Abstract

This study is an analyze disparity of maternal mortality in Indonesia with spatial analysis using intermediatefactor such as maternal health service, reproduction status, socio economic and demography. This studyaimed to spatially analyze disparity of maternal mortality, analyze the influence and risk of maternal mortality onintermediate factors, and provide recommendations on maternal health problems in Indonesia. This study usingecological study design (agrerat studies), spatial analysis, bivariate analysis, and multivariate analysis. Unit ofanalysis in this study is districts/cities in Indonesia. The results showed that disparity of maternal mortality causedby intermediate factor gap between districts/cities in Indonesia, with the highest risk of maternal mortality occurredin eastern Indonesia. The dominant variables related to maternal mortality, namely population density withOR:0,283 (95% CI:0,185-0,430) and childbirth by skilled health personal with OR:1,745 (95% CI:1,081-2,815).The high risk of maternal mortality occurred in districts/cities with low coverage of fourth pregnancy (K4), lowbirth attendance (PN) coverage, low postpartum (KF) coverage, average number of children, low average education,and high poverty

Abstract

Drug-resistant tuberculosis (TB) patients have a greater risk of loss to follow-up (LTFU) than drug-sensitive TB patients, due to their longer treatment duration. This study aimed to determine the influence of decentralization and patient type on LTFU among multidrug-resistant TB (MDR-TB) patients in Indonesia. A retrospective cohort study was conducted at all MDR-TB treatment healthcare facilities in Indonesia from  014 to 2015. Using total sampling technique, 961 patients were examined and sampled. Of these patients, 86.03% were decentralized. Patients were classified into types as follows: 35.17% were “relapse” patients, 5.52% were “new,” 13.94% were classified as “after LTFU” patients, 23.10% were “treatment failure category 1” patients, 20.29% were “treatment failure category 2” patients, and 1.90% were classified as “other types” patients. Decentralization reduced LTFU risk by up to 46% (HR = 0.54, 95% CI 0.35–0.84). LFTU in “after LTFU,” “treatment failure category 2,” and “other types” patients was higher by 50%, 53%, 74%, respectively compared to LFTU occurrence in “relapse” (baseline) patients. Among “treatment failure category 2 patients, female patients were 2.13 times more likely to have an occurrence of LFTU, while male patients were 0.55 times as likely to have an occurrence of LFTU, compared to “relapse” type patients of the same sex.

Abstract

The World Health Organization estimated that chronic obstructive pulmonary disease (COPD) will be the highest non-communicable disease worldwide by2030, and pesticide exposure is one of major risk factors. This study aimed to determine effect of pesticide exposure on lung dysfunction among paddyfarmers. A case-control study was performed on April - May 2016 in Purworejo. The case group comprised 66 farmers suffering from lung dysfunction during2015 based on medical records, while the control group comprised 59 neighboring farmers showing normal lung function. Both case and control groups weretested using a spirometer and COPD assessment test (CAT) questionnaire. Logistic regression analysis of pesticide quantity (OR = 0.75; 95% CI = 0.317 -1.754) and duration of spraying (OR = 1.11; 95% CI = 0.430 - 2.891) adjusted for confounders showed no clear associations. Pesticide remained a potentialhealth risk for lung dysfunction due to farming duration (OR = 5.61; 95% CI = 1.124 - 27.990) adjusted by age, personal protective equipment, records of lungdisease, smoking habit, dust exposure, and indoor air pollution. The farming duration was revealed to be a risk factor, but no clear association was found between quantity and duration of spraying to lung dysfunction.

