Abstract

The process of preparing a patient to return home is known as discharge planning. Discharge planning is a complex process and aims to prepare a patient for the transition in the hospital until the patient returns home. Based on Haris 2016's research, it was stated that 40% of patients were quite satisfied with discharge planning, 45.3% were satisfied, 14.7% said they were not satisfied. Based on the results of interviews conducted at the Syamrabu Hospital, Bangkalan Regency, it was revealed that health promotion and education as part of discharge planning was given only when the patient returned home, and was not clearly documented in a structured and integrated format. The method used in reforming agency activities related to discharge planning is a pilot study approach. This method starts from a situation analysis, making planning of action, implementation, evaluation, and gap analysis using a literature review. The data collection instruments used interviews, observation, document study and questionnaires. Interviews, observation and questionnaire distribution were carried out to the Head of the Nursing Division, the Head of the Nursing Section, the Head of the Room, the Chairperson. Furthermore, all teams are gathered together to be given direction or training related to the latest Discharge Planning Management. The participants of the activity were 20 nurses at the Syamrabu Hospital, Bangkalan Regency who attended the training activities. While participating in the activity, the nurse was very enthusiastic about the material presented. The nurse was also able to carry out the latest discharge planning. The training results were obtained before counseling was given to the knowledge of nurses in the sufficient category of 25% and the category of lacking 75%. After being given counseling, it increased into a good category, namely by 75%. Before being given training, the nurse's ability to be in the category of 0% and after being given the training, it increased to be in the good category, namely by 100%.

Abstract

Primary dysmenorrhea is a feeling of pain in the lower abdomen that occurs due to hormonal imbalance, without any abnormalities in the pelvic organs. Primary dysmenorrheam will affect 60–75% of young women. In Indonesia, the incidence of dysmenorrhea reaches 64.25%, 54.89% experiences primary dysmenorrhea, and the remaining 9.36% experiences secondary dysmenorrhea (Santoso, 2008). Based on a preliminary study conducted on grade VII students of SMPN 5 Bangkalan, it was found that 87% of students had mild dysmenorrhea, 9% of students had moderate dysmenorrhea, 4% had severe dysmenorrhea. The method used to carry out community service is by collecting data on the number of grade VII students at SMPN 5 Bangkalan, then conducting interviews with students about understanding primary dysmenorrhea and how to overcome it. The activity was continued with abdominal stretching exercise which was held at SMPN 5 Bangkalan. The participants of the activity were 80 students of SMPN 5 Bangkalan who participated in the training activities. While participating in the activities, students were very enthusiastic about the material presented. The students were also able to do abdominal stretching exercises to treat primary dysmenorrhea. The results of the pain scale of 80 respondents found that 20 experienced primary dysmenorrhea with mild pain, mostly 10 students (50%) before training. Meanwhile, after the training, all 20 students were painless (100%).

Abstract

Choking is an emergency condition that must be treated quickly. If left for too long without handling the body can experience a lack of oxygen (hypoxia) and can lead to death (Kalcare, 2014). Based on research conducted by Rovin (2013), there were 12,400 cases of children under 14 years of age from 2001 to 2009 who came to the ER because of choking, found 34 were taken to the ER (emergency room) and 57 children died every year because they could not. adequate aid when choking. Based on a survey conducted at Posyandu RW 6 in the Puskesmas Kamal area, through interviews with parents that have been conducted, it was found that many parents still do not know about first aid for choking children and its prevention. The method used to carry out community service is coordination with the posyandu coordinator as well as the appointment of cadres and determining the schedule of activities for providing material on the implementation of training in handling choking children. Then a cadre here will tell parents how to handle a choking child. In this training activity, there were 8 cadres in the Posyandu RW 6 area and cadres in the posyandu in the Kamal Puskesmas working area. During the activity, the cadres were very enthusiastic about the material presented. The cadres were also evaluated before and after being given material on the handling of choking in children. Apperception activities were carried out, namely the exploration of cadres' knowledge regarding the material. After evaluating the results, the cadres understood how to treat choking children

