Abstract

Introduction: Extraordinary policies have been launched by the government to reduce Maternal Mortality Rate/ Infant Mortality Rate (MMR/IMR) , however this has not been comparable with the expected results. The purpose of this study was to determine the effectiveness of Katoga's multi-level education on the competence of cadres, public figure, and families in preventing, detecting early and handling emergency pregnancy to reduce MMR/IMR. Methods: This research study used action research with stratified respondents, 10 health cadres who will provide training to 30 community leader respondents and subsequently public figure will provide training to 60 selected families with simple random sampling. The independent variable was multi-level education and the dependent variable was competence in early detection of emergencies in pregnancy. Data were collected using a questionnaire and analyzed by t-test. Results: The results showed that the increasing of the competence in preventing, early detecting and handling emergency pregnancy after receiving multilevel education training in health cadres from the previous value of 70 to 93; in community leaders from 61.1 to 80.5 and in families from 58.0 to 78.9. There was a significant increase in competency with multi-level education training in increasing competency in a larger population with a cadre pValue of 0.003; community leaders 0,000 and families 0,000. Conclusions: Community empowerment in the health sector through multi-level education can spread knowledge in preventing, detecting high-risk pregnancies early and managing maternal emergencies so as to facilitate the task of health nurses to obtain information about pregnant women detected as high-risk populations.

Abstract

Introduction: : Response time is the handling speed calculated when the patients arrive at the hospital until they are treated. It is influenced by several factors according to Gibson Performance Theory. The research approach aimed to analyze individual factors including: skills, emergency training, gender, age, length of working, and education, organizational factors: rewards, and psychological factors: nurse motivation that influences the response time of the nurses in emergency departments at general hospitals in Madura. Methods: The research method used was a descriptive-analytic study with  a cross-sectional approach. The sampling technique is total sampling, which is 101 nurses. Independent variables are skills, emergency training, gender, age, length of working, education, rewards, and motivation. Dependent variable is response time. Data were collected using a questionnaire and direct observations on nurses, data were analyzed using the Multiple Regression Logistic test. Results: The results show that there was no correlation between age (p= 0.996); education (p= 0.913); length of working (p= 0.921); training (p= 0.830); skill factor (p= 0.999); and motivation factors (p= 0.471) with response time. Whereas gender (p= 0.020); and reward factor (p = 0.020) were related with response time. Conclusion: In order to improve patients’ handling procedure, hospitals should give support  to nurses, like promotion or salary. Hospitals are expected to record documentations about the number of patients according to triage category and how fast nurses handle them because it may be an evaluation for rooms.

Abstract

Introduction: The clinical learning environment is a situation for the implementation of student practical learning. Satisfaction with the clinical learning environment is very important to foster a positive learning experience. This study aims to analyze the influence of the clinical learning environment on nursing student satisfaction. Methods: The study design used was an observational analysis with a cross-sectional approach to clinical nursing students in East Java Province with 451 student respondents. A consecutive sampling method was conducted. The Clinical Learning Environment Supervision Teacher (CLES T) instrument was used to assess learning environment variables and Clinical Learning Environment Inventory (CLEI), in particular, the satisfaction dimension was used to assess the satisfaction variable. Data analysis used the Spearman rank test and multiple linear regression. Results: There is a relationship between the clinical learning environment (pedagogic learning, leadership style, nursing care in the ward, the relationship of supervision, the role of nurse teacher) and the satisfaction of nursing clinic students (p = 0.000). Multiple linear regression analysis showed that the dimensions of pedagogic learning (t = 3.261; p = 0.001) and the role of the nurse teacher (t = 2.693; p = 0.007) had an effect on nursing student satisfaction. Conclusion: Student satisfaction is an important factor in the implementation of student clinical learning, so it is necessary to carry out good management by educational institutions and practice fields regarding the learning environment and improving the pedagogic atmosphere and the role of nurse teachers in clinical learning.

Abstract

Introduction. Therapeutic communication skill is the main competency of nurses. Lack of therapeutic communication capability will cause difficulties in engaging therapeutic relationship between nurse and patient. This study aimed to investigate the effectiveness of Standardized Patient Experience (SPE) in enhancing therapeutic communication skill of undergraduate nursing students compared to Early Clinical Exposure (ECE). Method. This study was quasi-experimental study used 47 undergraduate nursing students as the sample and divided into control and experiment group. The data was analysed by using t-independent test with significant value of 0.05. Results. There was no significant difference between the improvement of therapeutic communication skill in SPE and ECE group (p=0,576), the mean of therapeutic communication skill scores were significantly improved in each group (p=0,40; p=0,15 in SPE and ECE, respectively). Students’ satisfaction level in SPE group is significantly higher than the ECE (p=0,003). Discussion. The SPE method is as effective as the ECE in teaching therapeutic communication, so that SPE can be used as alternative if teaching opportunity in clinical practice was not possible to be conducted. Abstract

Introduction :Adolescence as a transitional period can be a problem because there are many changes in it, both physically, psychologically, and intellectually, that make them as a population vulnerable to perform risky behaviors, included sexual risk behavior. Peers have an important role in the social life of adolescents because teenagers spend more time with friends and generally teens also make their peers as a role model in behaving. This study aims to determine the effect of peer-based education in the prevention of sexual risk behavior among adolescents in rural areas Methods:This type of research is a pre-experimental quantitative study with a one-group pre-post test design. The research was conducted on April-June 2017 with 61 respondents which were selected by purposive sampling method. The research instrument used questionnaire. Results :The result of data analysis used Paired Simple T Test was found that there were significant differences in the prevention of sexual risk behavior among adolescents in rural area before and after been given sexual education through peer education with p value (0,002).Proper timing of education, a conducive environment and a good concentration in learning make education work optimally. Conclusions :There is a significant effect of peer education in the prevention of sexual risk behavior among adolescents in rural areas. It is recommended to conduct research in other regions with different characteristics

Abstract

Introduction: Maceration is one of the problems affecting the healing process of diabetic foot ulcers (DFUs).  In complementary therapies, sea cucumber is known to have higher absorbency. However, there are few studies on the reducing of periwound maceration and inflammation-related indicators in DFU.Methods: This was an observational study at a wound care clinic in Indonesia. Thirty-one DFU patients received treatment with sea cucumber were enrolled in the study. As control samples, 35 DFU patients treated with honey were also included. All were new occurrence DFU and had maceration at the baseline. The maceration area and inflammation-relate indicators including TNF-, MMP-2, and MMP were measured on baseline and week 4.Results: There were no significant differences between two groups at the baseline.  In the multiple linear regression model, sea cucumber significantly associated with the proportion of maceration reduction than honey at week 4 (β=0.520, p=0.001). There were no significant differences between two groups in levels on TNF-α, MMP-2, and MMP at week 4.Conclusions: Our results demonstrated that sea cucumber might be appropriate for reducing DFU periwound maceration.
