Abstract

Objective: Hypospadias is a malformation in urethra which has many range of severity. A patient with Isolated hypospadias (IH), a mild disorder of sex development (DSD) has a hypospadias phenotype only. Hypospadias is considered as multifactorial disease in which genetic factors contribute to its development. Chromosome analysis in DSD including hypospadias is conducted for gender assignment and other possible genetic contributions. This analysis solely could not elucidate all genetic causes of hypospadias. Polymorphism of V89L in SRD5A2 is suggested as one of genetic risk factors of hypospadias. To determine the genetic risk factor and pattern of inheritance, a good pedigree construction is required. Material & methods: 35 eligible subjects with IH admitted to Center for Biomedical Research (CEBIOR) during 2012-2015 were randomly selected. 35 normal male as control were included in this study. Data on three generation pedigrees were collected from medical records in 35 affected subjects. Chromosome analyses were done by using G-banding technique. Polymorphism analysis of V89L in SRD5A2 gene was done using PCR-RFLP technique in all samples. Results: From the 35 affected subjects, the most frequent phenotype was penile hypospadias (47%), a pair of twins were monozygotic and one had a cousin diagnosed with urogenital abnormalities (i.e micropenis and chordae). All subjects had 46,XY chromosome. No chromosomal aberration was found. No positive correlation between polymorphism of V89L in SRD5A2 and risk of hypospadias (PR of CC+CG vs GG=1.0, 95% CI: 0.342−2.921, p value=1.0). Conclusion: The pedigree data from our study implies tendency of genetic involvement in hypospadias cases. There were no chromosomal aberrations in hypospadias cases. The finding on polymorphism of V89L in SRD5A2 gene does not support that of previous studies.

Abstract

Objective: Excessive use of MSG in everyday life can cause infertility to men. Exposure to MSG can cause cells to experience oxidative stress which subsequently triggers the generation of free radicals. Free radicals can be resisted with antioxidants. Material & Methods: This research was a laboratory experimental test with a post-test only control group design using male mice as the experimental object. The statistical test used the One Way ANOVA test. Results: In this research, 5 male mice were assigned into 4 groups and 1 control group. Using the One Way ANOVA test, it was found that there were significant differences in ≥ 2 treatment groups in the motility group of spermatozoa with active movement (p < 0.001) and the motility group of spermatozoa with weak movement (p = 0.036). The two groups showed significant differences. The Post hoc LSD test in the motility group with active movement showed significant differences between the control group with MSG treatment (p < 0.001) and the treatment group given Moringa leaves extract of 600 mg/kgW (p < 0.001) and 1200 mg/kgW (p < 0.001). Conclusion: Moringa leaves extract can effect the improvement of the motility of spermatozoa exposed to monosodium glutamate at a given dose of 300 mg/kgW and 600 mg/kgW.

Abstract

Latar belakang; Nyeri merupkan pengalaman sensori dan emosional yang tidak menyenangkan karena kerusakan jaringan yang aktual ataupun potensial. Penatalaksanaan nyeri dapat dilakukan baik secara farmakologi ataupun non farmakologi. Penatalaksanaan nyeri secara farmakologi diketahui memiliki efek samping seperti adanya ketakutan pasien akan adiksi dan ketergantungan, depresi pernafasan, dispepsia, pendarahan lambung dan kerusakan saluran cerna serta gangguan pada ginjal. Oleh karena itu, penting artinnya untuk melakukan terapi non farmakologi sebagai terapi komplementer dan alternatif terhadap penatalaksanaan nyeri. Akupresur merupakan tehnik non farmakologi yang telah terbukti berdasarkan penelitian dapat mengurangi tingkat nyeri pada berbagai keluhan nyeri.Tujuan dari penulisan artikel ini ini adalah untuk mengetahui pengaruh akupresur sebagai manajemen nyeri non farmakologis untuk menurunkan tingkat nyeriMetode; Metode pada penulisan artikel ini adalah literature review dari data based pubmed, scientdirect dan google scholar. Artikel dengan bahasa Inggris full text dan batasan penerbitan artikel dari tahun 2009-2019.Hasil; Terdapat penurunan tingkat nyeri pada subjek yang diberikan terapi akupresur pada berbagai keluhan nyeri seperti nyeri disminore, nyeri post SC, nyeri pada persalinan, nyeri kepala, nyeri post TKR, Low back pain, nyeri kanker payudara. Kata kunci : Pain Management, Non Farmachological Therapy, Acupressure

