Abstract

Pendahuluan: Instalasi forensik seharusnya terdapat bukan hanya di rumah sakit umum, tetapi juga di Rumah Sakit Gigi dan Mulut untuk menindaklanjuti kasus yang membutuhkan ilmu forensik kedokteran gigi. Tujuan penelitian adalah mempelajari gambaran unsur manajemen instalasi forensik di Rumah Sakit Kepolisian Pusat Sukanto, Rumah Sakit Bhayangkara Sartika Asih serta Laboratorium dan Klinik Odontologi Kepolisian sebagai acuan pembentukan Instalasi Forensik Kedokteran Gigi. Metode: Penelitian ini lakukan secara deskriptif dengan metode observasi. Lokasi penelitian di Rumah Sakit Kepolisian Pusat Sukanto, Rumah Sakit Bhayangkara Sartika Asih, Laboratorium dan Klinik Odontologi Kepolisian. Hasil: Dari hasil penelitian menunjukan Instalasi Forensik Rumah Sakit Kepolisian Pusat Tingkat I Sukanto, Rumah Sakit Bhayangkara Sartika Asih serta Laboratorium dan Klinik Odontologi Kepolisian memiliki banyak kesamaan dari aspek unsur manajemen. Simpulan: Manajemen Instalasi Forensik yaitu Rumah Sakit Kepolisian Pusat Sukanto, Instalasi Forensik Rumah Sakit Bhayangkara Sartika Asih serta Instalasi Forensik Odontologi Laboratorium memiliki banyak kesamaan dari segi unsur manajemen dan dapat menjadi acuan untuk pembentukan unsur manajemen instalasi forensik kedokteran gigi.Kata kunci: Unsur manajemen, instalasi forensik, instalasi forensik kedokteran gigi. ABSTRACTIntroduction: Forensic installations should be available not only in public hospitals, but also at the Dental and Oral Hospital to follow up on cases that require dental forensic science. The purpose of the study was to study the description of elements of forensic installation management at the Sukanto Hospital, Central Police, Bhayangkara Sartika Asih Hospital and the Police Odontology Laboratory and Clinic as a reference for the establishment of a Dentistry Forensic Installation. Methods: This research is done descriptively by observation method. Research locations at Sukanto Hospital, Central Police Department, Bhayangkara Sartika Asih Hospital, Police Odontology Laboratory and Clinic. Results: From the results of the study showed the Forensic Installation of the Central Police Hospital Sukanto, Bhayangkara Sartika Asih Hospital and the Police Odontology Laboratory and Clinic have a lot in common with aspects of management. Conclusion: Management of forensic Installations, Sukanto Hospital Central Police Hospital, Bhayangkara Sartika Asih Hospital Forensic Installation police have many similarities in terms of management elements and can be used as a reference for the management of dentistry forensic installations.Keywords: Management elements, forensic installation, odontology forensic installation.

Abstract

Pendahuluan: Perkiraan usia dilakukan untuk mengetahui usia seseorang. Saat ini perkiraan usia banyak digunakan, kepentingannya adalah untuk forensik dan bidang kedokteran gigi. Tujuan dari penelitian ini adalah mengetahui apakah Metode Al Qahtani mampu mengidentifikasi usia melalui radiograf panoramik di RSGM FKG Unpad. Metode: Penelitian ini dilakukan secara deskriptif sederhana dengan menggunakan software atlas Metode Al Qahtani, dengan melihat perkembangan gigi dan tingkat erupsi dari masing-masing radiograf panoramik. Hasil: Sebanyak 94 sampel yang diteliti, hanya 66 sampel dengan hasil usia gigi berdasarkan Metode Al Qahtani yang sama dengan usia pasien, sedangkan dari sisanya terdapat 28 sampel dengan usia gigi Metode Al Qahtani yang berbeda dengan usia pasien. Simpulan: Metode Al Qahtani dapat mengidentifikasi sebagian besar (70.21%) usia melalui radiograf panoramik di RSGM FKG Unpad dengan hasil usia yang bervariasi pada kelompok usia anak.Kata kunci: Identifikasi usia, metode Al Qahtani, radiograf panoramik ABSTRACTIntroduction: Estimated age is done to determine someone’s age. At present age estimates are widely used, its importance is for forensics and the field of dentistry. The purpose of this study was to determine whether the Al Qahtani Method was able to identify age through panoramic radiographs at RSGM FKG Unpad. Methods: This study was carried out in a simple descriptive manner using the atlas software of the Al Qahtani Method, by observing the development of teeth and the eruption rate of each panoramic radiograph. Results: A total of 94 samples were studied, only 66 samples with dental age results based on the Al Qahtani Method which were the same as the patient’s age, whereas of the remaining 28 samples with teeth age, the Al Qahtani Method was different from the patient’s age. Conclusion: Al Qahtani method can identify the majority (70.21%) of age through panoramic radiographs at RSGM FKG Unpad with varying age results in the age group of children.Keywords: Age identification, Al Qahtani method, panoramic radiograph.