Abstract

Tuberculosis is one of the causes of morbidity and mortality that often occurs in children. WHO 2018 data states that there are 1.1 million cases of TB in children each year. One of the causes of TB in children is nutritional status. Poor nutritional status led to weak immunity and thus easier to be infected by tuberculosis. This study aims to analyze the correlation of nutritional status and the occurrence of TB in children aged 1-5 years old in Indonesia. This research is a quantitative study with a cross-sectional design using Riskesdas 2018 data. The sample of the study was children aged 1-5 years old with a total sample of 27779. The confounding variables were sex, residence area, BCG immunization, parents’ education status, parents’ employment status, the existence of smokers, and the physical condition of the house. Bivariate analysis using Chi-Square test and regression logistic for multivariate analysis. The variable associated with TB in children was nutritional status (p-value 0.02) PR 1.78 (95% CI; 1.1-2.9). Other variables related were the area of residence (p <0.05) PR 2.336 (95% CI 1.449-3.768) and the employment status of the father (PR 3.943 95% CI 1.584-9.815). There was a correlation between nutritional status and pulmonary tuberculosis in children aged 1-5 years in Indonesia. Further research is needed by using different designs and other variables. Abstrak TB paru merupakan salah satu penyebab kesakitan dan kematian yang sering terjadi pada anak. Data WHO 2018 menyebutkan terdapat 1,1 juta kasus TB anak terjadi setiap tahunnya. Salah satu penyebab TB anak adalah status gizi. Status gizi yang buruk membuat imunitas anak rentan sehingga dapat terserang TB paru. Penelitian ini bertujuan unuk melihat ada tidaknya hubungan status gizi terhadap kejadian TB paru anak usia 1-5 tahun di Indonesia. Penelitian kuantitatif studi crossectional dengan menggunakan data Riskesdas 2018. Sampel penelitian adalah anak usia 1-5 tahun dengan jumlah sampel 27779. Variabel perancu jenis kelamin, wilayah tempat tinggal, imunisasi BCG, status pendidikan orang tua, status pekerjaan orang tua, keberadaan perokok, dan kondisi fisik rumah. Analisis bivariat menggunakan uji Chi-Square dan multivariate menggunakan analisis regresi logistic ganda. Variabel yang berhubungan dengan TB paru anak adalah status gizi (p value 0,020) PR 1,78, (95% CI; 1,1-2,9). Variabel lainnya yang berhubungan adalah wilayah tempat tinggal (p value 0,00) PR 2,336 (95%CI 1,449-3,768) dan status pekerjaan ayah (PR 3,943 95%CI 1,584-9,815). Terdapat hubungan antara status gizi terhadap kejadian TB paru anak usia 1-5 tahun. Diperlukan penelitian lebih lanjut dengan desain yang berbeda dan variabel lainnya.

Abstract

ABSTRACT Dengue Hemorrhagic Fever (DHF) is an endemic disease in the tropical area. The spread of the disease can occur fast if it is not controlled. DHF is caused by the dengue virus that is transmitted through Aedes aegypti dan Aedes albopictus. According to Bangka District Health Office, It was reported that the number of DHF cases in 2017 was 51 and 50 cases in 2018 (until March 2018). The objective of this study was to know the association between the water container and DHF cases in West Bangka District in 2018. This was a case- control study with a total respondent of 183 respondents and the ratio of case and control was 1:2. The results showed that the presence of Aedes larvae (OR=0,007; p-value=0,007; 95% CI: 1,59-19,96), presence of water container (OR=5,12; p-value=0,01; 95% CI: 1,47- 17,86), water container opened/closed (OR=2,72; p-value=0,063; 95% CI: 0,94-7,84) were associated with DHF cases. Houses where Aedes larvae were founded, in or outside the house (with container index >20%) have a 5.6 times higher risk to be contracted with DHF compare to houses with no Aedes larva. In addition, houses with water containers can be founded near the houses have a 5.1 times higher risk to be contracted with DHF. Houses with opened water containers were associated with DHF with 2.7 times higher risk to be infected with DHF. Therefore, community participation in eliminating mosquitoes breeding places needs to be encouraged. ABSTRAK Upaya pengendalian demam berdarah dititik beratkan pada penggerakan potensi masyarakat untuk dapat berperan serta dalam Pengendalian Sarang Nyamuk (PSN) melalui 3M plus. Tempat Penampungan Air (TPA) merupakan salah satu tempat perkembangbiakan jentik Aedes Aegyti, semakin banyak TPA yang digunakan berpotensi untuk menjadi tempat perkembangbiakan jentik. Berdasarkan data Dinas Kesehatan Kabupaten Bangka Barat jumlah kasus DBD pada tahun 2017 sebanyak 51 kasus dan sampai bulan maret 2018 sebanyak 50 kasus terlaporkan. Tujuan penelitian adalah untuk mengetahui pengaruh tempat penampungan air dengan kejadian DBD di Kabupaten Bangka Barat. Desain penelitian adalah case control dengan jumlah total responden adalah 183 responden dengan perbandingan kasus dan kontrol adalah 1:2. Hasil penelitian menunjukkan tempat penampungan air terbuka/tertutup (OR=2,72; p-value=0,063; 95% CI: 0,94-7,84), keberadaan jentik (OR=0,007; p-value=0,007; 95% CI: 1,59-19,96), tempat penampungan air (OR=5,12; p-value=0,01; 95% CI: 1,47-17,86). Rumah dengan tempat penampungan terbuka mempunyai risiko 2,7 kali lebih besar untuk transmisi DBD, rumah yang ditemukan jentik di tempat penampungan air dalam maupun di luar rumah (CI >20%) mempunyai risiko 5,6 kali lebih besar untuk terkena DBD dan rumah yang disekitarnya ditemukan tempat penampungan air berisiko 5,1 kali lebih besar untuk menderita DBD. Pengelola program DBD agar terus melaksanakan kegiatan – kegiatan penanggulangan dan tatalaksana kasus DBD.

