Abstract

Kegiatan asuhan mandiri Toga atau yang juga dikenal dengan asman TOGA diintegrasikan dengan program PKK yang dalam pelaksanaannya dikoordinir oleh kader Asman dan didampingi fasilitator dari Puskesmas. Program ini dibawah pembinaan oleh Dinas Kesehatan Kabupaten/Kota. Desa Canditunggal yang terletak di wilayah Kecamatan Kalitengah Kabupaten Lamongan mempunyai kebun TOGA yang asri terbentang di tengah jalan desa. Kebun TOGA ini merupakan hasil kegiatan ibu-ibu PKK yang juga merupakan kader asman TOGA. Pada awalnya kebun TOGA ini hanya untuk keindahan saja. Untuk itu pada kegiatan pengabdian masyarakat yang dilakukan kali ini bertujuan untuk memberdayakan ibu-ibu PKK tersebut agar dapat memanfaatkan TOGA khususnya dalam membuat produk dari daun katuk menjadi produk bernilai ekonomi untuk memperbanyak ASI. Kegiatan dilakukan dengan pemberian materi dan pelatihan pembuatan produk. Antusiasme, kreatifitas dan inovasi dari ibu-ibu PKK kader asman TOGA Desa Canditunggal ini merupakan potensi yang sangat besar untuk keberhasilan pemanfaatn TOGA untuk menciptakan produk-produk yang bernilai ekonomi.  Kata Kunci: Asman, daun katuk, TOGA

Abstract

Pelayanan kesehatan konvensional dan komplementer terpadu dilaksanakan bagi warga binaan lanjut usia di Wilayah Karang Gayam Teratai, Surabaya dengan memberikan konsultasi kesehatan konvensional dan komplementer dengan ramuan herbal, akupunktur dan pijat. Tujuan program pengabdian masyarakat ini adalah peningkatan wawasan tentang pengaplikasian sistem pengobatan konvensional berpadu dengan pengobatan komplementer. Titik berat pengobatan konvensional berupa kepatuhan menjalankan perintah dan nasehat dokter Pusat Kesehatan Masyarakat setempat, yaitu kepatuhan minum obat dokter, pengaturan pola hidup terutama pola makan, olahraga dan istirahat. Perpaduan dengan pengobatan komplementer berupa pemanfaatan tanaman obat, akupunktur dan pijat yang sesuai dan menunjang pengobatan dokter. Jumlah pasien 30 orang yang semuanya mengalami gangguan penyakit kronik, yaitu diabetes mellitus, tekanan darah tinggi, kadar asam urat tinggi, kadar kolesterol darah tinggi. Berdasarkan data menunjukkan tidak terdapat satu pasien pun yang mengalami perubahan gangguan kesehatan pada ke empat indikator darah secara keseluruhan yang diukur. Melalui program ini dapat disimpulkan, bahwa pola hidup tidak sehat menjadi penyebab utama masalah kesehatan. Warga membutuhkan perhatian dalam bentuk konsultasi, dimana mereka dapat berdiskusi secara aktif, dan bukan tindakan pemeriksaan kesehatan dan pemberian obat saja. Pelayanan terpadu sistem konvensional dan komplementer perlu dipertimbangkan sebagai cara untuk meningkatkan kesehatan masyarakat.Kata kunci: Pelayanan Terpadu; Komplementer; Herbal; Akupunktur; Pijat.AbstractIntegrated conventional and complementary health care services have been conducted among elderly residents of Karang Gayam Teratai Regency in Surabaya which provided patients with necessary information on conventional combined with herbal medicine, acupuncture, massage. The aim of the program was to enhance the understanding of people in the community about the application of complementary medicine along with conventional medicine. Conventional medicine alone, or in combination has to be thoroughly taken through understanding about all necessary instructions, and conduct healthy life style. 30 patients participated in the program, where all of them contracted various kinds of chronic diseases, including high blood pressure, high blood sugar, cholesterol, uric acid levels. It is concluded from the program, that unhealthy life style is the main cause of their health problems. People showed great appreciation to every effort in giving them special time for active discussion and attention. Effective doctor-patient communication is a central clinical function in building a therapeutic doctor-patient relationship, which is the heart and art of medicine. This is important in the delivery of high-quality health care. Much patient dissatisfaction and many complaints are due to breakdown in the doctor-patient relationship.Keywords: Complementary Medicine; Conventional Medicine; Acupuncture; Massage; Herbal.