Abstract

The effect of eurycoma longifolia and pimpinella alpina on spermatogenesis in male Spraque Dawly Background: Alternative medicine used to overcome infertility among others is using traditional herbs. Crops which often used are pasak bumi (Eurycoma longifolia) and purwaceng (Pimpinella alpina). Several studies reported that pasak bumi and purwaceng could improve testosterone, luteinizing hormone and follicle stimulating hormone level in animal. The aim of the study was to analyze whether the extract of Eurycoma longifolia and Pimpinella alpina had effect on motility and spermatogenesis in white male Spraque Dawly mouse.Material and Method: Thirty white male mice, aged 40 days, with mean body weight of 200 grams were divided into 10 groups randomly. Each mouse was placed into individual cage and was given 25 mg Pimpinella alpina, 25 mg Eurycoma longifolia Jack and plain water in control group. Food was given ad libitum. All the treatment was given every morning for 53 days successively then the testis and epididymis were taken for analysis. Result: Administration of Eurycoma longifolia and Pimpinella alpina increased the number and motility of sperms as well as the spermatogenesis. However, there was no difference between the effect of Eurycoma longifolia and Pimpinella alpine.Conclusion: Extract of Eurycoma longifolia and Pimpinella alpina can improve spermatogenesis.ABSTRAKLatar belakang: Pengobatan alternatif untuk mengatasi infertilitas antara lain adalah dengan menggunakan pasak bumi (Eurycoma longifolia Jack) dan purwaceng (Pimpinella alpina). Pada beberapa penelitian keduanya terbukti meningkatkan kadar hormon testosteron, luteinizing hormone dan follicle stimulating hormone. Tujuan dari penelitian ini adalah untuk mempelajari apakah pemberian ekstrak pasak bumi dan purwaceng berpengaruh terhadap peningkatan derajat spermatogenesis serta jumlah dan motilitas sperma pada tikus putih jantan Spraque Dawly.Metode: Tiga puluh ekor tikus putih jantan, umur 40 hari dengan berat badan rata-rata 200 gram dibagi menjadi 10 kelompok secara acak. Tiap-tiap kelompok terdiri dari 3 ekor tikus. Setiap tikus ditempatkan dalam kandang individual. Makanan yang diberikan secara ad libitum. Masing-masing kelompok diberi perlakuan ekstrak akar pasak bumi sebanyak 25 mg, purwaceng sebanyak 25 mg dan akuades sebagai kontrol. Semua perlakuan tersebut diberikan tiap pagi selama 53 hari berturut-turut, kemudian diperiksa testis untuk derajat spermatogenesisnya dan epididimisnya. Untuk pemeriksaan jumlah dan motilitasnyaHasil: Pemberian ekstrak pasak bumi maupun purwaceng meningkatkan jumlah, motilitas spermatozoa serta derajat spermato-genesis tikus bila dibandingkan dengan kontrol (p<0,01). Efek pemberian pasak bumi tidak berbeda dengan pemberian purwaceng (p>0,01).Simpulan: Ekstrak Eurycoma longifolia dan Pimpinella alpine meningkatkan derajat spermatogenesis.