Abstract

Pendahuluan: gingival enlargement umumnya terjadi akibat inflamasi kronis yang disebabkan oleh faktor lokal, seperti induksi plak pada gingiva dan faktor sistemik seperti  pengaruh hormon dan obat-obatan sistemik tertentu. Hidrosefalus merupakan kondisi yang diakibatkan adanya volume cerebrospinal fluid (CSF) berlebih yang tidak terserap sempurna, dalam jumlah yang sangat tinggi pada ventrikel di otak dan terkadang juga terdapat di ruang subarachnoid, yang menyebabkan terjadinya dilatasi ventrikel secara progresif. Kondisi ini menghasilkan peningkatan tekanan intrakranial dan sering merusak jaringan di sekitarnya. Pasien hidrosefalus menunjukkan pembesaran kepala dan mengalami keterlambatan pertumbuhan. Tujuan penelitian laporan kasus ini adalah menjelaskan keberhasilan terapi fase inisial perawatan periodontal pada gingival enlargement pasien anak dengan hidrosefalus. Laporan kasus: Pasien hidrosefalus laki-laki berusia 12 tahun, mengalami pembesaran gingiva pada rahang atas, sering berdarah pada saat tersentuh sikat gigi ataupun terkena sentuhan lainnya, terjadi kurang lebih sejak dua bulan yang lalu, pasien tidak mengonsumsi obat-obatan secara sistemik. Hasil pemeriksaan klinis dan radiologis dapat ditegakkan diagnosis inflammatory gingival enlargement rahang atas disertai periodontitis kronis gigi 27, dengan diagnosis banding periodontitis kronis. Gingival enlargement merupakan faktor predisposing pembentukan plak. Gingival enlargement pada pasien ini dirawat dengan terapi inisial yaitu Oral hygiene Instruction (OHI), scaling, root planing, dan kontrol. Simpulan: Terapi fase inisial periodontal berupa OHI, scaling, root planing, dan kontrol, berhasil menghilangkan gingival enlargement pada pasien anak dengan hidrosefalus.Kata kunci: Gingival enlargement, hidrosefalus, perawatan inisial periodontal. ABSTRACTIntroduction: Gingival enlargement generally occurs due to chronic inflammation caused by local factors, such as plaque induction on the gingiva and systemic factors such as hormonal influences and certain systemic drugs. Hydrocephalus is a condition that results from the incompletely absorbed excess volume of cerebrospinal fluid (CSF), with very high amounts in the ventricles in the brain and sometimes also in the subarachnoid space, leading to progressive dilation of the ventricles. This condition results in increased intracranial pressure and often damages surrounding tissue. Hydrocephalus patients show head enlargement and growth delay. This case report was aimed to describe the success of the initial phase of periodontal therapy in gingival enlargement of paediatric patients with hydrocephalus. Case report: A 12-year-old male hydrocephalus patient, had enlarged maxillary gingiva, often bled when touched by a toothbrush or when exposed to other touches, occurred for about two months prior, and the patient did not take any medication systemically. The clinical and radiological examination results can confirm the diagnosis of inflammatory maxillary gingival enlargement with chronic periodontitis in tooth #27, with a differential diagnosis of chronic periodontitis. Gingival enlargement is a predisposing factor for plaque formation. Gingival enlargement in this patient was treated with initial therapy, namely oral hygiene instruction (OHI), scaling, root planing, and control. Conclusion: The initial periodontal therapy phase in the form of OHI, scaling, root planing, and control, succeeded in eliminating gingival enlargement in paediatric patients with hydrocephalus.Keywords: Gingival enlargement, hydrocephalus, initial periodontal therapy.

