Abstract

Village AIDS Cadres (KDPA) Program has been conducted in Denpasar district since 2010. Despite the importance of community participation for a successful community based program, no research has been conducted to explore community perception regarding KDPA program.This research is aimed to explore community perceptions regarding acceptability, utilization, and adoption of the KDPA program from community perspective.The design used in this research is descriptive with qualitative data collection method through six focus group discussions in six villages in Denpasar. Data was analysed using thematic analysis.KDPA is well-accepted as it is perceived as an important issue to be tackled in the community. However, many opinions arise concerning who should be given the role "cadre". Lack of program introduction to the community, also high stigma and discrimination resulted in low utilization of the program. Villages with active KDPA have conducted a variety of HIV-related activities rarely carried out by the less active villages. There is Lack of community participation on HIV-related activities. Inequity of access to HIV information exists between adults and teenagers, between men and women, and between the member of traditional and non-traditional hamlets. Program adoption is hampered by the lack of commitment of village authorities and limited resources of KDPA program.The utilization and adoption of KDPA program are sub-optimal. Introduction of KDPA program to the community and provision of program resources should be improved.

Abstract

ABSTRAK Pada era Jaminan Kesehatan Nasional salah satu upaya yang dilaksanakan untuk menanggulangi penyakit kronis adalah Prolanis. Puskesmas se-Kota Denpasar telah melaksanakan program prolanis tetapi terdapat perbedaan rasio kunjungan peserta prolanis tertinggi dan terendah. Tujuan penelitian ini untuk mengetahui gambaran persepsi peserta BPJS Kesehatan terhadap program pengelolaan penyakit kronis di Puskesmas I Denpasar Barat dan Puskesmas II Denpasar Timur. Penelitian ini merupakan penelitian deskriptif kualitatif dengan purposive sampling menggunakan metode wawancara mendalam dan focus group discusion. Penelitian ini dilaksanakan di Puskesmas I Denpasar Barat dan Puskesmas II Denpasar Timur dari bulan Mei-Juni 2017. Informan penelitian berjumlah sembilan belas orang yang terdiri dari kepala puskesmas, pemegang program prolanis dan peserta BPJS Kesehatan yang tergabung dalam kelompok prolanis. Hasil penelitian menunjukkan peserta prolanis di Puskesmas I Denpasar Barat merasa rentan terkena penyakit diabetes/ hipertensi dan dampak penyakit tersebut menimbulkan ketakutan dan kecemasan pada peserta karena efek keseriusan penyakit sudah dilihat/ dirasakan. Besarnya manfaat yang dirasakan mampu menutupi hambatan yang dialami, selain itu dukungan teman dan rasa kekeluargaan meningkatkan partisipasi peserta prolanis. Maka dari itu puskesmas yang rasio kunjungan prolanis rendah perlu mengundang keluarga yang menderita diabetes/ hipertensi untuk memberikan testimoni. Kata Kunci : Prolanis, Persepsi, Puskesmas ABSTRACT In the era of National Health Insurance, one of the efforts undertaken to tackle chronic illness is Prolanis. Health centers throughout Denpasar have implemented prolanis programs but there are differences in the ratio of visits of the highest and lowest prolanis participants. The purpose of this study was to determine the description of BPJS Health participants' perceptions of chronic disease management programs in Puskesmas I Denpasar Barat and Puskesmas II Denpasar Timur. This research is a qualitative descriptive study with purposive sampling using in-depth interviews and focus group discussion methods. The research was conducted at the Health Center I of West Denpasar and Health Center II of East Denpasar from May-June 2017. The informants of the study consisted of nineteen people consisting of the head of the puskesmas, the prolanist program holder and BPJS Health participants who were members of the prolanist group. The results showed prolanis participants in Puskesmas I Denpasar Barat felt vulnerable to diabetes / hypertension and the impact of the disease caused fear and anxiety in participants because the seriousness of the disease had been seen / felt. The amount of benefits felt is able to cover the obstacles experienced, in addition to the support of friends and a sense of family to increase participation of prolanist participants. Therefore a puskesmas with a low prolanis visit ratio needs to invite families suffering from diabetes / hypertension to provide testimonials. Keywords: Prolanis, Perception, Puskesmas

