Abstract

During storage, erythrocytes suffered from biomechanical alterations called the “storage lesion”, which may caused hemolysis. The hemolysis released LDH into the plasma. The LDH that was released during hemolysis made it an adequate instrument to assess the quality of in vitro blood products. The aims of this study were to analyse the alteration of LDH level at day 1, 3, 7, 14, and 28 in the WB and PRC, to analyse the correlation between LDH level with storage duration, and also to analyse enhancement differences of LDH level between WB and PRC.This research was an observational study with a cross-sectional design. As the samples there were 11 bags of WB and 10 bags of PRC. Blood products were kept in bloodbank with the temperature range of 2–6° C. The LDH level was measured with the Beckman Chemistry Analyzer. There were statistically significant alterations of LDH level started from day 7 of storage in both blood products (p<0.05). The significant strong correlation between LDH level with the storage duration were found r=0.772; r=0.835 (p<0.05) in WB and PRC respectively. The enhancement differences were found to be higher and significant in the PRC than in the WB started from day 7 of storage (p<0.05). As conclusion, LDH in WB and PRC were signifantly increased during storage, and correlate with storage duration.

Abstract

Understanding of hemostasis has developed substantially in the last century from stasis in vitro to in vivo concept. Hemostasis theory develops from classic theory, discovery of coagulation factors leading to cascade/waterfall theory, as well as to in vivo cell based theory which explains the limitations of cascade theory. Phases of cell based hemostasis theory include initiation, amplification, propagation and termination with the role of tissue factor, platelet activation and coagulation factors in thrombin and fibrin synthesis. Common hemostasis tests used nowadays are important in evaluating bleeding risk but this matter still can not explain cell based hemostasis theory comprehensively so we need to find new tests to evaluate in vivo hemostasis.

Abstract

Prevalence of type 2 diabetes mellitus and chronic kidney disease is increasing nowdays. Cystatin C is a more sensitive marker in detecting kidney disturbance at an early stage. Microalbuminuria indicates that there is an increasing risk in kidney disorders in diabetes mellitus patients. HbA1c is used to assess average blood glucose in three (3) months. An evaluation about the relation among Cystatin C, HbA1c, and ACR is needed to decide which test will be appropriate for the patient. Cystatin C was determined using Particleenhanced nephelometric immunoassay (PENIA) method from BN Prospec. High Performance Liquid Chromatography (HPLC) method from BioRad was used for HbA1c test, while random urine sample was used for Albumin Creatinine Ratio (ACR) test. Urine and serum creatinine were tested by immunoturbidimetric assay from Cobas C501. Correlation among Cystatin C, HbA1c, and ACR was estimated using Spearman’s correlation test. There was a significant correlation among Cystatin, HbA1c, and ACR. Spearman correlation value showed a positive weak correlation. There was a strong relation between Cystatin C and HbA1c, but weak relations between Cystatin C and ACR, and between HbA1c and ACR. It can be concluded that due to the weak correlation between ACR and kidney function as well as between ACR and HbA1c, ACR is not a good assessment to predict diabetic nephropathy progressiveness.

Abstract

Regarding to the ageing population in most countries, degenerative and metabolism diseases included osteoporosis are becoming increasingly and need special attention in all country especially in the developing country such as Indonesia. Current WHO recommendation of using a gold standard for establishing osteoporosis is by Dual X-Ray Absorptiometry (DEXA), but it is expensive and not yet widely available. There are some alternatives of biochemical markers that can reflect the bone turn over. One of them is Cterminal cross-linking telopeptide of type I collagen (CTX) or betacrosslabs, a marker of bone resorbtion. There are still limited studies on correlation of CTX and osteoporosis in peri menopausal and post menopausal women. The aim of this study is to know whether there are any differences of CTX level among peri menopausal and post menopausal women with different bone density. This research was a cross sectional study and aimed to know whether there is any correlation between CTX level and Bone Mineral Density Score measured with DEXA. The study was conducted in June-August 2010. The included subjects were women with age more than 45 years old, without apparent history of neoplasm, impaired renal function, bone metabolic diseases, and not currently pregnant. CTX level were measured using immunochemiluminescent. Their data were analysed using One Way ANOVA. This study showed that there are differences of CTX levels among women with osteoporosis, osteopenia, and normal (0.633±0.25 ng/mL vs 0.514±0.21 ng/mL vs 0.406±0.21 ng/mL p=0.02). Osteoporosis women have higher CTX level compared to Osteopenia and Normal women.

