Abstract

The prevalence of gingivitis increased in the puberty periods (Puberty gingivitis). Most of students of pesantren were in puberty age. The students of pesantren have a limited access to get periodontal treatment. This study evaluated the effectiveness of control plaque instruction and scaling with hand instrument for the reduction of puberty gingivitis in students of pesantren. The study sample consisted of 61 females aged puberty at a pesantren in Jember. Prior the study, the Plaque Index and Gingival Index were recorded. The sample then received plaque control instructions and scaling with hand instruments. Follow-up exams were made one month after treatment. The data were analyzed by Wilcoxon sign rank test. The Plaque Index and Gingival Index score decrease significantly between baseline and examinations at 30 days (p<0.05). The result showed that plaque control instructions and scaling with hand instruments is efficient to reverse puberty gingivitis. Plaque control instructions and scaling with hand instruments is a simple and inexpensive method of puberty gingivitis treatment in students of pesantren. Abstract

Maternal Porphyromonasgingivalis infection on periodontal tissue can result in Porphyromonasgingivalisdissemination to umbillical cord. Porphyromonas gingivalis presumably gain access to the systemic circulationvia local tissue inflammation, and may affect the placental development and the fetus itself. This study aimed toanalize the effect of periodontal infection with Porphyromonas gingivalis on placental development, and todetermine its effect on fetal growth in a pregnant rat model. Female rats were infected with live-Porphyromonasgingivalis at concentration of 2x109 cells/ml into subgingival sulcus of the maxillary first molar before and/orduring pregnancy. They were sacrified on gestational day (GD) 20. Fetuses were evaluated for weight andlength. All placentas were fixed in 10% buffered formalin, processed for paraffin embedding, and stained withhematoxylin and eosin.The histopathological analysis of placentas on GD 20 showed that trophoblast cells inlabyrinth and junctional zone had a greater density in control group than Porphyromonas gingivalis-infectedperiodontal maternal group. The nucleated-erythrocytes were found more abundant in the fetal blood vessels ofPorphyromonas gingivalis-infected periodontal maternal group than in the fetal blood vessels of control group.In conclusion, the impaired placental morphology influenced the normal function of placenta to maintain thegrowth and development of fetus. The decreased placental weightresulted in the decreased of fetal weightandlength.

Abstract

Cocoa beans are one of the most widely developed herbal plants, one of which is edel cocoa beans. Processing of cocoa beans can be through a fermentation process or not. The fermentation process can significantly reduce the concentration of total polyphenols and other active components in cocoa beans through oxidation and exudation processes. Cocoa beans are one of the plants that are rich sources of polyphenols. These polyphenols, especially flavonoids, have various benefits against several pathological disorders, such as cardiovascular disease, inflammatory processes, and cancer. In dentistry, cocoa bean extract gel has been shown to increase the number of fibroblast cells in the socket after tooth extraction of male Wistar rats, so that it has potential as a therapeutic agent. The requirement for a material to be used as therapy is that it must be safe and non-toxic, so it is necessary to carry out a biocompatibility test to ensure safety for the human body.Keywords: toxicity; fibroblast cells; cocoa bean extract

Abstract

Adhesion process of Porphyromonas gingivalis on neutrophils significantly play a role in the onset of periodontal infection. Papaya leaves contain several active substances that might impact to bacterial adhesion. This study was aimed to determine inhibition potency of papaya leaf extract (PLE) to adhesion of Porphyromonas gingivalis to neutrophils and at different concentration. This was an experimental laboratories in vitro study on neutrophils cells from peripheral blood. Samples divided into 5 group: group I/control (without PLE), group II (PLE 25%), group III (PLE 50 %), group IV (PLE 75%), and group V (PLE 100%). Isolated neutrofil was incubated by PLE in 3 hours, then incubated to the P. gingivalis in 2.5 hours. The index of adhesion was determined by calculating average number of P. gingivalis on 100 neutrophils. The result found that inhibition potency of PLE was significantly different to all groups. The conclusion was PLE inhibited P. gingivalis adhesion on neutrophils; and there were differences of inhibition potency of bacterial adhesion of P. gingivalis bacteria among PLE at 25%, 50%, 75% and 100% concentration, which higher concentration had higher inhibition effect.   Keywords: adhesion, neutrophils, papaya leaf, Porphyromonas gingivalis