Abstract

ABSTRACT The result of 2017 Indonesian Health Demographic Survey (SDKI 2017) shows the number of traditional contraception uses in Bali Province has increased from 7% in 2012 to 13% in 2017. The use of traditional contraception methods in Bali is double that of national figures. The purpose of this study is to determine the factors associated with the choice of traditional contraception methods in urban and rural areas of married women in Bali Province. This study is a secondary data analysis from the 2017 SDKI. The sample size is 357 women ages 15-49 years. There are 22 independent variables that are studied, and the dependent variable is the use of traditional contraception methods. The results of multivariate analysis showed that significantly related factors are uppermiddle category wealth quintile (p = 0.027, OR = 11.16, 95% CI = 1.31-94.54), top category wealth quintile (p = 0.049, OR = 9.04, 95% Ci = 1.01-80.77), obtained family planning information from the pharmacist (p = 0.034, OR = 2.1, 95% CI = 1.05-4.17), side effects of contraception (p = 0.002, OR = 2.95, 95% CI = 1.48-5.88), decision making to use contraception with partner (p = 0.029, OR = 2.43, 95% CI = 1.09-5 , 43), and decision-makers using family planning only husband/partner (p = 0.015, OR = 4.24, 95% CI = 1.32-13.56). The variables that become protective factor is the newspaper reading variables (p = 0.008, OR = 0.42, 95% CI = 0.23-0.79). The conclusion that can be drawn from this study is the use of traditional contraception methods in married women in Bali Province influenced by wealth quintiles, habits of reading the newspaper, obtaining contraception information from pharmacists, side effects of using contraception and those who make decisions about using contraception. ABSTRAK Hasil Survei Demografi Kesehatan Indonesia (SDKI) 2017 menunjukkan angka penggunaan KB tradisional di Provinsi Bali mengalami peningkatan dari 7% pada 2012 menjadi 13% pada 2017. Penggunaan alat/cara KB tradisional di Bali dua kali lipat lebih banyak dibandingkan angka nasional. Tujuan penelitian ini adalah untuk mengetahui faktor-faktor yang berhubungan dengan pemilihan alat/cara KB tradisional di daerah perkotaan dan perdesaan pada wanita kawin di Provinsi Bali. Penelitian ini merupakan analisis data sekunder dari data SDKI 2017. Besar sampel adalah 357 wanita usia subur (WUS) dengan usia 15-49 tahun. Terdapat 22 variabel bebas yang diteliti, sedangkan variabel terikatnya adalah penggunaan alat/cara KB tradisional. Hasil analisis multivariat menggunakan analisis regresi logistik menunjukan bahwa faktor -faktor yang berhubungan secara signifikan terhadap pemilihan alat kontrasepsi tradisional adalah kuintil kekayaan kategori menengah atas (p=0,027, OR=11,16, 95% CI=1,31-94,54), kuintil kekayaan kategori teratas (p=0,049, OR=9,04, 95% Ci=1,01-80,77), mendapatkan informasi KB dari apoteker (p=0,034, OR=2,1, 95% CI=1,05-4,17), efek samping penggunaan kontrasepsi (p=0,002, OR=2,95, 95% CI=1,48-5,88), mengambil keputusan menggunakan KB bersama pasangan (p=0,029, OR=2,43, 95% CI=1,09-5,43)), dan pengambil keputusan menggunakan KB hanya suami/pasangan (p=0,015, OR=4,24, 95% CI=1,32-13,56). Variabel yang menjadi faktor protektif adalah variabel membaca koran (p=0,008, OR=0,42, 95% CI=0,23-0,79). Simpulan yang dapat ditarik dari penelitian ini adalah penggunaan alat/cara KB tradisional pada wanita kawin di Provinsi Bali dipengaruhi oleh kuintil kekayaan, kebiasan membaca koran, mendapatkan informasi KB dari apoteker, efek samping penggunaan kontrasepsi dan pihak yang mengambil keputusan dalam menggunakan KB. Kata kunci : Penggunaan alat/cara KB, KB tradisional, WUS di Bali.

