Abstract

Background: Pneumonia is acute inflammation of the lung parenchyma. The long duration of hospitalization is associated with increased morbidity, nosocomial infections and treatment costs. Methods: The study was conducted from May to November 2019 at Saiful Anwar Hospital, Malang, with 40 pneumonia patients in the non-intensive community inpatient room divided into 2 groups. The treatment group performed pulmonary rehabilitation measures consisting of breathing exercise, effective coughing techniques, clapping, postural drainage and respiratory muscle training using spirometry incentives. Results: The duration of hospitalization for the treatment group was 2.25 days shorter (p<0.001). The average IL-6 level in the 5th day of the treatment group was lower than without treatment, that was 54.43 pg/ml (p=0.221). The mean of pressure threshold loading in the group without treatment was 31.5 cmH2O on first day and 36.35 cmH2O on the 5th day (p <0.001) and on first day treatment group was 32.9 cmH2O and 39.35 cmH2O on the 5th day (p=0.001), and the mean value of the 5th day of the treatment group was higher than without treatment (P=0.015). the mean of the 5th day of the treatment group was higher than without treatment (p=0.06).Conclusion: Pulmonary rehabilitation as adjunctive therapy shortens the duration of hospitalization. In both groups there was a decrease in IL-6 levels on the 5th day compared to zero and the lower IL-6 levels were not significant in the treatment group. There was a significant increase in the value of pressure threshold loading inspiration and expiration day fivecompared to day zero in the two groups and there was a significant increase in the Pressure Threshold Loading Inspiration mean treatment group compared to no treatment on the 5th day, but the Pressure Threshold Loading Expiration rate showed no significant increase.

Abstract

Background: Invasive candidiasis occurs in immunocompromised individuals as an opportunistic infection in patients with lung cancer. Although culture and histopathology remain the standard diagnosis of fungal infections, other tests are still needed to provide faster results. Human 1,3-β-D-Glucan (BDG) uses ELISA to detect candidiasis. β D-Glucan level from BAL fluid is positive when the cut-off is p 130 pg / mL. This study aims to determine differences in the levels of Human 1,3β-D-Glucan from Bronchoalveolar Lavage (BAL) fluid between the immunocompromised and immunocompetent groups in patients with suspected lung cancer at RSU Dr. Saiful Anwar Malang.Method: A cross-sectional study was conducted on 33 lung cancer patients who had risk factors for invasive candidiasis in Dr. Saiful Anwar Hospital Malang.Result: 33 study patients had significant differences in BAL levels of Human 1,3β-D-Glucan in the immunocompromised and immunocompetent groups (p = 0.009). In the different tests, there was a slight difference in the levels of BAL Human 1,3β-D-Glucan but it was not statistically significant based on age and sex in the immunocompromised and immunocompetent groups (p = 0.632, p = 0.338, p = 0.472, p = 0.667).Conclusion: Patients suspected of lung cancer have risk factors for invasive candidiasis with higher BDG levels due to immunoparalysis. There were significant differences in the BAL Human levels of 1,3β-D-Glucan in the immunocompromised and immunocompetent groups.

Abstract

The experience of breastfeeding mothers in RSIA X MalangPurpose: Several factors cause a low level of exclusive breastfeeding in Indonesia. One of them is breastfeeding, which is not immediately resolved by health workers in the hospital. Mothers who have just given birth in an MCH hospital expect help and assistance from health professionals to not have difficulty breastfeeding. This study is to uncover the experience of nursing mothers in an MCH hospital. Method: This phenomenology research took place in a hospital in Malang in April-May 2019 with five informants by purposive sampling. Data were from in-depth interviews and analyzed using an interpretive phenomenology analysis. Results: There were seven themes: satisfied and comfortable giving birth, health workers in hospitals did not deliver breastfeeding information and assistance, more comfortable breastfeeding because it was practical and economical, grateful to have struggled to provide breastfeeding so that children were healthy, disappointed and worried about breastfeeding, pain, and tiredness of breastfeeding, the desire to obtain breastfeeding support and information from the hospital. The mother expects to obtain breastfeeding support in the form of information and assisting breastfeeding so that the mother understands how to breastfeed and does not experience breastfeeding problems when returning home. Conclusion: Our study hospital did not consistently serve as a friendly environment for breastfeeding mothers.