Abstract

Sepsis is the most common cause of ICU mortality in USA. Mortality of sepsis in developing countries is still very high, about 50- 70% and has became a 80% incidence in septic shock. There was a decrease of CD4+ T lymphocyte count in patients with sepsis caused by apoptosis indicating septic patients suffered from immune functional impairment. CD4+ T lymphocyte count can reflect the severity of sepsis and predict the prognosis of the patients with sepsis effectively. Eighty eight (88) patients who met sepsis criteria were studied. The researchers collected clinical variables of all patients within 24 hours diagnosis of sepsis, and calculated APACHE II score. At the same time, blood sample were taken to measure the CD4+ T lymphocyte count. The data were analyzed using independent Student-T-test and ROC curve was used for prognosis. There is a significant difference in CD4+ T lymphocyte count between non survival and survival group (non survival group 203±178 cells/μL, survival group 442±303 cells/μL, p<0.001), and the percentage of CD4+ T lymphocyte (non survival group 25.05±11.55%, survival group 34.38±9.15%, p<0.001). There is an under ROC curve for CD4+ T lymphocyte count was 0.81, and for the percentage of CD4+ T lymphocyte was 0.748. Cut off value for CD4+ T lymphocyte count was 204 cells/μL, and the percentage of CD4+ T lymphocytes was 25.23%. Based on this study, the CD4+ T lymphocyte count can be used as a predictor of prognosis in sepsis patients.

Abstract

Abdominal obesity is closely linked to the occurrence of metabolic syndrome. In pathomechanism of metabolic syndrome, adipocity plays an important role as an active metabolic endocrine organ. This is accomplished the secretion of various hormones, enzymes, cytokines, and components that play a role in the rennin angiotensin system (RAS). One of the mechanisms linking the occurrence of hypertension in obesity is through the increased activity of RAS. Angiotensin II is the major effector of hypertension. The effect of high glucose exposure in adipocytes culture to angiotensin II secretion up to now is yet unknown. Adipociyties culture from rat visceral adipose tissue were exposed to 5 mM glucose concentration (as a physiological condition), 11 mM and 25 mM glucose concentration as the high glucose condition. Measurement of the angiotensin II level which is secreted in the culture medium was done by ELISA method. The mean (SD) levels of angiotensin II were 56.4 (4.28), 66.05 (2.24), and 69.22 (3:49) ng/mL respectively, for adipocities cultures exposed to 5 mM, 11 mM and 25 mM glucose concentration. High glucose exposure could increase the secretion of angiotensin II significantly in adipocytes culture. This suggests that the condition of hyperglycemia affects adipocytes dysfunction that play a role in the metabolic syndrome pathomechanism.

Abstract

The standard method for absolute lymphocyte CD4 count is by the use of flow cytometric analysis, but due to the high costly matter,a simpler method and less costly fee of PanLeucogating dual platform is needed. The aim of this study is to know how to replace the single platform by PanLeucogating dual platform by determining the agreement between both methods and to test the proportion of patient who have CD4 <200 cell/ul measured by PanLeucogating and single platform method. A cross sectional study was carried out in the Departement of Clinical Pathology Faculty of Medicine Gadjah Mada University Yogyakarta. The inclusion criteria of the study are HIV/AIDS patients who their CD4 were measured. Absolute count of CD4 was using FACS Calibur measured with both PanLeucogating and single platform method. The bias, correlation, regression and limit of agreement between both methods were analyzed using Bland Altman analysis to decide whether the two methods are interchangeable. The result of the study revealed an excellent correlation between the two methods (r=0.996; y=- 0.906 + 0.0.987). Bland Altman analysis revealed bias=5 cells/uL; standard deviation=18.7 and Limit of agreement (LOA)=-31.6 – 40.75. The Mean Percentage Difference (MPD)=2%; dan%LOA=-7.96 – 12.5%. PanLeucogating method has a good agreement with the single platform method and can be used interchangeable. There was no difference of the proportion of patient who has CD4 <200 cell/ul measured by Panleucogating method and single platform.