Abstract

Background: The sex determination process requires distinct signaling pathways to generate either testis or ovaries from the same precursor structures, the primordial gonad. Deviations of this signaling mechanism may result in disorders/differences of sex development (DSD). The 46, XX testicular DSD is a rare genetic condition identified by a discrepancy between genetic and phenotypic sex caused sex reversal syndrome. Case Presentation: We describe the case of a 5 years-old 46, XX boy with ambiguous genitalia. On physical examination he had severe hypospadias, bifid scrotum, micropenis and palpable bilateral testes. Cytogenetic analysis of patient reveals a 46, XX karyotype. Hormonal assay showed low level of FSH, LH and Testosterone and there was no evidence of Mullerian structures based on pelvic imaging. The histopathology of gonadal tissue showed a Leydig cell hyperplasia which gives the impression of Sertoli cell nodule. Polymerase chain reaction (PCR) analysis failed to identify the presence of SRY gene, therefore a diagnosis of 46, XX Testicular DSD with SRY-negative was established. Conclusion: This report presents a rare case of SRY-negative 46, XX Testicular DSD in a boy with ambiguous genitalia. A comprehensive management including clinical, cytogenetic and molecular analyses have indicated that undiscovered genetic or environmental factors needs to be elucidated. It is important to carry out further molecular testing to establish precise diagnosis of DSD and to provide appropriate genetic counseling for patients and their family.

Abstract

Background : Disorder of sex development (DSD) patients require comprehensive management to improve quality of life. A standardized management protocol for patients in Indonesia is not yet available resulting in patients infrequently received a proper diagnosis. This study reported a multidisciplinary management DSD in Indonesia based on minimal diagnostic facilities and expertise in developing country.Objectives : The purpose of the study is to review the management of DSD patients in Indonesia relates to providing appropriate gender assignment and to improving patients quality of life.Methodology : We analyzed the records of DSD patient admitted to the division of Human Genetics Center for Biomedical Research (CEBIOR) Faculty of Medicine Diponegoro University, Semarang, Indonesia from May 2004 - December 2015. Data were collected and analyzed for physical examination, family pedigree karyotyping, hormonal assays and  psychosocial.  Other examination such as ultrasonography, Xray and Cytoscopy were also recorded for selected cases. Bimonthly, Sexual Adjustment Team (SAT) meeting was recorded.Results : From the total 617 DSD cases we found 426 cases (69,04 %) with 46, XY DSD, 117 cases (18,96%) with 46,XX DSD and 74 cases (12%) with sex chromosome DSD. Most of the patients in the group of 46, XY DSD are Unknown Male Undervirilization (UMU) with 256 cases (60.09%). As the majority cases of 46, XX DSD was Congenital Adrenal Hyperplasia with 81 cases (69.23%). The remaining cases were Androgen Action Disorder (AAD) with 140 cases (32.86%), 46, XY DSD Gonadal Dysgenesis with 30 cases (7.04%), Androgen Excess Disorders with 3 cases (2.56%), Defect of Mullerian Development with 19 cases (16,24%), 3 cases (2.56%) of Androgen Excess and 3 cases (2.56%) of 46, XX Gonadal Dysgenesis.Conclusion : Comprehensive management for DSD Patients help patient in diagnosis, gender assignment and support patient to improve quality of life. This multidisciplinary of DSD team is the only team in Indonesia that can be used as a model for other center in Indonesia as well as other developing countries with minimal diagnostic facilities.

Abstract

Background: Hypospadia is believed to be a multifactorial disease.  The risk factors that may induce the formation of hypospadias are environmental factors, endocrine disruptors, and genetic factors. The aim of this study was to describe the profile of hypospadias patients who visited the Center for Biomedical Research (CEBIOR), Faculty of Medicine, Diponegoro University, Semarang, Indonesia.Methods: This study is an observational study, using patients’ medical record in CEBIOR from 2010 to 2012. The hypospadias cases were classified into syndromic, isolated and severe hypospadias based on their phenotype. The history of pregnancy, birth characteristics, and family history of the patients were described.Results: There were 120 cases of hypospadias, consisted of 48.33% severe hypospadias, 41.67% mild isolated hypospadias and 10% syndromic hypospadias. There were 38.33% hypospadias cases whose mothers were being exposed to repellant usage and 39.17% cases whose fathers were smoking. Forty (33.33%) probands’ mothers were aged above 35 years old when they gave birth to their affected son.Conclusion: Majority of hypospadias cases were severe and mild isolated. Environtmental factors including maternal age more than 35 years old, use of repellant, and smoking fathers were found in this study.

