Abstract

Introduction: The high cost of drugs or specific wound dressings for chronic wounds can be healed at least in two weeks depending on the stage of the wound and blood sugar levels. Hence the need for an innovative processing of natural resources (NR) to produce a chronic wound care products that are accessible to all people.  One of the current trends is to use protease enzymes (sap extract papaya) that serves as autolitic debridement in the treatment of chronic wounds. The aims of this research was to indentify characteristics of protease enzyme present in sap of papaya. Method: This study was a descriptive exploratory that doing pure protease enzyme extract of papaya sap. Results: Molecular weight measurement using a kit Pre-stained Protein Markers (Broad Range) for SDS PAGE, Nacalai tesque No. 02525 showed a homogeneous molecular weight that slightly above 28 kD. Measurements of the enzyme levels using NanoDrop Spectrophotometer ND kit-1000 (with a distilled water marker) showed a relatively homogeneous enzyme level with a range between 43% - 61% and 53, 88% in average. The addition of papaya sap extract into each tube containing 7 ml gelatin obtained results: the control group there were clots perfectly on gelatin; groups with the addition of 2 drops (± 0.5 ml) obtained 50% hydrolyzed gelatin; and groups with the addition of 4 drops (± 0.5 ml) obtained 75% hydrolyzed gelatin. Discussion: The results showed that papaya sap extraction synonymous with protease enzyme. Papaya sap extraction activity (protease enzymes) on hydrolyzing gelatin shows a qualitative picture. Therefore, it can be used as autholitic debridement. Further, the following study is essential to identify structure and activity of the enzyme with a more determinant (temperature, time, and dose).Keywords: sap papaya extraction, protease enzymes

Abstract

Introduction: Nurses’ organizational commitment in health services still low. The low organizational commitment of nurse impact burnout syndrome. Burnout syndrome leads to a reduce in the quality of care or service provided by nurses that could lead to low working motivation and increased attendance. The purpose of this studied was to saw the organizational commitment to reduce burnout syndrome of nurse. Methods: This researched was a descriptive studied with a sample of 126 respondents with stratified random sampling in Gresik hospital. Data were collected through filled out questionnaires and then analyzed descriptively. Results: The result showed that high organizational commitment of nurse (affective commitment, continuance commitment, and normative commitment) could reduce burnout syndrome of nurse (emotional exhaustion, depersonalization and a lack of personal accomplishment). Conclusion: The nurse organizational’s commitment impact to reduce burnout syndrome of nurse in hospitals. Intervention to reduce fatigue could be through direct personal intervention, direct organization as well as a combination of both. Intervention through personal focus reinforces the strategy of individuals in the faced of stressed, while the organization was directly focused on decreasing job demands. Some things that could be done to overcome burnout syndrome was by doing nurse empowerment. The benefits that nurses could received from empowerment were nurses capable of critical thinking, resolving problems, and developing a leadership stance among other empowerment promoting leadership positions, self esteem and professionalism of nurse. Keywords: organizational commitment; burnout syndrome ABSTRAK Pendahuluan: Komitmen organisasional perawat dalam pelayanan kesehatan masih rendah. Komitmen organisasional yang rendah akan menyebabkan burnout syndrome pada perawat. Burnout syndrome menyebabkan penurunan kualitas perawatan atau pelayanan yang dilakukan oleh perawat yang dapat mempengaruhi motivasi kerja yang rendah dan peningkatan absensi pada perawat. Tujuan dari studi ini adalah mengetahui komitmen organisasional untuk menurunkan burnout syndrome pada perawat. Metode: Penelitian ini merupakan studi deskriptif dengan sampel 126 responden perawat pelaksana dengan stratified random sampling di RS Gresik. Data dikumpulkan melalui kuesioner yang telah diisi dan kemudian dianalisis secara deskriptif. Hasil: Hasil penelitian menunjukkan bahwa komitmen organisasional yang tinggi pada perawat (komitmen afektif, komitmen kontinuan, dan komitmen normatif) dapat menurunkan burnout syndrome perawat (kelelahan emosional, depersonalisasi dan penurunan prestasi diri). Kesimpulan: Komitmen organisasional perawat memiliki pengaruh untuk mengurangi burnout syndrome perawat di rumah sakit. Intervensi untuk mengurangi burnout syndrome terutama pada dimensi kelelahan dapat melalui intervensi yang dilakukan secara pribadi, organisasi serta kombinasi keduanya. Intervensi dilakukan melalui fokus pribadi perawat untuk memperkuat strategi individu dalam menghadapi stres, sementara organisasi secara langsung difokuskan pada penurunan tuntutan pekerjaan. Beberapa hal yang dapat dilakukan untuk mengatasi burnout syndrome adalah dengan melakukan pemberdayaan perawat. Manfaat yang dapat diterima perawat dari pemberdayaan adalah perawat akan mampu berpikir kritis, menyelesaikan masalah, dan mengembangkan sikap kepemimpinan yang meliputi pemberdayaan dalam mempromosikan posisi kerja, harga diri dan profesionalisme perawat. Abstract