Abstract

Background. Porphyromonas gingivalis (P. gingivalis) is plaque bacteria caused periodontitis with proteolitic characterised that makes some damages to the periodontal tissues. Lymphocytes are chronic inflamed cells which have specific characteristic when periodontitis was occured. Papaya is known as herbal plants, which contains substances which have antimicroba and antiinflamation activities in its leaves. The purpose of this research is to determinate the effect of papaya leaves extract to the number of limfocyte cells to the male-wistar rats gingival that was periodontitis. Twenty male-wistar rats that were divided to the five groups with different treatments. The first group is control of other group, the second group is treatment group that was induced with P. gingivalis and assembled with a ligature, while the remaining of groups were added with the extract of papaya leaves sondation by certain concentration 25% for the third group, 50% for the fourth group, and 75% for the last group after getting some treatments as the second group. The result of the research showed that the highest lymphocytes cells was found to group that was induced by P. gingivalis and assembled with a ligature. In the contrary, the lowest lymphocytes cells was found to the fifth group that was treated with the extract of papaya leaves 75%. It concluded that papaya leaves extract decreased the number of lymphocytes in periodontitis male-wistar rats gingival. The most effectively of papaya leaves extract that was used is 75%. Advanced research about the compound of the extract of papaya leaves which most influential and the mechanism to the decrease of the number of lymphocytes cells are needed. Abstract

Oral health among elderly people needs a specially attention because it will affect their general health. A good knowledge of oral hygiene is essential to prevent oral disease. Therefor maintenance of oral hygiene was the best way to maintain the health condition of the elderly people. The purpose of this research was to determine the correlation between oral health knowledge of elderly people with oral health status in the working area ofPuskesmas Kalisat Jember. This study was observasional research on 80 elderly people aged 50-70 years old who came to 12 elderly posyandu of Puskesmas Kalisat Jember. Data were collected by structured questionnaires about oral health knowledge and clinical examination to determine oral hygiene status based on Oral Hygiene Index-simplified (OHIS). Pearson correlation was used for statistical analysis. Most of participant demontratedoral health knowledge score as sufficient category (52,5%) and the oral hygiene status was moderate (57,5%). The correlation between the level of oral health knowledge with OHI-S index was significant (p< 0,05). Among elderly people, the knowledge of oral health correlated with the oral hygiene.Keywords: elderly, dental health, Index OHI-S, Puskesmas Kalisat Jember

Abstract

Background: The decreased level of estrogen during menopause may be one of the risk factors of periodontal disease. The influence of estrogen to periodontal tissue disturbance is mediated by the presence of estrogen receptor on tissue. The precise mechanism how the estrogens mediate this effect is still unclear. Purpose: The aim of this study was to determine the correlation between estrogen receptor α and ß on the periodontal pocket of women who had severe chronic periodontitis measured based on the periodontal pocket depth. Methods: Twenty four periodontitis patients from menopausal and productive women according to the criteria were examined upon her periodontal status and immunoexpression of estrogen receptor α and ß on their periodontal pocket wall. Results: The result showed that in the menopausal and productive women, immunoexpression of estrogen receptor α and ß was not correlated with the periodontal pocket depth (p>0.05). However, the pocket depth seemed to show higher correlation with immunoexpression of estrogen receptor α than that with estrogen receptor ß, r=0.37 vs. r=0.12 for menopausal women, and r=41 vs. r=0.11 for productive women. Conclusion: It was concluded that no significant correlation was found between the estrogen receptor and periodontal pocket depth both on menopausal and productive women, presumed that estrogen has little role in the severity of periodontitis based on periodontal pocket depth. However, the estrogen receptor α has valuable effect on the severity of periodontal disease more than the estrogen receptor ß.Latar belakang: Berkurangnya kadar estrogen pada masa menopause merupakan salah satu faktor resiko penyakit periodontal. Peran estrogen dalam kerusakan jaringan periodontal dimediatori oleh reseptor estrogen α dan ß yang terdapat dalam jaringan. Akan tetapi, mekanisme estrogen mempengaruhi efek ini sampai saat ini masih belum jelas. Tujuan: Penelitian ini bertujuan untuk menentukan korelasi antara reseptor estrogen pada poket periodontal wanita menopause penderita periodontitis kronis dengan keparahan periodontitis yang ditentukan berdasarkan kedalaman poket. Metode: Dilakukan pemeriksaan status periodontal dan immunoekspresi reseptor estrogen α dan ß pada dinding poket periodontal dari 24 wanita menopause dan belum menopause penderita periodontitis yang sesuai dengan kriteria yang telah ditetapkan. Hasil: Hasil mendapatkan bahwa immunoekspresi reseptor estrogen α dan ß tidak berkorelasi dengan kedalaman poket periodontal wanita menopause dan belum menopause (p>0,05). Meskipun demikian, kedalaman poket periodontal tampak lebih berkorelasi dengan reseptor estrogen α daripada dengan reseptor estrogen ß, dengan nilai r=0,37 vs r=0,12 pada wanita menopause, dan r=0,41 vs r=0,11 pada wanita belum menopause. Kesimpulan: Dapat disimpulkan bahwa tidak terdapat korelasi yang signifikan antara reseptor estrogen dan kedalaman poket periodontal wanita menopause dan belum menopause sehingga diduga bahwa estrogen mempunyai sedikit pengaruh pada keparahan periodontitis. Meskipun demikian, reseptor estrogen α tampak lebih berperan terhadap keparahan penyakit periodontal dibandingkan reseptor estrogen ß.