Abstract

Morbiditas dan mortalitas apendisitis akut disebabkan oleh perkembangan apendisitis akut menjadi perforasi apendiks. Hal-hal yang menyebabkan kerentanan apendiks belum banyak diteliti dan belum diketahui penyebab pastinya. Tujuan penelitian ini adalah mengetahui faktor-faktor apa saja yang dapat memprediksi terjadinya perforasi apendiks. Penelitian dengan desain kasus kontrol menggunakan data sekunder berupa rekam medis penderita apendisitis akut dewasa di RS Al-Ihsan Kabupaten Bandung tahun 2013–2014 dengan jumlah kasus (perforasi apendiks) 36 kasus dan kontrol (nonperforasi) 93 kasus. Analisis data yang dilakukan meliputi deskriptif, chi-square, receiver operating characteristic, dan regresi logistik multivariat. Dua faktor prediksi yang bermakna sebagai faktor prediksi perforasi apendiks dalam analisis regresi logistik multivariat adalah suhu badan di atas 37,5°C dengan odds ratio (OR) 7,54 (IK 95%: 2,01–28,33), jumlah leukosit di atas 11.500/mm3 dengan OR 12,12 (IK 95%: 4,03–36,48). Perlu validasi pemeriksaan suhu badan di RS, penelitian lebih lanjut untuk mencari faktor prediksi lainnya, persiapan operasi segera untuk pencegahan komplikasi perforasi apendiks, dan pemberian informasi ke masyarakat bahwa sakit perut dapat bersifat gawat darurat. PREDICTIVE FACTORS FOR PERFORATED APPENDIX IN ACUTE APPENDICITIS ADULT PATIENTS IN AL-IHSAN HOSPITAL BANDUNG REGENCY 2013–2014Appendix perforation is the causation for acute appendicitis morbidity and mortality. Factors that may cause appendix vulnerability has not been extensively studied before and the main cause is still yet unknown. The goal of this study was to analyze what factors that could be used to predict appendix perforation. This study was a case control study using 2013–2014 medical records in Al-Ihsan Hospital Bandung Regency as data. Case group pooled from 36 perforated appendix adult (above 15 years old) patients, while control group pooled from 93 non perforated appendix adult patients. Data analysis conducted were descriptive, chi-square, receiver operating characteristic, and multivariate logistic regression. There were two prediction factors which significantly associated with perforated appendix. Those were body temperature above 37.5°C with odds ratio (OR) 7.54 (95% CI: 2.01–28.33), and leucocytes count above 11,500/mm3 with OR 12.12 (95% CI: 4.03–36.48). Further studies and body temperature validation on each hospital are needed to find other prediction factors, preparing pre operative equipment for immediate definite measure like surgery to prevent the complication of perforated appendix, and education to people that abdominal pain is not always causing by gastric problem and it might be a case of emergency.

Abstract

Dengue hemorrhagic fever (DHF) is an infectious disease caused by the Dengue virus. DHF cases in Mataram City from 2016 to 2020 tend to increase where most of the DHF patients are treated at the Mataram City Hospital. Death due to DHF infection mostly occurred in patients with Dengue Shock Syndrome (DSS) and mortality from DSS was reported to be 50 times higher than in DHF patients without DSS. Prompt diagnosis and appropriate treatment are important determinants for the prevention and control of dengue cases, so knowing the predictors of risk factors can prevent/reduce deaths from dengue virus. This study used a case-control design. Cases were DHF patients diagnosed with DSS by the treating doctor, while the controls were DHF patients diagnosed not DSS by the treating doctor. The research data was obtained from the results of laboratory tests stored in the medical records of patients treated at the Mataram City Hospital from January 2016 to December 2020. The design of the analysis was aimed at obtaining the odds ratio (OR) value followed by multivariate analysis to determine the risk factors that could detect DSS earlier. The variables that were statistically significant in the prediction of the final model were the increase in hematocrit value > baseline with OR= 17.1 (95% CI: 4.033-72.600), a decrease in platelet value < 100.000/µL with OR = 6 (95% CI). : 2,306-15,699), and decreased leukocyte value < baseline with OR= 5.1 (95% CI: 2,209-11,838). While the most dominant variable is the increase in hematocrit value > baseline with OR = 17.1 (95% CI: 4.033-72.600) and p value = 0.000.

