Abstract

Since the year 2000 there has been a steep increased in the number of HIV/AIDS patients in Indonesia, coming mostly from intravenous drug users. Antiretroviral treatment has been proved to decrease mortality and increase quality of life of HIV/AIDS patients. The treatment is given according to clinical condition of the patients, number of CD4 and viral load. In this study, CD4 and viral load were examined in 71 asymptomatic HIV patients originated from injecting-drug users. CD4 counting was performed by indirect immunoflouresence method using monoclonal antibody, and viral load was tested using PCR technique. Among 56 patients who has the number of CD4 more than 200/mm3, 30 patients (55.4 %) has viral load more than 55,000 copies/ml and 35.7% has viral load 5,000-55,000 copies/ml. Correlation between the number of CD4 and viral load gave the r value of 0.194. It is concluded that there is no association between the number of CD4 and viral load in drug user HIV/AIDS patients. The treatment of HIV/AIDS for these patients should be given according to the viral load. (Med J Indones 2002; 11: 143-7)Keywords: viral load, CD4, injecting-drug user

Abstract

Penelitian open-label, non-komparatif ini dilakukan untuk mengevaluasi efektivitas dan keamanan recombinant human G-CSF produksi Asia sebagai profilaksis primer dalam pencegahan neutropenia derajat berat pada pasien usia lanjut (>60 tahun) dengan limfoma non-Hodgkin (LNH) derajat sedang dan lanjut (stadium II,III,IV) yang mendapat terapi CHOP (siklofosfamid, doksorubisin, vinkristin). Profilaksis primer recombinant human G-CSF (rhG-CSF) produksi Asia dapat mengurangi median durasi neutropenia derajat 4 pada siklus sitostatistika ke-1 dan ke-2 menjadi tiga hari, sementara median durasi neutropenia derajat 3 pada siklus sitostistika ke-1 menjadi dua hari dan pada siklus sitostatistika ke-2 menjadi dua setengah hari, dari median durasi neutropenia grade 4 dan grade 3 tanpa G-CSF, yaitu empat dan lima hari berurutan. Febrile neutropenia ditemukan pada 7 pasien yang mendapat rhG-CSF produksi Asia (24.1%), lebih rendah jika dibandingkan studi tanpa rhG-CFS (31.3-34% FN). Tiga pasien mendapat rhG-CSF produksi Asia (10,3%) dirawat inap akibat febrile neutropenia, lebih rendah jika dibandingkan rawat inap pada studi tanpa rhG-CSF (24-28%). Kejadian yang tidak diinginkan terbanyak adalah mual dan muntah yang terjadi pada 9 (31%) pasien. Sebagai kesimpulan, penggunaan rhG-CSF produksi Asia untuk profilaksis primer pada pasien LNH usia lanjut yang mendapat regimen CHOP dapat mengurangi durasi neutropenia, mengurangi kejadian febrile neutropenia, dan angka rawat inap akibat febrile neutropenia.Kata kunci : Efektivitas, keamanan, G-CSF, LNH pada usia lanjut

Abstract

Background: This study aimed to compare the treatment outcomes between the use of breast-conserving treatment (BCT) and mastectomy for T1-2N0 breast cancer patients.Methods: This study retrospectively reviewed T1-2N0 breast cancer patients who received treatment between January 2001 and December 2010 at Department of Radiotherapy Cipto Mangunkusumo Hospital and Jakarta Breast Center. The endpoints of this study were overall survival (OS), local  recurrence (LR), contra-lateral breast cancer (CBC), distant metastasis (DM), and disease-free survival (DFS).Results: Among the 262 eligible patients, 200 (76.3%) patients underwent BCT while 62 (23.7%) patients underwent mastectomy. There were no differences between BCT and mastectomy groups in 5-Y OS (88.2% vs 86.7%, p = 0,743), LR (7.4% vs 2.7%, p = 0.85), CBC (3.4% vs 5.3%, p = 0.906), DM (17.7% vs 37.7%, p = 0.212), and DFS (78.5% vs 60.7%, p = 0.163). In multivariate analysis, grade 3 was associated with worse OS (HR 2.79; 95% CI 1.08 – 7.21, p = 0.03) and DFS (HR 2.32; 95% CI 1.06 – 5.06). Premenopausal women were associated with decreased risk of DM (HR 0.37; 95% CI 0.17 – 0.80) and DFS (HR 0.38; 95% CI 0.19 – 0.78).Conclusion: BCT and mastectomy showed similar outcome in terms of OS, LR, CBC, DM, and DFS. Abstract

Pancreatic carcinoma commonly occur in patients over 60 years. It is usually manifested as abdominal pain, jaundice, and pancreatic mass. In this report, a pancreatic carcinoma occurred in young woman is presented. A surgical drainage was done and followed by Whipple resection. Abstract

Introduction. Health-related quality of life among HIV patients can be used as one of several indicators of successful therapy. WHOQOL-HIV BREF questionnaire is one of the instruments used to assess the patients’ quality of life that has been used in many countries, but never been tested for its validity and reliability in Indonesia. This study aims to determine the validity and reliability of the questionnaire WHOQOL-HIV BREF in bahasa Indonesia as a tool for measuring the quality of life of HIV patients. Methods. A cross-sectional study was conducted in HIV Integrated Service Unit Cipto Mangunkusumo General Hospital (RSCM) Jakarta in November 2016 with consecutive sampling method. The study was conducted in two phases: first phase for the language and cultural adaptation process and second phase to test the validity and reliability of the questionnaire. Results. Fifty six respondents filled the questionnaire, 69.6% of them were male. Using a multi-trait scaling analysis, correlation coefficient value had a high correlation with the total score domain. This meant that it had a good validation. Correlation between questionnaire domain WHOQOL-HIV BREF and SF-36 questionnaire domain obtained 6 significant domains that shown strong correlation coefficient (r = 0.60 to 0.79; p <0.005). Reliability of the questionnaire was assessed by intraclass correlation coefficient of each domain, which was range from 0.401 to 0.484 with Cronbach Alpha 0.513 to 0.798 . Conclusions. The questionnaire WHOQOL-HIV BREF in the Indonesian language is valid and reliable.
