Abstract

Preeclamsia patient experienced deterioration of glomerolus filtration and plasma protein concentration. Bupivakain is bound strongly with the plasma protein that is 95%. This research was aimed to know the pharmacokinetics profiles of isobaric bupivacaine with the noncompartemen model in preeclampsia patients who underwent Sectio Caesarea (SC) with lumbar epidural technique in RSUP Dr. Sardjito and to know correlation between protein levels and pharmacokinetics profiles of isobaric bupivacaine in preeclampsia patients. Firstly, albumin concentration of patients was checked in the laboratory and then isobaric bupivacaine 0.5% in 15ml (75mg) was given by lumbar epidural technique. The samples of blood were taken through veins vessels at 0; 15; 20; 25; 30; 45; 60 and 90 minutes. The concentration of 0.5% isobaric bupivacaine in those samples of blood was determined with High Performance Liquid Chromatography (HPLC) methods. Furthermore, pinprick test was taken to measure block sensoric effect in T10-T6 areas. Pharmacokinetics parameters of bupivacaine (AUC, AUMC, MRT) were calculated based on data of bupivakain concentration in the blood versus time. The results of this research showed that the AUC0-∞ (total AUC) was 116±37,6 (μg/ml) minutes; AUMC0-∞ (total AUMC) was 9187,5 (μg/ml) minutes2; and MRT0-∞ was 102,5 minutes. Average block sensoric effect was at T8. There was no linear correlation between the protein levels and pharmacokinetics profiles of bupivacaine (p>0,05). The conclusion was that there was no linear correlation between the protein levels and pharmacokinetics profiles of bupivacaine that was given by lumbar epidural in preeclampsia patientswho underwent section caesarea.Keywords : bupivakaine, pharmacokinetic profile, lumbar epidural, section caesarea.

Abstract

Rumah sakit merupakan fasilitas pelayanan kesehatan bagi masyarakat yang senantiasa dituntut untuk melakukan peningkatan kualitas pelayanan untuk memenuhi kepuasan pasien. Penelitian ini bertujuan untuk mengkaji dan menganalisis kepuasan pasien rawat inap di ruangan Penyakit Dalam terhadap pelayanan di Instalasi Farmasi RSUD dr. Soedarso Pontianak periode Desember 2011 – Ferbruari 2012. Responden dalam penelitian ini adalah pasien maupun keluarga pasien yang pernah mendapatkan pelayanan di Instalasi Farmasi RSUD dr. Soedarso Pontianak. Jumlah sampel dalam penelitian ini berjumlah 172 orang. Proses pengumpulan data primer dilakukan dengan menggunakan instrumen penelitian dalam bentuk kuesioner dan  pengolahan data untuk mengetahui validitas dan reliabilitas kuesioner menggunakan program SPSS 17 forwindows. Variabel  bebas yang digunakan yaitu dimensi berwujud, dimensi keandalan, dimensi ketanggapan, dimensi jaminan, dan dimensi  empati. Sedangkan variabel terikatnya yaitu tingkat kepuasan  pasien rawat inap di ruangan Penyakit Dalam terhadap pelayanan  di Instalasi Farmasi RSUD dr. Soedarso. Analisis kepuasan pada  penelitian ini menggunakan model Weighted Servqual. Hasil  penelitian diperoleh indeks kepuasan secara keseluruhan sebesar  -0.92. Sedangkan indeks kepuasan untuk setiap dimensi adalah  sebagai berikut: -1.15 untuk dimensi jaminan, -0.91 dimensi  keandalan, -0.88 dimensi empati, -0.86 dimensi berwujud dan -0.76 untuk dimensi ketanggapan. Indeks kepuasan baik secara  keseluruhan maupun setiap dimensi menunjukkan nilai negatif  yang berarti pasien masih belum merasa puas terhadap pelayanan  di Instalasi Farmasi RSUD dr. Soedarso Pontianak. Kata kunci: tingkat kepuasan, harapan, kenyataan, dimensi kepuasan