Abstract

AbstrakBertani menjadi pencaharian warga Desa Wajik, Kecamatan Lamongan, Kabupaten Lamongan, Jawa Timur. Kawasan desa itu ditumbuhi berbagai tanaman, seperti mangga, delima, jambu biji. Kesuburan tanah belum digunakan secara optimal untuk bertanam tanaman obat, yaitu yang termasuk dalam Taman Obat Keluarga (TOGA). Beberapa tanaman obat untuk pagar, seperti beluntas, belum dimanfaatkan secara optimal. Ini karena pengetahuan tentang tanaman obat dan khasiatnya belum dipahami, sehingga belum diutamakan untuk mencapai keadaan sehat. Tujuan program pengabdian masyarakat ini adalah untuk mengoptimalkan pemanfaatan herbal untuk kesehatan. Penekanan diberikan pada peningkatan wawasan warga terhadap pemanfaatan herbal untuk kesehatan keluarga. Sasaran audiens adalah kadertim penggerak PKK desa yang berperan sentral dalam kesehatan keluarga. Kegiatan diselenggarakan melalui ceramah, pelatihan dan demonstrasi pembuatan ramuan tanaman obat, dan aplikasi aromaterapi. Pre dan post test dalam bentuk kuesioner tentang tanaman obat. Dari hasil tes diketahui pengetahuan peserta tentang tanaman obat dan khasiat cukup baik. Lebih dari 50% peserta belum mengetahui cara pengolahan pasca panen tanaman obat dan ramuan tanaman obat untuk kesehatan wanita. Kesimpulan program ini adalah terjadi peningkatan pemahaman peserta tentang tanaman obat untuk tujuan kesehatan.Kata kunci: Desa Wajik, Kesehatan, Optimalisasi, Tanaman obat.AbstractWajik Village is located in Lamongan Regency in East Java Province, and it has a population of almost 1700 people. Various plants such as mango, banana, pomegranate, and papaya trees grow well. Unfortunately, the Indonesian Government’s Program of TOGA, stands for Taman Obat keluarga (medicinal plant garden), meant to urges people to grow some beneficial medicinal plants in their own gardens was not successfully implemented. Eventhough some plants are found for hedges, such as betel, Pluchea indica plants, they are reluctant to apply them for maintenance of health. The purpose of the enlightenment programme aimed at enhancing the knowledge on medicinal plants in order to improve and maintain their health status. Participants of the program were cadres of PKK (Pembinaan Kesehatan) organization of the village. The program consisted of discussion, workshop and demonstration. Pre and post tests showed their understanding about medicinal plants and recipes, but did not understand post-harvest handling and some jamu products for women’s health. It wasconcluded, that the understanding of participants over the importance of herbal medicine and traditional medicine for health maintenance were low. It is concluded, that knowledge given during the program was able to enhance their understanding about herbal medicines for health.Keywords: Herbal medicines, Health, Optimization, Wajik Village.

Abstract

Background: Antimalarial screening against nine species of the genus Cassia showed that the methanol extract of leaves Cassia spectabilis have the highest activity. Since it will be used as a traditional medicine, hence it is needed further studies of antimalarial activity of these plants by choosing a safer solvent, namely ethanol. Objective: In vitro anti-malarial activity against Plasmodium falciparum was conducted using the method of Trager and Jensen. Methods: The serial solution tested were: 100, 10, 1,  0.1 and 0.01 µg/ mL, while the in vivo test was performed based on Peter’s test (The days suppressive test) that using P. berghei (strain ANKA) infected mice. Results: The results showed that ethanolic extract of C. spectabilis leaves has inhibitory activity against P. falciparum with IC50 value of 12.52 µg/ mL and against P. berghei with ED50 value of 131.5 mg/kg body weight. Conclusions: A further study to see the potential of ethanol extract from C. Spectabilis leaves as anti-malaria is warranted.