Abstract

ABSTRAKDalam upaya mencapai Jaminan Kesehatan Semesta (Universal Health Coverage) tahun 2019 maka seluruh elemen kesehatan diharapkan dapat bekerja sama, termasuk rumah sakit RSIA Harapan Bunda sebagai salah satu fasilitas kesehatan tingkat lanjut yang bekerja sama dengan BPJS Kesehatan tercatat mulai pada bulan Maret 2015. Tujuan dari penelitian ini adalah untuk mengetahui gambaran implementasi program JKN di RSIA Harapan Bunda. Penelitian ini merupakan penelitian dengan desain deskriptif kualitatif. Pengumpulan data dilakukan dengan wawancara mendalam kepada responden yang telah ditentukan dengan metode purposive sampling sebanyak 13 orang. Hasil penelitian menunjukkan bahwa RSIA Harapan Bunda perlu persiapan dari bagian manajemen selama 6 bulan agar dapat menerima pasien JKN. Persiapan dari pembentukan tim penanggung jawab pelaksanaan JKN di rumah sakit dan sarana prasarana. Keuntungan secara nominal selama 1 tahun pelaksanaannya belum didapat hanya masyarakat semakin mengenal RSIA Harapan Bunda sehingga kunjungan pasien bertambah. Petugas medis memberikan pelayanan yang sama kepada pasien umum dan pasien pengguna JKN, berdasarkan pada SOP yang telah ditetapkan oleh rumah sakit. Petugas penunjang medis telah berbekal pelatihan tentang penggunaan program INA-CBGs di rumah sakit.Pasien sebagai penerima pelayanan kesehatan merasa sangat terbantu dengan adanya program JKN. Prosedur dalam memperoleh pelayanan kesehatan sudah diketahui, namun masih ada pasien yang belum memahami dengan baik sehingga ada kesalahpahaman dengan petugas pendaftaran rumah sakit. Sosialisasi untuk penyatuan persepsi dan komitmen dari setiap komponen rumah sakit agar seluruh karyawan dapat memahami program ini dan mengetahui hal-hal apa saja yang harus diupayakan untuk mewujudkan pelayanan kesehatan yang cost effective. Abstract

One of the reasons for establishing Public Service Agency (BLU) policy at Community Health Care Centers is the number of complaints for poor quality services. This policy was implemented in January 2010 by the Gianyar Government in every Community Health Care Center in Gianyar.  This is the realization of the Gianyar Government's attention in improving the quality of the Community Health Care Center Services.  This study aims to determine the public perception of the quality of services provided by the Community Health Care Center in Gianyar District. This study was a cross-sectional analytic study and conducted at four Community Health Care Centers with BLUD’s status in Gianyar District with 105 respondents selected by multistage random sampling method. The data collection included survey questionnaires and analyzed descriptively by chi square test. The results showed that overall the respondents had a "good" perception towards the elements of Community Health Care Center services with BLU status in Gianyar District. However, there are three elements that were "perceived as poor" comprising responsiveness (14.3%), accuracy (11.4%) and environmental comfort (12.4%). The results of the chi square test showed that there were significant differences between the age factors with the respondent perceptions of service quality in Community Health Care Center with BLU status (p = 0.004), but gender, education level and occupation did not have a meaningful relationship. This research suggests that the Health Office should increase the intensity of coaching and facilitation of procedures related to the Community Health Care Center so that the expected performance of attendant care becomes more patterned.