Abstract

Outpatient services are a reflection of hospital-provided services. A public hospital in Yogyakarta is leading to a new development by adding new facilities and buildings. Objective of this study is to identify the outpatient service outline, the location of the bottleneck and its causes to provide some suggestions of continuous improvement with Lean Hospital approach. This study was conducted on outpatient service department during November 2016 to April 2017. This research is a qualitative descriptive study. Samples were taken by purposive technique using patient sample until it reached data saturation that is 61 respondents. Value Stream Mapping was used to identify outpatient service flow, waste and the location of the bottleneck. Fishbone Diagram was used to analyze the cause of the bottleneck. This research applied Baseline, Assess, and Suggest Solution from BASICS Method. The result shows the Total Cycle Time is 4,79 hours; Total Lead Time is 9,31 hours; and Total Waiting Time is 4,52 hours. Waste observed is as listed: waste of waiting, inventory, defect, transportation, motion, overproduction, overprocessing, and human potential. Bottleneck was located at the Outpatient Clinic, Outpatient Pharmacy, Laboratory, and Radiology. From the Fishbone Diagram the root causes of the problems are found and solutions are proposed to improve the performance in terms of: (1) Manpower, the employee working in the outpatient service, (2) Material, the infrastructure used for service, (3) Method, include management of service (Standard Operational Procedure), (4) Equipment, tools used in the outpatient service and (5) Environtment which is an area of the outpatient service include service area and waiting rooms.

Abstract

Pengukuran kinerja perlu dilakukan di instalasi farmasi. Balanced scorecard menggunakan empat perspektif yaitu perspektif keuangan, pelanggan, proses bisnis internal, serta pertumbuhan dan pembelajaran. Tujuan penelitian ialah mengukur kinerja instalasi farmasi menggunakan balanced scorecard serta menyusun peta strategi yang paling sesuai untuk pengembangan instalasi farmasi. Penelitian termasuk penelitian non eksperimental dengan rancangan deskriptif. Pengambilan data dilakukan secara retrospektif dan prospektif. Pengambilan data dilakukan pada bulan Desember 2014 sampai Februari 2015 menggunakan kuesioner, lembar pengamatan, dan pedoman wawancara. Kinerja pada balanced scorecard diukur melalui indikator dalam perspektif keuangan, pelanggan, proses bisnis internal, serta pertumbuhan dan pembelajaran. Hasil pengukuran kinerja dikombinasi dengan analisis strengths weaknesses opportunities threats (SWOT) digunakan dalam pembuatan peta strategi. Hasil penelitian menunjukkan kinerja yang baik pada indikator pertumbuhan pendapatan, kepuasan pelanggan, keterjaringan pelanggan, pertumbuhan pelanggan, ketersediaan obat, kepatuhan formularium, persentase stok mati, persentase perbekalan farmasi expired date (ED) dan rusak, analisis unit dose dispensing, produktivitas karyawan, turn over karyawan, kepemimpinan, dan kerja tim. Indikator yang memerlukan perbaikan kinerja antara lain inventory turn over ratio (ITOR), dispensing time, kepuasan kerja karyawan, pelatihan karyawan, budaya organisasi, keselarasan, dan kapabilitas sistem informasi. Posisi instalasi farmasi berada pada kuadran III matriks SWOT dengan strategi memperbaiki kelemahan untuk mengambil kesempatan. Peta strategi memprioritaskan penguatan perspektif pertumbuhan dan pembelajaran untuk meningkatkan kualitas pelayanan farmasi sehingga terjadi peningkatan pertumbuhan pelanggan dan pertumbuhan pendapatan. Peningkatan profit dapat tercapai melalui pertumbuhan pendapatan yang meningkat dan efisiensi biaya pada perspektif keuangan.