Abstract

Background: Hepatitis C Virus infection is a serious health problem that leads to chronic liver disease, liver cirrhosis, hepatocellular carcinoma, which causes high morbidity. Direct-Acting Antiviral Agents have been used as anti-hepatitis C Virus therapy. However, it was covered only in limited patients due to the high cost. Moreover, serious side effects and resistance cases were also reported in some HCV genotypes. Objective: This research aimed to find new anti-HCV from some Indonesia plants collected from Balikpapan Botanical Garden, East Borneo. Methods: Twenty-one leaf and stem barks extracts were successively extracted in n-hexane, dichloromethane, and methanol. Extracts were screened for their anti-HCV activity under in vitro culture cells in the concentration of 30 µg/mL. Plant extracts were inoculated in the Human Hepatocellular 7it and infected with HCV Japanese Fulminant Hepatitis strain 1a. Determination of 50% Inhibitory Concentration (IC50) value was further conducted at concentration of 100; 30; 10; 1; 0.1; 0.01 µg/ml of extracts. Results: In vitro anti-HCV activity revealed that among 21 plants extract, 11 extracts, namely, n-hexane extract from Luvunga scandens leaves, DCM extract from the leaf of L. scandens, Artocarpus sericicarpus, Artocarpus dadah, Eusideroxylon zwageri, Neolitsea cassiaefolia, methanol extract from A. sericicarpus and A. anisophyllus leaves, DCM extract from A. anisophyllus and A. elmeri stem bark, methanol extract from A. dadah stem bark, having potential inhibition with IC50 range 0.08 ± 0.05 to 12.01 ± 0.95 µg/mL. Conclusions: These results indicate that the eleven extracts could be good candidates as sources of anti-HCV agents.

Abstract

Hepatitis C Virus (HCV) infection is a serious public health problem since HCV is the ribonucleic acid (RNA) virus that  easy to mutate. The HCV standard treatment  has rapidly developed but the possibility of resistance and effectiveness of treatment needs to be considered. The medicinal plants are a source of various compounds that may potentially cure diseases including infectious diseases. Since a long years ago, medicinal plants were famous as an inherited treatment that believed to cure the disease. One of the medicinal plants is Scoparia dulcis (S. dulcis) that belongs to Scrophulariaceae family and traditionally used as remedies for digestive problems, hypertension, diabetes mellitus, bronchitis, and as an analgesic & antipyretic agent. The previous report showed that S. dulcis was known active as an antiviral against Herpes Simplex Virus (HSV) type 1 in vitro and in vivo. The aim of the study is to determine the biactivity potential of S. dulcis against HCV. Scoparia dulcis was extracted using 80% ethanol (EE) then further separated by liquid-liquid fractionation using dichloromethane (DCMF), ethyl acetate (EAF), butanol solvent (BF) and water (WF). The in vitro anti-HCV analysis was performed with Huh7it cells and HCV JFH1 (genotype 2a) by determining inhibition concentration 50 (IC50). The toxicity (Cytotoxicity Concentration 50, CC50) test was performed by 3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyltetrazolium bromide (MTT) assay and mechanism of action were analyzed using time addition experiment.   Phytochemical groups as the suspected active compounds of S. dulcis were identified by Thin Layer Chromatography (TLC) and observed under UV 254 nm, UV 365 nm, before and after sprayed using H2SO4 10% and heated at 105oC for 5 minutes. The IC50 test result of 80% EE and DCMF showed anti-HCV activity with a value of 12.7±4.8 µg/ml and 5.8±0.69 µg/ml, while EAF, BF, and AF respectively resulted in IC50 value of  >100 µg/ml that suggested there was no inhibition effect on HCV JFH1.  The DCMF was the most active fraction but toxic to the cell with CC50 value >23 µg/ml and selectivity index (SI) >3.9. According to the time addition experiment data, DCMF of S. dulcis inhibited post entry step HCV JFH1 infection that it means the possibility was to inhibit virus replication and or virion release. Scoparia dulcis contain chlorophyll, flavonoids and terpenoids as the suspected active compounds for inhibition of HCV JFH1 infecton. Futher study of post-entry inhibitions of HCV infection was needed.

