Abstract

Skrining peresepan pasien berbasis teknologi informasi sangat dibutuhkan sekarang ini. Penggunaan teknologi dalam pelayanan resep dapat meningkatkan kinerja tenaga kefarmasian di instalasi rawat jalan dalam melakukan skrining resep pasien. Tujuan dari penelitian ini untuk menganalisis kebutuhan sistem informasi manajemen skrining resep dan membuat model desain sistem informasi manajemen skrining resep sesuai dengan kebutuhan puskesmas wilayah kota Yogyakarta. Pengembangan sistem dalam penelitian ini menggunakan metode prototipe dimulai dari analisis kebutuhan dan desain sistem. Pengambilan data analisis kebutuhan sistem dilakukan secara retrospektif (data resep bulan Mei 2015) dan prospektif (wawancara dengan kerangka PIECES (performance, information, economic, control, efficiency dan service)), setelah mendapatkan analisis kebutuhan sistem selanjutnya pembuatan desain sistem dengan menggunakan diagram konteks, data flow diagram (DFD) dan entity relationship data (ERD). Kriteria sampel dalam penelitian ini adalah resep pasien BPJS rawat jalan dan petugas farmasi yang bertanggung jawab di instalasi farmasi rawat jalan di puskesmas wilayah kota Yogyakarta. Penelitian ini dianalisis secara deskriptif kualitasif. Berdasarkan hasil analisis kebutuhan sistem, skrining klinis merupakan skrining yang paling tinggi menimbulkan error. Sedangkan hasil studi kelayakan berdasarkan kerangka PIECES (Performance, Information, Economic, Control, Efficiency, dan Service) dengan adanya sistem informasi skrining resep ini dapat membantu tenaga farmasi dalam melakukan skrining resep dan meningkatkan waktu proses pelayanan resep sehingga sistem ini layak untuk dikembangkan. Berdasarkan desain sistem, desain input dan desain output, desain sistem informasi manajemen skrining resep pasien rawat jalan ini memenuhi kriteria sehingga siap untuk diimplentasikan ke dalam sistem.Kata kunci: skrining resep, desain sistem, puskesmas

Abstract

Technology and information systems can help an organization to execute various activities accurately, punctually and in good quality, so that can increase the efficiency, effectivenesss and competitiveness of the organization. Success in implementing information systems can be determined by an evaluation of information systems that have been applied. The purpose of this study was to evaluate the application of information systems that have been implemented at Immanuel Hospital Bandung, using information systems success models developed by DeLone and McLean. This study was a descriptive study using cross sectional design. Data was obtained from questionnaire to employees and in depth interview to Director of Hospital, Head of Pharmacy Departement and Head of Technology and Information system departement. The research examined variable is user satisfaction to the accuracy, completeness, relevance, timeliness, auditability, and the reliability of pharmacy management information system at Immanuel Bandung Hospital. The data which have been collected from questionnaires then analyzed using qualitative analysis. The results of the study showed that the user satisfaction management information system in Immanuel Bandung Hospital were in medium category. The average user satisfaction ranges between 2.50 to 3.33 of 5 scales. The highest level of user satisfaction of information systems management was on timeliness aspects, auditability, accuracy and completeness. Relevance and reliability had the lowest satisfaction.Keywords : evaluation, pharmacy management information system, user perceptions.

Abstract

In increasing social health services there are a lot of effort that have been done by the government such as providing facilities of health services such as pharmacy. As an effort to make phamacist can give a profesional pharmacy practice based on the decision of Health Minister Republic of Indonesia No.27/Menkes/SK/IX/2004 about the Standard of Pharmacy Practice in pharmacy community the goal is to find out the figure of Standards of Pharmacy Practice implementation in pharmac community at Yogyakarta City. The results of the research generally showed that the standards of pharmacy practice in Yogyakarta has not been implemented well. Total score for the standards pharmacy practice based on pharmacists respondent 21% (good), 79% (enough); employees respondent 21% (good), 75% (enough), 4% (bad) and patients respondent 38% (good), 42% (enough), 20% (bad). The inhibiting factors towards the implementation standards of pharmacy practice such as education programme, seminar or another thing that rarely held that make the opportunity for self development very limit, the socialization are rare and control function from institution that have authority are weak so monitoring and evaluation should do periodicaly. Keywords : Standards of Pharmacy Practice, Pharmacy, Pharmacist, Employees, Patients