Abstract

The role of pharmacists in health care has shifted not only providing medicine but also adjust the individual needs of patient,including counseling. This study aimed to determine the relationship between pharmacist counseling on the patients knowledgeabout hypertension and its treatment, patients adherence in taking antihypertensive medication, and the relationship between patientcompliance in the therapeutic target hypertensive patients.The study was conducted in UGM Pharmacy with two groups design, intervention and control, and pretest-posttest design using consecutive sampling method from hypertensive patient between July to December 2014. The data included bloodpressure, gender, age, education level, body mass index, the duration of patients suffered hypertension, other symptoms, presence ofcomorbidities, smoking status and alcohol consumtion, daily exercise, insomnia, stress, adherence measured using Morisky MedicationAdherence Scale (MMAS), and knowledge level about hypertension and its treatment.The study were followed by 76 people, consisted of 38 people in the intervention group and 38 in the control group. Characteristicsof the subjects were normally distributed between the two groups except the sex (p = 0,027) and stress levels (p=0,016). The counselingby a pharmacist in the intervention group significantly improved patient compliance (p=0,000) and increased patient knowledge(p=0,003) in the intervention group. Medication adherence in hypertensive patients showed that the achievement of targeted therapiesof systolic blood pressure (p=0,001) and diastolic (p=0,001) increased significantly.Keywords: hypertension, pharmacist, counselung, compliance.Keywords: hypertension, pharmacist, counseling, compliance

Abstract

Analysis on Pharmaceutical Management Information System (PMIS) application should be conducted for measuring or estimating the potential benefit purposes, observing potential problems that organization and its members might be faced, especially on pharmaceutical inventory management of a hospital. The results of the analysis may be used as the references in order to improve the existing potentials, so the PMIS will be able to carrying on the objective, vision and mission(s) of an organization. The HOT-Fit analysis model plays important role within information system namely Human, Organization, and Technology and harmonization among them in order to create Net Benefit from PMIS implementation. This research was conducted in Pharmaceutical Departement of Dr. Yap Eye Hospital Yogyakarta. This research was conducted within non-experimental quantitative- qualitative approach and applied case study framework. Questionnaires were analyzed by applying descriptive statistic methodology, Pearson correlation analysis, regression, multivariate statistic, and path analysis. Meanwhile, qualitative data were analyzed in order to establish the resume. The results of research showed that application of information system had not fulfilled the user expectation yet. On other side, harmonization among user-organization- technology really needs improvement to happen. There were strong correlations between Human-Technology and Organization-Technology. Human and Technology simultaneously affected the Net Benefit by 88,7%. Human and Organization simultaneously affected the Net Benefit by 87,8%. Organization- Technology simultaneously affected the Net Benefit by 75,3%. The most affective variable toward PMIS successful implementation were Human and Technology which means that certain improvement in the quality of technology & users ability would improve the probability to success of PMIS implementation in Dr. Yap Eye Hospital Yogyakarta.Keywords : Information system analysis, HOT-Fit, Dr. Yap Eye Hospital Yogyakarta

Abstract

One of male contraception methods in family planning program is vasectomy; however, its achievement has been still far behind due to several obstacles in its implementation. In Bantul, Dlingo is a rural sub-district with the highest achievement of vasectomy participants during the last three years (2007-2009). Such a condition is contrary to that found in Sewon as an urban sub-district. This study aimed to identify factors influencing the differences in the achievement, and its results would be beneficial in improving the participation level of vasectomy male contraception. This was a qualitative study comparing the conditions of the two sub-districts through the use of primary and secondary data. Subjects in the research considered information accuracy and validation; hence, its number was subject to the expected results. Data validity testing was performed by utilizing triangulation. Data analysis techniques involved data reduction, presentation, and verification, or tentative conclusion drawing possibly answered the research problem. The result of the research which influenced vasectomy male participation involved un-wealthy or limited economic conditions. This was supported by officers’ attitude and behavior, peer support, public/ religious leaders and community association that actively provided greater contribution in improving participation, and association as the media of socialization and motivation. The results recommended data updating, dialogues with public leaders/religious leaders, and also socialization concerning vasectomy. Tomaintain and increase the male contraception participation level, it is necessary to perform establishment, activation, supervision on the association of male contraception participants, with regard to welfare improvement. Recognition awarded to vasectomy participants and Family Planning Field Officer for their achievement is also necessary.Keywords: Vasectomy, Socialization, Recognition