Abstract

Background: Tuberculosis is an infectious disease caused by Mycobacterium tuberculosis (Mtb) which can be found on multiple organs, but mostly infect the lung. AFB smear is used to diagnose and evaluate therapy response, to show bacterial burden and the patient’s infectious status. Combination of antituberculosis drugs for a long period of time could cause poor compliance and increased risk of resistance. Mtb infection induce immune response and release cytokines, one of which is interleukin-2 (IL-2) that regulate T lymphocyte cell. Higher IL-2 levels is found on patient with high bacterial burdern.  This study aims to see the assossciation between IL-2 levels before and 30 days after DR-TB treatment, to see pre and post-treatment IL-2 levels with conversion of AFB smear, and correlation between pre-treatment IL-2 levels and AFB smear. Method: This was a cohort prospective study at RSUD dr Saiful Anwar Malang. Thirty nine DR-TB patients underwent AFB smear before and 30 days after treatment, while IL-2 levels was also measured by Enzym-Linked Immunosorbent Assay (ELISA) technique. Results: The levels of IL-2 before treatment was significantly higher compared to 30 days after treatment (P<0.001). There was no significant IL-2 levels difference between conversion and non-conversion group (P=0.23), and a weak, not-significant correlation between IL-2 levels and AFB smear (r=0.28; P=0.06). Conclusion: The levels of IL-2 was significantly higher before than after treatment and it cannot be used to determine the positivity of acid-resistant bacilli smears in the sputum of patients with DR-TB. The limited timeframe and biomarker in this study raise the possibility to observe IL-2 as well as other biomarkers after intensive phase of TB treatment for future studies.

Abstract

This study aimed to analyze the influence of service quality and brand image to re-visit interest. Samples were patients who were ambulatory and hospitalized at the Clinic Bidan Marlina Tangerang Selatan totaling 130 respondents. Regression analysis techniques were used to analyze the influence of service quality and brand image to re-visit interest is using SPSS (Statistical Package for the Social Sciences). Service quality and significant positive effect on the re-visit interest in Bidan Marlina Clinic Tangerang Selatan, every 1% increase in the variable quality of service will be followed by the increase in re-visit interest of 0.265 percent. While the brand image variable positive and significant effect on the interest re-visit the clinic Bidan Marlina Tangerang Selatan, every 1% increase in variable brand image will be followed by the increase in re-visit interest of 0.740 percent. Furthermore, the results also showed that there is a positive and significant influence on the independent variables of service quality and brand image together to re-visit interest. Service quality and brand image affects 65.7% of the interest to re-visit the clinic Bidan Marlina Tangerang Selatan.

Abstract

Abstract - This study aimed to analyze the impact of innovation and price on purchase decisions. The research was carried out at different places to get a full range of respondents. The sample was the owner of the vehicle Yamaha motorcycle in Tangerang Selatan totaling 150 respondents. Regression analysis techniques were used to analyze the impact of innovation and price of the purchasing decision is by using SPSS (Statistical Package for the Social Sciences). The analysis showed that the product innovation and significant positive effect on purchasing decisions, each increase of one unit in the variable product innovation will be followed by an increase of 0.401 unit purchasing decisions. The analysis also showed that prices positive and significant influence on purchasing decisions, any adjustment of the unit either affordability of the price, the suitability of the price of the product quality, price competitiveness, and suitability prices with benefits will influence the purchase decision of Yamaha motorcycles for 0538 unit. This shows that the innovation products made by Yamaha and Prices set affects 46.6% of the consumer decision in purchasing undergo a Yamaha motorcycle in Tangerang Selatan.Keywords : Innovation , Price, Purchase Decision, Motorcycles, Yamaha.

Abstract

Buying behavior in choosing health care is a long process. Segmentation is one of the things that underlies the decision of the patient. This study aims to determine the buying behavior of patients who do the first child labor and its relationship with buying behavior in RSIA Puri Bunda (PB) Malang. This research uses the cross-sectional method. The sampling technique used the total sampling method, ie all inpatients who gave birth to the first child at the time of this study were executed. The number of samples was 55 respondents in April-May 2017. Data were analyzed using multinomial logistic regression. The results showed that customer segmentation of RSIA PB was from Blimbing sub-district, 21-25 years old, last high school education, income <Rp.1.500.000,00, housewife, Islam, class of 3rd class and payment method using BPJS . Buying behavior of respondents is included in complex type and dissonance reducing buying behavior. Geographic, demographic, psychographic and behavioral segmentation is not related to buying behavior. Segments that have been detected are expected to get attention from the hospital to determine the marketing efforts that will be done next. In its marketing efforts, RSIA PB should pay more attention to customers who have a high-involvement type in order to provide all the information expected by the customer.
