Abstract

The Covid-19 outbreak has been declared as a public health emergency of international concern by the World Health Organization (WHO). The pandemic has also changed the process of health services provided to patients, including medical record services. This study aimed to examine the process of medical record services at the Gondokusuman II Primary Health Center (PHC) in Yogyakarta City during the Covid-19 pandemic. This qualitative research explored medical record services during pandemics. Three staff were recruited as respondents using the purposive sampling technique. Data were collected through observation and interviews. There were differences in patient registration services before and after the pandemic. Before registering, patients will be screened first. If they show Covid-19 symptoms, the patients are directed to the infectious clinic and the provision of medical records will be prioritized. To support the implementation of health protocols, the primary health center provides personnel protective equipment adequately. However, the reference for health protocols related to medical record services at the PHC level is not yet available. So, reference issued by the Ministry of Health is used respectively. Therefore, specific regulations need to be immediately compiled and disseminated to ensure that the prevention of Covid-19 transmission can be carried out more precisely.

Abstract

Background: Filariasis is a parasitic disease caused by microscopic, thread-like worms (Wuchereria bancrofti, Brugia malayi, and Brugia timori) which transmitted by mosquito bites. WHO records more than 1.4 billion of world population settle in areas with filariasis infection risk which are spread in 73 nations included Indonesia. Filariasis in Indonesia spread in 418 districts while 235 districts are stated as endemic areas with 14.932 cases. In West Sumatera, the Agam District becomes one of filariasis endemic areas with the highest rate with cases prevalence to 12.63 per 100.000. The environment condition of the Agam District situates the area with mountains, plains, rivers, lakes, farms, and rice fields.Objective: The study aimed to implement the use of Geographic Information System for vulnerability area mapping based on risk factors of filariasis in Agam District.Methods: The study is analytic observational, designed with case control study. Odds Ratio (OR) used to find out risk factor estimation of filariasis prevalence. This study involves 36 cases with 36 controls summed to 72 samples. Analysis of the data was used univariate, bivariate, multivariate and vulnerability area analysis spatially.Results: Geographic Information System (GIS) can be used to determine the level of vulnerability area of filariasis in Agam District. Statistical data such as low education (OR: 4.52), low knowledge (OR: 4.14), profession (farmer, labour, and fisherman) (OR: 4.38), and low income (OR: 4.43) along with that the behaviours of community such as high outdoor activities at night (OR: 3.75) and reservoir animal farming (OR: 3.57) are recorded as filariasis risk factors. Environment condition shows that plantation area (OR: 19.46), where mosquito breeding is commonly located, is the risk factor too. Based on multivariate analysis, the filariasis risk factor in the Agam District is the existence of plantation area (OR: 19.46) as well. The research found that Culex (67.26%), Aedes (18.06%), Armigeres (14.19%), and Anopheles (0.48%) were vectors of the disease. The clustering of filariasis cases was located in Subang – Subang and Muaro Putuih. The vulnerability zones found in Agam District such as Sub-district Tanjung Mutiara, Lubuk Basung, IV Nagari, Palembayan, Palupuh, Baso and IV Koto.Conclusions: The risk factors of filariasis in Agam were low education, low knowledge, profession (farmer, labour, and fisherman), low income, high outdoor activities at night, reservoir animal farming, and plantations area as mosquito breeding sites approximately 200 metres from residence. Filaria vector types in Agam such as Culex, Armigeres, Aedes and Anopheles. The vulnerability area and clustering of filariasis known by using Geographic Information System.Keywords: Geographic Information System (GIS), Filariasis, Risk factor, Vulnerability, Agam District.

Abstract

Indonesia masih menetapkan bahwa masalah kesehatan ibu dan anak (KIA) sebagai program prioritas yang harus diselesaikan karena angka kematian ibu (AKI) dan angka kematian bayi (AKB) belum memenuhi standar indikator. Sistem surveilans KIA belum terintegrasi sehingga data dan informasi yang dihasilkan juga belum bisa dimanfaatkan secara maksimal untuk mendukung pengambilan keputusan terhadap masalah KIA. Tujuan penelitian ini yaitu merancang dan mengembangkan sistem informasi surveilans program KIA berbasis DHIS 2. Jenis penelitian adalah action research dengan mengikuti langkah-langkah protoyping dalam mengembangkan sistem informasi surveilans program KIA berbasis DHIS 2 (District Health Information Software 2). Penelitian dilaksanakan pada bulan Agustus 2019 - Februari 2020 di Puskesmas Kabupaten Kulon Progo. Subjek penelitian ini yaitu Petugas KIA Puskesmas sebanyak 6 orang yang ditentukan berdasarkan purposive sampling. Objek penelitian ini adalah sistem informasi surveilans KIA. Prototipe sistem informasi surveilans program KIA berbasis DHIS 2 telah melalui tahap identifikasi kebutuhan, desain sistem, implementasi, dan pengujian. Hasil uji coba sistem membuktikan bahwa pengguna dapat menerima sistem tersebut dan sistem berjalan dengan baik. Sistem informasi surveilans program KIA berbasis DHIS 2 sudah berhasil dikembangkan dan diterima oleh pengguna. Oleh karena itu, pihak puskesmas disarankan agar mengimplementasikan sistem ini dalam kegiatan surveilans KIA.

Abstract

Stunting merupakan salah satu masalah kesehatan masyarakat yang menjadi prioritas pemerintah di bidang kesehatan. Hasil Riset Kesehatan Dasar (Riskesdas) 2018 menunjukkan bahwa kejadian stunting di Indonesia sebanyak 30,8% dan tahun 2019 mengalami penurunan menjadi 27%. Namun angka tersebut masih jauh dari standar yang ditargetkan pemerintah dan WHO yaitu dibawah 20%. Hasil analisis masalah bidang kesehatan di Desa Sidoharjo Kecamatan Samigaluh Kabupaten Kulonprogo tahun 2020 ditemukan bahwa kasus stunting balita Desa Sidoharjo cukup tinggi (30 kasus) yang disebabkan karena pemahaman masyarakat yang masih kurang mengenai stunting. Kegiatan pengabdian masyarakat ini bertujuan untuk meningkatkan pengetahuan dan pemahaman masyarakat mengenai stunting pada balita. Kegiatan dilakukan dengan metode sosialisasi yang mencakup penyuluhan kesehatan, pemberian bantuan makanan, dan pendataan balita stunting. Kegiatan dilaksanakan pada bulan September 2020 di tengah masa pandemi Covid-19 dengan mengikuti protokol kesehatan. Sasaran kegiatan ini yaitu warga desa secara umum dan khususnya yaitu kader kesehatan, dan ibu balita. Kegiatan diawali dengan melakukan pendataan balita stunting, kemudian dilanjutkan dengan penyuluhan kesehatan (pemasangan spanduk, leaflet, banner, dan penempelan sticker), pemberian bantuan makanan tambahan pada 30 balita stunting, dan pemasangan alat cuci tangan di Polindes dan Balai Desa yang bisa dimanfaatkan warga sebagai tindakan preventif stunting. Kegiatan ini dapat meningkatkan pengetahuan dan pemahaman masyarakat mengenai kasus stunting meskipun terdapat keterbatasan karena pandemi Covid-19 dan bisa dilanjutkan dengan pemanfaatan teknologi informasi untuk diseminasi informasi stunting pada tahun berikutnya.
