Abstract

PT. X adalah perusahaan manufaktur yang menghasilkan komponen boiler yang mengerjakan 500 karyawan dan 1500 karyawan subkontraktor dengan berbagai macam peralatan kerja dengan tingkat resiko terjadi kecelakaan yang tinggi. Meskipun perusahaan telah menerapkan zero accident, ternyata kecelakaan kerja masih sering terjadi. Penelitian ini dimaksudkan untuk mengidentifikasi kesalahan manusia saat bekerja dengan overhead crane juga mengidentifikasi efek terjadinya kesalahan manusia setiap prosedur pengoperasian overhead crane untuk meminimalisasi terjadinya kesalahan manusia. Dengan menggunakan metoda THERP ternyata  didapatkan besar HEP untuk benda kerja yang berbentuk bulat, persegi, beraturan dan tak beraturan masing-masing sebesar 0.051; 0.05; 0.046; dan 0,043. Maka direkomendasi bahwa untuk tugas-tugas yang kritis diperlukan administratif kontrol pada setiap prosedurnya, pengawasan oleh inspektor atau pemberian label dan tanda SWL pada setiap pengangkatan accesories yang digunakan, perawatan pada mesin, pelatihan, peringatan kembali mengenai prosedur pada operator.Kata Kunci: kesalahan manusia, over crane, analisa kehandalan

Abstract

Manual material handling if done inappropriately can pose a risk of injury to the body, mainly in the spine, which is usually referred as musculoskeletal disorders (MSDs). Based on direct interview with warehouse workers of PT. X, it is known that in the lifting or transfering mineral water activity often causes pain in the waist and spine. This study is intended to analyze the work posture and the load level that is safe for workers to reduce the risk of MSDs, the analysis is carried out with the Recommended Weight Limit (RWL) method. From the calculation gained RWL origin = 15.36kg / 34.07lbs and destination RWL = 18.54kg / 41.12lbs. Then the origin LI value = 0.9 and destination LI = 0.8. From the results of the Lifting Index can be concluded that the risk level of this work is low and there is no problem with the lifting work. The weight of load raised does not exceed the recommended lifting limit, so there are no repairs to the work needed, but the activity must keep to get attention so the LI value can be maintained and the activity does not bear the risk of spinal injury.

Abstract

PT. X merupakan perusahaan yang bergerak di bidang fabrikasi dan manufaktur penghasil boiler yang terletak di Surabaya Jawa Timur. Sesuai dengan data laporan kecelakaan yang didokumentasikan oleh Environment Health Safety Officer (EHS) PT. X selama tahun 2015-2019 terdapat 50 kasus kecelakaan. Dari beberapa kasus yang telah terdokumentasi oleh EHS tersebut, terdapat beragam penyebab, ada beberapa kasus kecelakaan yang terjadi berulang kali dengan penyebab yang hampir sama. Bahkan telah dilakukan investigasi setelah terjadinya kecelakaan namun kecelakaan masih saja berulang. Hal ini membuktikan bahwa terdapat faktor lain yang tidak teridentifikasi atau adanya rekomendasi yang tidak efektif dari analisa yang telah dilakukan. Metode Human Factors Analysis and Classification System (HFACS) adalah suatu alat analisa kecelakaan yang digunakan untuk menganalisa suatu kecelakaan pada aspek Human Factor. Metode ini dikembangkan oleh Wiegmann dan Shappell pada tahun 2003. Metode ini didasarkan oleh model Swiss Cheese. Hasil analisis kecelakaan menggunakan metode HFACS, telah dibuktikan bahwa kecelakaan yang terjadi dikarenakan adanya kegagalan pada tindakan tidak aman yaitu kesalahan yang terjadi karena skill, kesalahan pengambilan keputusan, salah persepsi, pelanggaran yang dilakukan secara rutin, pelanggaran luar biasa. Berikutnya, kegagalan pada pre kondisi untuk tindakan tidak aman yaitu lingkungan fisik, lingkungan kerja (teknologi), crew resource management, mental yang merugikan perusahaan, fisik yang merugikan perusahaan. Selanjutnya, pada tahapan pengawasan yang membahayakan yaitu pengawasan yang tidak memadai, pengawasan yang gagal untuk mengetahui masalah, pelanggaran pengawasan. Tahapan yang terakhir adalah organisasi yaitu sumber daya manajemen, dan proses dalam organisasi. Hasil uji koefisien kontingensi terdapat asosiasi pada tahapan sumber daya manajemen dan pelanggaran pengawasan. Hasil uji kontingensi menunjukkan bahwa pada kasus ini hampir semua kegagalan mempengaruhi kegagalan di tingkat lain. Dengan demikian maka sebaiknya tidak hanya manajemen yang dikontrol, melainkan semua elemen yang berpengaruh harus tetap dikontrol sehingga dapat mencegah terjadinya kecelakaan yang disebabkan oleh human error.