Introduction: Compliance is a level of person's behaviour in the therapeutic regimen, one of which is following the 3J diet. Dietary compliance for patients with DM may be caused by demographic and long-time factors with DM disease. The purpose of this research is to provide an overview of factors that influence the behavior of the Javanese type-2 tribe DM Diet compliance. Method: The study used a descriptive quantitative research design. Data was obtained through interviews with a questionnaire. Participants were people of type-2 DM who are in Javanese tribe in the work area of Krembung Puskesmas and Porong Sidoarjo Regency, selected purposive sampling a number of 105 participants. Data analysis in this study uses descriptive analysis. Results: The result of this research obtained by patients DM type-2 was dominated by women, age groups were susceptible to suffering DM is above age 65 years, with primary education level. Low dietary compliance was also a domination of the results of this study. Conclusions: Good meal planning will help patients with type-2 DM to be able to maintain dietary compliance for a long period of time, so that educational advice on the diet should be adjusted to the patient's prior dietary compliance level so that it can The patient is easily directed towards maximum compliance. Keywords: Diet, compliance, demographics. ABSTRAK Pendahuluan: Kepatuhan merupakan tingkatan perilaku seseorang dalam menjalani regimen terapeutik, salah satunya yaitu mengikuti diet 3J. Kepatuhan diet bagi penderita DM kemungkinan disebabkan faktor demografis dan lama pasien menderita penyakit DM. Tujuan penelitian ini adalah untuk memberikan gambaran faktorfaktor yang memengaruhi perilaku kepatuhan diet penderita DM tipe-2 suku Jawa. Metode: Penelitian ini menggunakan desain penelitian kuantitatif deskriptif. Data didapatkan melalui wawancara menggunakan kueisioner. Partisipan adalah penderita DM tipe-2 yang bersuku Jawa diwilayah kerja Puskesmas Krembung dan Porong Kabupaten Sidoarjo, dipilih secara purposive sampling sejumlah 105 partisipan. Analisis data dalam penelitian ini menggunakan analisis deskriptif. Hasil: Hasil penelitian ini didapatkan penderita DM tipe-2 didominasi oleh wanita, kelompok usia yang rentan menderita DM yakni diatas usia 65 tahun, dengan tingkat pendidikan SD. Kelompok penyakit penyerta terbanyak diderita penderita DM yakni hipertensi. Rendahnya kepatuhan diet juga menjadi dominasi dari hasil penelitian ini. Kesimpulan: Perencanaan makan yang baik akan membantu penderita DM tipe-2 untuk dapat mempertahankan kepatuhan diet dalam jangka waktu panjang, sehingga saran pemberian edukasi mengenai diet harus disesuaikan dengan tingkat kepatuhan diet pasien sebelumnya sehingga bisa pasien mudah diarahkan menuju ke kepatuhan yang maksimal. Abstract