Abstract

Health services in the era of JKN are organized by all Health Facility in cooperation with BPJS. Patient satisfaction is a critical indicator whether a program is a success or not. Satisfaction is the pleasure of someone who comes from the comparison between a product or performance with expectations, if performance is below expectations then the customer is not satisfied. This study aims to determine the effect of membership, service, and financing of patient satisfaction with BPJS in Primary Health Facilities. This research is an analytical descriptive research with cross sectional survey design. The data was taken quantitatively by using questionnaires and completed with qualitative data through interviews to deepen the findings of the field. Sampling technique used is purposive sampling with inclusion criteria. The number of respondents samples used were 278 respondents and the sample of the study sites was determined by random sampling method including health center, clinic and pharmacy network. The data were analyzed by using multiple linear regression analysis in the form of t test and F test. The result of research at Primary Health Facilities Regency showed that there was positive and significant influence to patient satisfaction of BPJS, including membership variable (p = 0,02), variable of service (p = 0,00), and financing variable (p = 0,00), while F test simultaneously also shows there are positive and significant influence to patient satisfaction BPJS with value (p = 0,00).

Abstract

Non-steroidal anti-inflammatory drugs such as diclofenac sodium were common drug given worldwide to reduce pain in osteoarthritis. The long-term used of diclofenac sodium side effects was on liver, kidney, gastrointestinal and other organs. The earlier study result have shown the effectiveness of the combination of temulawak rhizome extract, ginger, soy and shrimp shells in osteoarthritis patients. This third phase of clinical study purpose was to monitor the side effects of temulawak rhizome, ginger, soybean and shrimp shells extract in combination compared to diclofenac sodium in patients with osteoarthritis. Forty seven patients with knee osteoarthritis diagnosed by American College of rheumatology criteria were included in this study. This subject were devided randomly into two groups to receive either the combination of temulawak rhizome 100 mg, ginger 100 mg, soy 50 mg and shrimp shells 50 mg extract (n=23) or diclofenac sodium 25 mg (n = 24) three times a day orally for 14 days. The side effect that were monitored were side effect that appear in the gastrointestinal organs, cardiovascular, central nervous system and other organs for 14 days therapy and laboratory tests of SGOT, SGPT, creatinin serum, urea, hemoglobin, leukocytes and platelets were performed before and after 14 days. Chi-Square test were used to evaluate the side effects and independent t test or Mann-Whitney tests were used to evaluate changes of levels of SGOT, SGPT, serum creatinine, serum urea, hemoglobin, leukocytes and platelets before and after intervention. Percentage of side effects on the gastrointestinal organs, cardiovascular, central nervous system and other organs in combination of ginger rhizome 100 mg, 100 mg of ginger, 50 mg of soy and 50 mg of shrimp shell extract group was not significantly different compared to diclofenac sodium 25 mg. SGPT activity in trial group was decrease significantly (p <0.05) compared to diclofenac sodium 25 mg but no significant difference in liver function (SGOT), kidney function (creatinine and urea), and bone marrow function (hemoglobin, leukocytes, and platelets).Keywords: Osteoarthritis, combination of temulawak rhizome, ginger, soy and shrimp shells, diclofenac sodium, side effects, liver function, kidney function and bone marrow.

Abstract

Heart failure is a complex clinical syndrome and the risk of high morbidity and mortality in the population. HeartFailure is a disease requiring long therapy, so cost for heart failure treatment is very high.This study design was descriptive non experimental. Data was collected retrospectively. Data was analyzedby descriptive quantitative, and statistical method using correlation analysis. Data was classified based on costingsystem and number of comorbid.The mean and proportion of overall total costs of heart failure on non poor patients were greater than poorpatients and its value increases along with increasing number of comorbid. Laboratory cost dominate total cost for poor group without or with comorbid. While for non poor group was laboratory cost on patient without comorbid (29,80%) and with 1 comorbid (25,93%), on patient with 2 comorbid was medicine cost (21,99%) and onpatient with more than 2 comorbid was hospitalization cost (23,21%). Correlation test among patient characteristicindicating that six variable had significant correlation. Correlation test using a multivariate analysis showed thatvariable which significantly correlated with total cost on poor group was number of comorbid, and on non poorgroup was number of comorbid and LOS. Keyword : Heart failure, cost, type of costing system, number of comorbid