Abstract

Based on data from the Health and Safety Executive, it can be concluded that as many as 90% of accidents are caused by human error. Coal mining is an industry that has a high risk of work accidents. If the mining process is not following procedures, then lives will be at stake. Human error probability assessment needs to be done because most accidents that occur in coal mining are caused by human error. HRA (Human Reliability Analysis) is part of the risk assessment process which aims to calculate the HEP value. HRA has been used in many studies to assess the risks involved in large, complex, and dangerous systems. The Success Likelihood Index Method (SLIM) is a method of analysis for human reliability. Prevention and control of human error need to be done at every stage of work. SLIM can analyze HEP at each stage of the work. SLIM aims to obtain a HEP value. To get the HEP value, we must first find the SLI (Success Likelihood Index) value. Finding the SLI value comes from a weighting questionnaire and PSF (Performance Shaping Factor) assessment that has been filled in by an expert judgment. After the HEP value is obtained, it can be seen which jobs fall into the safe risk level and which the risk cannot be accepted. Furthermore, risk reduction is carried out by making a task analysis of jobs that have a high hazard risk. Based on the research results, it can be seen the factors that cause a human error, namely unsafe conditions, unsafe actions, personal factors, and job factors. While the task that has the highest HEP value is on task 4, which is equal to 0,006932. The task with the smallest HEP value is task 1, amounting to 0,006478.

Abstract

As aneffort to prevent work accidents, companies need to implement a risk management that can be appliedthrough Hazard Identification Risk. Assessment and Determining Controls (HIRADC) contained in ISO45001: 2018. This research was conducted with the aim of knowing the suitability of the application ofHIRADC in polygon warehouse construction with ISO 45001: 2018 clause 6.1.2 “Hazard Identificationand Assessment of Risks and Opportunities”. The type of research used is descriptive with data collectionthrough direct observation to the field and interviews with several workers. Based on observations in thefield, the impact of potential hazards and risks can have a direct impact on workers. The controls appliedtend to be in terms of administration and use of PPE. Broadly speaking, the controls that have beenimplemented by PT.X are carried out effectively so that the implementation of HIRADC is in accordancewithISO45001:2018clause 6.1.2

Abstract

Preparedness is a part of the disaster management process which aims to prevent and minimize disaster risk which is pro-active before a disaster occurs. Fire disasters can occur anywhere, including in the Islamic boarding schools, this is because in the Islamic boarding schools there is not only a teaching and learning process, but there are additional activities such as fulfilling their daily needs such as the students also live in the cottage during their studies. This research was conducted in July-August 2020 which aims to analyze the relationship between the level of knowledge and attitudes of students towards fire disaster preparedness at Islamic Boarding School Pondok Karya at Manado City. Type of this research using analytic survey with a cross sectional approach, with a total sample of 89 people was taken using accidental techniques. The results of this study showed that as many as 60 students (67.4%) have a level of preparedness that is ready to cope with fire disasters with a good level of knowledge (47.2%) on fire management procedures although only 33 students (37.1%) knew how to use fire extinguisher. Statistical test result using chi square test show that there is a relationship between the level of knowledge (0.002) and attitude (0.000) with the level of preparedness in fire disaster preparedness at Islamic Boarding Scholl Pondok Karya at Manado City. Recommended for Islamic Boarding School Management are required to provide education on fire disaster management preparedness to students at least once a year, so that if a fire disaster occurs, the risk can be minimized.

Abstract

Based on data compliance with the use and release of PPE Covid-19 pandemic for emergency room nurses during October to December 2020, the use of PPE decreased from October to November, the release of PPE decreased from November to December. This study aims identify the description of the use PPE for emergency room nurses at Jemursari Islamic Hospital. The study used quantitative descriptive design with cross sectional study approach. The research population nurses in emergency room at Jemursari Islamic Hospital, Surabaya. The sample was 32 respondents using total sampling technique. The data used reports of compliance the use and release of PPE obtained through the PPI team and data knowledge, attitudes and actions towards through distributing questionnaires to respondents using the Descriptive Statistics test. The results showed that the majority of nurses were aged <= 30 years (75%), with a D3 education level (63%) and a tenure of > 4 years (72%). The knowledge and attitudes of emergency room nurses about the use of PPE when working are in the good category, while the behavior of most respondents in the complete category is using PPE by 62.5%. The majority of IGD nurses at the Surabaya Islamic Hospital Jemursari are aged <= 30 years, education level is D3 and working period is > 4 years. Knowledge, attitudes and behavior of nurses in the good category and the majority behave completely in wearing PPE. The agency is expected to maintain training on the use of PPE, tighten supervision and reward disciplined nurses.