Abstract

Background: Hepatitis c virus infection is a global health problem which chronically infected 71 million people in the world. This infection has a risk of becoming liver cirrhosis and hepatocellular carcinoma. Since the current HCV therapy has been developed by direct-acting antivirals (DAA), however, most patients get limited access due to the high cost. Therefore, further development anti-HCV agent still greatly needed. Ruta angustifolia is a natural resource which was reported to possess anti-HCV activity. Ribavirin is an antiviral agent used to treat several virus infections, either DNA or RNA. Ribavirin was known to inhibit HCV infection by regulated immune system in host cells and interfering the replication of HCV by inhibit HCV RdRp. Objective: The current study evaluated the combination treatment of R. angustifolia extracts and ribavirin by in vitro culture cells of Huh 7it. Method: The study was conducted under an invitro cell culture of Huh 7it and infected with JFH1a. Result:  The result demonstrated an enhancement effect of the extract by increasing the anti-HCV activity 3.5-fold higher compared to ribavirin alone. The 50% inhibitory concentration of ribavirin by single treatment was 10.43 ± 0.18 µg/mL, while in combination with Ruta angustifolia extract was 2.80 ± 0.03 µg/mL. Further analysis of the combination by CompuSyn software mediated a synergistic effect among the combination with a combination index value of 0.691. Conclusion: These results suggested that combination of Ribavirin and Ruta angustifolia should be considered in developing anti-hepatitis C virus agents.

Abstract

Sterile dosage forms are pharmaceutical preparations that require to be free of microorganisms and particles. In health services, sterile preparations often undergo a series of changes in form from their original conditions to new products through dissolution or mixing with other sterile preparations. To maintain sterility, sterile preparation dispensing skills are needed. Regulation of the Minister of Health of the Republic of Indonesia No. 72 of 2016 and No. 74 of 2016 concerning Pharmaceutical Service Standards in Hospitals and Health Centers are intended to improve the quality of pharmaceutical services, guarantee legal certainty for pharmacists and protect patients and the public from irrational drug use in the context of patient safety. Pharmaceutical service standards in hospitals cover the management of pharmaceutical preparations, medical devices, consumable medical materials and clinical pharmacy services. One of the standards in clinical pharmacy services is aseptic/sterile dispensing sterile. The community service aims to increase the knowledge and skills of pharmacists at Bandar Lampung health facilities and its surroundings in managing sterile preparations.The community service is held through a webinar with a zoom platform followed by discussion and aseptic dispensing demonstration. The sterile dispensing demonstration are carried out by dividing the participants into four groups in the breakout room and each group was guided by a facilitator. In this activity, participants were provided with material on formulation, compatibility, and stability of pharmaceutical preparations, dispensing sterile preparations, aseptic techniques, and problems in sterile dispensing sterile at the hospital. The activity was attended by 63 pharmacists from 25 hospitals in Bandar Lampung and its surroundings, also academics in Tanjung Karang. The evaluation results showed an increase in participants' knowledge of 19,3% and participants expected that activities could be carried out periodically with various topics. The activities are expected to improve the patient's health status and enhance the role of the Faculty of Pharmacy, Airlangga University in the National Health System.

Abstract

Luvunga sarmentosa, commonly known as saluang belum, is widely used in Kalimantan to relieve pains, rheumatism, boost the immune system, and fever. The research on the free radical scavenging and analgesic effect of the L. sarmentosa stem extract has not been reported. This study aimed to evaluate the free radical scavenging and analgesic activity of the ethanol extract of L. sarmentosa. The L. sarmentosa stem was extracted using 70% ethanol and tested for free radical scavenging using the 1,1-diphenyl-2-picrylhydrazyl radical (DPPH) and analgesic activity, acetic acid-induced writhing test, and hot plate test in an animal model. The results showed that the 70% ethanol extract of the L. sarmentosa had an anti-free radical scavenging and analgesic activity. The extract has weak free radical scavenging with an IC50 value of 293.45 µg/mL. Analgesic activity using the writhing test indicated that the extract significantly reduced the writhes count after oral administration in a dose-dependent manner compared to the negative control. Extract at a dose of 550 mg/kg BW can reduce the writhing test by 67.60% compared to others. In contrast, the diclofenac sodium reduced the number of writhes by 74.74%. While in a hot plate, the extract at a dose of 550 mg/kg BW produced a maximum possible analgesia (MPA) of 17.64%, lower than the MPA of diclofenac sodium (51.01%). Analgesic activity of the extract has higher inhibition on the writhing test than on the hot plate. The extract could be responsible for the peripheral mechanism by inhibiting the prostaglandin biosynthesis.