Abstract

Pendahuluan: Sinus maksilaris, yang disebut juga Antrum Highmore merupakan sinus yang sering terinfeksi. Satu di antara penyebabnya adalah karena sinus ini merupakan sinus paranasal yang terbesar dan bentuknya bervariasi di setiap individu. Radiografi panoramik merupakan satu di antara teknik radiografi yang dapat melihat gambaran kedua sinus dan hubungannya terhadap gigi serta relatif aman karena paparan radiasinya tidak sebesar teknik radiografi lain. Penelitian mengenai prevalensi sinusitis maksilaris odontogenik telah banyak dilakukan tetapi peneliti belum menemukan adanya penelitian serupa di wilayah Bandung, Jawa Barat. Penelitian ini bertujuan untuk mengetahui prevalensi suspek sinusitis maksilaris odontogenik ditinjau dari radiograf panoramik di RSGM FKG Unpad. Metode: Jenis penelitian ini adalah deskriptif. Pemilihan sampel dilakukan dengan teknik purposive sampling. Hasil: Hasil penelitian dari 44 sampel yang diteliti, terdapat suspek radiologis sinusitis maksilaris odontogenik sebanyak 16 radiograf. Simpulan: Simpulan dari penelitian ini adalah bahwa prevalensi suspek kasus sinusitis maksilaris odontogenik pada arsip radiograf panoramik pasien yang mengalami infeksi pulpo apikal yang datang ke Instalasi Radiologi RSGM Unpad pada periode Juli-September 2013 adalah sebesar 36,36% dengan suspek kasus banyak terdapat pada populasi usia dewasa muda dan lanjut, dengan proporsi jumlah yang sama pada populasi perempuan dan laki-laki, dan lebih banyak melibatkan infeksi dari gigi molar pertama dan kedua. Kata kunci: Panoramik, prevalensi, sinus maksilaris, odontogenik. ABSTRACTIntroduction: Maxillary sinus, also called Antrum Highmore, is a sinus that is often infected. One of the causes is because this sinus is the largest paranasal sinus and its shape varies in each individual. Panoramic radiography is one of the radiographic techniques that can see both sinus images and their relationship to teeth and is relatively safe because exposure to radiation is not as large as other radiographic techniques. Research on the prevalence of odontogenic maxillary sinusitis has been carried out but researchers have not found similar studies in the Bandung, West Java region. This study aims to determine the prevalence of odontogenic maxillary sinusitis suspicions from panoramic radiographs at RSGM FKG Unpad. Methods: This type of research is descriptive. Sample selection is done by purposive sampling technique. Results: The results of the 44 samples studied were radiological suspects of 16 radiographs of odontogenic maxillary sinusitis. Conclusion: The conclusions of this study are that the prevalence of odontogenic maxillary sinusitis cases in panoramic radiographs of patients who have apical pulpo- rine infection who came to the Radiology Hospital Unpad installation in the period of July-September 2013 was 36.36% with many cases suspected in the population. young and advanced adulthood, with the same proportion of women and men, and more involving infections from first and second molars.Keywords: Panoramic, prevalence, maxillary sinus, odontogenic.

Abstract

Introduction: Epidemiological studies show a significant increase in mandibular fractures in the past few decades. Various radiographic projections are indicated for the diagnosis of mandibular fractures. This study was aimed to gather information on the type of radiographic examination commonly prescribed in the diagnosis of mandibular fracture among the oral maxillofacial surgeons and factors influencing the choice of radiographic prescription. Methods: A descriptive study with survey method was conducted on 30 respondents in the Oral Surgery Departments of three hospitals in Bandung, by purposive sampling technique. The data collected using the self-structured questionnaire method and assessed according to frequency distribution. Results: Highest choice of radiographic prescription in the survey for each location of fractures in the mandible was panoramic (83.3 to 100%), and the least is CBCT (10 to 16.7%). The most factors influencing the choice of prescription were availability (53.3 to 56.7%), image accuracy (50 to 60%) and cost (46.7 to 56.7%) of the radiographic examination. Conclusion: Most of the dentists in this study prescribe panoramic radiographs in mandibular fracture assessment based on availability, image accuracy, and cost of the radiographic examination.

Abstract

Introduction: Abnormalities of the dental pulp can have several different diagnoses. Therefore, the dental pulp characteristics must be known in more detail and clear so that diagnosis be established more precisely and accurately.  One characteristic of the pulp can be seen from the density value through the 3D Cone Beam Computed Tomography (3D CBCT). Methods: The study  was  conducted  with  a  simple  descriptive  method.  The population is all the data 3D CBCT of patients who visited the Universitas Padjadjaran Dental Hospital (RSGM Unpad) in 2012. Samples of the research were 75 pulps  with normal, inflammation, and necrosis conditions and calculate the average density value. Results: Density values for dental pulps in the normal teeth between 465 - 775 HU, the inflammation teeth between 243.5 - 396 HU, and necrosis teeth between - 461.5 - -170 HU. Conclusion: There are differences in dental pulp density between the normal pulp, inflammation and necrosis through 3D CBCT.