Abstract

Epilepsi merupakan suatu penyakit kronik dengan gangguan yang bersifat heterogen, multifaset yang menjadikan layanan farmasi kliniknya menjdi kompleks. Tantangannya antara lain adalah  masih adanya 25-30% yang belum terkendali dengan obat, masalah dalam farmakokinetika klinik, dan adherence yang  kurang baik. Tujuan dari penelitian ini adalah untuk mengevaluasi layanan farmasi klinik dengan mengukur pencapaian berbagai outcome. Penelitian ini adalah penelitian observasional pada  pasien epilepsi dengan kejang umum yang dilaksanakan secara prospektif dengan disain cross sectional, yang mengamati adherence, kadar obat dalam serum, serta frekuensi kejang. Pasien mendapat layanan farmasi klinik selama 6 bulan berupa asesmen ada tidaknya Drug Related Problem (DRP) pada setiap kunjungan poliklinik yang dilaksanakan 1 bulan sekali,  pemberian rekomendasi terapi kepada klinisi, konseling dengan alat bantu, monitoring dengan berbagai metode. Terdapat  peningkatan mean skor adherence antara skor 1 dan 6 bulan dari 3,95 menjadi 4,07 setelah 6 bulan terapi, meskipun tidak  signifikan secara Wilcoxon-Sign. Secara umum adherence pasien dalam 6 bulan terapi tetap (berkisar 80%). Kadar fenitoin serum  rata-rata pada 1 bulan 6,67 ± 6,65 mg/L tidak berubah secara  signifikan secara Wilcoxon-Sign pada 6 bulan terapi 6,07 ± 5,51 mg/L. Hasil pengujian korelasi antara kadar fenitoin dengan skor MMAS-8 menunjukkan tidak adanya korelasi rho= -0,051, P=0,73. Remisi terminal 6 bulan diperoleh sejumlah 43 pasien atau 86% dan sisanya sebanyak 7 orang masih menunjukkan  kejang, sedangkan reduksi kejang ≥50% sebesar 46 pasien (92%). Layanan farmasi klinik memberikan dampak positif dalam meningkatkan pencapaian hasil terapi pasien. Kata Kunci: epilepsi, farmasi Klinik, kepatuhan.

Abstract

Currently, the most effective therapeutic agent as a Stress Related Mucosal Disease (SRMD) prophylaxis therapy is the group of Proton Pump Inhibitor (PPI). In Indonesia, PPI agent most commonly used is pantoprazole and omeprazole, for they are already available in the form of injection and also available in generic product. This study aimed to determine the effectiveness and the therapy cost of omeprazole and pantoprazole used as a prophylaxis agent of SRMD. This study used observational analytic method using a retrospective cohort study. The subjects were patients in ICU of Bethesda Hospital in Yogyakarta with age ≥ 18 year old who received the therapy of omeprazole or pantoprazole as a prophylaxis of SRMD during January 2012 – September 2016 who meet the inclusion and exclusion criteria. The effectivity of therapy was seen by the bleeding events. The cost was calculated based on the amount of fee required by the patient, both the use of the medicine as a prophylaxis therapy of SRMD and for the bleeding therapy of SRMD. The value of Expected Monetary Value (EMV) was then calculated using the method of Decision Tree. Subject in this study were 152 patients, in which there were 76 patients on each group therapy of pantoprazole and omeprazole. The comparison between the number of patients who did not experience bleeding between group of pantoprazole and group of omeprazole is 85.5%: 81.6% (p = 0.512). The value of EMV in the pantoprazole group is Rp. 431.490,76, while the omeprazole group is Rp. 382.042,57. There is no difference in the effectivity between pantoprazole and omeprazole as a prophylaxis of SRMD, and the cost for SRMD prophylaxis is cheaper in the omeprazole therapy than in the pantoprazole therapy.