Latar belakang: Diabetes melitus tipe II merupakan salah satu penyakit tidak menular yang dapat menyebabkan kematian dan memiliki risiko tinggi terjadi komplikasi. Penatalaksanaan empat pilar diabetes tipe II meliputi edukasi, terapi gizi medis, latihan jasmani dan intervensi non farmakologi. Salah satu penanganan non-farmakologi yang sering dilakukan adalah Diabetic Self Management Education (DSME) dan senam kaki diabetik, tetapi kombinasi keduanya belum pernah di teliti. Gabungan beberapa terapi disebut terapi kombinasi. Penelitian ini bertujuan untuk mengetahui pengaruh terapi kombinasi DSME dan senam kaki terhadap Ankle Brachial Index (ABI) pada penderita diabetes tipe II. Metode: Desain penelitian adalah penelitian Experimental dengan rancangan pretest-posttest control group design. Kelompok intervensi di beri terapi kombinasi DSME dengan senam kaki dan kelompok kontrol dengan pemberian Range of Motion (ROM). Teknik sampling non-probability dengan metode consecutive sampling dengan 48 responden yang terbagi dalam 2 kelompok. Hasil Rerata ABI kelompok Intervensi dan Kontrol sebelum perlakuan adalah 0,84 mmHg dan 0,82 mmHg, sedangkan setelah perlakuan adalah 1,09 mmHg dan 0,89 mmHg. Uji independent t test menunjukkan nilai p value 0,000 berarti ada perbedaan rerata selisih ABI kedua kelompok. Kesimpulan kombinasi Diabetic Self Management Education (DSME) dengan senam kaki efektif dalam peningkatan Ankle Brachial Index (ABI) pada penderita diabetes tipe II. COMBINATION THERAPY OF DIABETIC SELF MANAGEMENT EDUCATION (DSME) WITH DIABETIC FOOT EXERCISE TOWARDS ANKLE BRACHIAL INDEX (ABI) ON PATIENTS DIABETIC TYPE II Background: Type II diabetes mellitus is a non-infectious disease which cause death and have a high risk complications. Management of 4 pillars of type II diabetes includes education, medical nutrition therapy, physical exercise and non pharmacological interventions. One of the non-pharmacological treatments is the combination of Diabetic Self Management Education (DSME) and diabetic foot exercises. Study aims is to determine effect of a combination of Diabetic Self Management Education (DSME) with diabetic foot exercises on Ankle Brachial Index (ABI) in type II diabetics patients. Method: Research design was Quasy Experimental with pretest-posttest control group design. Intervention group was 24 patients type II diabetic with therapy combination of DSME and foot exercises, the control group was given Range of Motion (ROM) as therapy. Results: Mean of ABI intervention and control group before treatment are 0.84 mmHg and 0.82 mmHg, while after treatment are 1.09 mmHg and 0,89 mmHg Independent t test shows p value 0.000, that there is a differences of mean of ABI both group. It can be concluded that combination of DSME with foot exercises is effective to increase Ankle Brachial Index (ABI) at patients type II diabetics. Abstract

Introduction: Diabetes mellitus (DM) is a chronic disease with the characteristic of treatment complexity. The toughest challenge for DM patients is dietary adherence. On the other hand, the socio-culture is one of the factors that influences dietary adherence. The aim of this study was to examine the dietary adherence of adults with type 2 Diabetes mellitus (T2DM), particularly to dietary programs.Methods: This study used a qualitative case study design with a positivist approach. The total participants consisted of 14 T2DM patients obtained through snowball sampling. The research phenomenon was the adherence of adults with type 2 Diabetes mellitus to dietary programs. The data collection was conducted through in-depth interviews with question guidelines analyzed using qualitative thematic analysis.Results: This study obtained five themes regarding the dietary adherence of T2DM patients such as activity, motivation, intention, behavior, and the benefits of dietary adherence. The focus points were activity, motivation, and the intention to comply with the dietary recommendations, including reducing the fatty and fried foods consumed. The patients who felt the benefits of complying with the dietary program found following the dietary adherence to be easier.Conclusions: Activity, motivation, intention, behavior and the benefits of the dietary adherence program all make it easier for the patient to comply with their diet. This result suggests that health workers should provide education to the patients on the importance, benefits and the way to comply with the T2DM dietary program.

