height, being hit by material, being electrocuted, being scratched by workers, being punctured by material, and being hit by heavy equipment.

Abstract

Worker safety is a critical factor in the continuity of an oil palm industry because the progress of the palm oil industry is also influenced by the availability of safety guarantees for its workers. Based on the results of the study, it was shown that the highest percentage of accident risk assessment was at the sterilizer work station (boiling FFB) using the HIRARC method, namely in the activity of pulling the cable sling from the winch and then attaching the hook to the lorry from the transfer carriage with 25 the risk of accidents occurring with a percentage of 33.33 % and categorized into extreme, for the second highest accident risk assessment, namely in the activity of pulling a lorry using a winch and in the activity of opening the sterilizer door and lowering the sterilizer bridge with a work accident risk of 16 times with a percentage rate of 21.33% and categorized into high incident and the risk of work accidents, namely in the activity of releasing the hook and carrying it then installing it to the last lorry and in the activity of pulling and carrying sling cables from the winch with a risk of accidents of 9 times the incidence of work accidents with a percentage rate of 12.00% and categorized into incident medium. Keywords: Minimizing the Risk of Occupational Accidents and the HIRARC Method

Abstract

This research focuses on analyzing the types of conductors and electrical installation safety at Sanctoo Suites & Villas Singapadu, where this research aims to determine the type of conductor, current carrying strength (CRC) flowing in the conductor, protection MCB, contactor, TOR, ELCB, and Surge Arrester installed already meets PUIL standards.After this research is completed, it is hoped that this research can provide consideration to the company that the importance of the electrical installation system, especially the installation of electric power for safety and comfort, by using the installation of electrical equipment according to standards.

Abstract

PT. Pallets is a manufacturing company that processes plastic waste into products that have value, namely plastic pallets. A plastic pallet is a flat city-shaped container used as a base for storing and transporting goods in logistics warehousing. This study aims to improve the quality of plastic pallet welding products of the HDX1210R type at PT. Pallets. This study uses the Six Sigma method with the DMAIC (Define - Measure - Analyze - Improve - Control) approach with the help of fishbone diagrams, Pareto diagrams, and FMEA. From the company's historical data from March 2022 to March 2023, 3 types of defects were obtained, with loose welding as the highest type of defect, with a percentage of 89% of the total product defects. The overall DPMO average value is 49908.1405, and the average sigma value is 3.191. From FMEA calculations, the highest RPN value is obtained for frequently changing over machines with a scale value of 480. The proposed improvements can be made by changing the hitter model and size, modifying the mold (pallet molding) by adding the welding area on the pallet and making SOPs for producing HDX1210R-type pallet welding.

Abstract

This study aims to determine the effectiveness of holding time variation of fir charcoal with oil cooling media SAE 20 on the toughness of low carbon steel ST 37 due to the pack carburizing process. This research method, namely impact test, and macrostructure. In the pack carburizing process using fir charcoal and SAE 20 oil cooling media on the toughness and macrostructure of low carbon steel ST 37. The holding time used is 1,2,3,4 and 5 hours. After the carburizing process, a toughness test and macrostructure observation were conducted. From the results of the toughness test obtained the value of each: 17.513 kJ/mm2 at a holding time of 1 hour, 11.920 kJ/mm2 at a holding time of 2 hours, 13.196 kJ/mm2 at a holding time of 3 hours, 7. 917 kJ/mm2 at a holding time of 4  hour, and 3.081 kJ/mm2. Then for macro photo observations greatly, it can be seen that holding time variations greatly affect the holding time, the more tenuous the steel structure