Abstract

One of the disorders of pregnancy that can occur in pregnant women, fetuses and after childbirth is tetanus infection. Tetanus is an acute and sometimes fatal infectious disease caused by neorotoxin (tetanuspasmin) produced from clostridium tetani, whose spores enter the body through wounds. Based on data from the World Health Organization (WHO) worldwide, the Maternal Mortality Rate (MMR) during childbirth, there are 500,000 maternal deaths per year and 10,000,000 neonatal deaths per year. Indonesia has implemented the Development of the Immunization Program (PPI) since 1977 with the aim of including the Elimination of Neonatal Tetanus (ETN) with the aim of reducing the incidence of neonatal tetanus to 1 / 10,000 live births (Ditjen PP & PL, 2012). Based on a survey through interviews with mothers information about pregnant women at the Bangkalan Community Health Center that has been carried out is that there are still many pregnant women at the Bangkalan Community Health Center who do not yet know the importance of TT examination. The method used to carry out community service is by collecting data on the number of pregnant women at the Bangkalan Community Health Center. Then conduct interviews with mothers about the understanding of antenatal care, and TT examination. Then make a time contract with pregnant women. The activity was continued with training for free Pregnancy Check-Up and TT injection held at the Bangkalan Community Health Center. Participants in the activity were 35 pregnant women at the Bangkalan Community Health Center who participated in the training and free TT injection. While participating in the activity, the mother was very enthusiastic about the material presented. She was also able to ask questions related to the material presented. The results of the activity showed that most of the mothers' knowledge was 21 mothers (60%) and the mother's TT immunization status was incomplete, namely 20 mothers (57.1%).

Abstract

Asphyxia Neonatorum is a condition in which the baby is unable to breathe spontaneously and regularly after birth. This is due to fetal hypoxia in the uterus and this hypoxia is associated with factors that arise in pregnancy, childbirth or sooner birth. Based on research from the World Health Organization (WHO), around the world there are 10,000,000 infant deaths, especially neonates per year (Katiandagho & Kusmiayi, 2015). The method used to carry out community service is to collect data on the competence of health workers in the Baby Room at Syarifah Ambami Ratu Ebu Bangkalan Hospital through interviews with the Head of the Room, several nurses and midwives and management to find out the needs of the Hospital. In addition, it is also necessary to observe the skill competence of nurses and midwives when caring for babies with asphyxia During the process of the training program, all health personnel who are scheduled to take part in the training can attend from the start of the activity to the end. Abstract

The risk of mastitis is one of the problems that occur during the breastfeeding process that affects the offering of breast milk (ASI). One of the factors that can increase the risk of mastitis incidence is breastfeeding technique. Based on the World Health Organization (World Health Organization) the estimated incidence of mastitis in breastfeeding mothers is around 12.6% -33% and the global prevalence is around 10%. Data on breastfeeding problems from April to June 2012 in Indonesia showed 22.5% had sore nipples, 42% of mothers had milk dams, 18% of mothers had blocked milk, 11% had mastitis, and 6.5% of mothers had breast abscesses. It was caused by the mother's mistake in breastfeeding her baby. Based on the data and information obtained from the midwife, Lukluatun, there were 20 breastfeeding mothers and 7 of them had mastitis. The method used to carry out community service is by collecting data on the number of post-partum mothers at BPM Luklutun Mubriqoh, then conducting interviews about mastitis knowledge in postpartum mothers, counseling about mastitis, and demonstrating breast care and treatment. Implemented at BPM Luklutun Mubrikoh. The luklutun midwife collaborated with STIkes Ngudia Husada Madura in an effort to find a solution so that the Maternity department offered a solution, namely conducting counseling on mastitis and training on the correct way of breastfeeding and breast care with mastitis.

Abstract

Family planning is an effort to increase awareness of community participation through maturing age at marriage, regulation, birth, fostering, family resilience and improving family welfare to create a happy and prosperous small family (Yuhedi and Kurniawati, 2013). Based on the results of information obtained from the Arosbaya Health Center, the prevalence of Family Planning (KB) in Bangkalan Regency in 2017 reached 72.97 percent. In 2018 the priority level is 71.99 percent. Meanwhile, from 2019 to April, the prevalence rate is 67.88 numbers. From year to year, community participation in family planning programs tends to decline. It is known that the prevalence is a measure of the level of participation and public awareness. Privalence is the percentage of fertile age couples (PUS) participating in family planning. If the percentage is high, the level of public awareness is also high. However, in Bangkalan, the participation rate is still low. However, the Family Planning Office for Women's Empowerment and Child Protection (KB P3A) in Bangkalan Regency is actively conducting socialization to the community. From the existing problems, we are planning to go on a family planning IUD safari, namely through the installation of a free IUD. The target of family planning counseling and counseling is fertile age couples and community leaders, with a total of approximately 50 people. Meanwhile, the free IUD is intended for women of reproductive age who wish to regulate pregnancy. The highlight of this family planning safari was held for 2 days. The output target to be achieved from this community service is the publication of reputable scientific journals.