Abstract

Introduction: Child abuse often occurs among children and leaves a negative impact on the rest of their life. The purpose of this study was to determine the significance of the manifestation of violence on the teeth, jaw, and facial from the view of odontology forensic. Methods: This study was descriptive research. Forty subjects were involved, which were taken using the purposive sampling method. Data obtained in the form of questionnaires and presented in tabular form. Results: The result has shown that 20.32% of the subjects had oral injuries. 20.32% of the subjects had oral bleeding, followed by bleeding gums, tooth bleeding, tooth mobility, tooth fractures, swollen gum, and swollen lips—injuries to the facial area including laceration, incision and burns. The highest incidence was 70.59% of facial injuries involving the middle third of the face, followed by the upper third, and then lower third of the face. Conclusion: The most manifestation of violence among the child abuse case involved are observed in dental and facial areas.

Abstract

Introduction: Age estimation has an important role in the process of human identification and needs to be performed in an efficient, fast, and accurate ways. Age estimation based on the pulp cavity volume of canines was referring to the theory stated that the apposition of secondary dentine during human life could be determined as an age indicator. This study was aimed to estimate the human age based on the pulp volume of canines for chronological estimation. Methods: Forty-one canine teeth from Cone Beam Computed Tomography (CBCT) 3D image sample from Dental Hospital of the Faculty of Dentistry Universitas Padjadjaran segmented into dental pulp and enamel were prepared for measurement of pulp volume using the ITK SNAP volumetric software. Descriptive analysis of pulp volume and regression equations were generated from regression analysis of pulp volume and tested for age estimation. Result: Dentine thickness was found to be increased from adolescence age range until adulthood age range. Logarithmic regression between pulp volume was statistically significant (p < 0,001). The coefficient of determination (R2) was 0.753 with a standard deviation of 8.421. Mathematics model for estimating age was: Age = 79.523 – (18.194 x ln(Pulp Volume)). The pulp cavity and dentine thickness were decreasing along the age. The same equation was also used in the same sample. There were several differences between chronological age and estimated age approximately 8.4 years following the regression result of R2 (75.3%). The rest of the sample (24.7%) have a big difference, and this could be affected by modifying variables such as occlusal force and human error measurement procedures. Conclusion: The human age can be estimated by the pulp volume of canines.Keywords: Age estimation, pulp volume, canine, ITK-SNAP, CBCT.

Abstract

Introduction: Every individual has a different lip print pattern that facilitates personal identification. Every individual has the physical characteristics that are different from other individuals. The physical characteristics can be an identity for the individual. The purpose of this research was to investigate the lip print pattern among Deutromalayan subrace with a modification of lip print formulation technique. Methods: A descriptive research was conducted among 82 male and female students of 2008 batch students of Faculty of Dentistry Universitas Padjadjaran. The research was an observation of lip print from each subject in the digital view from the scanning of a lip seal. The observation was carried out on all parts of the fissure in the subject’s lip print. The data was then analysed to a lip print pattern formulation based on fingerprint formulation. Results: Lip print pattern of Deutromalayan subrace was different among each other and clarified in a formulation consisted of elements from the central fissure type, amount of median fissure, fissure tracing and all lip print type. Conclusion: Lip print pattern among Deutromalayan subrace was identified by modification of lip print formulation technique that was clarified on the formulation.

Abstract

Introduction: Many children with post palatoplasty had crossbite posterior. This study was aimed to assess the nasopharynx area and the posterior crossbite severity level of children with cleft lip and palate (CLP) who received palatoplasty treatment compared to normal children. Methods: The study was observational analytic. The research subject was 14 children with CLP post-palatoplasty and 14 normal children. The object of research was 28 study models and secondary data of lateral cephalometric radiograph of children with CLP post palatoplasty and normal children. The measurement of PTM-ad1-Ad2-PTM and PTM-So-Ba-PTM were used to measure the nasopharyngeal area. Study models were assessed to analyse the level of severity of posterior crossbite. Results: The average of the soft tissues (the nasopharynx) area children with CLP post-palatoplasty was 35.02 mm2, which was lower than the normal child (35.73 mm2). Similarly, the average of the hard tissues (the nasopharynx) area children with CLP post-palatoplasty was 301.40 mm2, which was smaller than the normal children (315.54 mm2). Statistical analysis of the nasopharynx area resulted in non-significant difference. All children with CLP post-palatoplasty was suffered from posterior crossbite. The level of severity posterior crossbite, which was categorised as good was 42.9%, poor criteria was 35.7%, moderate criteria was 14.3%, and very good criteria was 7.1%. Conclusion: There is no difference between the average size of the nasopharynx area on children with CLP post-palatoplasty and normal children. The level of severity posterior crossbite after palatoplasty in CLP children mostly included in the good criteria.