Abstract

Cervical cancer disease is one of cancers that attack many women in the world and ranks second afterbreast cancer. Symptoms of cervical cancer may cause degradation quality of patients life.Chemotherapy is one of treatments of cervical cancer patients. Chemotherapy provides therapeuticeffects, however, it can causes a decrease in life quality due to its side effects. BOMP is one ofregimens that can be used as one of the management of cervical cancer. Assessment of quality ofpatients life with cervical cancer are needed to see the changes in the quality of life in patients besidesknowing the functional status changes from time to time. Monitoring the effects of treatment, andcollecting the data on quality of life can be used as initial data for consideration in formulating theappropriate action for the patient, And also assist physicians in selecting an effective and welltolerated drug for patients.The study was conducted in February to June 2014 in Obstetrics Clinic andCempaka Timur Room General Hospital (RSUP) Sanglah in observational design with cross sectionalprospective. The sample selection is done by consecutive sampling. The methods used for datacollection using questionnaires EORTC QLQ with interview techniques before and afterchemotherapy in patients suffering cervical cancer of squamous cell stage IIB - IIIB who had BOMPchemotherapy regimens for 3 series in RSUP Sanglah. Research for quality of life is conducted ingeneral and the 15 domains that affect the quality of life of patients. In this study, 12 patients indicatedthe inclusion criteria. There were significant differences in the value of the quality of life of patientswith cervical cancer squamous cell stage IIB - IIIB before and after BOMP chemotherapy with 0.001p value. BOMP chemotherapy regimens can improved the quality of life of cervical cancer patientsincreasing from 44.833 ± 6.235 to 60.333 ± 9.168. In 15 ratings domain included decrease the qualityof life of pain domain, nausea domain, vomiting domain, decreased appetite domains, fatigue domain,physical function domain, functional role domain, social functioning domain, sleeplessness domain,constipation domain and domain of financial difficulties and on the other hand emotional domainincrease.

Abstract

ABSTRACT Midwifery as paramedic in a village plays an important role in primary health care in order to pursue Indonesia Sehat 2010. Midwifery should provide good services with adequate tariff for society. As one indicator of service, the use of unit cost method has become an important part in tariff calculation. It was found that knowledge of the local Midwifery about unit cost was quite low. The activity of community services was carried out at IKK-IKP department Faculty of Medicine, on 01st September 2007. This activity aimed to improve knowledge and ability of midwifery of how to use Unit Cost in tariff calculation. The method was practicing on calculate several health care services cost. The result of this activity was midwifery can create adequate tariff base on unit cost.

Abstract

The images of teenagers in the world, especially in South Asia and Southeast Asia have less information about reproductive and sexual health because of the assumption that it is unnecessary even considered harmful if they knew it. It’s not surprise that the teen birth rate is still quite high. previous research in Indonesia on adolescent premarital sexual behavior, obtain a yield of about 25% - 51% of teens have sex before marriage. (Utomo ID 2009; 40 (2) :. 133-46.). Studies in Bali obtain the results of teenage boys in high school (SMA) and junior high school (SMP) more premarital sex (40.3% and 29.4%) than girls (3.6% and 12.5%). Teenage boys in Bali more permissive toward premarital sexual behavior than girls and about 5% of teens have sex before marriage (Faturochman 1992; 1 :. 12-7) Adolescents have a right to a healthy life. They need to have access of the tools that can protect their health, such as condoms, contraception and youth-friendly health services are comprehensive. Governed by Law no. 36/2009 Article 4 and 5, which states that everyone has the same right to gain access of health and this rule does not see marital status. The purpose of this study was to describe the general perception of access and use of contraception among adolescents in the city of Denpasar.