Abstract

Type 2 diabetes mellitus is a chronic metabolic disease that continues to develop over time and requires complex treatment. while undergoing treatment DM, diabetic patients need supports in order to be able to carry out their treatment on an ongoing basis. This study aims to describe supports obtained by Type 2 DM patients during their treatment and caring. This is a qualitative research study with phenomenological approach. Sampling was conducted from social situation with a sample size 20 participants. Data were collected using semi-structured in-depth interviews and analysis were used Colaizzi methods. The results showed three themes. Theme 1) family supports; 2) social supports, 3) health workers supports. Family supports is the most important part, but the supports from health workers also play a role in increasing the motivation of type 2 DM patients in undergoing treatment and diet. it is important to explore more deeply and more specifically how the role of health workers in the care of DM patients for further studies

Abstract

Arthritis Gout is a disease affecting the joints characterized. It was characteristic of monosodium urate monohydrate (MSU) accumulation in tissue synovial or other tissues. The prevalence of gouty arthritis in various countries in the world in the last few decades continues to increase. In patients, the main problem that occurs with gouty arthritis is pain and stiffness in the joints that recur, causing reddish swelling around the joints due to increased uric acid. Alternative treatment for arthritis gout in nursing interventions such as acupressure and bay leaf extract (Syzygium polyanthum). It could reduce and relieve pain in gout arthritis patients. This study examines the analysis of complementary nursing interventions of acupressure and bay leaf extract (Syzygium polyanthum) on pain in arthritis gout clients. This study used true experimental with approach pre-test and post-test control group design. Data collection involved 30 respondents with arthritis gout clients, selected through technique probability sampling with simple random sampling, divided into three groups. The intervention group received the therapy acupressure and bay leaf extract (Syzygium polyanthum). The first control group received the therapy acupressure, and the second control group received bay leaf extract therapy (Syzygium polyanthum). Pain intensity decreased in the intervention, control one, and managing two groups. But based on the One Way ANOVA test on the pain variable, it showed that the intervention group had the most significant reduction with a value (4.70±1.05) in the control group 1 (3.10±0.99) and control group 2 (3.0±1.05) with a significant p-value of 0.001 (<0.05). Conclusion giving complementary nursing intervention acupressure and bay leaf extract (Syzygium polyanthum) effectively reduces pain in arthritis gout patients

Abstract

The recovery of intestinal peristalsis in post cesarean section patients shows the recovery of digestive system function after receiving spinal anesthesia, so it should be restored as soon as possible. This study aims to analyze the effect of foot reflexology on increasing intestinal peristalsis in postoperative sectio caesarea patients. This study applied a non-equivalent control group design, involving 30 patients. Data on intestinal peristalsis were collected by auscultation, then analyzed using the Mann-Whitney U test. The mean value of the frequency of intestinal peristalsis in the group that received the intervention in the form of reflexology on the soles of the feet was 3.67 times/minute; while for the control group, the mean frequency of intestinal peristalsis was 1.67 times per minute. The results of the Mann-Whitney U test resulted in a p-value = 0.000. This shows that there was a difference in the frequency of intestinal peristalsis in postoperative cesarean section patients between the intervention group and the control group; So it is concluded that reflexology on the soles of the feet is effective in increasing the frequency of intestinal peristalsis in post-cesarean section patients. Abstract

In Malang City, Indonesia, the number of motorized vehicles is increasing from year to year which causes congestion and air pollution. In this case, there are volunteer traffic control assistants (Supeltas), who are suspected of having affected their oxygen saturation. The purpose of this study was to analyze the relationship between the length of time exposed to air pollution with oxygen saturation in the blood of traffic controller assistant volunteers in Malang City. The design of this study was cross-sectional, involving 30 respondents. Data on the length of time worked were collected through interviews, in addition to checking oxygen saturation through blood gas analysis. The results showed that most of the respondents had oxygen saturation less than normal, with 12-18 months of work time, and 3 hours of work per day. Contingency coefficient test results show p-value = 0.000; which means there is a relationship between the length of time exposed to air pollution with oxygen saturation in the blood.