Abstract

Skizofrenia merupakan salah satu gangguan kejiwaan yang diakibatkan oleh kerusakan pada otak. Salah satu penanganan skizofrenia adalah dengan memberikan antipsikotik untuk mencegah gejala yang timbul. Penelitian ini bertujuan untuk melihat gambaran pola penggunaan antipsikotik pada pasien skizofrenia di ruang rawat inap Rumah Sakit Jiwa Tampan Pekanbaru. Penelitian ini merupakan penelitian observasional jenis studi deskriptif dengan menggunakan data rekam medik. Penelitian ini dilakukan pada 81 rekam medik pasien periode Januari 2015 – Juni 2015 yang menerima pengobatan antipsikotik dan dianalisis dengan analisis univariat. Data yang diperoleh menunjukkan terapi kombinasi lebih banyak digunakan (95,08%) dari pada terapi tunggal (4,96%). Pada terapi tunggal antipsikotik yang paling banyak digunakan adalah haloperidol dan risperidon dengan persentase yang sama (2,46%) dan pada terapi kombinasi antipsikotik yang paling banyak digunakan adalah haloperidol dan klorpromazin (37,03%). Kategori pengobatan yang paling banyak digunakan adalah pengobatan antipsikotik tipikal (56,79%), atipikal (3,7%) dan kombinasi tipikal dan atipikal (39,5%).Kata kunci: skizofrenia, antipsikotik, Rumah Sakit Jiwa Tampan Pekanbaru

Abstract

Dalam sistem pelayanan kesehatan di EraJaminan Kesehatan Nasional (JKN), pemerintah menerapkan proses pengadaan obat berdasarkan elektronik katalog (e-katalog) melalui mekanisme e-purchasing dan manual-purchasing. Penerapan sistem akan berpengaruh pada efisiensi pengadaan dan ketersediaan obat yang dapat dinilai dari persepsi farmasi, pelaksana pengadaan dan perawat. Penelitian bertujuan untuk melihat hubungan penerapan e-katalog terhadap efisiensi pengadaan dan ketersediaan obat di RSUD Kelas B Yogyakarta. Penelitian merupakan penelitian deskriptif analitik dengan desain survei cross sectional. Data diambil secara kuantitatif dengan menggunakan kuesioner dengan responden farmasi, pelaksana pengadaan dan perawat. Metode sampling untuk responden perawat sebanyak 120 responden sedangkan responden farmasi dan pelaksana pengadaan dengan populasi sebanyak 21 responden. Data dianalisis menggunakan uji regresi linear untuk melihat hubungan penerapan e-katalog dengan efisiensi pengadaan dan ketersediaan obat.Pengaruh e-purchasing terhadap efisiensi pengadaan pada responden farmasi dan pelaksana pengadaan nilai signifikansi (p) untuk indikator persiapan, pelaksanaan dan manfaat kendala 0,001, 0,024, 0,007 sedangkan manual-purchasing nilai signifikan untuk indikator pelaksanaan 0,014, dengan koefisien korelasi (r) indikator persiapan e-purchasing sebesar 0,690, menunjukkan hubungan antara persiapan e-purchasing dengan efisiensi sangat kuat, sedangkan untuk responden perawat pengaruh penerapan e-katalog baik secara e-purchasing maupun manual-purchasing memiliki hubungan terhadap ketersediaan obat nilai signifikansi (p) 0,000, dan nilai koefisien korelasi (r) 0,765 yang menunjukkan hubungan yang kuat antara penerapan e-purchasing dan manual-purchasing dengan ketersediaan obat. Penerapan e-katalog baik secara e-purchasing dan manual-purchasing meliputi indikator persiapan, pelaksanaan, dan manfaat kendala memiliki hubungan yang signifikan dengan efisiensi pengadaan dan ketersediaan obat di RSUD Kelas B di Yogyakarta.

Abstract

Drugs supply management aims to control the drugs supply and make sure that the drugs in good condition, fairly distributed, and fulfill the types and quantity of the drugs needed by health service. Health department of Cilacap District faced many obstacles in drugs supply management during 2008-2010. Therefore, evaluation of drugs supply management in health department of Kabupaten Cilacap during 2008-2010 certainly needed to make the better drugs supply. The research conducted with observational method using descriptive monitoring. Data were collected retrospectively using data of planning, drugs supply and storage, and drugs consumption in health department of Cilacap District. Indicators that used in the research were indicators that determined by health Department of Indonesia in 2006. Primary data were gathered through interview with 3 groups: policy group, drugs supply group, and drugs consumption group or drugs management. Values of drugs management indicators –based on pedoman supervisi dan evaluasi obat publik dan perbekalan kesehatan were gathered through evaluation. The evaluation results show that in 2008, 2009, 2010, the indicators were not fit with the standard indicator, such as: indicator of drugs’ planning accuracy (147%, 108% and 145%), drugs charge per person in Cilacap District (Rp.1.342, Rp.2.000, Rp.4.615) accuracy level of financing the drugs supply (94,60%, 95,04%, 98,30%), and percentage of financing drugs supply (24,8%, 14,8%, 49%). Indicators that fit with the standard indicators were such as: Level of drugs supply (16, 5, and 7 months), accuracy level of drugs item with DOEN 52,4% during 2008-2010, generic drugs supply (89,6%, 79,1%, 88,2%), accuracy level of drugs distribution 100% during 2008-2010, accuracy level of LPLPO delivery time, drugs supply is fit with the drugs demand (109%, 160%, 167%), and drugs charge per visit per prescription (Rp.7.512, Rp. 8.180, Rp. 7.536). Keywords: Drugs Supply Management, Evaluation, Indicators, Health Department of Kabupaten Cilacap