Abstract

AbstrakRS X merupakan salah satu rumah sakit swasta di Tangerang yang berhasil mengimplementasikan JKN (Jaminan Kesehatan Nasional) dengan efisiensi biaya tanpa mengabaikan mutu layanan. Penelitian ini bertujuan untuk mengetahui strategi RS X dalam pelaksanaan Program JKN dengan metode Balanced Scorecard. Penelitian dilakukan pada Mei 2018 dengan pendekatan kualitatif. Data dikumpulkan melalui wawancara mendalam kepada 7 informan pihak RS dan 2 informan dari BPJS Kesehatan. Subjek dipilih berdasarkan purposive sampling dan dianalisis dengan analisis tematik. Hasil penelitian menunjukkan ada beberapa strategi yang telah dikembangkan dan diterapkan yaitu dari (1) Perspektif keuangan (menambah jenis layanan, meningkatkan kapasitas pada rawat inap dan poliklinik, menerapkan prinsip low cost dan meningkatkan modal kerja, menambah slot dokter, melengkapi perawatan medis menerapkan prinsip low cost, dan meningkatkan modal kerja); (2) Perspektif pelanggan (menentukan target market, mengembangkan 5 nilai tambah, dan manajemen komplain);  (3) Perspektif proses bisnis internal (pembuatan standar operasional prosedur, menyusun formularium dan clinical pathway, melakukan pengendalian dan audit operasional, menetapkan bisnis model, pembentukan tim casemix, serta manajemen klaim); (4) Perspektif pembelajaran dan pertumbuhan (reward dan gaji bagi karyawan, lingkungan kerja kompetitif serta memperhatikan jenjang karir karyawan). Melalui strategi tersebut, RS X mampu mengimplementasikan Program JKN dengan baik dan tetap membukukan surplus tanpa mengorbankan kualitas pelayanan. AbstractOne of the private hospital successful implementation the National Health Insurance with cost efficiency without neglecting the quality of service is X Hospital. This study aimed to finding out the strategy of X Hospital in the implementation of the National Health Insurance with Balanced Scorecard. The research was conducted in May 2018 using qualitative approach. Data was collected through in-depth interview to 7 informants from hospital and 2 informants from BPJS Kesehatan. The subjects were selected purposively and the data was analyzed using thematic analysis. The results show that there are several strategies that have been developed and implemented, including: (1) financial perspective (adding types of services, increasing the capacity of inpatients and polyclinics, increasing doctor practice slots, completing medical equipment, applying the principle of low cost and increasing working capital); (2) customer perspective (choosing target market, develop 5 values propotition, complaint management); (3) internal business processes perspective (develop standard operating procedures, develop drug formulary and clinical pathways, carry out operational and audit controls, build business models, form case mix teams and claim management); (4) learning and growth perspective (develop training, giving reward and good salaries, build a competitive work environments, and pay attention to employee career paths). Through this strategy, X Hospital is able to implement the JKN Program well and still record a surplus without sacrificing service quality.Keywords: Balanced scorecard, hospital strategy, national health insurance

Abstract

A lot of misinformation about COVID-19 on social media possibly hinder the practice of healthy behaviors that are essential to protect individuals. This situation is exacerbated by the lack of health literacy in Indonesia. This study aimed to determine the influence of social media information exposure on the knowledge and behavior of Indonesians in compliance with COVID-19 protocols. An online survey of 909 people was conducted from July 2nd to August 10th, 2020. The data collected were knowledge and behavior of preventing COVID-19, while independent variables were socio-demographic characteristics and exposure to social media information. Inclusion criteria were defined as follows: minimum age of 18 years and domiciled in the country during the data collection period. The analysis used for the data collected were univariate and multivariate. The result showed that gender, age, marital status, and social media presence significantly affect a person's knowledge about COVID-19 with a p-value of 0.05. Enforcement behavior of health and healthy living protocols is significantly influenced by the respondent’s gender, marital status, education level, the island of residence, and exposure to online information. The frequency of exposure to information affects people's knowledge and behavior to implement health protocols and healthy living in the COVID-19 era, and it is further influenced by socio-demographic characteristics.