Abstract

Salah satu tantangan program TB di Indonesia adalah semakin meningkatnya trend angka putus berobat pada pasien TB MDR/RR. Pada tahun 2011-2015, angka putus berobat berada di rentang 25-30%. Angka ini juga berada di atas angka putus berobat global DKI Jakarta merupakan salah satu provinsi yang memiliki jumlah kasus TB (termasuk TB MDR/RR) yang cukup besar. Sebagai provinsi dengan jumlah kasus yang besar, belum ada penelitian di DKI Jakarta mengenai faktor-faktor yang menyebabkan kejadian putus berobat pada pasien TB MDR/RR. Desain penelitian ini adalah kohort retrospektif. Data bersumber dari kasus TB MDR/RR di DKI Jakarta yang tercatat di E-TB Manager yang memulai pengobatan di tahun 2014-2015. Analisis statisik menggunakan uji Chi-Square. Ukuran asosiasi yang digunakan adalah risiko relative (RR). Angka putus berobat pasien TB MDR/RR di DKI Jakata tahun 2014-2015 adalah 37,1%. Umur diketahui berhubungan secara statistik dengan kejadian putus berobat sedangkan jenis kelamin, riwayat pengobatan TB, dan status HIV tidak berhubungan dengan kejadian putus berobat. Kata Kunci: kohort retrospektif, resisten obat, perilaku pencarian pengobatan

Abstract

Background: HIV / AIDS is still a global health problem which needs to be addressed including prevention of HIV / AIDS transmission from mother to child. Prevention of transmission is carried out by implementing the Prevention of Mother to Child Transmission or PMTCT program. The PMTCT program is considered successful in reducing the risk of mother-to-child transmission, but this has not been done well and thoroughly, various kinds of challenges have occurred in the implementation of the PMTCT program.Objective: To know the challenges that occur in implementing the PMTCT programMethod: This study uses a systematic review based on Preferred Reporting Items For Systematic Reviews & Meta-Analyzes (PRISMA)  to identify all the literature published using relevant keywords.Results: challenges in implementing the PMTCT program are the lack of information on HIV treatment, lack of family support, the heavy workload experienced by health workers and the limited availability of HIV testing equipment and drug stocks.Conclusion:  The success of efforts to prevent HIV / AIDS transmission from mother to child does not depend on one party only, but involves several parties, the active role of health workers in providing education and information about HIV / AIDS to mothers and their families is not enough but must be added with active roles and support from family members to HIV mothers as the primary motivator for good behavior in accordance with the guidelines for PMTCT.

Abstract

Dari hasil salah satu studi diketahui bahwa penggunaan obat nyamuk memiliki efek lebih besar dalam menanggulangi penyakit Demam Berdarah dibandingkan penggunaan larvasida dan fogging. Tujuan studi ini untuk mendapatkan informasi tentang penggunaan obat nyamuk di masyarakat. Studi ini merupakan studi deskriptif yang pengumpulan datanya dilaksanakan pada Februari-Maret 2013 di DKI Jakata dan kota Depok. Dari hasil studi ini didapatkan bahwa: Dari 83 responden yang mengisi kuesioner (terdiri dari 65% perempuan, 45% berpendidikan SLTA dan 11% salaah satu anggota keluarganya pernah menderta demam berdarah), didapatkan bahwa Jenis obat nyamuk yang sering digunakan adalah obat nyamuk lotion (32,5%), disusul dengan spray (26,5%), semprot/cair (18,1%), elektrik (15,7%) dan bakar (1,2%). Obat nyamuk yang dianggap paling efektif; lotion (29%), spray (19%), elektrik (16%) dan semprot/cair (13%). Obat nyamuk yang dianggap paling aman; lotion (31%), elektrik (30%), spray (15%) dan semprot/cair (12%). Alasan penggunaan obat nyamuk oleh masyarakat yaitu; murah, mudah di dapat dan memiliki aroma khusus. Studi menyarankan agar masyarakat menggunakan obat nyamuk dengan baik, benar dan aman.Kata kunci: obat nyamuk, studi deskriptif, obat nyamuk efektif dan aman