Abstract

Self Care is an implementation of activities based on self-awareness and conducted by the individual itself to meet the needs in maintaining life, health, and well-being in accordance with the state of health and illness.Employment of independence conducted by patients Diabetes Mellitus (DM) called self care DM with one of the goals in performing independent actions is to control the Diabetes Mellitus so as to minimize the occurrence of complications.Edukasi is a source of information that can help diabetes mellitus patients in taking decisions appropriate self-care nursing actions.The purpose of this study is to determine the influence of peer education against self care in patients with Diabetes Mellitus. Type of quantitative research with quasi experimental research design pre-test and post-test control group. The number of samples in this study were 21 patients with Diabetes Mellitus in the Hospital according to inclusion criteria and exclusion in the study. Sampling using purposive sampling technique, data collection using questionnaire The Personal Models Of Self Care Diabetes Questionnaire that consists of 9 items of questions. The research analysis used univariate and frequency distribution. The results showed that patients with Diabetes Mellitus self care category as much as 16, 7% and self care with less good category as much as 83, 3 %. Based on the results of the study, it can be concluded that self care nursing diabetes mellitus patients in performing self actions to minimize the occurrence of complications in the category less good.

Abstract

Postpartum pain and fatigue, impaired mobility, motivation, and psychological disorders of mother cause disturbances in the excretion of the breast which have an impact on the health of mother and baby. The SPEOS (Endorphin, Oxytocin, and Suggestive Massage Stimulation) methods is an alternative to help increase breastfeeding that combination of endorphin, oxytocin, and suggestive massage stimulation carried out sequentially which will feel the relaxation that stimulates the brain to release endorphin, prolactin and oxytocin hormones so that breast milk becomes smooth. The objective of this study to determine the effectiveness of the SPEOS methods in increasing breast milk production in postpartum mothers. A literature review through a review of nursing articles to identify SPEOS methods in increasing breast milk production in postpartum mothers. The author identifies research questions, identifies relevant articles, select articles, maps data and summarizes, analyzes and reports data. The databases used are Scopus, Medica Local Update, One Search and Google Scholar. Articles were filtered according to the inclusion and exclusion criteria. 7 articles include the inclusion and exclusion criteria requirements, then the article was synthesized to see which was affected by SPEOS method, the duration of the SPEOS method, and the time the SPEOS method war administered. This literature review shows that the SPEOS methods effective in increasing breast milk production in postpartum mothers. The SPEOS method not only helps physically but psychologically through oxytocin, endorphin massage, and suggestive. The SPEOS method is carried out for a minimum of 3 days to get an increase in milk production. This method should be done 2 times a day for maximum results for at least 14 minutes. The SPEOS methods are effective in increasing breast milk production in postpartum mothers.

Abstract

Postpartum stress mostly experienced by first-time mothers. This stressful condition indicates that some new mothers experience emotional changes, sometimes feeling happy and at times almost feeling sad for no reason. This study aims to understand a deeper understanding of interventions for postpartum anxiety. A literature review was conducted by looking for literature related to the theme taken. The articles obtained were 9 quantitative studies, 7 articles from Indonesia and 2 articles from other countries. Most interventions are massage therapy and various types of relaxation (aromatherapy, hypnotherapy, yoga) to reduce the level of anxiety of postpartum mothers. This review helps provide several interventions that can be carried out on postpartum mothers in a nonpharmacological, simple and effective interventions that can be carried out in postpartum mothers in reducing the level of anxiety of postpartum mothers, besides that it can improve sleep quality, reduce levels of depression, reduce stress, can increase breast milk production and increase comfort in postpartum mothers.