Abstract

Good occupational health and safety (K3) needs to be done in an effort to reduce the possibility of accidents when doing work. To reduce the number of work accidents, it is necessary to control risk in the workplace through risk management, one of the methods is HIRARC. PT XYZ is a company engaged in providing environmental laboratory services. This company has several fields namely administration, analyst, test sample taker. Thearea most at risk is the part of the test sample taker. The purpose of this study is a Risk Assessment on the work of taking test samples at PT. XYZ. This type of research is qualitative with descriptive observational method, by describing the results of hazard identification and risk assessment on the work of taking test samples, in order to obtain a recommendation for hazard risk control using the HIRARC (Hazard Identification Risk Assessment Risk Control) method. Data collection techniques were carried out by observation, in-depth interviews and documentation. Sampling work carried out by test sampling officers PT. XYZ has 26 potential hazards. Test sample taker officer PT. XYZ has a low including the risk of skin irritation, eye irritation, minor injury, moderate includes the risk of muscle pain, fractures.and high includes the risk of injury to death, fire, heat stress, dehydration, respiratory infections. The recommended control refers to the control hierarchy, namely elimination, substitution, engineering, administrative and personal protective equipment. The conclusion of the test sampling job shows that the hazard identification stage of this work produces 26 potential hazards and has a risk level of low, moderate, high. The recommended risk control that has the most priority is the implementation of the OHS management system because the company is required to apply 64 basic criteria

Abstract

Losses caused by fires that occur in industry are very large, because they involve high asset values, and can cost lives. Fires can occur anywhere, one of which is in the office building of PT. XY which is classified as a light fire hazard. One form of security and prevention against fires is the installation of fire extinguishers. The purpose of this study was to evaluate the suitability of the application of a light fire extinguisher. The type of research in this study is qualitative, namely by comparing the actual condition of light fire extinguishers in the field with the Minister of Manpower and Transmigration Regulation Number 04 of 1980 through observation, interviews, documentation and measurement of the height and distance of the fire extinguisher. The results showed that the number of fire extinguishers as a whole was not appropriate, the type of fire extinguisher had been fulfilled properly, the condition of the fire extinguisher was still not up to standard, the placement of the fire extinguisher was still quite good at the distance between fire extinguishers but there were still some that were not suitable, the maintenance of the fire extinguisher was well maintained but still needs control and the level of knowledge of workers regarding the use of fire extinguishers is quite good but needs to be improved again because there are still some employees who have not received fire training. The conclusion that PT.XY, the, has implemented fire prevention by installing fire extinguisher, there are several indicator components that are not appropriate. Suggestions that can be given are to improve the fire extinguisher indicators that are not appropriate and always carry out routine monitoring to improve fire extinguisher maintenance and provide fire training to workers.

Abstract

Musculoskeletal disorders (MSDs) are disorders of the skeletal muscles. According to the Bureau of Labor Statistics (BLS), there were 387,820 cases ofmusculoskeletal disorders (MSDs) from the total injuries and illnesses of workers in 2011. This study aims to analyze the level of risk, occupational risk at work. position using the Rapid Upper Limb Assessment (RULA) method. to production workers at PT. Jayamas Medica Industri, Sidoarjo Regency. This study used an analytic observational research design with a cross sectional approach. The population of this study are workers in the manufacturing section. The resulting sample is 29 using the Slovin formula and the sampling technique using a random sampling system. The variables of this research are personal and work factors, work posture, RULA method, and Nordic Body Map (NBM) questionnaire. This analytic data only generates the frequency and percent distribution of each variable and is interpreted as tables and stories. The results showed that most of the respondents complained about the lower neck as much as 79% and the upper neck as much as 69%. With this, the researchers calculated using the RULA method, the results obtained from the RULA method were respondents with serious complaints that needed further examination. Summarizing the results and discussing workers' complaints or disturbances, the recommended suggestions are stretching, designing the workplace according to the needs of workers, conducting training and socialization activities. Therefore, the researcher designed the workstation as a table and chair.