Abstract

Perubahan model pembayaran dari model fee for service menjadi kapitasi dan sistem pembayaran langsung di fasilitas kesehatan penunjang masih menimbulkan masalah mutu pelayanan. Akibatnya masyarakat masih ragu dengan mutu pelayanan yang diberikan. Penelitian ini bertujuan untuk melihat sistem pembayaran JKN dilihat dari sudut pandang pasien di Apotek Program Rujuk Balik (PRB), apotek jejaring, dan apotek klinik pratama. Metode penelitian ini adalah deskriptif analitik dengan desain survey cross sectional. Data diambil secara kuantitatif menggunakan kuesioner dan data kualitatif melalui wawancara. Kuesioner yang terdiri dari indikator kualitas, jumlah, ketersediaan, dan pembayaran obat diberikan kepada 205 pasien rawat jalan bulan Agustus sampai September 2015 di 18 spotek di D.I Yogyakarta. Data dianalisis menggunakan uji statistik terkomputerisasi untuk mengetahui perbedaan persepsi pasien dan mengetahui hubungan sosio-demografi serta jenis kepesertaan terhadap persepsi pasien. Hasil analisis didapatkan bahwa terdapat perbedaan persepsi pasien pada indikator kualitas (p = 0,000) dan indikator ketersediaan (p = 0,000), namun tidak terdapat perbedaan persepsi pasien pada indikator jumlah (p= 0,667) dan indikatorpembayaran (p = 0,057). Terdapat pengaruh karateristik pasien terhadap persepsi pasien berdasarkan jenis apotek ( p = 0,006) dan usia (p = 0,019), namun tidak terdapat pengaruh sosio-demografi pasien: jenis kelamin, pendidikan, pekerjaan, dan penghasilan perbulan terhadap persepsi, dengan nilai p berturut-turut sebagai berikut: 0,937; 0,233; 0,605; 0,945, begitu juga dengan jenis kepesertaan pasien (p = 0,050) yang tidak berpengaruh terhadap persepsi.Kata kunci: sistem pembiayaan, pelayanan, persepsi, pasien

Abstract

Digoxin is a medicine with narrow therapeutic index. Digoxin is widely used for heart failure treatment. Adjustment of digoxin dose is needed in patient with renal dysfunction. This research were done to evaluate digoxin dose in heart failure patient with renal dysfunction. This research was retrospective and prospective research, performed in Dr. Sardjito hospital during January 2010 to March 2011. Data were collected from patient medical record and then evaluated with descriptive method. From 43 patients, percentage men compare to women were 56 % : 44 %. Most patients come from age of 50 – 59 (34.88%). Dose calculation based on pharmacokinetic concept found that some of digoxin dose in patients were exceed digoxin therapeutic concentration. From 19 women patients there were 11 patients use digoxin dose exceed digoxin therapeutic concentration when calculate by pharmacokinetic while from 24 men patients there were 14 patients use digoxin dose exceed digoxin therapeutic concentration when calculate by pharmacokinetic. Dose recalculation method was mostly based on literature data particularly for pharmacokinetic parameter values i.e. volume distribution, half life, and clearance. Therefore, the corrected doses suggested in this thesis should be regarded as theoretical estimates. Furthermore, it was also found that patients did not get appropriate loading dose of digoxin. However, drug dosing for digoxin were individual dose, therefore overall found that patient clinical outcome was generally better. Keywords : digoxin, heart failure, renal dysfunction, clinical outcome