Abstract

Dysregulation of apoptosis plays an essential role either in leukemogenesis or treatment response. Caspase-3 is a cysteine protease that functions as the final common mediator of apoptosis. The expression of the active caspase-3 is presumed as a predictor of prognosis and is able to predict the chemotherapy sensitivity. The aim of this study is to identify and to know the profile of active caspase-3 in Acute Myeloid Leukaemia (AML) and Acute Lymphoblastic Leukaemia (ALL), to correlate its expression in marrow and peripheral blood mononuclear cells, and to verify the extent of its use as a complete remission predictor after induction treatment. The study subjects consisted of patients who were diagnosed as AML and ALL with marrow and peripheral blood examination performed at the Department of Clinical Pathology Dharmais Cancer Hospital and CiptoMangunkusumo Hospital. Based on this study, it is revealed that the active caspase-3 expression in mononuclear marrow cells was higher in AML compared to ALL (p=0.033), active caspase-3 expression in marrow showed a strong correlation (r=0.764; p=0.001) to peripheral blood mononuclear cells in ALL and a medium correlation (r=0.594; p=0.042) in AML. The expression of the active caspase-3 in ALL patients was lower in complete remission patients compared to the non-complete remission patients. Regarding to this study it is recommended to measure the active caspase-3 along with molecules integrating in apoptosis signaling pathways such as cytochrome-c and in the formation of apoptosome.

Abstract

Candida albicans β-D-glucans examination has been used for invasive fungal detection in human blood and was approved by the Food and Drug Administration. However, the method is rarely used in Indonesia and also because the cost is hardly affordable. A modification of the method using enzymatic reaction is hoped will provide a simple and affordable measurement in human blood serum. β-D-glucans as heterogeneous molecules constitute the major carbohydrates fractions of cell wall and readily detected in supernatants of Candida albicans cultures are hydrolyzed by β glucanase to form D-glucose. This additional glucose is measured using Megazyme GOPOD-Format Procedure® at 510 nm. Candida albicans were identified and cultured was derived from a patient of Dr. Moewardi General Hospital, Surakarta in July, 2012. The results show that β-D-glucans from Candida albicans is measureable to the amount of μg/100 μL serum using this modification principle

Abstract

Many reports have documented apoptosis index in hemodialysis patients, but to date, no single study has directly compared the apoptosis index of males to females. Data on mortality rate among hemodialytic patients in the hemodialysis center at the Department of Internal Medicine Dr Soetomo General Hospital, Surabaya, Indonesia show a high number predominated by female patients. Therefore, to answer the question of whether there is a gender difference in apoptosis index, the researcher studied leukocyte responses in male and female hemodialysis patients. The apoptosis index of the sample was measured by indirect immunoassay method. Cell lyses, followed by immunochemical determination of histone-complexed DNA fragments in a microtiter plate wells. The apoptosis quantization was obtained by determining the amount of colored product spectrophotometrically. One hundred and four non-diabetic subjects who received hemodialysis (HD), and 24 normal controls (NC), were evaluated. The apoptosis index in ESRD patients group and control group showed no significant difference (0.6172 vs 0.4008, p=0.114), neither did it vary in both sexes and age groups. When the sex factor was analyzed (after exclusion from the diabetic ESRD patients), females apoptosis index was significantly higher than that of the males (0.7325 vs 0.55175, p<0.05). In conclusion, apoptosis index in females among non-diabetic patients undergoing hemodialysis is higher than that occur in males and controls.