Abstract

Blood is one of the most important components in the body, considering its function as a means of transportation. Lack of blood in the body can spur a number of diseases starting from anemia, hypotension, heart attacks, and several other diseases. Law number 36 of 2009 concerning health states that a health service facility is a tool and / or place used to carry out promotional, preventive, curative and rehabilitative health service efforts which are carried out by the government, local government and / or the community (Kemenkes RI , 2015). Based on a survey through interviews with students of SMKN 2 Bangkalan that had been conducted, it was found that many students at SMKN 2 Bangkalan still did not know their blood type. The method used to carry out community service is by collecting data on the number of students at SMKN 2 Bangkalan, then conducting interviews with students about their blood group and then making a time contract with the students. The activity was continued with a blood group examination for santri santri who did not know their blood type. The participants of the activity were 37 students at SMKN 2 Bangkalan who took part in the blood group examination. While participating in the activities, the students were very enthusiastic about the activities being carried out. The results of the activities most of the gender of the students were male as many as 21 people (56.8%). And, most of the students' blood groups were O as many as 13 people (35.2%)

Abstract

Electric shock is one of the emergencies in children that often occurs because the power source tends to be easily accessible by children. When the baby / child touches an electric current, with the hands, feet or mouth, an electric current flows through the body parts. According to the National Fire Protection Association, about 2,400 children suffered severe trauma and burns from electric shocks because they pushed their hands into the power outlet plugs. The incidence of burns from electric shock accounts for about 2-3% of all burns in children requiring emergency care. In addition, in Indonesia, around 12 children die from electricity each year. Based on a survey through interviews with parents of children in Mlajah Village, it was found that many parents of children in Mlajah Village did not know about the first handling of cases of electric shock. The method used to carry out community service is by collecting data on the number of parents who have children in Mlajah Bangkalan Village, then conducting interviews with mothers about the understanding of being electrocuted in children and first handling of being electrocuted in children, then making a time contract with the mother. The activity was continued with training on the first handling of an electric shock on a child which was held at the Mlajah Bangkalan Village Hall. The participants of the activity were 30 mothers in Mlajah Village who participated in the training for the first handling of an electric shock to a child. While participating in the activity, the mother was very enthusiastic about the material presented. She was also able to demonstrate how to remove objects that caused electric shocks. Also, mothers also understand the concept of electric shock and its first treatment.

Abstract

Febrile seizures are seizures that occur due to an increase in body temperature (rectal temperature above 38° C) due to an extracranial process without intracranial infection or other causes. Fever seizures often occur in children aged 6 months to 5 years because their immune system is still low in children under five (Wilson, 2016). The incidence of febrile seizures is estimated to be higher in Asia, approximately 20% of cases are complex febrile seizures (Manjoer et al., 2000). According to the Health Profile of Bangkalan Regency (2017), one of the diseases that can cause death in children under five is pneumonia. Peneumonia is a disease that can be caused by viruses and manifests in the presence of fever. Based on a survey through interviews with parents of babies in Patemon Village, information was obtained that many parents of babies in Patemon Village did not know how to properly handle febrile seizures. The method used to carry out community service is by collecting data on the number of parents who have babies in the village of Patemon Bangkalan, then conducting interviews with parents about problems related to awareness and knowledge about handling febrile seizures, then making a time contract with the mother. The activity was continued with a pre-test, audiovisual-based counseling, and training in the management of febrile seizures in infants, the activities were carried out at the Posyandu in Patemon Village. There were 20 participants from Patemon Village who participated in the extension activities. Participants are given a pre test and post test using the instrument based on the material given. Overall, the material presented can be understood by the participants. The results of the activity showed that before being given training, the knowledge of mothers was the same between the enough and insufficient categories, namely 40% and after being given the training, it increased into a good category, namely by 50%.
